The impact of neonatal morbidity and admission for neonatal care, on the longer term outcomes of LPls has not been fully explored. This thesis has sought to bridge an identified gap in the literature relating to this significant group of neonatal care graduates. The overall aim of this thesis was to consider the significant population of LPls who require admission for neonatal care. Firstly, maternal and perinatal risk factors and neonatal outcomes were considered through a descriptive analysis of neonatal data from the Neonatal Intensive Care Outcomes Research and Evaluation (NICORE) database. Delayed child development is a cumulative process that starts as early as in the womb and, once begun, may be difficult (or very costly) to reverse during school years and adulthood. This explains why early childhood development (ECD) has been recognized as having high returns over the life cycle in both developing and developed countries. Yet, very little is known with confidence about the economic justification for ECD programs in general and in developing countries in particular. The economic justification must depend on the comparison of benefits and costs of each ECD program. In this chapter, we review what is known and, perhaps more importantly, what is not known about components of economic analysis of ECD programs.

This finding is significant because early birth due to planned birth now accounts for almost half of births before 39 to 40 weeks gestation. The decision for planned birth is predicated on beliefs that it is safer for the baby or mother to deliver early. It is of paramount importance to ensure that there are no unintended harms from such a significant shift in clinical practice. Although increased risks of poor health in childhood are associated with early planned birth, early childhood development is another important aspect of child well-being that predicts adult social, educational, and health outcomes. Objective: In this pilot study, we described the gross motor development of infants aged 4 to 24 months with congenital heart disease (CHD) and assessed through a systematic develop­mental screening programme, with individualised motor interventions. Methods: Thirty infants who had cardiac repair underwent gross motor evaluations using the AIMS at 4 months, and the Bayley-III at 12 and 24 months. Results: Based on AIMS, 80% of 4-month-old infants had a delay in gross motor development and required physical therapy. Gross motor abilities significantly improved by 24 months. Infants who benefited from regular physiotherapy tended to show better improvement in motor scores. Conclusion: Our study highlights the importance of early motor screening in infants with CHD and suggests a potential benefit of early physical therapy in those at-risk. Further research is needed to assess the effectiveness of systematic developmental screening and individualized inter­vention programmes at identifying at risk

Downloadable! In this paper we explore the impact of birth weight on childrens cognitive and behavioural outcomes using data from the UK Millennium Cohort Study. In order to deal with the endogeneity of birth weight we use an estimator based on the eliminant method. When coupled with ordinary least squares, this estimator allows us to bound the effects of birth weight. The results show that birth weight has significant but very small effects on male cognitive development at age 3 and on female cognitive and behavioural outcomes at age 3. We also find that birth weight affects age 5 outcomes only through previous achievements, and that the overall impact fades out over time. These findings call into question the effectiveness of birth weight as a policy target.

Abstract: Childrens optimal development is hampered by risks in the childs environment that get exacerbated by poor economic and social systems like the ones in Bosnia and Herzegovina (B&H). Good systems detect risk factors and atypical development, and support the individual needs of families and young children through parent education and direct early intervention in the first few years of the childs life. Such systems rely on science, data, and use validated measuring tools. With the support of USAID and in partnership with UNICEF and the relevant ministries in B&H, EDUS worked on developing such systems, which can provide evidence-based early detection and intervention based on the science of behavior. Few data from randomised prospective studies address whether early diet influences later neurodevelopment in man. As part of a larger multicentre trial, 502 low birthweight infants were assigned randomly, for a median of 30 days, to receive a preterm formula or unfortified donor breast milk as sole diets or as supplements to their mothers expressed milk. Surviving infants were assessed at nine months after their expected date of delivery without knowledge of their feeding regimen. The mean developmental quotient was 0.25 standard deviations lower in those fed donor breast milk rather than preterm formula. In infants fed their mothers expressed milk, however, the disadvantage of receiving banked milk compared with preterm formula as a supplement, was greater when the supplement was over half the total intake, and approached five points, representing 0.5 standard deviations for developmental quotient.
