In recent decades the community mental health movement has achieved a dramatic reduction in the census of state and county mental hospitals in the United States, and hundreds of federally-funded community mental health centers have been established nationwide. At the same time, national controversy has arisen in response to what in places has seemed the haphazard process of implementing "deinstitutionalization" and the fate of many chronically mentally ill persons who are without needed social services and psychological care. Despite the widespread attention that this contemporary program has received, theoretical analysis of the complex social, scientific, intellectual, and political origins of Americas community mental health policy remains deficient. This article examines the background and development of the Community Mental Health Centers Act of 1963, tracing how an important shift in national policy toward the mentally ill grew out of changing perceptions-among policymakers, professional ...

7. Defendant cites the Departments Declaratory Ruling in Howard Center for Human Services, Inc. v. Commerce and Industry Insurance Co. and AIG (undated opinion) as establishing a precedent against finding statutory employer status in situations similar to the current claim. In Howard Center, the Department concluded that a community mental health center was not the statutory employer of the developmental home providers with whom it arranged placements for developmentally disabled adults. Upon careful reading of the Commissioners Ruling, I find that the facts at play in the current claim differ significantly from those underlying the Howard Center ruling. To be sure, the community mental health center under scrutiny in that ruling took an interest in ensuring that the adults it placed were appropriately cared for at home. Beyond addressing basic safety issues, however, it did not dictate the home environment. Nor did it supervise the home providers directly or impose rigorous requirements as to ...

RIN: 0938-AS82). (i) Cost-benefit analysis. The Centers for Medicare and Medicaid Services (CMS) estimate that the policies in this final rule with comment period will result in a 1.7 percent overall increase in outpatient prospective payment system (OPPS) payments to providers. CMS estimates that total OPPS payments for calendar year (CY) 2017, including beneficiary cost-sharing, to the approximate 3,906 facilities paid under OPPS (including general acute care hospitals, childrens hospitals, cancer hospitals, and community mental health centers (CMHCs)) will increase by approximately $773 million compared to CY 2016 payments, excluding CMSs estimated changes in enrollment, utilization, and case-mix. CMS estimated the isolated impact of its OPPS policies on CMHCs because CMHCs are only paid for partial hospitalization services under OPPS. Continuing the provider-specific structure that CMS adopted beginning in CY 2011 and basing payment fully on the type of provider furnishing the service, it ...
This guide provides an overview of practical program evaluation activities that can assist mental health centers in improving their administrative and program capabilities. Definitions of terms and assumptions are followed by an outline of the stages of the program evaluation cycle. Program evaluation activities, the roles of the administrator and evaluator, alternative ways to organize and deploy staff for program evaluation activities, and evaluator qualifications are presented. The importance of the administrators leadership skills in creating a positive climate for program evaluation is stressed. Program monitoring is suggested as one way to create a balance between the need to improve management procedures and the need to maintain or improve the quality of client care. Additionally, various uses of evaluation findings are discussed. (Author/NRB)

Findings of excess cardiometabolic morbidity and mortality in persons with severe mental illness (SMI) have led to a growing interest by Community Mental Health Centers (CMHCs) in improving the medical care of the populations they treat. However, these organizations face a number of financial and organizational barriers to implementing and sustaining such programs. In previous and ongoing work, the study team has documented the promise of team-based models in improving health and health care in this population. This study will test a novel approach for improving mental health consumers based on a partnership model between a CMHC and a Community Health Center (CHC). This partnership will capitalize on collocation of services, the primary care expertise of the CHC, and favorable reimbursement conditions, to develop a program that is both clinically robust and financially and organizationally sustainable A total of 300 CMHC clients with a severe mental illness and one or more active cardiometabolic ...

Description: The purpose of this study is to test the utility of Talcott Parsons AGIL Model, i.e., Adaptation, Goal Attainment, Integration, and Latency (Pattern Maintenance) in evaluating the program effectiveness of a community mental health center (CMHC). The model provided a conceptual framework for the selection of appropriate variables. The dependent variable in this study is the overall evaluation of the CMHC as measured through the perception of community leaders. Fourteen hypotheses were constructed to identify and test the relationship among the AGIL criteria and the use of a selected set of independent variables. Data for this study were collected from primary and secondary sources. Secondary data were obtained from the Texas Department of Mental Health and Mental Retardation in Austin and the CMHC center in Eton. Primary data were collected through personal interviews of general community leaders and influential persons in health-related activities in the community. The selected ...

