An analysis of the cost effectiveness of screening should take several variables into account, such as the prevalence and incidence of infection in the population, the risks of transmission, the mortality and morbidity of those infected with the virus, and the expected survival rate of patients receiving blood components from donors infected with the virus.. The calculation of the expected seroprevalence of human T cell leukaemia/lymphoma virus types I and II in new blood donors was based on the prevalence in donors who did not come from endemic countries.16 A more strict assessment of donors has been introduced in Sweden and potential donors from areas endemic for infection with HIV, human T cell leukaemia/lymphoma virus, or hepatitis B are excluded from donating blood. However, an increasing number of Swedish citizens are born to immigrants from endemic areas.. The incidence of infection among already established blood donors was estimated from the prevalence in Swedes and was based on the ...
As diagnostic tests for HTLV become more reliable, the worldwide patterns of distribution of HTLV infections are becoming more clear. Many samples initially thought to be HTLV positive because of some WB reactivities were subsequently found to be negative by other techniques such as PCR (13, 29).. WB techniques commonly use HTLV-1 viral lysate spiked with recombinant antigens derived from HTLV-1 and HTLV-2 protein sequences. To detect the humoral immune response, which varies among infected individuals, an optimal concentration of each HTLV antigen is required. However, detection and typing of HTLV antibodies are not always feasible by WB due to inherent difficulties for optimization of the technique. In addition to the variability of the immune response to HTLV infections, the parameters measured by serological assays for HTLV seem to widely occur in other infectious and noninfectious diseases. Thus, the results for many ELISA-reactive samples remain unresolved by WB techniques. Other tests, ...
The HTLV WB seroindeterminate frequency varies according to HTLV-1/2 endemicity, i.e., to the geographical area studied. Among blood donors in areas of low endemicity (Europe and the United States), the seroindeterminate WB patterns consist of faint isolated Gag reactivity (2, 12, 32). They occur at a frequency similar to true HTLV-1 seropositivity (ranging from 0 to 0.022% among blood donors) (26). In such populations, the HGIP appears to be very rare (26). Although some uncertainty remains, WB-indeterminate blood donors are generally counseled that they are not infected with HTLV (9, 13, 25, 35, 55). By contrast, in tropical areas such as Central Africa, Melanesia, and some regions of southeast Asia and South America, the prevalence rate of the indeterminate WB reactivities is high, representing in some cases more than 50% of all WB profiles (8, 30). Of the indeterminate WB patterns, HGIP makes up a large proportion. In the present study, HGIP represented the most common WB pattern, with 42% ...
From 1986 to 1993, 160 patients with nodal and extranodal lymphoma, excluding those who were positive for human T cell leukaemia/lymphoma virus type I antibody, were treated at Kobe University School of Medicine Hospital. No patients were positive for HIV-1 antibody. We excluded 27 patients (21 with nodal lymphoma, six with extranodal lymphoma) because adequate data were not documented. Thus 133 patients (74 with nodal lymphoma and 59 with extranodal lymphoma) remained in the study. The median age at diagnosis was 62 in patients with nodal lymphoma and 60 in patients with extranodal lymphoma. The distributions of age, sex, and histological findings were not significantly different between the group of patients with nodal lymphoma and the group with extranodal lymphoma except that the prevalence of patients with a follicular-type lymphoma was higher in the first group. The body mass index (weight (kg)/(height (m)2)) was 21.8 (SD 2.90) in the group of patients with nodal lymphoma and 22.6 (3.58) ...

N2 - The health effects of chronic human T-cell lymphotropic virus type I (HTLV-I) infection were examined in a cohort of Japanese men who had emigrated from Okinawa, Japan, and had been participants in a prospective study in Hawaii since 1965. In the present follow-up study carried out in 1987-1988, various health indicators were measured in the subjects, whose mean age was 72.5 years. Participation rates were lower in the HTLV-I seropositives than in the seronegatives (46.7% vs. 76.0%) in the ≥75-year age group. Lack of participation was significantly correlated with a high HTLV-I antibody titer. Among the participants, seropositive subjects were significantly more likely than the seronegatives to have lymphocytopenia (32.7% vs. 17.7%) and mild anemia (25.5% vs. 14.1%) after ...

I feel like my heart is truly broken, said his mother, Vivian Lefferts of Normal. After my son died, I thought How will I tell people about John? How will I keep his memory alive? A path chosen by Vivian and Larry Lefferts was to keep their sons memory alive by helping others to live. They agreed that their son would be a tissue donor. His two corneas and 35 bones and tissues have improved the lives of 37 people in 14 states. Its a way for his legacy to live on, said Eleanor Hess of Normal, Johns sister. They have spent much of the past eight years encouraging people to register to be organ and tissue donors by telling Johns story. inhibitor, just about everyone who responds eventually has the cancer become resistant.. Every treatment is likely to have some side effects, and some can be serious. She may go a very, very long time without it affecting her quality of life or survival. The key question I think needs to be asked is not should we do nothing or do something?, but rather, do you want to blow your best options while shes feeling fine and potentially without symptoms for years, or do you want to save your treatments for when you need them? Its not now or never, but now or later. And how bad would it be if she went on a treatment and had a serious or fatal complication? Fortunately, thats not likely, but its possible to have permanent problems or even die from chemo or tarceva or avastin complications. Its tremendously unfortunate if it happens, but if its a setting of facing an imminent disease-induced problem, thats less of a travesty than if were treating a scan rather than the person..
