Downloadable (with restrictions)! Recent theoretical research suggests that property taxation has incentive effects that can help control cost problems in the public sector. The institutional setting in Norway allows this first empirical investigation of the incentive effect of property taxation, since we can separate between local governments with and without property tax. The raw data of the variation in the unit cost level for utilities show that local governments with property tax have 20% lower unit cost. Using both linear regression and propensity score matching, we are not able to wash out the difference in unit costs. Our interpretation is that having a visible and controversial local tax related to property stimulates voter interest in local government activities and thereby may help cost control.
As the health care debate drags on, I often hear this from both supporters and opponents of reform: "theres really nothing in the reform bill to control health costs." Thats just not true of course. From the CBO estimates that the Senate reform bill actually reduces the federal deficit by over $100 billion to the substantial efficiencies and reform in Medicare that lower costs by hundreds of millions of dollars, there are many changes for the better.. However, what Congress has found in creating health reform is that there is no magic bullet out there to reduce costs. Thats why the Senate bill contains a slew of strategies and pilot programs aimed at achieving this goal. Some will have an effect - thats why the federal deficit is reduced. Others may or may not, but we have to try and then use the techniques with the best results for long-term cost control. Massachusetts, the one state with near-universal coverage, is already wrestling with the cost conundrum and they contracted with outside ...

Voila. Everyone who wants it can now afford insurance and has access to it. That was two paragraphs. We can put that in legislature-ese and make that a 30-page bill. The rest of the nonsense is about "cost control" which isnt going to happen in practice, since we cant compare costs to counterfactual reality (the world without "cost control" or other worlds with different "cost control"). This crap is going to be ridiculously expensive no matter what, and whatever costs we might be able to control (doctors using two little band-aids instead of one big one, prescribing the Really Awesome Cholesterol Drug instead of the Not Quite So Awesome Cholesterol Drug That Doesnt Work But Is Cheaper In Theory, throwing people in the Really Fancy Scanner rather than just having them sit on some film and swallow some U-238) are rounding errors in that.. Sound risk management and cost controls give confidence to consumers that the program will deliver on all of its promised future obligations. The funding structure of the program is important. Because the need for LTC increases with age, there is good reason for the sake of program sustainability to design the program using systematic prefunded pooling of homogenous risks in which participants make substantial contributions during their working years, continue to contribute during their retirement years, and receive most of their benefits in the last few years of life or possibly die without ever needing LTC. A reasonable fear of consumers is the risk that the program runs out of money precisely at the point where the participants are most in need and unable to care for themselves. This risk can be minimized by designing cost controls into the program. Controls may need to change when the markets environment changes over time. For example, if the reform were to restrict initial underwriting on ...

Controlling overhead costs is critical for all construction managers. Increased competition and slim profit margins have forced contractors to take a hard look at overhead cost management.. Budgeting and variance analysis. Technological changes have made budgeting systems, planning, and control techniques available to all contractors. Budgeting allows contractors to evaluate current operations, assess future costs, and provide a proactive approach to controlling overhead costs.. Overhead costs are either variable or fixed. Managers can use these cost relationships along with historical trends to forecast overhead expenses. A flexible budget can then be used to evaluate significant variances between actual costs incurred and projected amounts. This analysis also highlights problems and weaknesses in a contractors organizational structure.. Budgets can be used as a quantitative action plan to change employee behavior and decision-making processes. By prompting managers to emphasize cost control ...

Did you know that you could be tossing money away at the checkout line? Here are some tips to help you cut costs at the grocery store.
Some programs might have delayed effects, meaning that benefit managers wont see the results in the first year of implementation. Delayed results might take the form of cost savings or increased expense. It is even possible to see savings in the first year of implementation followed by a jump in health costs a year or two later.. In some forms of utilization management (the cost-containment process used by many health plans to prevent unnecessary health care expenses), managers might save a large amount during the first year because the health plan denies an expensive diagnostic or treatment procedure. However, if this results in a delayed diagnosis or untreated disease progression, there could be more costly medical bills in subsequent years when the health plan that denied those procedures is no longer the vendor. When outcomes are examined strictly on a fiscal basis, this type of trend might be lost, making it more difficult to identify the program with the best ROI.. Another example might ...
As P.T. Barnum was fond of saying, without promotion something terrible happens….nothing - Thats certainly not the only reason behind the recent announcement by the Cleveland Clinic and North Shore Long Island Jewish hospital to partner in a growing cardiac care network of excellence, but its a big reason. These self-promoted and described networks of excellence are forming in response to two important industry trends: (1) procedural episode carve outs, and (2) consumer price-sensitivity. For the first, employers who are looking to control the well documented variability in episode costs have been turning to carve outs as a way to accomplish that goal and gravitated towards marquee name facilities so that their network selection doesnt appear to be solely motivated by cost control. However, in doing so, they are far from optimizing the opportunity to control costs because the marquee names are also almost always the priciest. For the second, consumers who are subject to reference price ...

I had the pleasure to recently sit down with Purohit to examine how CIOs are managing their IT budgets for 2010. During the economic recovery, the cost-containment conundrum of "do more for less" -- that is, while still supporting all of your business requirements -- is likely to remain the norm.
The growing cost-containment pressures in major industrialized nations, coupled with continued technological advances in chromogenic substrates, monoclonal antibodies, immunoassays, molecular diagnostics, computers and laboratory automation will radically change the global coagulation diagnostics practice during the next five years. New specific and sensitive markers of coagulation will be increasingly used on automated instrumentation. Coagulation testing will also become more standardized, offering opportunities for quality control products and services. Moreover, the continuing contraction of the hospital system and technological advances will facilitate decentralization of the coagulation testing closer to the patient, thus creating additional opportunities and challenges for suppliers. ...

