On March 14, the Connecticut Supreme Court issued its ruling in Donna Soto, et al. v. Bushmaster Firearms International, LLC, et al. (SC 19832), a lawsuit stemming from the Dec. 14, 2012, mass shooting at Sandy Hook Elementary School in Newtown, Connecticut. That lawsuit saw the plaintiffs - administrators of the estates of nine of the victims - bring a variety of claims against defendant Bushmaster, the manufacturer of the XM15-E2S modern sporting rifle used by the shooter. These guidelines recommend using either a screening-based or a risk-based strategy to identify women who should receive intrapartum antimicrobial prophylaxis. To assess adoption of the GBS disease prevention guidelines, the Connecticut and Minnesota state health departments surveyed prenatal-care providers during January--April 1998. This is a 5-day tour that goes from the Connecticut shoreline up through State Forests and back down to the Rhode Island shoreline. The roads are mostly quiet and the scenery is gorgeous. The trip starts and ends in New Haven, with a Metro North ride at either end. Be aware that bikes are not permitted on rush hour trains in either direction on the New Haven line. We started and ended on a weekday, so we had to take a 9:04 train and thus started in New Haven a bit after 11:00. For this reason, we took the flatter of the two Day 1 routes. This stays on Rte. 1 for 11 miles, but theres a wide shoulder for most of it and traffic was not bad. The original route has a lot of climbing over the second half, including a 13.5% grade. There is a long history in Connecticut of providing permanent supportive housing to homeless populations. Ten years ago, the Corporation for Supportive Housing helped with data matching for the most vulnerable people in the state. The first match between HMIS and state Medicaid data was for a re-entry program for the criminal justice population. They used that data-matching model to identify housing needs and ultimately to decrease costs for high-cost Medicaid users. This cost savings resulted from a substantial reduction in the utilization of health care and shelter systems. The Connecticut Coalition to End Homelessness received a federal Social Innovation Fund grant to house the 160,000 highest-cost Medicaid beneficiaries who use housing services. Once this grant was in place, the state experienced a decrease in high-cost medical services use, such as emergency departments, ambulances, behavioral health care, and hospitals, but also an increase in outpatient and medication usage. Kristopher T. Kahle, M.D., Ph.D., is an Assistant Professor of Neurosurgery and Pediatrics and Cellular and Molecular Physiology at Yale University School of Medicine, and Director of Neonatal and Congenital Anomaly Neurosurgery in the Division of Pediatric Neurosurgery. He completed his MD and PhD degrees at the Yale School of Medicine under the mentorship of Richard Lifton, and neurosurgical residency at the Massachusetts General Hospital.
