Origins of the Third Psychiatric Revolution: The Community Mental Health Centers Act of 1963 | Journal of Health Politics,...
In recent decades the community mental health movement has achieved a dramatic reduction in the census of state and county mental hospitals in the United States, and hundreds of federally-funded community mental health centers have been established nationwide. At the same time, national controversy has arisen in response to what in places has seemed the haphazard process of implementing deinstitutionalization and the fate of many chronically mentally ill persons who are without needed social services and psychological care. Despite the widespread attention that this contemporary program has received, theoretical analysis of the complex social, scientific, intellectual, and political origins of Americas community mental health policy remains deficient. This article examines the background and development of the Community Mental Health Centers Act of 1963, tracing how an important shift in national policy toward the mentally ill grew out of changing perceptions-among policymakers, professional ...

Federal Certification. The Centers for Medicare & Medicaid Services (CMS) has issued revised survey priorities due to very substantial federal resource limitations. In light of the federal Medicare resource constraints, initial surveys for most provider and supplier types have been classified at the lowest priority level. For facilities that qualify for deemed status there is an alternate route to obtain certification and this may well be the fastest way to obtain certification this year. However, there are other types of facilities for which deemed status is not an option. The Survey and Certification Letter S&C-08-03 can be accessed for additional information.. The following are the requirements and procedures through which your agency may be approved to participate in Medicare as a provider of Community Mental Health Center services. Healthcare Licensing and Surveys (HLS) certifies and periodically recertifies providers of services to assure Medicare Conditions of Participation are met. This ...

7. Defendant cites the Departments Declaratory Ruling in Howard Center for Human Services, Inc. v. Commerce and Industry Insurance Co. and AIG (undated opinion) as establishing a precedent against finding statutory employer status in situations similar to the current claim. In Howard Center, the Department concluded that a community mental health center was not the statutory employer of the developmental home providers with whom it arranged placements for developmentally disabled adults. Upon careful reading of the Commissioners Ruling, I find that the facts at play in the current claim differ significantly from those underlying the Howard Center ruling. To be sure, the community mental health center under scrutiny in that ruling took an interest in ensuring that the adults it placed were appropriately cared for at home. Beyond addressing basic safety issues, however, it did not dictate the home environment. Nor did it supervise the home providers directly or impose rigorous requirements as to ...

U.S. GAO - Department of Health and Human Services, Centers for Medicare & Medicaid Services: Medicare Program: Hospital...
RIN: 0938-AS82). (i) Cost-benefit analysis. The Centers for Medicare and Medicaid Services (CMS) estimate that the policies in this final rule with comment period will result in a 1.7 percent overall increase in outpatient prospective payment system (OPPS) payments to providers. CMS estimates that total OPPS payments for calendar year (CY) 2017, including beneficiary cost-sharing, to the approximate 3,906 facilities paid under OPPS (including general acute care hospitals, childrens hospitals, cancer hospitals, and community mental health centers (CMHCs)) will increase by approximately $773 million compared to CY 2016 payments, excluding CMSs estimated changes in enrollment, utilization, and case-mix. CMS estimated the isolated impact of its OPPS policies on CMHCs because CMHCs are only paid for partial hospitalization services under OPPS. Continuing the provider-specific structure that CMS adopted beginning in CY 2011 and basing payment fully on the type of provider furnishing the service, it ...
ERIC - Using Program Evaluation in Mental Health Centers., 1978
This guide provides an overview of practical program evaluation activities that can assist mental health centers in improving their administrative and program capabilities. Definitions of terms and assumptions are followed by an outline of the stages of the program evaluation cycle. Program evaluation activities, the roles of the administrator and evaluator, alternative ways to organize and deploy staff for program evaluation activities, and evaluator qualifications are presented. The importance of the administrators leadership skills in creating a positive climate for program evaluation is stressed. Program monitoring is suggested as one way to create a balance between the need to improve management procedures and the need to maintain or improve the quality of client care. Additionally, various uses of evaluation findings are discussed. (Author/NRB)