Since Medicare Part D officially kicked in at the beginning of the year, the new program has given community mental health centers staff-and the people they serve-major headaches. Many behavioral health professionals remain skeptical that the federal government will make the regulatory and administrative changes needed to enable the more than 25 million participants to fully reap the intended benefit of the landmark program: access to prescriptions and reduced drug costs. The bottom line, say many behavioral health professionals, is that Part D has substantially increased their costs, taxed their resources, and frustrated their clients.. "From its inception, Part D has been an utter hassle for mental health service providers," says Jim Van Norman, MD, medical director of the Austin Travis County MHMR Center in Texas. When the program first began, he notes, more than 6.2 million dually eligible Medicare and Medicaid recipients automatically were enrolled in Part D plans. Yet "it was difficult to ...

An East Tennessee community mental health center, specializing in co-occurring substance abuse and mental health services, has an Intensive Outpatient Program (IOP) that utilizes a combination of cognitive behavioral therapy, motivational interviewing, and relapse prevention. Currently, 50% of individuals who enter the IOP program fail to graduate. The purpose of this study is to identify the factors associated with successful program completion. Methods: A secondary analysis was conducted on data from 116 participants in the IOP group. A logistic regression model was used to determine which factors were related to successful program completion. Results: The overall regression model was found to be statistically significant, (χ2 (20, N = 115) = 39.06, p | .05). Results indicate that age was a significant variable (p = .026), and with each increment in age, the odds of completion increased by almost 6%, when controlling for other variables in the model. Likewise, each increment of medication

The C and E department is useful to many individuals, and helps the other programs gain clients, but the C and E Department needs to show proof to the board that there is a connection by showing evidence of this. C and E department needs to utilize a process evaluation and an outcome evaluation, which will show that the programs it has, does generate clients for the other programs within Greenby. Some crimes are so horrific that even the most Christian of hearts struggle to find forgiveness. On July 30, 2008, Vince Li beheaded 22-year-old Tim McLean, a fellow passenger on a Greyhound bus in Manitoba, Canada. A psychiatric assessment concluded Li was suffering from untreated schizophrenia. The murder was the result of a severe psychotic episode. He was found not criminally responsible and admitted to the Selkirk Mental Health Centre for treatment. Public reaction continues to reflect misunderstanding of mental illness in general and schizophrenia in particular. When a review board first allowed Li supervised walks on the grounds of the Mental Health Centre, media focused on the anger and fears of local residents. The same review board has now recommended supervised walks in the town of Selkirk. Again, local media found no shortage of residents willing to voice their opposition on public airwaves.

For NDana Carter, the proposal to transfer patients from the citys Beverly-Morgan Park Mental Health Center to the center in Roseland is emblematic of the "callousness" of the cutbacks in Mayor Emanuels proposed budget.. The Beverly Area Planning Agency and other community groups will rally against the closing of the center on Monday, November 14 from 3 to 6 pm. at 111th and Longwood.. "Theres nowhere else in our community to receive public mental health services," said Matt Walsh, executive director of BAPA. We previously reported that cholinergic and serotonergic agents induce sleep changes partially consistent with monoamine models of sleep disturbances in depression. One potential cause of disturbed neurotransmission is genetic predisposition. The G(-1019) allele of the serotonin-1A (5-HT1A) receptor promoter region predicts an increased risk for depression compared to
Goal attainment scaling (GAS) is a therapeutic method that refers to the development of a written follow-up guide between the client and the counselor used for monitoring client progress. GAS was first developed by Thomas Kiresuk and Robert Sherman in response to the wide variety of evaluation models regarding mental illness and treatment. With the advent of GAS, Kiresuk and Sherman sought to create an evaluation program that could measure effectiveness across several different modalities and justify economic and labor resources based on effectiveness. Evaluation practices are important for justification and support for services, especially in mental health. The existing evaluation procedures had problems in definition and measurement, and each mental health center used its own definitions and measurements to evaluate. This created unspecified and informal evaluations. The variety of evaluation methods also made comparisons impossible. Thus, evaluation reform was needed. By focusing on broadly ...
Behavioral health EHR vendor Netsmart adds data analytic capabilities from InterSystems to its MyAvatar CareRecord EHR and PM software. Mental Health Center of Denver helped guide the technology integration - timed to coincide with the retirement of its legacy EHR - and served as a test site for the enhanced solution. While I couldnt uncover what type of EHR it originally used (it may have been homegrown), I did come across this Avatar implementation evaluation based on end-user surveys conducted twice during the first 12 months of testing/development. After a year, nearly 80 percent of users felt the system had become more usable, with "Workflow/Form Simplicity" being ranked as the top area for improvement. My favorite observation from the evaluation was this: "While it is unreasonable to expect that all staff will ever feel favorable towards the electronic health record, it is reasonable to expect that the favorable rating will increase over time with continued efforts to improve and enhance ...

