Title:Hypereosinophilic Syndrome, Churg-Strauss Syndrome and Parasitic Diseases: Possible Links between Eosinophilia and Thrombosis. VOLUME: 10 ISSUE: 5. Author(s):Alberto Maino, Raffaella Rossio, Massimo Cugno, Angelo V. Marzano and Alberto Tedeschi. Affiliation:Allergy and Clinical Immunology Unit, Fondazione IRCCS Ca Granda, Ospedale Maggiore Policlinico, Via Pace, 9 - 20122 Milan, Italy.. Keywords:Eosinophil, hypereosinophilic syndrome, Churg-Strauss syndrome, parasitic infestations, thrombosis, platelet activating factor, myocardial infarction, stroke, vasculitis, heparin. Abstract:Throughout the past decade, a possible role of eosinophils in blood coagulation and thrombosis has been suggested. We conducted a Pubmed (MEDLINE) search of case and series referring to any kind of thrombotic events described in three conditions characterised by persistent blood eosinophilia, i.e. the hypereosinophilic syndrome (HES), the Churg Strauss syndrome (CSS), and parasitic infestations from 1966 to ...
Objective: Hypereosinophilic syndromes are a heterogeneous group of uncommon disorders characterized by the presence of marked peripheral blood eosinophilia, tissue eosinophilia, or both, resulting in a wide variety of clinical manifestations, often without an identifiable cause. Churg-Strauss syndrome is a systemic vasculitis characterized by prominent peripheral eosinophilia, asthma and systemic involvement. The presence of mild to severe eosinophilia and systemic involvement raise the search of many trigger factor that need to be ruled out. Distinguishing CSS from idiopathic hypereosinophilic syndrome may be particularly challenging, especially in ANCA negative patients. Methods: The aim of the present study was to present a small case series of patients referred to a Rheumatology Unit for mild to severe eosinophilia and signs and symptoms of systemic involvement and to outline the clinical significance of molecular biology in the work-up of hypereosinophilia. Results: Eleven patients with ...

part of that is self education. IF you read only one thing about your ANCA related Vasculitis all year… then THIS should be it. Seriously, not kidding.. First off Granulomatosis with polyangiitis (GPA, Wegeners granulomatosis), microscopic polyangiitis (MPA) and eosinophilic granulomatosis with polyangiitis (EGPA, Churg-Strauss syndrome) are termed the antineutrophil cytoplasmic antibody (ANCA)- associated vasculitides. Even IF you are ANCA Negative (50% of CSS/EGPA patients you are still considered to have an ANCA Associated Vasculitis.. On June 23rd, 2016 a new document was published that updated the guidelines for treatment of ANCA Associated Vasculitis. This was the first major revision of treatment guidelines since 2009 and in many ways it is a game changer because of the newer biological medicines. It was created by European League Against Rheumatism (EULAR). Which includes doctors, nurses, researchers and even a patient.. Fifteen recommendations were developed, covering general ...

Introduction. Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic vasculitis that is commonly associated with cutaneous involvement (40%-81%).1-3. Case Report. The patient was a 53-year-old man with a history of rhinitis, asthma and eosinophilic pneumonia, diagnosed 4 years earlier, who presented with pruritic lesions on the scalp that had developed 7 days before. There were 4 circular plaques measuring 2-4cm, that were skin-colored or erythematous, with an infiltrated border and small scabs (Fig. 1A and C). Biopsy revealed a superficial inflammatory infiltrate made up of lymphocytes and eosinophils; there were neutrophils in the vessel wall, with no fibrinoid necrosis; we also detected histiocytes interspersed with degenerated collagen in the superficial and reticular dermis (Fig. 1B). Direct immunofluorescence and Ziehl-Neelsen staining were negative. We found leukocytosis with 15,900cells/μL; eosinophilia (24.7%; 3900cells/μL); and complement C3 of 83.7mg/dL. Antinuclear ...

Putting a name to what I have been dealing with for the past 17 years certainly made me feel a little less like I had imagined all of the bizarre issues. It seemed to justify the times that I moved with my job from one state to another; each time enlisting a new Dr. and trying to explain to them why I needed a steroid shot to stay healthy. Six months in to this Churg-Strauss (also known as EPGA and CSS) diagnosis and each new little diagnosis that pops up under the Churg-Strauss Syndrome umbrella, I dont feel so relieved anymore at knowing what it is. I want it to go away. I want to turn back time and pretend that I could have done something differently to have prevented it choosing my body for its home.. Likely what has happened is that I have moved into a different phase of the disease. I had a lull for a few years where I was able to get a random steroid shot occasionally and things were low key and I as fairly healthy. In my mind I had just gotten over the allergies.. In reality, the ...

