Keywords: Acute kidney injury, chronic kidney disease, end stage renal disease (ESRD), general anesthesia, hemodialysis, intraoperative hypotension, preventative nephrology.. Abstract: Background/Objective: Intraoperative hypotension (IOH) invariably follows the induction of general anesthesia during surgical operations. The current prevailing and predominant consensus is that IOH has immense clinical benefits such as reduced bleeding, less need for blood transfusions, and shorter surgery times. Simultaneously, it is assumed that IOH is devoid of adverse renal, hepatic and neurological consequences. Emerging new evidence and our experiences suggest a strong link between IOH and postoperative acute kidney injury (AKI). Method/Case Reports: We report on three case presentations to illustrate the impact of IOH on postoperative AKI. Conclusion: Our recent experiences suggest and show a link between IOH and postoperative AKI. Sun et al. (2015) recently demonstrated that postoperative AKI was ...

Methods We analysed data for patients undergoing major non-cardiac surgery in 2008 from The American College of Surgeons' National Surgical Quality Improvement Program database (a prospective validated outcomes registry from 211 hospitals worldwide in 2008). We obtained anonymised data for 30-day mortality and morbidity (cardiac, respiratory, CNS, urinary tract, wound, sepsis, and venous thromboembolism outcomes), demographics, and preoperative and perioperative risk factors. We used multivariate logistic regression to assess the adjusted and modified (nine predefined risk factor subgroups) eff ect of anaemia, which was defi ned as mild (haematocrit concentration ,29-,39% in men and ,29-,36% in women) or moderate-to-severe (≤29% in men and women) on postoperative outcomes.. Interpretation Preoperative anaemia, even to a mild degree, is independently associated with an increased risk of 30-day morbidity and mortality in patients undergoing major non-cardiac surgery." ...

Sara A Hennessy, MD2, Tjasa Hranjec, MD, MS1, Peter T Hallowell, MD2, Bruce D Schirmer, MD2. 2University of Virginia, 1University of Southwestern Texas. Introduction: Approximately 3-13% of patients with abdominal surgery develop a ventral hernia and over 350,000 ventral hernias are repaired annually. A surgical site infection (SSI) after ventral hernia repair (VHR), either open or laparoscopic, is a highly morbid complication associated with increasing health care costs, hernia recurrences, mesh infections and their complications. With the many advances in preoperative, operative and post-operative care the current incidence of surgical sites infections after laparoscopic VHR is unclear. We hypothesized that the incidence of surgical site infections is significantly reduced with a laparoscopic ventral hernia repair.. Methods and Procedures: The prospectively collected American College of Surgeons' National Surgical Quality Improvement Program database was retrospectively reviewed for all VHRs ...

Risk of bias was assessed according to the Cochrane tool and a modified Downs and Black quality analysis for RCTs and CCTs, respectively. Results: Sinus membrane perforation and hemorrhagic events following vascular lesions were the only intraoperative complications reported by the selected studies with overall occurrences of 15.7% and 0.4%, respectively. Three different surgical devices (rotary instruments, piezoelectric osteotomes, and manual bone scrapers) were used to perform the lateral antrostomy. Ultrasonic devices and ...
This is the first study to compare the incidence and severity of transplant coronary artery disease in heart-lung and heart transplant recipients with the use of ICUS. The results of this study show that the incidence and severity of transplant coronary artery disease are less in heart-lung transplant recipients than in heart transplant recipients.. A major cause of mortality, and the main cause of retransplantation in heart transplant recipients, is transplant coronary artery disease. As many as 50% of the patients will develop significant angiographic disease by 5 years after transplantation. The majority of the heart-lung transplant recipients who survive the first year do not die as the result of cardiac disease or event.8 9 12 The present study confirms earlier reports that indicate that transplant coronary artery disease, as assessed by quantitative coronary angiography, does not play a major role in the morbidity and mortality of heart-lung transplant recipients. Methanol poisoning is a serious public health issue in developing countries, but few data are available in the literature on acute kidney injury (AKI) after methanol intoxication. This study examined the clinical features, spectrum and outcomes of AKI in patients with methanol intoxication and evaluated the predictors of mortality after methanol intoxication. A total of 50 patients with methanol intoxication were seen at Chang Gung Memorial Hospital between 2000 and 2013. Patients were grouped according to the status of renal damage as AKI (n = 33) or non-AKI (n = 19). Demographic, clinical, laboratory, and mortality data were obtained for analysis. Most patients were middle-aged (47.8 ± 14.9 years), predominantly male (74.0%), and habitual alcohol consumers (70.0%). Most incidents were oral exposures (96.0%) and unintentional (66.0%). Two (4.0%) patients attempted suicide by intravenous injection of methanol. Five (10.0%) patients suffered methanol intoxication after ingestion of methomyl pesticide

Background: In this study, we aimed to investigate frequency, patterns, etiologies, and costs of unplanned readmissions after left ventricular assist device implantation.. Methods: Between April 2012 and September 2016, 99 unplanned readmissions of a total of 50 consecutive bridge-to-transplant patients (45 males, 5 females; mean age 46.9±10.3 years; range, 19 to 67 years) who were successfully discharged after left ventricular assist device implantation were retrospectively analyzed. Patient demographic data, hemodynamic measurements before implantation, and readmissions after discharge were recorded. Hospitalizations due to major problems which were unable to be managed in routine outpatient clinic were accepted as unplanned readmissions. Survival analysis was performed.. Results: The readmission rate was 1.7 per year after discharge. Survival of patients who were readmitted within the first 90 days was found to be significantly lower than those without early readmission. The most common ...
Introduction: Laparoscopic cholecystectomy (LC) is one of the most common surgical procedures performed by non academic surgeons as well as general surgery residents, under the supervision of academic surgeons. Bile duct injury is a catastrophic complication of this procedure. There is a general perception that performance of LC in a facility with a surgical training program offers a safer environment due to the presence of an assistant surgeon.. Objective: To compare the rate of bile duct injury, conversion and overall mortality between hospitals with surgical training programs (Group I), and hospitals without surgical training programs (Group II) after LC.. Methods: Methods: Using the Florida State Inpatient Database we extracted and analyzed data for LC and conversion from LC to open cholecystectomy (OC) during the years 1997 thru 2006. Bile duct injury was indicated by a biliary reconstruction procedure performed during the same admission. Hospitals with surgical training programs were ...

