Around 130 000 people in France were infected with HIV in 2005. Among the 7000 people newly diagnosed in 2004, 58% became infected through heterosexual intercourse, 29% through homosexual intercourse, and 2% through intravenous drug use. Around half of those infected through heterosexual intercourse came from sub-Saharan Africa.. In France, testing is free in all healthcare settings, and anonymous counselling and testing is provided in dedicated sites all over the country. Besides prenatal routine testing, current policy focuses on high risk groups (men who have sex with men, sub-Saharan migrants) and behaviours (unprotected sex). This policy results in a large number of tests annually. However, around 40% of cases identified are in people with advanced infection, most of whom belong to groups not focused on by the current testing policy. We discuss the consequences of late testing on mortality and spread of infection and recommend expanding HIV testing to all primary care settings. ...
The main goal of this study was to analyse the determinants of late diagnosis of HIV infection. Secondly, we studied the role of the perception of risk and sexual orientation in HIV testing. Twenty-five people with late HIV diagnosis were interviewed. They were contacted through hospitals and non-governmental organizations (NGOs). To design the interview, we integrated the variables considered in the main models of health-related behaviour. We followed a mixed strategy of analysis. Firstly, we carried out the maticanalysis of the interviews, followed by quantitative analysis of the initially qualitative data. The results revealed that the most relevant determinants were the appraisal of the threat of HIV and the low perception of HIV risk. Also, the study found many missed opportunities for diagnosis in health-care setting. Low perception of HIV risk was related to unrealistic optimism, low levels of information about HIV, and the presence of stereotypes about people with HIV. High perception of HIV

Purpose: The aim of this study was to investigate the diagnostic delay and its determinants among oral cancer patients in Tehran, Iran. Materials and Methods: This study was conducted between September 2004 and September 2006 in three university hospitals, and included 100 consecutive patients with primary oral squamous cell carcinoma (international classification of disease, ICD-10 sites C01 to C06). Little information is available concerning how patient delay may be affected by mass disasters. The main objectives of the present study are to identify whether there was a post-disaster increase in the risk of experiencing patient delay among breast cancer patients in an area affected by the 2011 triple disaster in Fukushima, Japan, and to elucidate factors associated with post-disaster patient delay. Sociodemographic factors (age, employment status, cohabitant status and evacuation status), health characteristics, and health access- and disaster-related factors were specifically considered. Records of symptomatic breast cancer patients diagnosed from 2005 to 2016 were retrospectively reviewed to calculate risk ratios (RRs) for patient delay in every year post-disaster compared with the pre-disaster baseline.
