AU - Jahangiri, Leila. AU - Katz, Ralph V.. PY - 2012/8. Y1 - 2012/8. N2 - Background. In 2002 and 2009, two consensus statements-one from a symposium in Canada and one from England-were issued that recommended that the first-choice standard of care for an edentulous mandible should be the two implants-retained mandibular overdenture (IRMOD). The authors conducted a survey to determine if, in 2011, U.S. academic prosthodontic expertsopinions were aligned with those in the two consensus statements. Methods. The authors administered a Delphi method survey to an expert panel of 16 nationally representative academic prosthodontists to determine if there is consensus on the first-choice standard of care for an edentulous mandible between the IRMOD and a conventional mandibular complete denture (CD). Consensus agreement was ...
Background: There is uncertainty about the most important indicators of pulmonary exacerbations in CF.. Methods: Two parallel Delphi surveys in 13 CF centres (UK and Ireland). Delphi 1: 31 adults with CF, ≥ one exacerbation over 12 months. Delphi 2: 38 CF health professionals. Rounds 1 and 2 participants rated their level of agreement with statements relating to indicators of exacerbation; Round 3 participants rated the importance of statements which were subsequently placed in rank order.. Results: Objective measurements were of higher importance to health professionals. Feelings of increased debility were rated most important by adults with CF.. Conclusions: There were clear differences in perspectives between the two groups as to the most important indicators of an exacerbation. This highlights that CF health professionals should take more cognisance of specific signs and symptoms reported by adults with CF, especially since these may be a precursor to an exacerbation.. ...
To the best of our knowledge, this is the first study which has explicitly sought to understand current consensus on medication management strategies for ADHD within the constraints of real-world practice. Whereas the number of Delphi panelists (n=12) may be considered relatively small, this is sufficient to demonstrate consensus. There is no consensus in the literature on optimum sample size31 and panel sizes ranging from 10 to 1142 participants are apparent in published Delphi studies.32 However, Jones & Hunter19 recommend a Delphi panel size of between 9 and 12 participants. Although the sample size of the online survey, workshop and focus groups were also small, the combination of opinion of multiple stakeholders to inform the items of the Delphi survey enabled a more complete approach to understanding issues pertinent to clinicians, young people, parent and carers in relation to ADHD medication management. Given that the participant is often over-looked in determining best practice, our ...

The use of health information technology (HIT) to improve patient safety is widely advocated by governmental and safety agencies. Electronic-prescribing and smart-pump technology are examples of HIT medication error reduction strategies. The introduction of new errors on HIT implementation is, however, also recognised. To determine the impact of HIT interventions, clear medication error definitions are required. This study aims to achieve consensus on defining as medication errors a range of either technology-generated, or previously unaddressed infusion-related scenarios, common in the paediatric intensive care setting. This study was conducted in a 23-bed paediatric intensive care unit (PICU) of an Irish tertiary paediatric hospital. A modified Delphi technique was employed: previously undefined medication-incidents were identified by retrospective review of voluntary incident reports and clinical pharmacist interventions; a multidisciplinary expert panel scored each incident using a 9-point Likert
Delphi. The aim of this study was to obtain an expert consensus on which history factors are most important in the clinical diagnosis of lumbar spinal stenosis (LSS). LSS is a poorly defined clinical syndrome. Criteria for defining LSS are needed and should be informed by the experience of expert clinicians. Phase 1 (Delphi Items): 20 members of the International Taskforce on the Diagnosis and Management of LSS confirmed a list of 14 history items. An online survey was developed that permits specialists to express the logical order in which they consider the items, and the level of certainty ascertained from the questions. Phase 2 (Delphi Study) Round 1: Survey distributed to members of the International Society for the Study of the Lumbar Spine. Round 2: Meeting of 9 members of Taskforce where consensus was reached on a final list of 10 items. Round 3: Final survey was distributed internationally. Phase 3: Final Taskforce consensus meeting. OBJECTIVES: To gain consensus among an ethnically and linguistically diverse group of international child protection experts on the structure and content of a new survey tool for retrospective measurement of child abuse, and to determine the performance of the instrument through an international field trial with young adults.. METHODS: The questionnaire was developed through focus group discussions with international experts, and then subjected to a Delphi study in two waves to determine the perceived importance and translatability of items. The resultant questionnaire was translated into six languages and field tested in seven countries with convenient samples of young adults aged 18-26 years (N=842).. RESULTS: Child maltreatment experts from 28 countries provided input to questionnaire development. Satisfactory agreement on draft item inclusion and exclusion and the translatability of items was gained. The tool includes 15 primary questions about potentially abusive physical, sexual and ...
