Results: Of the respondents, 25% (7 persons) and 75% (21 persons) were men and women respectively. The majority of the respondents, i.e. 60% (17 persons), had a bachelors degree. Moreover, 21.6% of the sample had more than 5 years of professional experience. The items in the improved surgical safety checklist were as follows: including a space for the signature of the scrub nurse and personnel during the shift change and mentioning instruments used during an operation as well as different phases of surgery one by one. In general, the personnels perspectives on the improved checklist, compared with the present checklist, were statistically significantly different (p,0.05). That is to say the improved checklist is a more comprehensive guide than the existing checklist as for keeping a checklist of instruments applied and procedures adopted. Furthermore, it is crucial to mention names of instruments on the checklist completely and individually. ...

Background: A considerable proportion of children and adolescents attending mental health services suffer from severe affective and behavioral dysregulation. The Dysregulation Profile, which was identified based on certain subscales of the Child Behavior Checklist and later, also in certain subscales of the Strengths and Difficulties Questionnaire, has been defined as a marker of a broader form of severe psychopathology. DP has been described as a theoretical construct and research data have been provided to consolidate its operational description. However, its stability, which is one of the fundamental aspects for operational definition of a construct, remains unexplored ...
The Pediatric Symptom Checklist is a brief screening questionnaire used by pediatricians and other health professionals to recognize psychosocial problems and improve treatment in children.
Results: Modified checklists contained more total items as compared to the original, averaging 33.5 ±8.4 items versus 28 in the original. The most commonly added items were procedural, not teamwork-based. 50% of all checklists removed two or more of the original seven teamwork items. Surgeon-led items were more likely to be removed. The average number of teamwork items on US checklists was significantly lower than on non-US checklists (3.98 vs. 5.64, p,0.05). US checklists were also more likely to eliminate all surgeon-led items (21.4% of US vs. 12.7% of all checklists ...

Considering the benefits of checklists in other specialties [9,10], an endoscopy safety checklist (appendix 1) was implemented across the trust in 2012 for all GI endoscopic procedures.[11] Having implemented the checklist across our entire unit, we sought to evaluate compliance with the checklist to identify problems and optimise its effectiveness.. Many lessons were learnt through this process. Importantly, implementation of a checklist needs a detailed long-term plan with consideration of likely obstacles to ensure sustainability.. PDSA cycle 1: Checklist compliance was prospectively evaluated for consecutive patients undergoing an endoscopic procedure during a seven-day period. This was initially completed in February 2013. The medical record was assessed for the following parameters:. - Section of checklist completed (Time out / Sign out / Both). - Procedure time (AM or PM). - Grade and gender of the endoscopist. - Admission type: elective or inpatient.. PDSA cycle 2: Following this initial ...

The survey on staff satisfaction in relation to the use of SCLs was completed by 48 employees (54% of the staff), 10 doctors and 38 nurses, a difference that was not statistically significant (P=.501). When it came to the usefulness of SCLs, 75% (36/48) considered that SCLs were useful or very useful, 14.58% (7/48) felt indifferent, and 10.42% (5/48) considered them of little use or useless. Of all respondents, 68.75% (33/48) believed that SCLs had succeeded in preventing some AEs. In addition, 87.5% (42/48) of respondents did not feel that SCLs added significantly to their work burden. As for the level of overall satisfaction with SCLs, 54.16% (26/48) reported feeling satisfied or very satisfied, 31.25% (15/48) indifferent and 14.58% (7/48) dissatisfied. One hundred percent of respondents replied Yes to the question if your child was hospitalised in the neonatology department, would you like safety checklists to be implemented to improve their safety?.. For the new study, Pronovost and his team, using Medicare claims data, studied hospital mortality of patients admitted to ICUs in Michigan before, during and after what is known as the Keystone ICU Project, which features the checklist. They compared the Michigan information to similar data from 11 surrounding states. While data from both Michigan and the other states showed a reduction in hospital deaths of elderly patients admitted to ICUs over the five-year period from October 2001 to December 2006, the patients in Michigan were significantly more likely to survive a hospital stay during and after the Keystone project. These findings cannot definitively attribute the mortality reduction to the Keystone project, Pronovost says, but no other known large-scale initiatives were uniquely introduced across Michigan during the study period. This is perhaps the only large-scale study to suggest a significant reduction in mortality from a quality-improvement initiative, Pronovost says. The Keystone ...
BACKGROUND: The World Health Organization (WHO) Surgical Safety Checklist is reported to reduce surgical morbidity and mortality, and is mandatory in the U.K. National Health Service. Hospital audit data show high compliance rates, but direct observation suggests that actual performance may be suboptimal. METHODS: For each observed operation, WHO time-out and sign-out attempts were recorded, and the quality of the time-out was evaluated using three measures: all information points communicated, all personnel present and active participation. RESULTS: Observation of WHO checklist performance was conducted for 294 operations, in five hospitals and four surgical specialties. Time-out was attempted in 257 operations (87.4 per cent) and sign-out in 26 (8.8 per cent). Within time-out, all information was communicated in 141 (54.9 per cent), the whole team was present in 199 (77.4 per cent) and active participation was observed in 187 (72.8 per cent) operations. Surgical specialty did not affect time-out or

