TY - JOUR. T1 - The influence of infusions of 1-desamino-8-D-arginine vasopressin (DDAVP) in vivo on the anticoagulant effect of recombinant hirudin (CGP39393) in vitro. AU - Ibbotson, S H. AU - Grant, P J. AU - Kerry, R. AU - Findlay, V S. AU - Prentice, C R. PY - 1991/1/23. Y1 - 1991/1/23. N2 - Hirudin is a specific, potent inhibitor of thrombin that may be a valuable antithrombotic agent. The aim of this study was to investigate the hypothesis that the haemostatic effects of DDAVP counteract the coagulation defect induced by hirudin. The effect of DDAVP was studied in vivo on the anticoagulant action of recombinant hirudin (CGP39393) in vitro. Blood samples were taken at intervals from 10 normal volunteers infused with DDAVP. Factor VIII:C rose from (mean) 0.68 IU/ml before DDAVP to 2.19 and 2.16 IU/ml after 30 and 60 min infusion, respectively. Samples taken during DDAVP infusion showed a dose related decrease in the hirudin (0.5 and 1.0 microM) induced prolongation of the APTT, that ...

Context: The desmopressin (DDAVP) test has been proposed to discriminate Cushing s disease (CD) from Pseudo-Cushing states (PC); however, current information on its value is scarce and contradictory.. Objective: To assess the ability of the DDAVP test in distinguishing between these conditions, with emphasis on subjects with mild hypercortisolism.. Design: Retrospective/prospective study.. Setting: Division of Endocrinology, Polytechnic University of Marche, Ancona, Italy.. Patients: Fifty-two subjects with CD, 28 with PC and 31 control subjects (CT).. Intervention(s): DDAVP test and standard diagnostic procedures for the diagnosis of Cushing s syndrome.. Main outcome measure(s): Diagnosis/exclusion of CD.. Results: Interpretation of the DDAVP test based on percent and absolute increment of cortisol and ACTH did not afford acceptable values of both sensitivity (SE) and specificity (SP). CD diagnosis based on simultaneous positivity for basal serum cortisol ,331 nmol/l and absolute ACTH increment ...
This is a prospective, single center, Phase II biologic effects study of recombinant interleukin-11 (rhIL-11, Neumega) in subjects hemophilia A, moderate or mild; or with Von Willebrand disease unable to take desmopressin acetate (DDAVP) because they are unresponsive, allergic, or DDAVP is contraindicated. The purpose of the study is to establish the biologic efficacy and safety of rhIL-11 in those not able to take DDAVP. Study subjects will include adults, age ,= 18 years, with hemophilia A, moderate, defined as factor VIII 0.01-0.04 U/ml, or mild, defined as factor VIII ,= 0.05 U/ml; or with VWD defined by low VWF:RCo and /or low VWF:Ag, past bleeding history, and/or family history of VWD. A total of 10-16 subjects will be enrolled in order to assure that 10 complete the study. The specific aims of the study are: 1) to determine the biologic effect of rhIL-11 when given 4 consecutive days; 2) to determine the safety of rhIL-11 when used in subjects with hemophilia A, moderate or mild; or with ...
Treatment for VWD depends on the diagnosis and severity. The mainstay of treatment is DDAVP (desmopressin acetate), the synthetic version of a natural hormone vasopressin,. It stimulates the release of VWF from cells, which also increases FVIII.DDAVP comes in two forms: injectable and nasal spray. Because DDAVP is an antidiuretic, causing the body to retain water, fluid restrictions are important so patients dont develop hyponatremia, reduced sodium in the bloodstream.. There are a few clotting factor concentrates that are rich in VWF, and are recommended for patients with VWD. These therapies are given by intravenous infusion. In December 2015, the US Food and Drug Administration (FDA) approved Baxaltas Vonvendi®, the first recombinant VWF product. Unlike other products, it contains VWF only, not VWF and factor VIII. It is approved to treat on-demand and for control of bleeding in adults 18 and older.. Aminocaproic acid and tranexamic acid are antifibrinolytics agents that prevent the ...
