OBJECTIVES--To examine general practitioner consultations by demographic and socioeconomic variables and to derive a method of measuring the impact of relative deprivation on general practitioner workload. DESIGN--The study was based on general practitioner consultations reported in the general household surveys of 1983-7, covering a sample of 129,987 individuals in Great Britain. Odds ratios for general practitioner consultations were obtained for selected variables among children (0-15 years), men (16-64), women (16-64), and elderly people (greater than or equal to 65). These were then used to derive deprivation indices specific to electoral wards for use in general practice. SETTING--Great Britain, with particular findings illustrated by English electoral wards and the conurbations of London, Manchester, Merseyside, and the West Midlands. RESULTS--Council tenure increased the likelihood of consultation significantly in all four groups. Odds ratios were raised in children, men, and women with ...

What does a pediatric dentist do?. Pediatric dentistry offices are colorful, fun, and child-friendly. Dental phobias are often rooted in childhood, so it is essential that the child feel comfortable, safe, and trusting of the dentist from the outset.. The pediatric dentist focuses on several different forms of oral care:. Prevention - Tooth decay is the most prevalent childhood ailment. Fortunately, it is almost completely preventable. Aside from providing advice and guidance relating to home care, the pediatric dentist can apply sealants and fluoride treatments to protect tooth enamel and minimize the risk of cavities.. Early detection - Examinations, X-rays, and computer modeling allow the pediatric dentist to predict future oral problems. Examples include malocclusion (bad bite), attrition due to grinding (bruxism), and jaw irregularities. In some cases, optimal outcomes are best achieved by starting treatment early.. Treatment - Pediatric dentists offer a wide range of treatments. Aside from ...

The professional practice of behavior analysis is one domain of behavior analysis: the others being radical behaviorism, experimental analysis of behavior and applied behavior analysis. The professional practice of behavior analysis is the delivery of interventions to consumers that are guided by the principles of behaviorism and the research of both the experimental analysis of behavior and applied behavior analysis. Professional practice seeks maximum precision to change behavior most effectively in specific instances. Behavior analysts are mental health professionals and, in some states, may hold a license, certificate or registration as a behavior analyst. In other states, there are no laws governing their practice and, as such, the practice may be prohibited as falling under the practice definition of other mental health professionals. This is rapidly changing as Behavior Analysts are becoming more and more common. The professional practice of behavior analysis is a hybrid discipline with ...

During the past years it has been reported that musculoskeletal disorders (MSD) have increased in working life. The body region with the highest prevalence of reported pain is the neck region. Several studies show that MSD in the neck region varies between 20-68% among dentists and dental hygienists. The dental personnel work with a limited field of vision and space, which often results in an unnatural work posture. The aim of this study was to evaluate the prevalence of neck pain in dentists and dental hygienists working in the county of Västra Götaland, Sweden and to explore possible work-related and individual risk factors. The study was based on a questionnaire, which was sent out to all dentists and dental hygienists (n = 473) who worked at Folktandvården (response rate 60%). The overall prevalence of neck pain (neck pain every day the past month) was 23%. Female dentists and dental hygienists had an increased risk of neck pain compared to male dentists (OR 2.68, 95% CI 1.22 - 6.75). ...

Acamprosate has been shown to reduce drinking days in alcohol dependent patients and promote abstinence, with few reported side effects. A limitation of these studies, however, has been their lack of generalizability due to restrictive inclusion and exclusion criteria. Furthermore, most of the previous studies of acamprosate have been conducted in Europe, in a different treatment setting from the typical American Family Medicine center, where alcohol dependent patients are most likely to be first identified in the U.S.. The present study is designed to determine the efficacy of acamprosatefor alcohol dependence in a Family Medicine setting using minimal psychotherapeutic interventions-as would also likely occur in a primary care setting. The study will be a 12-week, double-blind, placebo-controlled, randomized trial comparing 666 mg acamprosate t.i.d. to placebo in patients at the UNC Family Medicine Center with alcohol dependence. Subjects will be seen by Family Medicine physicians and receive ...

The authors who recently reviewed the barriers that inhibit the implementation of hypertension management guidelines in Canada1 neglected to mention what might be one of the most important factors: the powerful influence of pharmaceutical manufacturers marketing campaigns on physician practice patterns.2 The freebie phenomenon was addressed in a news item in the same issue of CMAJ in which the review appeared.3 Flip through the pages of that particular issue and you will come across 5 glossy advertisements promoting angiotensin-converting-enzyme inhibitors or AT1 receptor blockers in the treatment of hypertension. Clinical practice guidelines are reflected only in footnotes in tiny print stating that the drugs being advertised are indicated when treatment with diuretics or β-blockers is ineffective or not appropriate.. If the groups that create clinical practice guidelines are wondering how to influence physicians practices more effectively across the country, perhaps they should take a ...