Visual deterioration occurs in two peaks. The first peak manifests as progression of the ongoing flare on an unchanged steroid dose, typically during the first 6 days.25 The second peak occurs after weeks or months of tapering treatment. Relapses increase with reduction of corticosteroid therapy and were seen in 19% of patients within 1 year.26. Reasons for progression of visual loss despite treatment may include hypoperfusion of the optic disc, treatment delay, inadequate steroid dose, quick taper or hypercoagulability with retinal artery infarction, possibly due to steroid therapy. Continuation of arteritis despite adequate corticosteroid dose may be considered part of the spectrum of TA or may even be a separate disease entity.. Differential diagnoses mimicking TA include systemic lupus erythematodes, Sjögrens syndrome, rheumatoid arthritits, Behcets disease, antiphospholipid antibody syndrome, polyarteritis nodosa, Churg-Strauss syndrome, Wegeners granulomatosis and other rheumatic ...

Heart disease responsive to steroids is well described in many disorders, including sarcoidosis, systemic lupus erythematosus, polyarteritis nodosa, myocarditis and Churg-Strauss syndrome. The underlying disorder is often obvious and the response is usually slow. We describe a woman who had severe left ventricular failure, cardiac dilatation and pericardial effusion which were rapidly rectified by steroid therapy. Steroid withdrawal led to recurrence of signs, which were reversed by recommencing steroids. The aetiology was not determined.

Eosinophils are granulocytes that contain basic granules (i.e. secretory vesicles) that kill large parasites and are linked to various forms of allergies. Eosinophilic pneumonia (EP) is a broadly defined disease that is characterized by an infiltration of eosinophils in lung alveolar tissue. EP includes Churg-Strauss Syndrome (a rare autoimmune disease), chronic EP and acute EP (the difference between the two is the presence of eosinophils in the blood/tissues and only the tissue, respectfully). Individuals with EP are often found to have an increased concentration of macrophages and dendritic cells, important innate immune response mediators. Symptoms include shortness of breath, weight loss, fever, and even respiratory failure, while causes range from parasitic infection, immune system dysfunction, medication, and environmental stimuli like tobacco smoke and dust. Although symptoms can be serious, few is known about the cellular mechanisms behind EP, specifically macrophage and DC recruitment ...

Results A 14-year-old male previously misdiagnosed as cutaneous anthrax was referred with a 2-month history of multiple wide and deep ulceronecrotic lesions in lower extremities, started after contact with animals (1a). Complete blood count (CBC) revealed anemia (hemoglobin: 8.5 g/dl, hematocrit: 26.9%), eosinophilia (4100/mm3) and thrombocytopenia (34000/mm3). Peripheral blood smear also revealed marked eosinophilia (42%). Infectious etiologies such as cutaneous anthrax, leishmaniosis, aspergillosis and nonspecific agents were excluded by gram staining, wound-blood-bone marrow cultures and serological investigations. Bone marrow aspiration was performed owing to the prominent eosinophilia, anemia and thrombocytopenia. However, the result was also pointing out eosinophilia without signs of eosinophilic leukemia. Although ANCAs were negative, the robust clinical manifestations concordant with EGPA (e.g. eosinophilia, increased Ig E level, history of asthma, vasculitic lesions) had led us to ...

On that tragic December day when the Taylors lost their mother, a 35-year-old husband and father of two, Mike Strauss, was preparing for his final Christmas. Strauss had Primary Sclerosing Cholangitis, a long-term progressive disease of the liver and gallbladder. Years earlier Strauss had refused a liver transplant because he was feeling so healthy and strong, he wanted the liver to go to someone who needed it more than he did. Fast forward to 2001 and Strauss was dying, his doctor predicting he only had a few weeks left. Bonnie Taylors liver saved Mike Strausss life.. Intermountain Donor Services (IDS) is an organization that puts the families of organ donors and living donors in contact with transplant recipients. Kami and Zach Taylor got a letter from the woman who received their mothers kidney, but it was soon after their mothers and neither Kami or Zach felt ready to connect with her. In 2003, Mike Strauss sent a letter to IDS and they forwarded it on to Taylors. This time, both Kami ...

Strausss introduction of the hidden culture in Seaweed is marvelously mysterious and definitely interesting on many levels. The main story background is brilliantly developed and pops to life on the pages, sparkling like the shining waters of the rich and vast deep blue sea. The characters are fun to get to know on a more personal level, as readers delve into each of their stories and bring them to life across the pages. Some are deliciously likable and easy to connect with, while others may have a tendency to fall a bit flat at times. Read struck six boundaries, mainly through the off-side, but became the first of two quick wickets when he drove at a wide delivery from seamer Gayan Wijekoon to provide a catch behind. Rikki Clarke soon followed when he pushed forward and was bowled through the gate by left-arm spinner Dinuk Hettiarachchi.. Strauss kept the scoring rate healthy alongside Anthony McGrath, leading the side in Vaughans absence, and another Yorkshireman Gareth Batty.. Batty hit two sixes off seamer Akalanka Ganegama in one over but was caught on the midwicket boundary going for a third and Strauss followed in the penultimate over.. Englands attack struggled to contain the Sri Lankan top order, however, as Michael Vandort, a two-cap Test player, and Saman Jayantha provided a rapid response to their chase.. Jayantha began the flurry of boundaries with a pulled six off Richard Johnson and Vandort followed that with two fours off Anderson in the next over, the fifth of the innings.. They kept up the tempo until a ...

