Background. Conversion disorder is largely managed by neurologists, for whom it presents great challenges to understanding and management. This study aimed to quantify these challenges, examining how neurologists understand conversion disorder, and what they tell their patients.. Methods. A postal survey of all consultant neurologists in the UK registered with the Association of British Neurologists.. Results. 349 of 591 practising consultant neurologists completed the survey. The authors prospectively studied consecutive neurological inpatients with either motor conversion symptoms or pseudoseizures of recent onset. Patients were administered a structured psychiatric diagnostic interview, a measure of perceived parental care, and a life events inventory. They found that patients with pseudoseizures (N=20, mean age=27 years): 1) were younger than patients with motor conversion symptoms (N=30, mean age=39 years), 2) were more likely to have a borderline personality disorder), 3) were more likely to have a lower perception of parental care and to report incest, and 4) reported more life events in the 12 months before symptom onset. Some evidence suggests that psychogenic non-epileptic seizures (PNES) are associated with increased mortality. The authors obtained death certificate information in a cohort of 260 patients who presented with PNES between 1999 and 2004. The follow-up period averaged 7.92 years, during which 17 patients died, 12/17 were under the age of 75 years, giving a premature (,75 years) mortality rate of 0.58%, compared with a Scottish mortality rate for the 40-75 years age group of 0.41% per year.