The New England Journal study was a prospective preintervention/postintervention study that looked at mortality rates and major complication rates after non-cardiac surgery. It involved hospitals in eight different countries, allowing the investigators to assess the utility of the checklist in both wealthy and poor countries. The intervention, of course, was introduction and use of the WHO Surgical Safety Checklist. Mortality at 30-days post-op decreased from 1.5% before introduction of the checklist to 0.8% after. Rate of any complication decreased from 11% to 7%. Both these outcomes were highly statistically significant. That s a relative risk reduction of approximately 36% for mortality and major morbidity! Of important specific surgery complications, both surgical site infections and unplanned reoperations decreased significantly. Improvements were seen at all participating sites ...
Logistic regression is similar in many aspects to linear regression, they differ in a very critical aspect. Logistic regression assumes outcome can be explained through weighted sum that goes through a special mathematical transformation, called logit. This transformation allows all weighted sum to be mapped into a value in between 0 and 1, which can be interpreted as a probability of a binary outcome. Thus, logistic regression is widely used in outcome variable that has two outcome, e.g., whether you have a disease or not. De Vries et al. [1] researched the relationship between mortality and iatrogenic illnesses that occur outside the surgical room. The researchers implemented a multidisciplinary surgical safety checklist in which six hospitals had to check for medication, operative side, and medication. Logistic regression was performed to assess the relationship between the checklist and mortality. The study showed an association between the comprehensive checklist and a reduction in surgical ...
examination and diagnosis all the way to actual treatment and follow-up - largely depends on following certain determined protocols that have been scientifically validated as producing the best possible results, with maximum efficiency and reproducibility. However, as medical and dental procedures are becoming more complex and lengthier by the day, even the most experienced clinicians can forget, miss or overlook certain critical steps in recommended procedures, to the detriment of the success of the intervention itself as well as the well-being of the patient. After all, we are all human. Precise, well-thought out checklists are a major valuable asset in improving our efficiency and ensuring we do not overlook important steps necessary for success of our interventions.. The idea of implementing checklists in medical practice has been on the radar for a long time, and more recently, a number of systematic studies have been conducted in the medical field to determine whether the use of checklists ...

Updated 16 December 2014. We found evidence of good outcomes for patients who underwent surgery at Hammersmith Hospital. There was a backlog of patients waiting for elective surgery with some patients who had experienced long waits for their surgery. The trustdid provide a plan to reduce the backlog of patients waiting for elective surgery. We found preoperative assessment for some surgical specialties was not managed effectively, which often led to cancellation of elective procedures. Data submitted by the trust showed a high rate of procedure cancellation.. The trust had not taken sufficient steps to ensure the five steps to safer surgery - from the World Health Organization (WHO) surgical safety checklist - was embedded in practice across Hammersmith Hospital, due to the low numbers of WHO checklist audits. We identified that surgical wards had a low number of nursing vacancies; they regularly reviewed skills mix and used a low volume of agency staff. The majority of staff received ...
Checklists have been shown to improve patient safety and clinical outcomes when used in combination with other interventions to increase adherence to proven safety strategies. The use of checklists to prevent diagnostic errors has been advocated as well. In this study, medical students, residents, and cardiology fellows were instructed to use a checklist while interpreting electrocardiograms (ECGs). Use of the checklist increased the accuracy of ECG interpretation for all groups and was most effective with the least experienced clinicians. As ECG interpretation errors are common and have serious clinical consequences, checklists such as this have the potential to improve patient care.. ...

View Poster. INTRODUCTION. With the increased use of simulation for surgical training, there is a need for objective forms of assessment to evaluate trainees. The Global Evaluative Assessment of Robotic Skills (GEARS) is the gold standard for assessing skills in robotic surgery, but there are no recognised checklist scoring systems. The purpose of this study was to develop and validate a checklist for evaluating suturing in robotic surgery. METHODS. A suturing checklist, containing needle driving and knot tying categories, was constructed following evaluation of participants performing urethrovesical anastomoses. Key procedural steps were identified from a review of expert surgeons, while assessing novice participants allowed identification of common technical errors. Using the checklist, 22 novices and 13 experts were marked in needle driving, while 18 novices and 10 experts were assessed in knot tying. All participants were scored separately by the GEARS score. Cronbach&[prime]s alpha measured ...

