In the developing world, 74 million unintended pregnancies occur annually, of which a sizable share, 30%, are due to contraceptive failure among women using some type of contraceptive method. Detailed information on contraceptive failure rates is critical to inform improvements in provision of contraceptive information, supplies and services, which can help women and couples to use methods correctly and consistently. We undertook a study to update contraceptive failure rates and provide a more comprehensive global picture. We provide new estimates of contraceptive failure in 43 counties across seven subregions using recent data from Demographic and Health Surveys (DHS ...

This is quite an understatement of the importance of medical oversight. Chronic conditions, especially those like diabetes mellitus and asthma, require follow up and guidance from their physicians. Having diabetes is quite a bit more complicated than mere numbers and glucometer checks. Diabetes education, foot checks, eye exams and hemoglobin A1C are just a few of the regular checks that your physician performs. Additionally, with asthma, the disease can change and require a change of medications. It is also important patients receive education and guidance to avoid exacerbations and trigger events. Yeah, they still need to see their doctor ...

However, most contraceptive delivery programs are implemented by health departments at lower administrative levels, reflecting a persisting gap between the availability of and need for subnational mCPR estimates. Methods: Using woman-level data from multiple semi-annual national survey rounds conducted between 2013 and 2016 in five sub-Saharan African countries (Burkina Faso, Ethiopia, Ghana, Kenya, and Uganda) by the Performance, Monitoring and Accountability 2020 project, we propose a Bayesian ...

The aim of the study was to investigate the determinants of use of modern contraceptive among married women in Ethiopia. Our study is based on the data taken from a nationally representative survey EDHS of 2011. The sample includes 9,438 married women aged 15-49 years. Cross tabulations were carried out at the bivariate level to assess the association between contraceptive use and each of the explanatory variables and binary multiple logistic regression analysis was used to identify the factors influencing modem contraceptive use among married women in Ethiopia. The bivariate analysis reveals statistically significant association between all explanatory variables i.e age of woman, region, religion, place of residence, education level of woman, number of living children, desire for more children, wealth status, and decision maker for modern contraception, educational level of husband, modern contraceptive knowledge and exposure to media. Results for binary multiple logistic regression analysis reveals
Background: Unwanted pregnancy and HIV infection are issues of significant concern to young people. Limited data exists on contraceptive decision-making and practices among HIV-infected and HIV-negative young people in low resource settings with generalized HIV epidemics. Methods: From July 2007 until April 2009, we recruited, and followed up over a one year period, a cohort of 501 HIV-negative and 276 HIV-infected young women and men aged 15-24 years residing in Kampala and Wakiso districts. We compared contraceptive use among HIV-infected and HIV-negative young people and assessed factors associated with contraceptive decision-making and use, using multivariate logistic regression modelling to estimate odds ratios (OR) and 95% confidence intervals (CI). Results: Contraceptive use among sexually active HIV-infected young people was 34% while it was 59% among the HIV-negative group. The condom was the most frequently used method of contraception. Only 24% of the HIV-infected used condoms ...
Despite progress in recent years, girls and women remain disadvantaged relative to boys in many parts of the world, with multiple negative potential implications. At the heart of gender inequality are social norms and attitudes about gender that give rise to and perpetuate discriminatory institutions, and ultimately inhibit institutional and behavioral change. Despite this, little is understood about the how gender norms and attitudes evolve and adapt. One important question is why is gender inequality acute and persistent in particular societies but not others? A related question is how readily do gender attitudes evolve when women are given more opportunities to advance and thereby influence perceptions about womens relative ability? This project will contribute to our understanding of both questions by collecting new and unique data that will be used to investigate how gender attitudes respond to improvements in girls schooling and a narrowing of the gender gap in cognitive ability that ...

Significant proportions of U.S. women switch contraceptive methods. Indeed, when all method-related reasons for stopping use are considered, two-year discontinuation rates are as high as 90% for some methods; rates are particularly high among unmarried women.1 As a result, the rate of method switching is the primary determinant of the prevalence of use of specific contraceptives.
How effective are the various methods? Some contraceptive methods are more effective in preventing pregnancy than others, while only condoms offer any protection against sexually transmitted infections. The following figures will give you some idea of which kinds of contraception are the most efficient at protecting you against pregnancy.
Contraceptive methods involve the whole range of measures used to prevent pregnancy. They may be reversible or irreversible. The reversible ones can be

As the United States celebrates Black History Month, one enduring challenge we face as a nation is the persistent health disparities between black and white Americans. The area of sexual and reproductive health, in particular, is riddled with inequities, including differences in contraceptive failure rates, unintended pregnancy and abortion among teen and adult women, and rates of infection with HIV and other STIs among both men and women. While much progress has been made, our efforts and attention need to be directed toward equalizing health outcomes for all Americans.
And then right in the middle of everything she finds out that she is pregnant. Her life is not a leisurely one with time to start studying biology and childcare and her own psychology and the big questions like how can this pregnancy be framed as a positive thing, rather than a contraceptive failure and a huge incoming expense? She had a job. A family. Travel plans. So whatever research she did not probably include a few salient facts: It is advised that you get a doctors clearance before you fly in the third trimester. (They look at the cervix. If the cervix is tight and thick and solid you are good to go. Lots of women fly in the third trimester.) As all those T.V. parents say: "This isnt an argument, its a discussion!" I understand your point, and I feel that were on opposite sides of the fence. I simply dont believe that weve evolved past the point of our primal urges enough (or at all) to make the link between procreation and sex appeal any less relevant. As Nyokki pointed out, guys in our society arent normally after girls to consciously get them pregnant. I daresay they normally avoid it with the contraceptive methods you mentioned. If they care enough to do so. The urge to "hook up" is strong enough to explain 99% of the stupid crap we do as guys. In my mind, it all relates back to animalistic hierarchy and mating. Were not as evolved as we like to think we are. Every time a guy gets hurt trying to look cool, its all about trying to assert his place in the pack. Every time he ends up looking like a dork in front of a girl, its a misguided attempt at "clubbing her over the head." Metaphorically speaking of course. In my ...