Of the five projects identified by either the funding recipients or Departmental staff as having been delayed by tender results that were over budget, multiple tenders were received in four out of five cases. Bills of Quantities were provided to the tenderers in 4 out of 5 cases, and full design documentation was provided to them in four out of five cases. This suggests strongly that the project designs were over budget even before they were issued for tender and that all the processes normally employed to ensure competitive tender results did not overcome poor design management during documentation. It also suggests that the quantity surveyors, in preparing tender Bills of Quantities, did no more than measure the causes of the budget over-runs. These resources might have been better assigned to or supplemented by cost planning and cost control services ...
I have always argued that manufacturers need to be the biggest experts on the diseases they are pursuing and their treatments. This by necessity must include the value proposition of their technologies beyond the statistically significant improvements over placebo required by the FDA for approval. We must develop objective milestones by which to judge worthiness of technologies in development at every point in the development process. Those who do, will adapt to and succeed in this atmosphere of cost controls. Those who do not do so at their own peril. As scientists, citizens, consumers and investors, we should make sure that manufacturers are engaging in this ongoing evaluation of their wares with a critical eye to what value they intend to bring to the society ...

The researchers studied the sources of cost growth among the privately insured by analyzing the contributions to higher spending of changes in prices and changes in the number and types of services performed. They also examined how changes in prices and changes in the number and types of services have differentially affected different categories of spending and different demographic groups. These findings for the privately insured were also compared to trends in cost growth in public programs. The researchers explored which policies or benefit designs will be more effective in reducing spending, as well as whether costs are driven more by increased utilization of certain types of services or by increases in the prices of particular services. In addition, the researchers suggested that the findings would be useful in developing policies to expand coverage by identifying the sources of cost increases that may affect coverage rates. The objective of this study was to provide information for ...

For a variety of reasons, it is not surprising that government spending in the District and elsewhere has grown in recent years. Even without program changes, government spending typically grows from year to year as a result of inflation that is, rising costs for the services that government provides. It is worth noting that health care costs, which represent a substantial portion of government spending, have risen dramatically in recent years. Nationally, Medicaid costs are expected to rise 11 percent this year. Finally, state spending in some areas, such as education, has risen nationally in direct response to growing public demands.. These figures should not be interpreted to mean that District policy makers need not be concerned with restraining expenditure growth. Indeed, the size of the current budget shortfall and the fact that weak revenue growth could continue for several years mean that significant spending reductions will have to be made. In addition, administrative and management ...
Until insurance levels the field and stops paying big medical centers for this type of non hospital type procedure and paying everyone else a much lower price, we will ocntinue to get the profligation of hospital offered services in the outpatient realm that only serve to drive up the cost of care. Insurance and goverment claims it wants to control cost, but turns around day after day and pays 2-3 times the cost of the same service to a hospital owned enterprise simply because the hospital owns it! Does this make any sense? Do you care, if you are footing the bill, if a hospital derived clinic or a privately owned urgent care center sews up you finger? You want the best value for your money! Insurance companies muck the whole situation up by paying the larger organizations higher reimbursement regardless of whether they do a better job or not, but simply because they can leverage their size. The end result of such a policy will eventually be extremely expensive hospitals that charge more than ...

The Carter administrations hospital cost control bill will be met with the "united opposition" of both hospitals and physicians, the president of the 6,000 - member American Hospital Association said yesterday.. The hospital associations opposition to the bill, announced at a press conference yesterday by president J. Former Goldome Chief Cooper Urges Cost Control As Cultural Discipline for Survival-Minded Banks. Thomas A. Cooper took the reins at Goldome in Buffalo in 1989, when the institution was awash in a sea of red ink.. Largely because of ruthless expense reduction, Mr. Cooper -- former president and cost-cutting specialist at San Francisco-based BankAmerica Corp. -- lifted Goldome into the black. In 1988, the bank posted a net loss of $119 million. By contrast, in 1990 the bank earned $37.5 million.. Nonetheless, Goldome remained starved for capital, and the Federal Deposit Insurance Corp. last month parceled out the Buffalo banks franchise to Albany-based KeyCorp and First Empire State, also of Buffalo.. Shortly before stepping down from his post as chief executive officer at Goldome, Mr. Cooper summed up some of his accomplishments.. Many people talk about cost cutting the way they used to talk about the four-minute mile. Too often, they start by assuming that it simply cant be done.. But it has ...

The purpose of the Advisory Committee is to provide feedback on our research plans, the data used in this project, and the analysis conducted on that data. The Advisory Committee helps to ensure that the right questions are being asked, that assumptions are challenged where appropriate, and ultimately, that reports and recommendations to the Legislature are meaningful and useful toward the end-goal of achieving cost containment in the Commonwealths healthcare market. ...
The costs of information technology (IT) in large multinational companies (MNCs) often constitute a significant portion of the companys total yearly turnover. IT departments are on one hand expected to return value to business, but are on the other hand often prone to cost reductions. Thus, it is not unusual that IT management chooses to focus on cost cutting rather than cost control in order to meet the companys financial targets. Understanding cost drivers in IT is not always evident, making it difficult for managers to know what performance indicators that should be tracked. This study addresses the issue of cost control and performance measurement in IT departments, in the form of a case study carried out at the IT department of a large Swedish MNC in the telecommunications industry. The case company is divided into two parts: Enterprise and Engineering, where Enterprise provides the organization with IT in form of printers, personal computers, applications, IT support and communication ...