The Health Outcomes Management and Evaluation (HOME) Study - Full Text View - ClinicalTrials.gov
Findings of excess cardiometabolic morbidity and mortality in persons with severe mental illness (SMI) have led to a growing interest by Community Mental Health Centers (CMHCs) in improving the medical care of the populations they treat. However, these organizations face a number of financial and organizational barriers to implementing and sustaining such programs. In previous and ongoing work, the study team has documented the promise of team-based models in improving health and health care in this population. This study will test a novel approach for improving mental health consumers based on a partnership model between a CMHC and a Community Health Center (CHC). This partnership will capitalize on collocation of services, the primary care expertise of the CHC, and favorable reimbursement conditions, to develop a program that is both clinically robust and financially and organizationally sustainable A total of 300 CMHC clients with a severe mental illness and one or more active cardiometabolic ...

Description: The purpose of this study is to test the utility of Talcott Parsons AGIL Model, i.e., Adaptation, Goal Attainment, Integration, and Latency (Pattern Maintenance) in evaluating the program effectiveness of a community mental health center (CMHC). The model provided a conceptual framework for the selection of appropriate variables. The dependent variable in this study is the overall evaluation of the CMHC as measured through the perception of community leaders. Fourteen hypotheses were constructed to identify and test the relationship among the AGIL criteria and the use of a selected set of independent variables. Data for this study were collected from primary and secondary sources. Secondary data were obtained from the Texas Department of Mental Health and Mental Retardation in Austin and the CMHC center in Eton. Primary data were collected through personal interviews of general community leaders and influential persons in health-related activities in the community. The selected ...
Affordable Care Act - Teaching Health Center Graduate Medical Education (THCGME) Program | Government Grants | Grants Gov
This announcement solicits FY 2014 applications for the Teaching Health Center Graduate Medical Education (THCGME) program. The Affordable Care Act directly appropriates $230 million for Fiscal Years 2011 through 2015 to make payments to qualified teaching health centers to support the expansion of primary care medical and dental residency training in community-based ambulatory settings. These community-based settings include, but are not limited to, federally-qualified health centers (FQHCs) and FQHC Look-Alikes, community mental health centers, rural health clinics and health centers operated by the Indian Health Service or an Indian tribe or tribal organization, and entities receiving funds under title X of the Public Health Service (PHS) Act. This funding opportunity builds on evidence showing that family medicine resident physicians who train in Health Center (HC) settings are nearly three times as likely to practice in underserved settings after graduation when compared to residents who did not

Comparative effectiveness of cognitive and dynamic therapies for major depressive disorder in a community mental health setting...
There is substantial evidence that cognitive therapy is an effective intervention for the treatment of major depressive disorder. Although dynamic psychotherapies have been widely studied and are commonly practiced worldwide, there are few randomized comparisons of cognitive therapy and dynamic therapy for major depressive disorder. We are completing data collection on a randomized non-inferiority trial comparing the effectiveness of cognitive therapy and short-term dynamic psychotherapy in the treatment of major depressive disorder in the community mental health setting. Therapists employed in the community setting have been recruited for training in either short-term dynamic psychotherapy or cognitive therapy. Patients seeking services at the community site who meet criteria for major depressive disorder based on a blind independent diagnostic interview are randomized to 16 sessions of treatment. All patients are assessed at baseline and months 1, 2, 4, and 5 utilizing a comprehensive battery. This

A troubled beginning for Part D | Behavioral Healthcare Executive
Since Medicare Part D officially kicked in at the beginning of the year, the new program has given community mental health centers staff-and the people they serve-major headaches. Many behavioral health professionals remain skeptical that the federal government will make the regulatory and administrative changes needed to enable the more than 25 million participants to fully reap the intended benefit of the landmark program: access to prescriptions and reduced drug costs. The bottom line, say many behavioral health professionals, is that Part D has substantially increased their costs, taxed their resources, and frustrated their clients.. From its inception, Part D has been an utter hassle for mental health service providers, says Jim Van Norman, MD, medical director of the Austin Travis County MHMR Center in Texas. When the program first began, he notes, more than 6.2 million dually eligible Medicare and Medicaid recipients automatically were enrolled in Part D plans. Yet it was difficult to ...