Although CBD of injectable contraceptives is no longer a novel idea in sub-Saharan Africa, the process of translating community-based health care policies into effective programs is not always straightforward. For example, policy makers in Mozambique were supportive of CBD, but there was limited country-specific experience and no country-level data to support the policy. Consequently, this study generated evidence to support CBD of DMPA in northern Mozambique and found that provision of injectable contraceptives by APEs and TBAs was feasible, safe, effective, and acceptable among women. Very few morbidities at the injection site and no deaths were reported. The study demonstrated that APEs and TBAs can improve contraceptive access and use in rural Mozambican communities; in fact, the majority of women in the study started using contraception for the first time during the study period, and satisfaction with community-based providers was high and improved over the entire study ...
Despite the enormous benefits of family planning services, the contraceptive utilization still remains low in Sub-Saharan Africa. There is regional variation in modern contraceptive utilization in Ethiopia. Therefore, this study was aimed to determine the prevalence of modern contraceptive utilization and determinants in Dabat demographic and health surveillance system site, northwest Ethiopia. A re-census was carried out in Dabat Health and Demographic Surveillance System (HDSS) site from October to December 2014. Data of 8271 married women collected in the re-census was used. The outcome variable was current utilization of any modern contraceptive methods whereas socio demographic and economic variables were the potential determinants considered. Bi-variable and multivariable binary logistic regression along with odds ratio and 95% confidence interval were used to describe the strength of association. Prevalence of modern contraceptive utilization among married women in Dabat DHSS site was found to be
The transmembrane migration method has permitted quantification of the effect of drugs on human sperm motility. Calcium ionophores and calcium antagonists have paradoxical effects on motility depending on the stage of sperm maturity. Drugs with membrane stabilizing properties inhibit sperm motility. Propranolol, for example, has been shown to have contraceptive activity when administered intra-vaginally. Pharmacokinetic studies have shown that the systemic availability of vaginally administered propranolol is markedly greater than after oral administration. Seminal fluid concentrations of propranolol after oral administration are similar to those in plasma, but cervicovaginal mucus concentrations are much higher, and residence times much longer. The clinical consequences of these observations await elucidation.. ...

Societal norms about gender roles contribute to the economic disadvantages facing women in many developing countries. This paper evaluates an intervention aimed at eroding support for restrictive gender norms, specifically a multi-year school-based intervention in Haryana, India, that engaged adolescents in classroom discussions about gender equality. Using a randomized controlled trial, we find that the intervention increased adolescents support for gender equality by 0.25 standard deviations, a sizable effect compared to other correlates of their gender attitudes such as their parents views. Program participants also report more gender-equitable behavior; for example, boys report helping out more with household chores. Worldwide, 214 million women have an unmet need for modern contraception (i.e., they do not wish to get pregnant and are using no contraceptive method or a traditional method).6 Access to FP methods varies significantly by region (see Table 1). Unmet need for FP is highest in regions like sub-Saharan Africa and Oceania where contraceptive prevalence is lowest.7 Adolescent fertility rates have declined slowly and remain particularly high in sub-Saharan Africa, where child marriage remains common, and in Latin America and the Caribbean.8 While the percentage of pregnant women receiving the recommended minimum number of four antenatal care visits has been on the rise, it is 52% in developing countries and lower still in sub-Saharan Africa and Southern Asia.9 Each year, approximately 47,000 women die from complications associated with unsafe abortion.10 Access to and use of effective contraception reduces unintended pregnancies and the incidence of abortion.11. Reasons for the lack of access to and, ...
Similar to any other technique of birth control, the DMPA contraceptives also have their advantages and disadvantages. First, a look at the advantages. This method is hassle free and effective. It has been used by over 90 million women and is reported to be as effective as sterilization. One injection can prevent pregnancy for 3 months. Secondly, it offers no hindrance to normal sexual life and can be made use of at any stage. Thirdly, it is extremely safe and can be even used by nursing mothers as early as six weeks after childbirth. Also, there is probably no increase in congenital abnormalities when contraception failure occurs. Fourthly, there are no side effects related to estrogen, which involve the risk of heart attack. Fifthly, it shields the body from ovarian and endometrial cancer and uterine fibroids. Lastly, it is reversible and one can plan a family after a gap of three to six months on stopping the usage.. The disadvantages include alteration in menstrual bleeding, which implies ...

Special consideration: Adolescents In general, adolescents are eligible to use any method of contraception and must have access to a variety of contraceptive choices. Age alone does not constitute a medical reason for denying any method to adolescents. While some concerns have been expressed regarding the use of certain contraceptive methods in adolescents (e.g. the use of progestogen-only injectables by those below 18 years), these concerns must be balanced against the advantages of avoiding pregnancy. It is clear that many of the same eligibility criteria that apply to older clients apply to young people. However, some conditions (e.g. cardiovascular disorders) that may limit use of some methods in older women do not generally affect young people since these conditions are rare in this age group. Social and behavioural issues should be important considerations in the choice of contraceptive methods by adolescents. For example, in some settings, adolescents are also at increased risk for STIs, ...