To the editor: Kobrinsky and associates (1) show that patients receiving 1-desamino-8-D-arginine vasopressin (desmopressin) had a decreased blood loss during Harrington rod spinal fusion surgery. The authors speculate that the decreased blood loss was due to various hematologic effects of desmopressin and conclude that "the use of desmopressin in other surgical procedures associated with significant blood loss is recommended.". This conclusion is probably unwarranted. Intraoperatively "a balanced technique with normotension" was used. Unless specifically contraindicated, induced hypotension with a mean blood pressure of about 70 torr is the accepted anesthetic technique (2). This is done to minimize intraoperative blood loss. ...
293(6). Aquaporin (AQP) 1 null mice have a defect in the renal concentrating gradient because of their inability to generate a hyperosmotic medullary interstitium. To determine the effect of vasopressin on renal medullary gene expression, in the absence of high local osmolarity, we infused 1-deamino-8-d-arginine vasopressin (dDAVP), a V(2) receptor (V(2)R)-specific agonist, in AQP1 null mice for 7 days. cDNA microarray analysis was performed on the renal medullary tissue, and 5,140 genes of the possible 12,000 genes on the array were included in the analysis. In the renal medulla of AQP1 null mice, 245 transcripts were identified as increased by dDAVP infusion and 200 transcripts as decreased (1.5-fold or more). Quantitative real-time PCR measurements confirmed the increases seen for cyclin D1, early growth response gene 1, and activating transcription factor 3, genes associated with changes in cell cycle/growth. Changes in mRNA expression were correlated with changes in protein expression by ...
In this study, published in the Canadian Journal of Anesthesia, researchers sought to determine the effects of desmopressin on coagulation and blood loss in patients undergoing elective partial hepatectomy. The research team performed a randomized, controlled and double-blind study on 59 patients who received either 0.3 µg•kg-1 of desmopressin, or an equal volume of normal saline. Treatments were infused intravenously over 20 min after induction of general anesthesia. The researchers found a significant increase in plasma levels of factors VIII and von Willebrand, after the infusion of study drug in both groups. The team also observed a wide range of intraoperative blood loss (400 to 7128 ml), with no significant differences between groups. In addition, there were no changes in plasma electrolyte levels or osmolality. ...
For diabetes insipidus: The usual recommended dose for adults is 10 µg to 40 µg daily. The usual recommended dose for children 3 months to 12 years of age is 5 µg to 30 µg daily. The dose may be given as a single dose or divided into 2 or 3 doses. The lowest effective dosage should be used. Do not drink any fluids for at least 1 hour before taking desmopressin and for 8 hours after using desmopressin.. For preventing bleeding: The usual recommended dose is one 150 µg spray in each nostril, or one 150 µg spray in one nostril for people who weigh less than 50 kg (110 lbs). If it is being used to prevent bleeding related to surgery, it is given 2 hours before surgery.. Follow the instructions in the package insert on how to use medication. This medication should not be sniffed or inhaled; it works by being absorbed through the nose. To ensure the best absorption of the medication, place a finger on the outside of the nostril you just sprayed to close it for 10 seconds. During this time, ...

The present results confirm that the intravenous administration of DDAVP induces a release of FVIII, vWF and t-PA in conscious dogs. Comparable increases in plasma levels of these proteins with similar kinetics have been previously described in conscious dogs (Vilhardt and Barth, 1991) and anesthetized monkeys (Kinter et al., 1992), whereas humans are more sensitive to DDAVP-induced release of vWF and t-PA (Mannucci et al., 1975). The possible influence of DDAVP on PAI-1 plasma activity has not previously been reported. Present results in conscious dogs do not demonstrate evidence of a involvement of PAI-1 in the effect of DDAVP to release hemostasis factors.. SR 121463 and OPC 31260, two novel, selective nonpeptide antagonists for V2 receptors, possess a high affinity for renal V2 receptors and inhibit AVP-induced cAMP formation (Manning et al., 1984; Serradeil-Le Gal et al., 1996; Yamamura et al., 1992). Both compounds also revealed potent in vivo activities: OPC 31260 exerted an aquaretic ...