These are the first recommendations for the management of hand OA to be developed by EULAR. As with the previous EULAR recommendations for management of knee OA7,8 and hip OA,9 we used an evidence based format that presents both research evidence and expert opinion, with clear separation between the two. We employed a Delphi technique to generate propositions and to reach consensus in an unbiased democratic fashion, and undertook a systematic evidence based medicine approach to identity and appraise the research evidence. The EULAR VAS scale was employed to show the strength of recommendation and the level of concordance within the Task Force for each proposition. This is based not just on the research evidence but also on the opinion of each expert, taking into account efficacy, safety, availability, logistical issues, and perceived patient acceptability. Although evidence based methodology continues to evolve, we consider that this current system of generating and presenting recommendations ...
Genetic counsellors have been working in some European countries for at least 30 years. Although there are great disparities between the numbers, education, practice and acceptance of these professionals across Europe, it is evident that genetic counsellors and genetic nurses in Europe are working autonomously within teams to deliver patient care. The aim of this study was to use the Delphi research method to develop a core curriculum to guide the educational preparation of these professionals in Europe. The Delphi method enables the researcher to utilise the views and opinions of a group of recognised experts in the field of study; this study consisted of four phases. Phases 1 and 4 consisted of expert workshops, whereas data were collected in phases 2 and 3 (n = 35) via online surveys. All participants in the study were considered experts in the field of genetic counselling. The topics considered essential for genetic counsellor training have been organised under the following headings: (1) ...
BACKGROUND: Successful implementation of newborn screening (NBS) for cystic fibrosis (CF) depends on robust protocols, good communication and appropriate management of recognised infants. In response to current varied practice, the ECFS Neonatal Screening Working Group developed a consensus on the early management of these infants using the Delphi methodology. METHODS: Following detailed literature review, statements were generated by a core group of experts and then assessed by a larger group using modified Delphi methodology. RESULTS: Forty-one statements were written by the core group. Eighty-six CF specialists contributed to the modified Delphi process. During three rounds, extra statements were added and consensus achieved on 44 (one statement did not achieve consensus). CONCLUSIONS: These statements will provide a framework for the management of screened infants in the first year of life. This process highlights the paucity of evidence on which to base management of these infants. To ...
This study involved a Delphi inquiry concerning the characteristics of resiliency specific to African American children/youth. The study was conducted with a large group of African American parents who were considered experts in resiliency because they had graduated from high school and had at least one child who had graduated from high school. Through a series of three Delphi surveys, the parents moved toward consensus concerning the most important characteristics of resiliency that contributed to their success and the success of their child(ren); as well as those that hindered their success and the success of their child(ren). Data were analyzed using descriptive statistics and qualitative analysis. The main characteristics of resiliency defined by the parents as contributing to or hindering their success or the success of their child(ren) included (a) spiritual/faith, (b) positive/negative personal traits, (c) family involvement/problems, (d) positive/negative educational supports, (e) ...
OBJECTIVE: Hip pain and injury as a result of activity can lead to the development of early hip osteoarthritis (OA) in susceptible individuals. Our understanding of the factors that increase susceptibility continues to evolve. The ability to clearly identify individuals (and cohorts) with activity-related hip pain who are at risk of early hip OA is currently lacking. The purpose of this study was to gain expert consensus on which key clinical measures might help predict the risk of early hip OA in individuals presenting with activity-related hip pain. The agreed measures would constitute a standardised approach to initial clinical assessment to help identify these individuals. METHODS: This Dephi study used online surveys to gain concordance of expert opinion in a structured process of rounds. In this study, we asked What outcome measures are useful in predicting hip OA in activity-related hip pain? The Delphi panel consisted of experts from sport and exercise medicine, orthopaedics, rheumatology,

Explicate the Management Strategies and Strategic Components to Identify Structural Practices (Participants: approximately 5775 online participants).. We propose to use an iterative online engagement strategy based on the Qualtrics [40] platform to explore variation in Management Strategies and Structural Practices focused on HAI-prevention across an initial sample of hospitals. In this step, we will build a survey instrument and deploy a computer-assisted Delphi method to conduct this assessment. The Delphi method is an iterative engagement technique where knowledgeable individuals are systematically asked a series of questions in multiple rounds of queries in five process stages, Preparation, Generation, Structure, Analysis, and Summary. Throughout this process, the research team will provide summaries of results to participants as findings emerge across the different participants perspectives. The Delphi study stages are explained below.. Stage 1, Preparation will involve identification of ...

