Cervical insufficiency is seen in 0.1% - 1% of all pregnancies and classical treatment involves vaginal cerclage. In some conditions, such as an extremely short, deformed or absent cervix, surgery needs to be done by transabdominal approach. In those first 2 incredibly stressful weeks, we ended up back at labor and delivery 3 times. The first time, Laurie woke up at 3 AM about 5 days after the surgery, shaking uncontrollably and hyperventilating. Our doctors were concerned it could have been signs of an infection, but it turned out to just be a completely understandable panic attack as the emotions and stress of the week came crashing down on Laurie. One of the nurses at labor and delivery told Laurie she needed to just get over it during that visit. A few days later, our OB was concerned that the antibiotic regimen used after the cerclage was placed had given Laurie C Difficile, but once again it was a false alarm as the tests were negative (though we also were given incorrect instructions on how to collect the stool sample for testing, and Laurie ended up having to do that multiple times). I dont remember exactly how it went down (partly because it is the worst part of the whole thing, I think, so I probably blocked it out), but I get stuck with one needle and can feel burning as the anesthesiologist injects what I think is a local anesthetic. Then I get stuck with another needle that hurts worse than the first (not sure how that happens), and Im in that uncomfortable position for several long minutes as it seems like hes trying to find the right spot. I dont remember the spinal taking that long any other time (and Ive had 2 other cerclages AND 2 C-sections), and I really dont know if he was having trouble because he didnt tell me. Unfortunately, the way that most women find out that they have a soft or incompetent cervix is to lose a baby. Recently, more OBs have started routinely measuring cervical length at about 20 weeks of pregnancy, so that treatment can be offered if the cervix is shortened. In women with a history of preterm birth, weekly progesterone injections are now offered with subsequent pregnancies, sometimes in addition to cerclage. A prophylactic cerclage is one that is placed before the cervix begins to shorten. The decision to place a cerclage is based on whether a previous premature birth was felt to be due to an incompetent cervix rather than preterm contractions. Progesterone is a better option to reduce contractions. Cerclage may help to give the cervix extra support, but will not prevent contractions. Methods: We evaluated a universal psychosocial screening intervention using a retrospective cohort (n=881 prenatal care patients). Pregnancy outcomes among patients screened beginning in 2008 (n=464) were compared with outcomes among women receiving care before universal screening was implemented (n=417). Data were obtained from medical records between 2007 and 2009. I would be interested to see the statistics regarding the success rates of emergency cerclages when placed at the first sign of trouble. Emergency cerclages have been reported to have a low success rate. In talking with other women who have incompetent cervix I have found that most who were able to get an emergency cerclage were initially refused a cerclage, did not get a cerclage for many days, or received them after the cervix was open, often with membranes protruding into the vagina. Preventative cerclages have much higher success rates because they are placed before changes in the cervix occur. I believe that if doctors could catch cases of incompetent cervix during the period prior to the cervix actually opening, that emergency cerclages might be just as effective as preventative cerclages ...
