Around 130 000 people in France were infected with HIV in 2005. Among the 7000 people newly diagnosed in 2004, 58% became infected through heterosexual intercourse, 29% through homosexual intercourse, and 2% through intravenous drug use. Around half of those infected through heterosexual intercourse came from sub-Saharan Africa.. In France, testing is free in all healthcare settings, and anonymous counselling and testing is provided in dedicated sites all over the country. Besides prenatal routine testing, current policy focuses on high risk groups (men who have sex with men, sub-Saharan migrants) and behaviours (unprotected sex). This policy results in a large number of tests annually. However, around 40% of cases identified are in people with advanced infection, most of whom belong to groups not focused on by the current testing policy. We discuss the consequences of late testing on mortality and spread of infection and recommend expanding HIV testing to all primary care settings. ...
The main goal of this study was to analyse the determinants of late diagnosis of HIV infection. Secondly, we studied the role of the perception of risk and sexual orientation in HIV testing. Twenty-five people with late HIV diagnosis were interviewed. They were contacted through hospitals and non-governmental organizations (NGOs). To design the interview, we integrated the variables considered in the main models of health-related behaviour. We followed a mixed strategy of analysis. Firstly, we carried out the maticanalysis of the interviews, followed by quantitative analysis of the initially qualitative data. The results revealed that the most relevant determinants were the appraisal of the threat of HIV and the low perception of HIV risk. Also, the study found many missed opportunities for diagnosis in health-care setting. Low perception of HIV risk was related to unrealistic optimism, low levels of information about HIV, and the presence of stereotypes about people with HIV. High perception of HIV
Prevalence and Risk factors associated with LT. There was no change in the proportion of patients defined as LT over time during the period of observation (p=0.37) (figure 2). We observed that patients older than 45 years (p= 0.03), unemployed (p= 0.04), married (p= 0.04), and having less than nine years of education (p= 0.001) were at higher odds of late diagnosis that their counterparts. We also observed a gradient of decreased risk of late diagnosis as socioeconomic status improved (p= 0.001 and p= 0.04 for middle and high socioeconomic level). In contrast, having being infected through homosexual sex was negatively associated to being a late tester (p= 0.04) (table II). However, when we fit a multivariable logistic regression model including all these variables only being older than 45 years old, being unemployed and having less than nine years of education were independently associated to late diagnosis of HIV infection (table II).. Discussion. In this cross-sectional study of HIV-infected ...
There is no validated way of measuring the prevalence and duration of patient delay, and we do not know how people perceive and define the time intervals they are asked to report in patient delay studies. This lack of a validated measure hampers research in patient delay and is counterproductive to efforts directed at securing early diagnosis of cancer. The main argument of the present paper is that current studies on patient delay do not sufficiently consider existing theories on symptom interpretation. It is illustrated that the interpretation of bodily sensations as symptoms related to a specific cancer diagnosis is embedded within a social and cultural context. We therefore cannot assume that respondents define delay periods in identical ways. In order to improve the validity of patient delay studies, it is suggested that research be strengthened on three counts: More research should be devoted to symptom interpretation processes, more research should seek to operationalise patient delay, and,

OBJECTIVES: Prompt HIV diagnosis and treatment are associated with increased longevity and reduced transmission. The aim of the study was to examine late diagnoses and to assess the quality of care following diagnosis. METHODS: National surveillance and cohort data were used to examine late HIV diagnoses and to assess the quality of care received in the 12 months following HIV diagnosis. RESULTS: In 2011, 79% (4910/6219) of persons (15 years and over) diagnosed with HIV infection had CD4 counts reported within 3 months; of these, 49% were diagnosed late (CD4 count , 350 cells/?L ...

INTRODUCTION: In the Community of Madrid there is 42.7% late HIV diagnosis. Primary care is the gateway to the health system and the frequency of serological tests requested by these professionals is unknown. The objectives were to establish the frequency of requests for HIV serology by medical and nursing primary care professionals in the Community of Madrid and the factors associated with these requests. METHOD: An on-line survey was conducted, asking professionals who participated in the evaluation study of strategies to promote early diagnosis of HIV in primary care in the Community of Madrid (ESTVIH) about the number of HIV-serology tests requested in the last 12 months ...