The level of evidence and strength of recommendation were classified according to the Jadad scale (3).The level of agreement was established through a Delphi technique. ...
The Research Group provided methodological and clinical expertise in the area of chronic pain and addiction medicine . They summarized evidence from the studies and drafted 49 initial recommendations that each included a discussion and related evidence. An iterative course of action ensued, using a Modified Delphi technique with the National Advisory Panel (NAP), to produce final recommendations. NAP member identities were blind to the Research Group and each other until the last round of review.. NAP received material via email and responded using an on-line survey tool to rate their opinion on relevance, feasibility, clarity, and their degree of agreement with each recommendation. They also provided open-ended narrative comments.. Consensus was defined as 80% of NAP members supporting a recommendation. Recommendations that did not receive this level of consensus were revised using feedback provided by NAP and re-rated in the next round. With each round of review, each NAP member received a ...
Oocyte donation is one of the assisted reproductive techniques that can undermine the mental health of the women donor. As such, the aim of this study is to design a mental health promotion program for oocyte donors. This is an exploratory mixed methods study (qualitative-quantitative) that consists of three phases. In the first phase, a qualitative study will be conducted to identify the needs and strategies of the mental health promotion program for the women donors. In this phase, the participants will be selected using purposeful sampling method and the data will be collected through semi-structured interviews. In the second phase, the initial draft of the program is designed and validated in the panel of experts and using the classic Delphi technique and, then, the program is finalized. In the third phase, the designed program will be implemented as a quasi-experimental study in two groups of program recipients (intervention) and control, and the effectiveness of the intervention program will be
BACKGROUND: Although over the past decade the management of invasive fungal infection has improved, considerable controversy persists regarding antifungal prophylaxis in solid organ transplant recipients. AIMS: To identify the key clinical knowledge and make by consensus the high level recommendations required for antifungal prophylaxis in solid organ transplant recipients. METHODS: Spanish prospective questionnaire, which measures consensus through the Delphi technique, was conducted anonymously and by e-mail with 30 national multidisciplinary experts, specialists in invasive fungal infections from six national scientific societies, including intensivists, anesthetists, microbiologists, pharmacologists and specialists in infectious diseases that responded to 12 questions prepared by the coordination group, after an exhaustive review of the literature in the last few years ...
48 Table 3 1. Comparison of traditional survey with Delphi method Evaluation criteria Traditional survey Delphi study Summary of Procedure The researchers design a questionnaire with questions rele vant to the issue of study. There are numerous issues concerning validity of the questions they must consider to develop a good survey. The questionnaire can include questions that solicit quantitative or qualitative data, or both. The researchers decide o n the population that the hypotheses apply to, and selects a random sample of this population on whom to administer the survey. The respondents (who are a fraction of the selected random sample due to non response by some) fill out the survey and return it The researchers then analyze the usable responses to investigate the research questions. All the questionnaire design issues of a survey also apply to a Delphi study. After the researchers design the questionnaire, they select an appropriate group of exp erts who are qualified to answer the ...
This study examined the possible effects of the EHR on the small physician practice. The design for this study was chosen because of the incomplete nature of the EHR. The modified Delphi design was selected for this study because it can avoid the ambiguity often seen in the classic Delphi model.10 Quantitative methods cannot fully ascertain the financial or patient outcomes until, and unless, the government fully implements meaningful use stage 3. Until this occurs, quantitative studies can show only the effect of the current stage, not the complete product. After a final stage 3 fully certified EHR system is in place, quantitative studies would be appropriate.. The criteria for expert panelists focused on four primary needs. First, a pool of US physicians (MDs and DOs) with a practice size of one to four physicians was necessary to understand the EHRs impact on small physician practices in the United States. Second, the study focused on those in a small practice for at least five years to ...