Introduction The volume and complexity of knowledge today has exceeded any single individuals ability to manage it consistently without error despite advances in technology, years of training, and specialization of functions and responsibilities. Checklists have been employed since the 1930s in aviation and high-risk industries to prevent accidents caused by human error. Only recently has their use been introduced to surgery after the World Health Organization (WHO) heightened awareness of the significant number of deaths, about a half million worldwide, that occur each year as a result of avoidable surgical error.1 With over 275,000 surgical procedures performed daily, an emphasis on perioperative safety is a necessity.2 The WHO developed the first surgical safety checklist for use in the operating room in 2007. Since implementation, the checklist has reduced morbidity and mortality by over 30%.3 Subsequently, many hospitals have adapted and implemented checklists to improve patient outcomes. Moreover

Translated versions of neck-specific questionnaires have been evaluated in 15 different languages. Generally the methodological quality of the translation process is poor, which was mainly due to the fact that the translated version was not pre-tested in the target population. Furthermore, none of the included studies performed a cross-cultural validation. This is necessary to evaluate whether the constructs underlying the original questionnaire are represented adequately by the questionnaire items in the new language. For each questionnaire, except for the Iranian NPDS and Spanish NDI, at least half of the information regarding measurement properties was lacking. Moreover, the evidence for the quality of measurement properties of the translated versions is mostly limited, due to methodological shortcomings of the included studies.. The COSMIN checklist has recently been developed and is based on consensus between experts in the field of health status questionnaires [9]. The COSMIN checklist ...

REBREATHER FORUM 3 CONSENSUS STATEMENTS. CHECKLISTS:. The forum acknowledged the overwhelming evidence demonstrating the efficacy of checklists in preventing errors in parallel fields that share similar technical complexity. Two recommendations regarding checklists were consequently agreed:. 1. CHECKLISTS. The forum recommends that rebreather manufacturers produce carefully designed checklists, which may be written and / or electronic, for use in the pre-dive preparation (unit assembly and immediate pre-dive) and post-dive management of their rebreathers.. - Written checklists should be provided in a weatherproof or waterproof form.. - The current version of these checklists annotated with the most recent revision date should be published on the manufacturers website. 2. CHECKLISTS. The forum recommends that training agencies and their instructors embrace the crucial leadership role in fostering a safety culture in which the use of checklists by rebreather divers becomes second ...
BACKGROUND: We previously developed and validated the Oxford NOTECHS rating system for evaluating the non-technical skills of an entire operating theatre team. Experience with the scale identified the need for greater discrimination between levels of performance within the normal range. We report here the development of a modified scale (Oxford NOTECHS II) to facilitate this. The new measure uses an eight-point instead of a four point scale to measure each dimension of non-technical skills, and begins with a default rating of 6 for each element. We evaluated this new scale in 297 operations at five NHS sites in four surgical specialities. Measures of theatre process reliability (glitch count) and compliance with the WHO surgical safety checklist were scored contemporaneously, and relationships with NOTECHS II scores explored. RESULTS: Mean team Oxford NOTECHS II scores was 73.39 (range 37-92). The means for surgical, anaesthetic and nursing sub-teams were 24.61 (IQR 23, 27); 24.22 (IQR 23, 26) and 24.55