Patients who live in rural areas are more likely to receive late-stage diagnoses and to die from their colon cancer, and less likely to receive an early diagnosis, chemotherapy, or thorough surgical treatment, according to a recent study presented at the 2012 Annual Clinical Congress of the American College of Surgeons.
Since the Covid pandemic forced much of the world into lockdown, Bhakdi says X-ZELL has double-downed on strategy and tried to maintain the momentum generated during Q120.. Coming off a very successful 2019 we had high expectations on 2020, and that has not changed, he explains - adding the young company was on route to closing a $3 million seed funding round when the crisis evolved into a global phenomenon.. With thousands of late-stage diagnoses every day and a solution right at our fingertips, we felt an obligation to keep going and help reduce the burden on our health systems.. However, keeping up business momentum amid a global pandemic did require some tough decisions, according to the father-of-three. In a move to limit our exposure to a full lockdown, we were forced to split up our leadership team and invest more aggressively in the expansion of X-ZELLs international footprint.. In late spring, Bhakdi and his family temporarily relocated to Thailand to work out of X-ZELLs ...

I really had a hard time with the chemo. I think it was because I was overdosed. Chemo is based on weight but I am super sensitive to medication (a quarter of a Percoset knocked me out). After the second one, they couldnt control the side effects. I was so week I could barely walk and the nausea was horrible. After the third one, my liver started to fail, I lost control of my bladder and bowels, my eyesight was effected, my finger and toenails fell off, my skin, especially on my left side, started to peal and I could only walk a few steps without falling. My GP thought I had a small stroke with some bleeding in my brain when my blood pressure went to 220/140. The day before my fourth scheduled chemo, we stopped it. My GP and I both believed another chemo could be fatal. She pointed out to me that this chemo was preventative; not curative and it was HER job to keep me alive while my oncologist/gynecologists job was to kill the cancer. I agreed with her 100%. It was world arthritis day, 12 October, when I finished my first 3 months of immuno-therpy of RA. I had juvenile RA during adolescence then long remission. Last decade, I suffered many activities of the RA. I said SUFFERED as I was wrongly diagnosed as Spondylo-arthropathy and many other false diagnoses and kept on non-steroidal anti-inflamatory and analgesics (NSAIs). Lastly, I started the proper therapy. Some side effects and little improvement could make me upset and worried about my physical activity with the disease. I feel like imprisoned with both symptoms and side effects. I know that late diagnosis and treatment is a cause. It is so hard to read the first statement in most Rheumatology books quoting There is no cure of RA. ...
It is of great importance that whenever you find any of the above symptoms, you must report to your physician to know as to whether it is lung cancer or other disease related to lung. The physician will perform a detailed workup on you so as to determine as to what is the cause and then decides on what actions need to be taken once the diagnosis is made. It is always the best that you go to your physician as soon as possible and do not delay it as it is of grave importance that it is reported at the earliest.. ...

BACKGROUND: the delay of HIV diagnosis represents an important risk of spreading HIV infection within the community and, at the same time, a loss of opportunity for undiagnosed subjects to start the antiretroviral therapy. OBJECTIVES: to evaluate the difference, over time, between early and late HIV diagnosis in a large University Hospital of Southern Italy and to emphasize the importance of spreading the culture of prevention, based on the improvement of the HIV screening test adherence, in order to reduce the incidence of late HIV diagnosis. DESIGN: retrospective cross-sectional study. SETTING AND PARTICIPANTS: all the HIV screening tests performed in the six-year period 2013-2018 by the HIV laboratory of a third-level University hospital of Sicily (Southern Italy) were considered. The tests were performed on four categories of patients: voluntary HIV screening participants, inpatients, outpatients, and healthcare workers. MAIN OUTCOME MEASURES: number of performed HIV tests and frequency of ...
Results During the study period, a tentative diagnosis of STEMI was established in 1061 patients, of whom 919 were triaged directly to the PCI centre. In 771 (84%) patients, a diagnosis of STEMI was confirmed. Patients transported ,10 km had a mean system delay of 82 min; this delay rose to 103 min for patients transported 50-75 km. Data on system delay was achievable in 682 patients in whom 553 (81%) were treated within 120 min of the FMC, and a system delay of ,120 min was achievable in 89% of patients living up to 95 km from the PCI centre. Even for patients living ,10 km from the PCI centre, only 16 (14%) had a system delay of 60 min or less.. ...
Purpose: The aim of this study was to investigate the diagnostic delay and its determinants among oral cancer patients in Tehran, Iran. Materials and Methods: This study was conducted between September 2004 and September 2006 in three university hospitals, and included 100 consecutive patients with primary oral squamous cell carcinoma (international classification of disease, ICD-10 sites C01 to C06). Data were obtained through questionnaire interviews and medical records of the patients were reviewed to obtain information on the date of diagnosis, primary tumour site and the stage of the tumour at the time of diagnosis. Statistical analysis was performed by t test, ANOVA and logistic regression. Results: The mean diagnostic delay was 7.2 months (SD 7.5, range 1 to 36 and median 4). The most important determinants of longer diagnostic delay were being single (OR = 4.8; 95% CI = 1.5 to 14.8; P < 0.05) and being at advanced tumour stages (OR = 5.3; 95% CI = 1.8 to 15.6; P < 0.01). The mean patient ...