The BARI 2D trial reported no significant differences in the primary end point of all-cause mortality or in the principal secondary end point of all-cause death/MI/stroke between patients with type 2 diabetes mellitus undergoing revascularization and those undergoing IMT or between strategies of IS and IP. However, in the CABG stratum, a significant reduction in major cardiovascular events (all-cause mortality/MI/stroke) was seen with CABG compared with IMT, largely because of a reduction in MI in patients within the IS strategy.7 In our original report, MI rates included only nonfatal MI events; in this article, the MI rates reported include both fatal and nonfatal MI events.. In BARI 2D, 43% of the total mortality was attributed to cardiac causes over the average 5.3-year follow-up. Cardiovascular deaths accounted for 37% to 53% of all-cause mortality in the Action to Control Cardiovascular Risk in Diabetes Study Group (ACCORD), Veterans Affair Diabetes Trial (VADT), Action in Diabetes and ...

Current International Diabetes Federation guidelines recommend a target HbA1c |7.0%, but many people with diabetes worldwide find this difficult to achieve, increasing their risk of developing complications. This publication examines the prevalence of diabetes complications and its association with baseline characteristics in people with type 2 diabetes who participated in the A1chieve study. A1chieve was a 24-week, multinational, open-label, observational study of 66,726 people with type 2 diabetes who had begun using biphasic insulin aspart 30, insulin aspart, or insulin detemir in routine clinical care. Participants were enrolled from 28 countries across four continents (Asia, Africa, Europe and South America). Baseline measurements of disease characteristics included: glycated haemoglobin (HbA1c), fasting (FPG) and post-prandial plasma glucose (PPG), high- and low-density lipoprotein cholesterol (H- or LDL-C), systolic blood pressure (SBP), and body mass index (BMI). Data on complications and use of
The term 'diabetes mellitus' refers to a group of disorders that relate to carbohydrate metabolism. The condition is characterised by hyperglycaemia due to decreased and therefore inadequate levels of insulin in the body, resistance to the effects of insulin, or a combination of both. The symptoms of marked hyperglycaemia include polyuria, polydipsia, weight loss, polyphagia and blurring of vision. The aetiological types of diabetes mellitus are type 1, type 2, gestational diabetes mellitus and other specific types. However, the two major categories of diabetes mellitus are type 1 and type 2. Type 1 (formerly known as insulin-dependent diabetes mellitus) patients are usually at risk of developing ketoacidosis and require insulin therapy. Patients who suffer from type 2 diabetes mellitus, formerly known as noninsulin-dependent diabetes mellitus, as opposed to those suffering from type 1 diabetes mellitus, still have functional pancreatic β cells which produce insulin. However, they suffer from a

INTRODUCTION: To determine the safety and efficacy of single-site robotic cholecystectomy compared to multi-port laparoscopic cholecystectomy within a high-volume tertiary health care center.. METHODS AND PROCEDURES: A retrospective analysis of prospectively maintained data was conducted on patients undergoing single-site robotic cholecystectomy or multi-port laparoscopic cholecystectomy between October 2011 and July 2014. All surgeries included in the study were performed by a single surgeon. Pre-operative (i.e. age, gender), peri-operative (i.e. operative time, intra-operative complications, conversions), and post-operative (i.e. length ...
This is the first demonstration - using direct measurement of bone strength in the body - of compromised bone material in patients with type 2 diabetes," says Sundeep Khosla, M.D. , Mayo Clinic endocrinologist and senior author of the study. "Clearly, the skeleton needs to be recognized as another important target of diabetes complications.". Previous studies in the field showed that patients with diabetes experienced fractures at levels of bone density above that of the regular population, hinting that something was different about the "quality" of their bone. The Mayo researchers validated that assumption in a clinical study of 60 postmenopausal women, 30 of whom had type 2 diabetes. Using a new tool (OsteoProbe®), the researchers performed micro indentation testing of the tibia (actually causing a microscopic crack) to measure bone material strength. Compared to the control group of women, aged 50 to 80, the group with type 2 diabetes had significantly lower bone material strength. There was ...
Patients with OSA are reported to have a higher rate of severe respiratory complications associated with upper airway obstruction during anesthesia and sedation or immediately after anesthesia. Children with OSA (especially those under three years of age, those with severe OSA, cerebral palsy or craniofacial anomalies) are at increased risks for post-operative complications, and require careful monitoring post-operatively.. Although the etiology of obstructive sleep apnea is mainly obstruction due to anatomical and neuromuscular abnormalities, we believe that a central element may contribute to OSA.. The aim of this study is to evaluate whether administration of caffeine to children with OSA, scheduled for elective T & A under general anesthesia contributes to a faster recovery, less post-operative complications, and a shorter stay in the PACU, DSU and the hospital. ...