Our study contributes to the literature that show that hyperosmotic stress induces an ER stress‐like response. Water restriction or dDAVP infusion, which both increase medullary osmolality, increased Grp78 and ATF4 expression in the inner medulla (van Balkom et al. 2004; Cai et al. 2006, 2010). Conversely, GRP78 protein abundance decreased in IMCD of animals subjected to vasopressin escape (Hoorn et al. 2005). Expression levels of ER stress‐responsive genes CHOP and ATF3 were increased by either NaCl or urea challenge in cultured IMCD cells (Kultz et al. 1998; Zhang et al. 1999; Tian and Cohen 2002) and several ER stress‐responsive genes were upregulated by NaCl challenge in renal fibroblast cell lines (Dihazi et al. 2011). Our study shows that overall, the extent of induction of ER stress‐responsive genes by either Tg or Tun was significantly higher than that induced by hyperosmolality. However, it should be noted that induction of ER stress by either chemical agent would be expected to ...
Prognostic factors associated with clinical presentation include the number of tumors, presence of ulceration, and fixation of tumors to underlying tissue. Survival outcome is significantly worse for dogs with ulcerated mammary carcinomas with a median survival time of 118 days and 1- and 2-year survival rates of 10.8% and 5.4%, respectively, compared to a median survival time of 443 days and 1- and 2-year survival rates of 52.6% and 45.0%, respectively, for dogs with non-ulcerated mammary carcinomas.. The use of desmopressin in the perioperative period significantly improves median survival times in dogs with grade II and III mammary carcinomas. In one prospective study of 18 dogs treated with desmopressin and 10 dogs treated with a saline placebo, the overall median survival time was significantly longer in dogs treated with desmopressin (809 days) compared to dogs in the placebo group (237 days) with 17% and 80% of dogs dying because of disease-related reasons in each group, respectively. ...

It can be taken by children as young as six years of age, who are advised to take 10 mcg (microgramme) in each nostril before going to sleep. The ideal dosage is arrived at only by trial and error, with the minimum being 10 mcg, going up to 40 mcg. The dose sometimes has to be increased after six months usage, when the effects seem to diminish. Any patient who has taken the drug that long has gone further than medicine; trials have tested the drug and reactions for only four to eight weeks usage ...

Audience: Endocrinologists, pediatricians, other healthcare professionals. [Posted 12/04/2007] FDA notified healthcare professionals and patients of the Agencys request that manufacturers update the prescribing information for desmopressin to include important new safety information about severe hyponatremia and seizures. Certain patients, including children treated with the intranasal formulation of the drug for primary nocturnal enuresis (PNE), are at risk for developing severe hyponatremia that can result in seizures and death. As such, desmopressin intranasal formulations are no longer indicated for the treatment of primary nocturnal enuresis and should not be used in hyponatremic patients or patients with a history of hyponatremia. PNE treatment with desmopressin tablets should be interrupted during acute illnesses that may lead to fluid and/or electrolyte imbalance. All desmopressin formulations should be used cautiously in patients at risk for water intoxication with hyponatremia ...
Desmopressin is a synthetic analog of human endogenous antidiuretic hormone, arginine vasopressin; its major action is to reduce urine production.16 Nørgaard et al17 postulated that some children with nocturnal enuresis produce high volumes of urine with low osmolarity at night, with no associated increase in secretion of antidiuretic hormone, hence the applicability of desmopressin given at bedtime to supplement antidiuretic hormone and increase overnight urine osmolality and decrease urine volume, leading to dry nights.18-20 According to several literature reviews, desmopressin treatments render immediate response; however, they are associated with high relapse rates after abrupt withdrawal.5,6 Critical care studies have postulated that suppressed endogenous vasopressin caused by vasopressin infusion and sudden withdrawal may lead to sustained vasopressin deficiencies.21 Endocrine withdrawal syndrome is an established physiologic phenomenon after discontinuation of hormone therapy, which ...
