Mixed warm and cold autoimmune hemolytic anemia runs a chronic course with severe intermittent exacerbations. Therapeutic options for the treatment of hemolysis associated with autoimmune hemolytic anemia are limited. There have been only two reported cases of the effective use of rituximab in the treatment of patients with mixed autoimmune hemolytic anemia. We report a case of severe mixed autoimmune hemolytic anemia that did not respond to steroids and responded to four weekly doses of rituximab (one cycle). A 62-year-old Caucasian man presented with dyspnea, jaundice and splenomegaly. His blood work revealed severe anemia (hemoglobin, 4.9 g/dL) with biochemical evidence of hemolysis. Exposure to cold led to worsening of the patients hemolysis and hemoglobinuria. A direct antiglobulin test was positive for immunoglobulin G and complement C3d, and cold agglutinins of immunoglobulin M type were detected. A bone marrow biopsy revealed erythroid hyperplasia. Background: Ataxia telangiectasia (AT) is one of the combined immunodeficiency syndromes with immunologic, neurologic, endocrinologic, hepatic and cutaneous abnormalities. Regarding the fact that autoimmune disorders; such as autoimmune hemolytic anemia (AIHA), are not generally expected in the course of AT, we present a patient with an unusual presentation of these two conditions. Case presentation: An otherwise seemingly normal girl, who had developed limping at the age of 11 months old, referred to Namazi Hospital, Shiraz, Iran, due to pallor and latitude at the age of 3 yrs and was diagnosed with AIHA. After 2 years of therapeutic course she developed ocular telangiectasia and ataxic gate. Conclusion: This case emphasizes the possibility of ataxia telangiectasia coexistence with autoimmune disorders and must be taken into consideration by physicians.

Background: Pneumococcal hemolytic uremic syndrome (P-HUS) is a rare but severe complication of invasive pneumococcal disease (IPD) in young children. We have focused on finding ways to improve transfusion safety. We have studied blood group alloimmunization, i.e. the development of antibodies to red blood cells that occurs in some patients after transfusion. These antibodies are important since they can make it difficult to find compatible blood. They also mediate destruction of transfused red blood cells and transfusion reactions. We found that these antibodies are more dangerous than previously thought due to their higher rate of disappearance over time than previously thought. Their disappearance causes them to become undetectable during transfusion compatibility testing, yet their levels increase dramatically following transfusion of incompatible red blood cells. We have also studied the tendency of some patients to develop more than one blood group antibody. Currently, we are investigating ways to better detect and prevent blood group antibodies. N2 - OBJECTIVE: To assess the utility of 24 and 48 hours transcutaneous bilirubin (TcB) index for predicting subsequent significant hyperbilirubinemia in healthy term neonates.METHODS: TcB indices were obtained for healthy, breastfed, term AGA newborns at 24 +/- 2, 48 +/- 2 and subsequently at intervals of 24 hours. Neonates with illness, on treatment and positive Direct Coombs test were excluded. Serum bilirubin levels were obtained whenever indicated. Neonates having serum bilirubin , or = 17 mg/dL were considered as significant hyperbilirubinemia. The 24 and 48 hour TcB indices, as risk predictors for such hyperbilirubinemia were determined.RESULTS: Study included 461 healthy term neonates. Single port laparoscopic surgery further improves cosmesis, pain control and recovery compared to standard multi-port laparoscopy.. Patient was maintained on prednisone 30 mg daily preoperatively. Preoperative imaging localized accessory spleen to left upper quadrant area. With the use of SILS™ Port (Covidien) accessory spleen was identified in splenic fossa and removed. The later represented globular mass of 35 grams, 4.6 x 4.3 cm in diameter. Procedure lasted 80 minutes. Estimated blood loss was 30 ml. Postoperatively patient did well and was discharged on postoperative day three requiring only four Lortab tablets during hospital stay. Clinicians have been found to hold predominantly negative attitudes toward routine outcome assessments (ROA). This study aims to assess changes in clinicians attitudes to ROA, and in particular, the provision of feedback from such assessments following a training workshop. Ninety-six mental health workers attended a training workshop on ROA, which was supported by the use of a CD-ROM video resource. Participants completed a questionnaire before and after training that assessed their attitudes toward ROA and the provision of feedback from these assessments. Attitudes toward feedback were predominantly positive at baseline. Significantly more positive attitudes on general attitudes and specific attitudes related to the provision of feedback were found following training. Generating positive clinician attitudes is the first step toward improving the processes and effectiveness of ROA. Controlled trials with a follow up of clinicians behaviour are needed to determine whether the changes found are

We have investigated the hemolytic mechanisms in a patient with acquired immune hemolytic anemia whose red cells appeared to be coated with IgA alone. The clinical course was similar to that of patients with hemolytic anemia mediated by warm-reacting IgG antibody. Splenic sequestration of red cells was demonstrated, and marked reduction of hemolysis occurred after corticosteroid therapy. And this is very significant for the fact that it appears David Coombs, Mannings defense lawyer, built a lot of his case around being able to discuss this evidence. But now the judge has -- shes been won over by this idea presented by the prosecutors that it is not part of the charges and that the defense should not be able to raise this evidence when they discuss it in court. BACKGROUND: The anti-HLA antibody can be detected using either a complement-dependent lymphocytotoxicity (CDC) assay or a flow cytometry crossmatch (FCXM) in renal transplantation. Discordant results are often obtained because the two methods detect different reaction phases between the donor lymphocytes and the recipient sera. This study was intended to confirm that antibody binding and cytotoxicity to the lymphocytes can be detected simultaneously in a single FCXM assay, cytotoxic FCXM.. METHODS: In the cytotoxic FCXM, the antibody binding to the lymphocytes was measured using anti-IgG-FITC, and the cytotoxicity using 7-aminoactinomycin D (7-AAD) after adding complement. For an evaluation of two test parameters, the cytotoxicity test moiety (dead-cell percentage) was compared with the anti-human globulin (AHG)-CDC, and the antibody-binding test moiety (sample/control fluorescence ratio) with the conventional FCXM in 77 positive and 30 negative crossmatches..