Hispanics are among the fastest growing population in the U.S., and are predicted to account for one third of the nation by 2060. Although melanoma is more common among Caucasians, Hispanics are at greater risk of late-stage diagnosis, increased tumor thickness, and poorer survival. To better understand public awareness of melanoma and evaluate change over the last 21 years, particularly among high-risk minority populations.A cross-sectional survey collecting information on knowledge and awareness of melanoma was conducted on 285 participants from May-November 2017.Approximately 39% of participants were unaware of melanoma. Sixty-five percent successfully identified early signs of disease. Approximately 86% of Fitzpatrick Skin Types (FST) I-II identified melanoma as a cancer, compared to 46.3% of FST III-IV and 57.6% of FST V-VI. Hispanics were less likely to know what melanoma was compared to Caucasians (OR 0.27 (0.65, 0.11), p=.0037). U.S. natives (OR 2.38 (5.56, 1.04), p=.0403) and patients ...
Worldwide, there are over 450,000 new cases of oral cancer reported each year. Late-stage diagnosis remains a significant factor responsible for its high mortality rate (>50%). In-vivo non-invasive rapid imaging techniques, that can visualise clinically significant changes in the oral mucosa, may improve the management of oral cancer. We present an analysis of features extracted from oral images obtained using our hand- held wide-field Optical Coherence Tomography (OCT) instrument. The images were analyzed for epithelial scattering, overall tissue scattering, and 3D basement membrane topology. The associations between these three features and disease state (benign, pre-cancer, or cancer), as measured by clinical assessment or pathology, were determined. While scattering coefficient has previously been shown to be sensitive to cancer and dysplasia, likely due to changes in nuclear and cellular density, the addition of basement membrane topology may increase diagnostic ability- as it is known that ...
Background Early diagnosis and immediate initiation of treatment are essential for an effective tuberculosis (TB) control program. Delay in diagnosis is significant to both disease prognosis at the individual level and transmission within the community. Most transmissions occur between the onset of cough and initiation of treatment. Methods A systematic review of 58 studies addressing delay in diagnosis and treatment of TB was performed. We found different definitions of, for example, debut of symptoms, first appropriate health care provider, time to diagnosis, and start of treatment. Rather than excluding studies that failed to meet strict scientific criteria (like in a meta-analysis), we tried to extract the solid findings from all of them to arrive on a more global understanding of diagnostic delay in TB. Results The main factors associated with diagnostic delay included human immunodeficiency virus; coexistence of chronic cough and/or other lung diseases; negative sputum smear; ...
SOD1 mutations are the most common cause of amyotrophic lateral sclerosis (ALS) in non-Caucasian patients. Detailed natural history profiles of SOD1-mutant patients will be beneficial for the strategy and interpretation of future SOD1-targeted clinical practice. Mutational distribution, age at onset (AAO), site of onset, diagnostic delay, disease progression (rate of ALSFRS-R decrease, ΔFS) and survival were analysed. Further comparisons between heredity of disease, gender, and mutations were performed. Sixty-six cases with 43 SOD1 mutations were included and analysed, with p.His47Arg as the leading mutation and seven novel variants identified. The mean (SD) AAO was 43.92 years (9.24) for all subjects, with a significant difference between patients carrying mutations in exon 2 (n = 24,46.83, 8.31) and exon 4 (n = 18, 37.75, 7.67) (p = 0.002). The median (IQR) diagnostic delay from symptom onset was 14.50 (6.00-36.50) months for all SOD1-mutant patients, 9.50 (4.75-24.25) months for males and 24.00 (9