For those anxious to get any work done to their mouths, different types of sedation dentistry are becoming more available, which make a trip to the dentist less scary and sometimes even relaxing. |b| By Meagan Thomas |/b|
Antibiotics are commonly prescribed to critically ill patients throughout the world, with rates as high as 60%.1 A major concern is the almost universal observation that 31 - 77% of these are inappropriate.1-5 The inappropriate use of antibiotics is associated with increased morbidity, mortality and cost, and is a major driver in the emergence of resistant pathogens. Antibiotic prescription practices in South African intensive care units (ICUs) have not been described in the public or private sectors. Apart from its economic implications, this information is relevant because of the emergence of extremely high levels of drug resistance among Gram-negative bacilli in South Africa.6. As a continuum of the National Critical Care Audit, the Critical Care Society of Southern Africa (CCSSA) undertook a 1-day prevalence study of infection as a first step to ascertain the national profile of sepsis among critically ill patients.7-11 One aspect of the study was to ascertain antibiotic prescription ...

Think of a pediatric dentist as the pediatrician of dentistry. Like pediatricians, pediatric dentists are specially trained to treat the specific needs of infants, children and teens. Pediatric dentists are recognized by the American Dental Association (ADA) as dental specialists; they complete four years of training plus two years in residency. For these reasons, the pediatric dental office is one of the most important resources for moms and dads. Its where parents can rest assured that their kids will receive the best in pediatric dental care.. Beyond their dental expertise, pediatric dentists also know how to interact with kids. This can be a huge help, especially during your childs first dental visit or if your child has dental anxiety. And most pediatric dental practices are designed just for kids. While each pediatric dental office is different, a ""kid-friendly environment"" usually means plenty of toys and other activities, pillows & blankets and goodie bags.. Consider a Kids ...

Background: Antibiotics are frequently prescribed for upper acute respiratory tract infections (ARI) in the emergency department. To reduce inappropriate overprescribing, it is necessary to understand factors influencing physicians decisions to prescribe antibiotics. Objective: Analyze the judgment policies of emergency physicians to determine factors predicting antibiotic use. Design: Paper case vignette study. Participants: 104 emergency physicians from Wisconsin. Measurements: We used judgment analysis to derive the policies of 104 emergency physicians from their responses to 20 case vignettes. We designed the cases such that each physicians use of clinical findings and patient factors could be inferred from the decisions they made about each case. Findings were compared to primary care practitioners (PCPs) in Colorado responding to the same paper cases to examine differences in factors influencing prescribing among the two groups. Results: The emergency physicians said they would prescribe an
Background: Clinical practice guidelines (CPG) are thought to be an effective tool in improving efficiency and outcomes of clinical practice. Physicians adherence to guidelines is reported to be poor. We evaluated the relationship between generalizability of guidelines on hypertension and physicians adherence to guidelines recommendations for pharmacological treatment. Methods: We used the Sixth Joint National Committees (JNC VI) guidelines on hypertension to evaluate our hypothesis. We evaluated the evidence from controlled clinical trials on which the JNC VI bases its recommendation, and compared the population enrolled in those trials with the American hypertensive population. Data on this population came from the National Health and Nutritional Examination Survey III. Results: Twenty-three percent of the NHANES population had a diagnosis of hypertension, 11% had hypertension requiring drug treatment according to the JNC VI. A dental surgeon is a regular dentist or also known as a general dentist. The general dentist will perform various procedures throughout their day, including restorative dentistry, veneers, tooth whitening, bridge work, crowns, root canals, and some forms of oral surgery. While they can perform all these procedures, oral surgery is not their main focus of practice.. An oral or maxillofacial surgeon uses some of the same equipment and instruments as the general dentist, however; most of their tools are more extensive and specialized to allow them to work more quickly, delicately and precisely. The oral dentist will also have had extensive training in anesthesiology, so they can offer intravenous sedation to make their treatments more comfortable.. Before the apicoectomy, you may require additional x-rays of your tooth and its surrounding bone. Most times, an antimicrobial mouth rinse is provided to reduce your inflammation, or you may be given antibiotics. The dentist will need to review your ...

My fiancee also had the privilege of working for an experienced dentist (,35 years in his own practice) and then having his two kids gradually take over the practice shortly after they graduated from dental school. This reinforced her personal belief that you should not necessarily trust a newly commissioned dentist. She had worked at the practice for two years prior to going to dental hygiene school, and during the summer period between her two academic years, so she experienced the change in the office. Her personal opinion about new dentists over the 1-2 years that these newly commissioned dentists had been working had not changed -- she would still rather have a dentist with ,4 or 5 years of experience. The biggest things she complained about were that the new dentists lacked the experience necessary to evaluate whether or not a patient needed a surgery/procedure, and that the new dentists were more concerned with the profit of the dental offices.. so they would default to recommending an ...