Thursday Sept 19 Alan is back in Reno at work. James is in New Braunfels a few days while his brother, Billy is in Methodist Hospital in San Antonio recovering from surgery on his cervical vertebrae. James main task is to drive his brother and sister-in-laws grandson to school and pick him up and watch him. Mark and I are "batching it." Yesterday I worked at the mental health center from 9 am to 8 pm with an hour off from 4:30 to 5:30 to run home to check on things. Mark has been tired and complaining of no energy and is in more pain without anything much to focus on right now. After another busy day beginning at 4 am this morning I went grocery shopping at 9 pm. I had a strange urge to lie down in the grocery aisle and take a nap, but instead I came home with the groceries to watch a science fiction movie with Mark and make some stew. Friday Sept 20 Previous footage of the night poker game for the film Gun 4 Hire shot at our house had to be reshot for various reasons. All the original cast of ...

Q: In a "nutshell," how would you describe your network?. A: We are a very mature network, over 21 years old. Were made up of community mental health centers and addiction providers in rural Indiana. We have ten members within our network, all located within the state of Indiana.. Weve accomplished a number of successful "Indiana Firsts." We became the first clinical provider network for the Indiana Division of Mental Health and Addiction back in 95. We were the first to obtain the state contract to do peer training, which is now integrated with community health worker training. We were the first to have a private telehealth network with the VA. We were the first to have a statewide health navigator network. Weve also been an education network forever.. Q: Are there any other types of organizations youre interested in recruiting for membership?. A: We really havent focused on "new" membership. What we focus on is "partnerships." There are organizations out there we would like to partner ...
Before the defense attorney and defendant left the courtroom, the resource coordinator or research associate gave each a sheet, available in English or Spanish, summarizing the offenders responsibilities. After receiving other paperwork from the court, offenders assigned to the two groups required to attend a batterer program proceeded to the defendant monitors office. One of the defendant monitors conducted a brief intake interview and offenders assigned to attend a batterer program (groups 1 and 2) were mandated to either the Domestic Violence Accountability Program (DVAP) run by Safe Horizon or the Mens Choices Program run by the Fordham Tremont Community Mental Health Center. The research associate also created a research case file including the date of the first scheduled appearance in DVC, case and offender identifiers, charge information, disposition, sentencing date, experimental assignment, victim name, and victim contact information. The offenders were tracked for at least twelve ...

Aims and method To compare the cost and quality of a memory-clinic-based service (MCS) with a traditional community mental health team (CMHT) service. Using a retrospective case-note review, we studied two groups, each with 33 participants. Consecutive referrals for diagnostic memory assessments over 4 months were evaluated. Participants were evaluated for up to 6 months.. Results The MCS was less costly than the CMHT service but the difference was not statistically significant (mean cost for MCS was £742, mean cost for CMHT service was £807). The MCS offered more multidisciplinary and comprehensive care, including: pre- and post-diagnostic counselling, more systematic screening of blood for reversible causes of dementia, more use of structured assessment instruments in patients/carers, signposting to the third sector as well as more consistent copying of letters to patients/carers.. Clinical implications An MCS service offered more comprehensive and multidisciplinary service at no extra ...

Prior to 1894, many physically ill patients were denied admission to the state hospitals as being too infirm to benefit from care and were kept in local almshouses. However, changes in Illinois laws required that they be treated by the state hospitals. As a result, in 1894, Wing Hall opened as a detached infirmary building, bringing the total hospital capacity to 1,107 beds.[19] In 1910, a 110-bed infirmary for female patients opened in an addition to the north end of the Center Building called "D-North" bringing the bed capacity to 1,210. These buildings were replaced in 1921 with the construction of a new general hospital located between the Central Building and the Annex.[20] In 1967, the general hospital was replaced by a new Medical and Surgical Building.[21] The new circular building was designed by Bertrand Goldberg, who also designed Marina City in Chicago.[22] The name Elgin State Hospital was adopted on January 1, 1910, shortly after the administration of all state charitable ...