Background: Quality indicators (QI) have been developed to define appropriate antibiotic use in hospitalized patients. A checklist based on these QIs could be a useful tool to promote appropriate antibiotic use in daily practice. Methods: We developed an antibiotic checklist, which applied to hospitalized adult non-ICU patients with a bacterial infection in whom intravenous (IV) antibiotic therapy was initiated. It was divided into two bundles; the first should be completed at start of treatment, and the second during the first 72 hours of treatment.. The checklist was implemented in nine Dutch hospitals using a stepped wedge study design (SWD) (figure 1). Outcomes before (baseline) and after introduction of the checklist (intervention) were compared with mixed effect models. The number of patients included in the baseline period was determined by a power analysis; in the intervention period all eligible patients were included. The primary endpoint was length of stay (LOS); secondary endpoint ...
Surgical care is a fundamental part of healthcare around the globe. Ensuring surgical safety in operation theatres is the need of the hour.
Depression among children and adolescents is common but frequently unrecognized. It affects 2 percent of prepubertal children and 5 to 8 percent of adolescents. The clinical spectrum of the disease can range from simple sadness to a major depressive or bipolar disorder. Risk factors include a family history of depression and poor school performance. Evaluation should include a complete medical assessment to rule out underlying medical causes. A structured clinical interview and various rating scales such as the Pediatric Symptom Checklist are helpful in determining whether a child or adolescent is depressed. Evidence-based treatment guidelines from the literature are limited. Psychotherapy appears to be useful in most children and adolescents with mild to moderate depression. Tricyclic antidepressants and selective serotonin reuptake inhibitors are medical therapies that have been studied on a limited basis. The latter agents are better tolerated but not necessarily more efficacious. Because the risk of
Follow me on Instagram at sharpharmade! . Sunday October 30, 2016 is Checklist Day. If you are living in the modern world, making use of a checklist is a powerful habit to embrace. So, Checklist Day is a great way to remind yourself how important these self-made reminders truly are. Interestingly enough, it is said that this day harkens back to some rather dark roots. During the 1930s, early aviators were searching for ways to allow flying to be much safer and avoid the all-too-common crash during takeoff. After several costly mishaps, one group of pilots can up with the idea to create a checklist before taxiing down the runway. It is said that the origins of Checklist Day can be traced back to this singular event. So, how can you celebrate Checklist Day appropriately? As you may have guessed, the best way to do so is by making a checklist. Still, remember to use this checklist to make absolutely certain that you never forget Checklist Day! ^sPm - #sharpharmade Fr Days of the Year . #fit ...

The Aberrant Behavior Checklist (ABC) is a caregiver rated questionnaire for assessing problem behaviors of children over the past week relative to typically developing children of the same age. Problem behaviors are rated on a categorical scale between 0 not at all a problem and 3 problem is severe in degree. Raters are instructed to consider both the severity and the frequency of the behavior in determining how severe a problem the behavior is. Thus, if a given behavior occurs more often than in other children of the same age and sex, scores greater than or equal to 1 are warranted. The total score can range from a minimum of 0 (no problem behaviors) to a maximum of 174, higher the number the worse the symptoms. The irritability subscale consists of 15 items with a minimal score of 0 - no irritability problems to 45 - all irritability items rated as severe. A rating of 18 or more on the irritability subscale is considered clinically significant ...

(2) Methods: We retrospectively described 14 patients with autism and SPD, including seven with comorbid GJH, who were hospitalized for major challenging behaviors with remaining behavioral symptomatology after the implementation of multidisciplinary approaches, including medication, treatment of organic comorbidities, and behavioral restructuring. Each patient received a CG to wear for at least 1 h (but most often longer) per day for six weeks. We assessed challenging behaviors in these participants with the Aberrant Behavior Checklist (ABC), sensory integration with the

The COMFORT scale is a measurement tool to assess distress, sedation and pain in nonverbal paediatric patients. Several studies have described the COMFORT scale, but no formal assessment of the methodological quality has been undertaken. Therefore, we performed a systematic review to study the clinimetric properties of the (modified) COMFORT scale in children up to 18 years. We searched Central, CINAHL, Embase, Medline, PsycInfo and Web of Science until December 2014. The selection, data extraction and quality assessment were performed independently by two reviewers. Quality of the included studies was appraised using the COSMIN checklist. We found 30 studies that met the inclusion criteria. Most participants were ventilated children up to 4 years without neurological disorders. The results on internal consistency and interrater reliability showed values of ,0.70 in most studies, indicating an adequate reliability. Construct validity resulted in correlations between 0.68 and 0.84 for distress, ...
Compelling people with evidence or demanding compliance does very little to drive adoption of new practices - handing out articles, making posters and Powerpoint presentations to educate, and conducting audits will continue to disappoint. What will help? Create real ownership (not demand buy-in), deeply understand and respect the motivations of stakeholders, and demonstrate results by getting early adopters to practice first. Heres an excellent case study in HBR from McLeod Health in South Carolina on creating adoption of a Surgical Checklist. I try not to make any major decisions at work after 3 pm. Why? Im tired and my brain isnt as sharp. It turns out, doctors get fatigued too, and it results in a difference in the care provided. Scientific research should be recorded with sufficient detail and semantic clarity to enable the information obtained from one investigation to be re-used in future investigations. The traditional way of recording science, based on the use of natural language, does not fully promote reuse as it permits too much ambiguity [1-3].. There are now a growing number of domain-specific data reporting standards for experimental data, especially in biology. These ensure that common experimental metadata are recorded, and partially deal with the ambiguity of natural languages by using standard taxonomies. The Minimum Information for Biological and Biomedical Investigations (MIBBI) project provides a resource for the existing checklists and fosters coordinated development [4]. These checklists are intended to promote transparency in experimental reporting, enhance accessibility to data and support effective quality assessment, thereby increasing the value of a body of work. Often the terminology used in ...