New Brochure on AIDS Testing. The press release refers to a New Brochure on AIDS Testing yet addresses only HIV testing. It does not define what is meant by AIDS Testing but presumably this refers to laboratory procedures used to document immune deficiency (AID = low T4 cells) or diagnostic methods for the AIDS indicator diseases. As Brent Leungs film discloses, many people are confused about the terms HIV and AIDS and are not always able to distinguish between them. The press release does not alleviate this situation.. SAN FRANCISCO (Rethinking AIDS), May 23, 2009 - The brochure outlines how tests for HIV (Human Immunodeficiency Virus) or AIDS have no scientific basis and are useless as a diagnostic tool determining who will get AIDS, and describes the dangers of coming up positive on the tests.. By failing to distinguish between testing for HIV and AIDS diseases the brochure asserts there is no scientific basis for the diagnosis of AIDS indicator diseases. This is incorrect. In ...
1248 of 17 661 eligible patients died during 91 203 person years follow-up. Life expectancy (standard error) at exact age 20 increased from 30.0 (1.2) to 45.8 (1.7) years from 1996-9 to 2006-8. Life expectancy was 39.5 (0.45) for male patients and 50.2 (0.45) years for female patients compared with 57.8 and 61.6 years for men and women in the general population (1996-2006). Starting antiretroviral therapy later than guidelines suggest resulted in up to 15 years loss of life: at age 20, life expectancy was 37.9 (1.3), 41.0 (2.2), and 53.4 (1.2) years in those starting antiretroviral therapy with CD4 count ,100, 100-199, and 200-350 cells/mm3, respectively ...

Pancreatic cancer is the tenth most common cancer in the UK; however, outcomes are poor, in part due to late diagnosis. We aimed to identify symptoms and other clinical and sociodemographic factors associated with pancreatic cancer diagnosis and diagnostic intervals. We did this prospective cohort study at seven hospitals in two regions in England. We recruited participants aged 40 years or older who were referred for suspicion of pancreatic cancer. Data were collected by use of a patient questionnaire and primary care and hospital records. Descriptive and regression analyses were done to examine associations between symptoms and patient factors with the total diagnostic interval (time from onset of the first symptom to the date of diagnosis), comprising patient interval (time from first symptom to first presentation) and health system interval (time from first presentation to diagnosis). We recruited 391 participants between Jan 1, 2011, and Dec 31, 2014 (24% response rate). 119 (30%) ...
This study showed navigation was successful in reducing the time from abnormal screening to diagnostic resolution among NHW, NHB, and Hispanic women of all ages, especially for women who had a biopsy as the definitive test. A nearly 4-fold reduction in time to diagnostic resolution was seen when comparing navigated to nonnavigated women who resolved with cancer.. Navigation significantly reduced the odds of having diagnostic delays for both uninsured and privately insured women, with a similar nonsignificant trend seen for women with government insurance. Navigation was most effective in reducing diagnostic times within the first 60 days following abnormal screening.. If we assume that women excluded from the analysis because of missing definitive diagnosis dates never received a definitive diagnosis or took a longer time to obtain a definitive diagnosis, then the disproportionate number of missing diagnosis dates-112 navigated versus 37 nonnavigated-may have biased the results toward seeing a ...

As we talked about above, one of the early symptoms is haemoptysis (coughing up blood). You should highly consider this early warning as a threat of a tumor, that may be curable. In case if you come across blood during coughing up, you should at once contact your General Practioner for getting essential advice, especially if you are a individual over 40 years in age with a smoking habit. Occasionally the early warning may be due to the effects of a secondary tumor somewhere else in the body ...

BACKGROUND: Prostate cancer (PCa) is the most commonly diagnosed cancer and the second leading cause of cancer-related deaths among adult males globally. The poor prognosis of PCa is largely due to late diagnosis of the disease when it has already progressed to an advanced stage marked by androgen-independence, thus necessitating new strategies for early detection and treatment. We construe that these direly needed advances are limited by our poor understanding of early events in the progression of PCa and that would thus represent ideal targets for early intervention. To begin to fill this void, we interrogated molecular oncophenotypes that embody the transition of PCa from an androgen-dependent (AD) to-independent (AI) state. METHODS: To accomplish this aim, we used our previously established AD and AI murine PCa cell lines, PLum-AD and PLum-AI, respectively, which recapitulate primary and progressive PCa morphologically and molecularly. We statistically surveyed global gene expressions in ...