Testing the Impact of Batterer Intervention Programs and Court Monitoring in the Bronx [New York City, New York], 2002-2004
Before the defense attorney and defendant left the courtroom, the resource coordinator or research associate gave each a sheet, available in English or Spanish, summarizing the offenders responsibilities. A memory clinic v. traditional community mental health team service: comparison of costs and quality | BJPsych Bulletin
Aims and method To compare the cost and quality of a memory-clinic-based service (MCS) with a traditional community mental health team (CMHT) service. Using a retrospective case-note review, we studied two groups, each with 33 participants. Consecutive referrals for diagnostic memory assessments over 4 months were evaluated. Participants were evaluated for up to 6 months.. Results The MCS was less costly than the CMHT service but the difference was not statistically significant (mean cost for MCS was £742, mean cost for CMHT service was £807). The MCS offered more multidisciplinary and comprehensive care, including: pre- and post-diagnostic counselling, more systematic screening of blood for reversible causes of dementia, more use of structured assessment instruments in patients/carers, signposting to the third sector as well as more consistent copying of letters to patients/carers.. Clinical implications An MCS service offered more comprehensive and multidisciplinary service at no extra ...

Elgin Mental Health Center - Wikipedia
Prior to 1894, many physically ill patients were denied admission to the state hospitals as being too infirm to benefit from care and were kept in local almshouses. However, changes in Illinois laws required that they be treated by the state hospitals. As a result, in 1894, Wing Hall opened as a detached infirmary building, bringing the total hospital capacity to 1,107 beds.[19] In 1910, a 110-bed infirmary for female patients opened in an addition to the north end of the Center Building called D-North bringing the bed capacity to 1,210. These buildings were replaced in 1921 with the construction of a new general hospital located between the Central Building and the Annex.[20] In 1967, the general hospital was replaced by a new Medical and Surgical Building.[21] The new circular building was designed by Bertrand Goldberg, who also designed Marina City in Chicago.[22] The name Elgin State Hospital was adopted on January 1, 1910, shortly after the administration of all state charitable ...

An economic evaluation of a specialist preventive care clinician in a community mental health service: a randomised controlled...
Clinical practice guidelines and policies direct community mental health services to provide preventive care to address chronic disease risks, however, such care is infrequently provided in routine consultations. An alternative model of care is to appoint a clinician to the dedicated role of offering and providing preventive care in an additional consultation: the specialist clinician model. Economic evaluations of models of care are needed to determine the cost of adhering to guidelines and policies, and to inform pragmatic service delivery decisions. This study is an economic evaluation of the specialist clinician model; designed to achieve policy concordant preventive care delivery. A retrospective analysis of the incremental costs, cost-effectiveness, and budget impact of a specialist preventive care clinician (an occupational therapist) was conducted in a randomised controlled trial, where participants were randomised to receive usual care; or usual care plus the offer of an additional

Measuring Continuity of Care for Clients of Public Mental Health Systems - Fortney - 2003 - Health Services Research - Wiley...
Objectives. The aims of this research were to generate a set of time-variant measures of continuity of outpatient care using administrative data, and to evaluate the validity of these measures for persons in the community with serious mental illness (SMI) who use public mental health services.. Data Sources. Individuals with SMI were identified using multistage random sampling from shelters, streets, and public mental health clinics in Houston, Texas.. Study Design. The study design was observational, cross-sectional, and retrospective. Based on a review of the literature, five distinct conceptual dimensions of continuity of care were defined: timeliness, intensity, comprehensiveness, stability, and coordination. Repeated measures of continuity were generated for each day of the year. Construct validity was assessed by comparing continuity for housed persons and homeless persons based on the assumption that homelessness is a risk factor for low continuity of outpatient care.. Data Collection. ...

