Although the community health service system is now established in China, the utilisation of the community health service institutions is low due to the lack of a gate-keeping role of the primary health service providers and referrals among the three-tiered health service institutions. In addition to this, patients who can afford to pay, often seek best services in big hospitals to guarantee the quality of care. Thus, the need of guiding the patients to the community health services and increasing the utilisation of the community health service institutions is becoming an urgent problem, which hinders the future development of community health services. This study focuses on the question of how to increase the utilisation of Chinese community health centres (HCs). A cross-sectional Base-line Survey of Chinese City Community Health Service System Building using the multi-staged cluster sampling was conducted to collect data from all HCs in 28 key contact cities. Relevant indicators of totally 1790 HCs

Timely and equitable access to treatments is a high priority both to those using, and to those delivering, healthcare services [1]. In childrens services both the benefits of early intervention and the possible harms associated with delayed intervention have been widely documented [2-5]. Parents dissatisfaction with services has been found to be triggered by lack of access to and availability of care rather than clinical outcomes [6]. Yet long waiting times and large caseloads are a challenge to childrens therapy services internationally [7-11].. To date, there have been few attempts to systematically study the determinants of waiting times in childrens community health services [12], or community health services more broadly [13-16]. Research in hospital-based healthcare indicates that waiting times are a function of throughput [17] (i.e. flow of patients from the start to the end of a care episode), and that throughput is influenced by a range of factors at different levels of care ...

Fianna Fáil Senator Dr. Keith Swanick has called on the Health Minister to seriously consider using the existing district hospital network to alleviate the pressure being experienced in our acute hospitals. Senator Swanick was speaking at the Institute of Community Health Nurses conference earlier today.. The Mayo based GP explained, The district hospital network is a useful asset in our health service that is not being effectively utilised. We need to maximise the services provided in these hospitals so that patients have an alternative to already overcrowded Emergency Departments and acute hospitals. District hospitals can be used in the treatment of minor medical conditions thereby alleviating the pressure on bigger units.. Almost every county has a district hospital making it an almost nationwide resource. These hospitals could be used for step down care, could accommodate certain outpatient clinics or be used as an intermediate care setting for older people who have been discharged from ...

Community health services in China have developed over the last few decades. In order to use limited health resources more effectively, we conducted a community health needs assessment. This aimed to provide an understanding of the communitys health problems and the range of potential factors affecting risk behaviours for the priority health problems. We used the precede-proceed model for the needs assessment. Triangulation of data, methods and researchers were employed in data collection. Main findings include: cardiovascular diseases (CVDs) were identified as the priority health problems in the study communities; risk factors associated with CVDs included smoking, physical inactivity and unhealthy eating behaviours, particularly amongst male residents with low education level; factors negatively affecting behaviours were classified into predisposing factors (limited knowledge, beliefs and lack of perceived needs), enabling factors (limited access to health promotion activities, unawareness of health

Prevention of type 2 diabetes mellitus (T2DM) is a global priority; however, there is a lack of evidence investigating how to effectively translate prevention research into a primary care setting.(1) To develop and validate a risk score to identify individuals at high risk of T2DM in the UK; and (2) to establish whether or not a structured education programme targeting lifestyle and behaviour change was clinically effective and cost-effective at preventing progression to T2DM in people with prediabetes mellitus (PDM), identified through a risk score screening programme in primary care.A targeted screening study followed by a cluster randomised controlled trial (RCT), with randomisation at practice level. We estimated the prevalence of AD among children age 0 to 5 attending community-based primary care practices to be approximately 24%, with a mean age of AD onset in the first year of life. Parents reported that AD severity was mild in more than half of participating children, and 20% of those with AD had their sleep disturbed at least once a week as a result of their AD. As anticipated, a higher prevalence of AD-associated comorbidities and a family history of atopic conditions were found among those with AD. The majority of parents were using some kind of moisturizer on their childs skin on a regular basis; children with AD were more likely to receive creamy and oily moisturizers, whereas children without AD were receiving lotions primarily. This large community-based study is the first study to describe the pediatric AD burden within community-based primary care practices and provides important insight into skin care practices that may be modifiable in future disease prevention ...
On an individual as well as an institutional basis, the health of young men of color has been neglected. Unfortunately, this neglect results in limited research, and therefore an incomplete understanding of effective strategies, optimal prevention and intervention programs, best practices, and estimations of models efficacious for promoting and providing health services for minority men. However, a promising area for the reversal of this trend is the furthering of community health strategies. Community health strategies are defined as promoting community involvement in and ownership of health problems and solutions to improve and strengthen the well-being of community members and their quality of community life. Because young men of color are vital members of the community, the improvement of life options for them will heavily rely on community health strategies. For example, community-based health promotion can facilitate outreach and access to publicly supported health care programs and access ...

In the same district in Malawi which I referred to earlier, I came across situations of mistrust between volunteers, health surveillance assistants and supervisors. This mistrust was a result of perceptions of dishonesty towards the upper level about financial incentives that were expected to come, but were felt to be not distributed properly. This situation was caused by multiple programmes that worked separately from each other, each with their own incentive structure. It shows that programme design, in this case the way incentives are handled, can - quite easily - knock down existing, but fragile, trusting relationships. Community health workers can end up being caught between different forces.. Too often, in the past and in the present, community health workers have to juggle between the health sector and the community. They are in a continuous balancing act to meet the interests of their surroundings. They work in a complex environment, where power relations and societal values and ...
