Prosthetic reinforcement in the surgical repair of pelvic prolapse by the vaginal approach is not devoid of tolerability-related problems such as vaginal erosion. The purposes of our study are to define the risk factors for exposure of the mesh material, to describe advances and to recommend a therapeutic strategy. Two hundred and seventy-seven patients undergoing surgery due to pelvic prolapse with transvaginal mesh technique were included in a continuous, retrospective study between January 2002 and December 2003. Thirty-four cases of mesh exposure were observed within the 2 months following surgery, which represents an incidence of 12.27%. All the patients were medically treated, nine of whom were found to have completely healed during the check-up performed at 2 months. In contrast, 25 patients required partial mesh exeresis. Risk factors of erosion were concomitant hysterectomy [OR=5.17 (p=10−3)] and inverted T colpotomy [OR=6.06 (p=10−2)]. Two technical guidelines can be defined from this

Presumably the patient was offered low-dosage testosterone replacement as an optional adjunct to estrogen replacement. The idea seems to be that itll help with libido, although I dont think this really replicates normal female endocrinology. Dosage should therefore be titrated to keep levels as close as possible to pre-surgical levels. Im guessing she was doing topical estrogen replacement to minimize side effects, so the provider thought, prescribe Androgel or whatever and the patient doesnt have to come in for injections. Still, dosage errors in topical testosterone replacement that are at the margin for male levels can be a pretty big delta for females. Plus, unlike shots, pellets, etc, the topicals purportedly deliver testosterone (or as much as will get through the skin) in a couple of hours. Moreover, the lead warning on topical testosterone proparations are all about females and children coming into inadvertent contact (intimate contact, laundry, etc.) and the importance of ...

Repair of a vaginal vault prolapse is done to manage symptoms such as sagging or drooping of the top of the vagina into the vaginal canal, urinary incontinence, and painful intercourse.. Vaginal vault prolapse often occurs with other pelvic organ prolapse. So tell your doctor about other symptoms you may be having. If your doctor finds prolapse of other pelvic organs during your pelvic exam, that problem may also be repaired during surgery.. Your doctor may do an examination while you have a pessary in your vagina. This exam will help him or her see if urinary incontinence would be a problem after surgery. If the exam shows that urinary incontinence will be a problem, another surgery can be done at the same time to fix the problem.. ...
Objective. The current literature suggests that robotic procedures appear to incur the greatest risk of vaginal cuff dehiscence. The purpose of this study is to compare the clinical outcomes in relation to vaginal cuff dehiscence in patients undergoing total robotic hysterectomy (TRH) compared to conventional total laparoscopic hysterectomy (TLH) and the use of bidirectional barbed suture. Methods. A retrospective cohort study was performed including women undergoing either conventional total robotic hysterectomy (TRH) or total laparoscopic hysterectomy (TLH) from January 1, 2004 to July 31, 2011 performed by a single surgeon. Age, weight, BMI, surgical procedure, EBL, vaginal cuff complication, dehiscence, DVT, and PE were evaluated. Chi square tests were used to determine associations in the contingency tables while for the continuous variables we used Student t test when appropriate or the Wilcoxon nonparametric test. All comparisons were two-sided where we used a Type I error of 0.05. Results. A
Warts are common dermatologicals disorder which are difficult to treat. In the present study the efficacy of topical 5% 5-fluorouracil was evaluated in various types of warts and compared with the conventional electrosurgical method which included both electrodessication and electrofulguration. 50 patients clinically diagnosed as warts were enrolled. 25 patients were treated with topical 5% 5-fluorouracil applied for 4 hrs twice daily for 3 weeks, while another 25 were treated with electrosurgery. It was observed that electrosurgery yielded better initial results than 5-fluorouracil (72 and 52% respectively), but at the end of 6 months, the results were favourable for 5-fluorouracil as compared to electrosurgery (44 and 36% respectively). This was due to the signficantly low recurrence rate with 5-fluorouracil in contrast to electrosurgery (8 and 48% respectively). Though electrosurgery yielded quicker initial results but 5-fluorouracil was better as long term measurer because of its ...

Some of the major indications for electrosurgery are described and illustrated in the following sections.. Electrosurgery to Facilitate Placement of Direct Composite Materials. Tissue management during direct restorative procedures is an area where dentists can benefit greatly from electrosurgery. During cavity preparation, patients are almost always anesthetized. Nonetheless, delivery of a small amount of anesthesia into the soft tissue is recommended when a soft-tissue electrosurgical procedure is planned. For example, when interproximal electrosurgery is planned prior to matrix placement around a maxillary tooth, the interproximal tissues still might not be sufficiently numb from a vestibular infiltration. One solution is to administer 2% lidocaine with 1:50,000 epinephrine circumcoronally around the tooth that is being restored. Intrapapillary and marginal injections will blanch the tissue and gain soft-tissue anesthesia circumcoronally, preparing the patient for a number of ...

Electrosurgery is the application of a high-frequency (radio frequency) alternating polarity, electrical current to biological tissue as a means to cut, coagulate, desiccate, or fulgurate tissue. (These terms are used in specific ways for this methodology-see below). Its benefits include the ability to make precise cuts with limited blood loss. Electrosurgical devices are frequently used during surgical operations helping to prevent blood loss in hospital operating rooms or in outpatient procedures. In electrosurgical procedures, the tissue is heated by an electric current. Although electrical devices that create a heated probe may be used for the cauterization of tissue in some applications, electrosurgery is usually used to refer to a quite different method than electrocautery. Electrocautery uses heat conduction from a probe heated to a glowing temperature by a direct current (much in the manner of a soldering iron). This may be accomplished by direct current from dry-cells in a penlight-type ...

Fact: The pelvic organs and tissues are linked by connective, supportive tissues that attach them to the pelvic bone structure. When a hysterectomy is performed and the uterus is removed, this structure becomes vulnerable. The tissues supporting the other pelvic organs - such as the bladder, cervix or rectum - can weaken causing these organs to collapse into the vagina. Women who have had a hysterectomy are prone to a specific type of prolapse known as vaginal vault prolapse, when the top portion of the vagina (vaginal vault) collapses into itself. In severe cases the vagina can even start to turn itself inside out. Exercising the PF can help strengthen these support structures.. Livia Nemeth. ...

Im back, 12 days after my total hysterectomy. After a few early challenges in hospital, Ive been doing really well, except that Im now having increasing vaginal bleeding. 3 days ago, I was experiencing sharp pain (havent had any to speak of so far), so called my Dr and went for exam and ultrasound. They found a small infection starting, so Im on day 3 of heavy antibiotics. I mentioned some light bleeding, and the Dr. said some was normal as the sutures in the vaginal cuff dissolve.

Repeated re-processing has a detrimental effect on diathermy instruments: the coating breaks down and pinholes or nicks appear. A leakage of current can occur, which could cause a burn to the patient or surgeon. Some electrosurgery manufacturers replace the damaged instrument with another of unproven provenance raising issues about accountability and operating history. Repaired diathermy forceps and cables do not require CE marking under the Medical Devices Directive 93/42/EEC.. To address these problems, Surgical Holdings has introduced a new system which offers these advantages:. ...
In an electrosurgery apparatus for treating a living tissue, while the living tissue is being treated, a variation in state of energy supplied from a cautery power supply to a monopolar treatment device is detected on the basis of detection data obtained from a current sensor and a voltage sensor. Based on the detected variation, living body information on the living tissue to be treated is obtained.