Exercise, Physical Activity and Mental Health | Mood Disorders
Exercise and physical activity play a crucial role in both maintaining ones mental health condition and in recovering from a mental illness. Breaking research indicates that exercise actually produces a chemical that stimulates the growth of brain cells, thus allowing for recovery from sever substance abuse disorders. Furthermore, physical activity and mental health recovery coincide in fostering a social network and encouraging self-reflection, both of which are crucial on the path to mental health recovery.. The human mind evolved in an environment which required it to travel over twelve miles daily. And no, that drive to work in the morning does not count…but that would make things easier, no? This evolution was due to survival instincts when humans migrated from the jungles into the flatlands. Humans also developed an adrenaline reaction which both encouraged movement and triggered immediate learning reactions; as Doctor Carl Clark from the Mental Health Center of Denver once stated, when ...

Fifteen-month follow up of an assertive community treatment program for chronic patients with mental illness | BMC Health...
In this study, we compared the effect of an ACT program versus that of a case-management approach on the psychiatric symptoms, global functioning, life satisfaction, and recovery-promoting relationships of patients at the Suwon Mental Health Center.. We hypothesized that patients in the ACT group would exhibit greater psychiatric symptom improvement as shown by BPRS scores than the control group receiving case management. The ACT group showed significant reductions in psychiatric symptoms, but these reductions were not significantly greater than those seen in the control group between baseline and the 15-month follow-up. In a previous study, patients who underwent the ACT program showed no significant change in hospitalization period or in symptoms [8]. Another study revealed that the ACT program for disabled patients with mental illness could significantly improve psychiatric symptoms [22]. An ACT program in Japan showed improvement of depressive symptoms and reduction of hospitalization period ...

Nail Biters, Beware: Teeth Grinding Is Next - Healthcanal.com : Healthcanal.com
Anxiety disorders affect approximately one in six adult Americans, according to the National Institute of Mental Health. The most well-known of these include panic disorder, post-traumatic stress disorder, obsessive-compulsive disorder, and social anxiety disorder. But what of brief bouts of anxiety caused by stressful social situations?. A new study by Tel Aviv University researchers, published recently in the Journal of Oral Rehabilitation, finds that anxiety experienced in social circumstances elevates the risk of bruxism - teeth grinding which causes tooth wear and fractures as well as jaw pain. According to the research, led by Dr. Ephraim Winocur of the Department of Oral Rehabilitation at TAUs School of Dental Medicine and conducted by TAU doctoral student Roi Skopski in collaboration with researchers at Geha Mental Health Center in Petah Tikva, Israel, interaction with people is likely to trigger bruxism in the socially anxious.. Similarly, a former college athlete who becomes angry and irritable in his 40s could be suffering a midlife crisis-or delayed reaction to head injuries sustained decades earlier. We now know that multiple concussions can have a sleeper effect for years. Then one day, out of the blue, you start acting explosive and depressed, due to a brain swelling known as chronic trauma encephalopathy, said Jerrold Pollak, a neuropsychologist at Seacoast Mental Health Center in Portsmouth, N.H., and lead author of an article on distinguishing mental from medical disorders in the Journal of Clinical Psychology Practice this spring ...

One hundred eighty drug-using individuals seeking treatment at the Substance Abuse Treatment Unit (SATU) of the Connecticut Mental Health Center will be randomized to (1) standard outpatient counseling at SATU (typically consisting of weekly group counseling), (2) individual clinician delivered CBT, or (3) web-based CBT4CBT. Treatments will be delivered over a 12-week period with a six-month follow-up after termination of the study treatments. The primary outcome measures will be reduction in substance use (frequency of drug use by time, confirmed by urine toxicology screens). Secondary outcomes will include treatment utilization and cost, several measures intended to detect whether web-based CBT4CBT retains key characteristics of traditional clinician-administered CBT (e.g., acquisition of coping skills, use of change strategies), participant characteristics which will be evaluated as potential moderators of outcome, as well as participant satisfaction and treatment credibility ...