Infections such as common colds, influenza, acute upper respiratory infections, bacterial gastroenteritis, and urinary tract infections are usually diagnosed according to patients signs and symptoms. This study aims to develop a scale for the diagnosis of infectious diseases based on the six excesses (Liu Yin) etiological theory of Chinese medicine (CM) by the Delphi method. A total of 200 CM-guided diagnostic items measuring signs and symptoms for infectious diseases were compiled from CM literature archives from the Han to Ming dynasties, CM textbooks in both China and Taiwan, and journal articles from the China Knowledge Resource Integrated Database. The items were based on infections and the six excesses (Liu Yin) etiological theory, i.e., Feng Xie (wind excess), Han Xie (coldness excess), Shu Xie (summer heat excess), Shi Xie (dampness excess), Zao Xie (dryness excess), and Huo Xie (fire excess). The items were further classified into the six excess syndromes and reviewed via a Delphi process to
Background: Value assessment of vaccination programs against serogroup B invasive meningococcal disease (IMD) is on the agenda of public health authorities. Current evidence on the burden due to IMD is unfit for pinning down the nature and magnitude of the full social and economic costs of IMD for two reasons. First, the concepts and components that need to be studied are not agreed, and second, measures of the concepts that have been studied are weak and inconsistent. Thus, the economic evaluation of the available serogroup B meningococcal (MenB) vaccines is difficult. The aims of this DELPHI study are to: (1) agree on the concepts and components determining the burden of MenB diseases that need to be studied; and (2) seek consensus on appropriate methods and study designs to measure quality of life (QoL) associated with MenB induced long-term sequelae in future studies. Methods: We designed a DELPHI questionnaire based on the findings of a recent systematic review on the QoL associated with ...
The aim of the present study was to identify core components in the care of immigrants with psychosis in Sweden. Experts (n=43) from different perspectives (immigrants, families, and health-care staff) were assembled and used to score the importance of statements regarding components in the care for a person with psychosis in three questionnaire rounds. After each round, the opinions were consolidated and compared to identify whether consensus was reached. Consensus was reached about the importance of being treated on equal terms, regardless of country of birth. Staff interest and respect, shown in different ways of understanding, was valued. Consensus could not be reached on approximately half of the statements, of which four tended to be ranked towards unimportant. Those included that staff should have specific cultural knowledge or that the patient should be allowed to decide whether to be cared for by male or female staff. Nor was it regarded as important to identify a persons religious or ...

Nevertheless, our study has at least seven limitations. Firstly, the investigators knew the results of the Manchester study in which SB, who moderated the NZ meetings, was involved. However, the NZ panel did not know these results (which had not yet been published) nor did they know that the criteria rated in round 2 in the UK had been used to inform the criteria they were asked to rate in round 1 in NZ. Previous assessments of interpanel reliability have involved panels in rating the same criteria, although from round 1.13 29. Secondly, we revised the wording of the appropriateness scale used in the Manchester study. However, we changed the description of the task rather than the task itself, and very few final criteria describe inappropriate actions. Almost all the criteria reported in our study and the UK study result from necessity ratings on a scale that was identical in both studies. Consequently, this paper has focused mainly on the necessity criteria.. Thirdly, we have assumed that the ...

ASCOs Guidance Products Clinical Practice Guideline: Addresses specific clinical situations (disease- oriented) or use of approved medical products, procedures, or tests (modality-oriented). Systematic reviews serve as the evidentiary basis for drafting principles of clinical care. Interpretation and extrapolation of evidence are often necessary. Consensus Guideline: A systematic review is conducted. If evidence identified is limited, inconsistent, indirect, or of poor quality, then the formal consensus-based methodology may be considered. While the decision to incorporate consensus recommendation(s) may vary, the common thread is lack of sufficient evidence. ASCO utilizes a modified Delphi process for creating consensus guideline recommendations. Additional information on the methodology can be found on the ASCO Guidelines Wiki Guideline Development page.ASCO Guidelines Wiki Guideline Development page
INTRODUCTION: Emergency general surgery (EGS) is responsible for 80-90% of surgical in-hospital deaths and the early management of these unwell patients is critical to improving outcomes. Unfortunately care for EGS patients is often fragmented and important care processes are frequently omitted. METHODS: This study aimed to define a group of important processes during EGS admission and assess their reliability. Literature review and semi-structured interviews were used to define a draft list of processes, which was refined and validated using the Delphi consensus methodology. A prospective cohort study of the 22 included processes was performed in 315 patients across 5 acute hospitals. RESULTS: Prospective study of the 22 selected processes demonstrated omission of 1130/5668 (19.9%) processes. Only 6 (1.9%) patients had all relevant processes performed correctly. Administration of oxygen to hypoxic patients (82/129, 64%), consultant review (202/313, 65%) and administration of antibiotics within 3 h for