Necrotizing vasculitis also called Systemic necrotizing vasculitus (SNV)[1] is a category of vasculitis,[2] comprising vasculitides that present with necrosis.[3] Examples include giant cell arteritis,[4] microscopic polyangiitis, and granulomatosis with polyangiitis. ICD-10 uses the variant necrotizing vasculopathy. ICD-9, while classifying these conditions together, doesnt use a dedicated phrase, instead calling them polyarteritis nodosa and allied conditions. When using the influential classification known as the Chapel Hill Consensus Conference, the terms systemic vasculitis or primary systemic vasculitides are commonly used.[5][6][7] Although the word necrotizing is omitted, the conditions described are largely the same. ...

The primary systemic vasculitides are a group of autoimmune conditions characterised by occlusion, stenosis or aneurysmal dilatation of blood vessels secondary to intra-mural inflammation. Current therapy has converted the outlook of these diseases from death or severe morbidity to a remitting-relapsing condition in most instances. Longer survival, relapsing course of disease and chronic glucocorticoid therapy probably contribute to an increase in cardiovascular events and morbidity. This article reviews the available data for effect of primary systemic vasculitis on cardiovascular end points like coronary artery disease, congestive cardiac failure, hypertension and aortic aneurysm in all age groups. We examine the interplay between the activated endothelium, autoimmune mechanisms and treatment factors to produce a direct insult or increased atherogenic potential of primary systemic vasculitis. Recommendations to deal with cardiovascular end points are made.. ...

To the editor: Drs. White and Schambelan (1) reported in the February 1980 issue two cases of hypertension with hyperreninemia complicating systemic necrotizing vasculitis. In one case, hypertension was easily controlled with captopril during the time that evidence of healing of renal vasculitis was obtained, likely due to cyclophosphamide and prednisone. Therefore, the authors advocated the use of captopril as an effective antihypertensive agent in such patients. Because hypertension was present, however, in the initial stages of the disease, some degree of hypertension control related to improvement in the underlying disease process cannot be fully ruled out. Indeed, Fauci and ...
No current diagnostic criteria exist for vasculitis. The Modified ACR Classification Criteria for vasculitides were published in 1990, and do not reflect current diagnostic tests or disease definitions, he said. Classification criteria are meant to identify already-diagnosed patients for clinical trials, not for clinical diagnosis. New diagnostic and classification criteria in vasculitis are now being developed by investigators who presented an early draft of their GPA criteria at the 2016 ACR/ARHP Annual Meeting. The 2012 Chapel Hill Consensus Conference definitions of vasculitides provide one way to distinguish these diseases from others.2. Diagnosis is based on clinical features, biopsy and testing for ANCA antibodies.. Just because the ANCAs are negative, dont rule out these types of vasculitides, said Dr. Springer.. ...

Jonathan Bask published a fascinating article in AdAge discussing the dangers of the social web for real businesses. While the article meanders to become the latest in a flurry of ills of social media articles, the real takeaway was social media doesnt replace business skills. At the behest of the queen of successful blogging Liz Strauss, this write-up discusses core business skills - specifically those of product marketing - and how they cannot be replaced with social media savoir faire. Somewhere along the way, social media folks thought they should completely reinvent business. That they knew better, and that could they completely disregard historys many lessons about how to build great products that work. Online idea markets have inherent weaknesses to them, including making bad ideas popular (personal branding, anyone?).. Before leaving the house, comedian Adam Strauss would routinely spend hours deciding which shirt to wear. One seemed too nice, one not nice enough, and another didnt fit quite right. First, Strauss tried telling stories about his experiments at comedy clubs, quickly realizing they ...

By Steve Neavling. A New York pharmacist is accused of sending powdered cocaine in a greeting card to a DEA agent after discovering his business was under investigation, the Justice Department said. The idea was to disrupt the investigation, but the plan failed. Dimitrios Lymberatos, 34, is charged with obstruction of justice and conspiracy to commit obstruction of justice for seeking to interfere with the investigation through intimidation, acting U.S. Attorney Audrey Strauss of the Southern District of New York said in a statement. Lymberatoss misguided message was received loud and clear - and he now faces the possibility of a lengthy prison term for his potentially harmful attempt to obstruct law enforcement, Strauss said.
