AU - Beals, Rodney K.. PY - 1982/9. Y1 - 1982/9. N2 - In a retrospective sequential study we determined the rate of infection occurring despite cephalothin or cephapirin chemoprophylaxis in orthopedic and cardiac surgery done from 1973 to 1977. The incidence of infection after prosthetic hip placement or open reduction of hip fracture was 3.4% and 1.0% in patients receiving cephalothin or cephapirin, respectively. The infection rate after prosthetic heart valve implantation was 3.5% in those receiving cephalothin and 1.6% in those receiving cephapirin. There was no significant difference in infection rate, duration of fever 3=38.0 C, or length of postoperative hospitalization. The efficacy of selected antistaphylococcal antibiotics in preventing colonization of human fibrin clots by staphylococci was studied. ...

Recombinant adeno-associated viruses (rAAVs) are commonly used vehicles for in vivo gene transfer. However, the tropism repertoire of naturally occurring AAVs is limited, prompting a search for novel AAV capsids with desired characteristics. Here we describe a capsid selection method, called Cre recombination-based AAV targeted evolution (CREATE), that enables the development of AAV capsids that more efficiently transduce defined Cre-expressing cell populations in vivo. We use CREATE to generate AAV variants that efficiently and widely transduce the adult mouse central nervous system (CNS) after intravenous injection. One variant, AAV-PHP.B, transfers genes throughout the CNS with an efficiency that is at least 40-fold greater than that of the current standard, AAV9 (refs. 14,15,16,17), and transduces the majority of astrocytes and neurons across multiple CNS regions. In vitro, it transduces human neurons and astrocytes more efficiently than does AAV9, demonstrating the potential of CREATE to ...
The objective of this study was to determine some metabolic and other factors predicting the risk of postpartum uterine disease (PUD), and the effects of puerperal metritis (PM) on metabolic status, reproduction and milk yield were analysed. A total of 105 Holstein-Friesian cows were included, and sampled on day , −14 prepartum and days 4, 10-14, 28-35 and 56-63 postpartum for metabolic tests. From day 4 the development of PUD, and from days 28-35 the ovarian activity was monitored. When grade ≥ 1 + ketonuria was present on day 4 postpartum, this indicated a higher probability of PUD [odds ratio (OR) 2.64; P , 0.05] including PM occurring on days 10-14 (OR: 2.65; P , 0.05). Plasma nonesterified fatty acid (NEFA) concentrations , 0.200 mmol/l on days , −14 prepartum indicated a higher risk of uterine diseases (OR: 3.44; P , 0.05). The odds of PUD increased, depending on whether a body condition score (BCS) loss of ≥ 1.0 occurred between days , −14 and 28-35 (OR: 2.82; P , 0.05), between ...

There was a recent discussion on the Kensmuir stockdog list on increasing fat in a dogs diet to enable the dog to withstand the colder temperatures. One poster indicated that she added fat from pasture-fed cattle (a good thing) or bacon fat (probably not so good). Not so sure why anyone would want to add more fat to commercial dry dog food. Just another example of: if the food was correct to begin with, there would be no need to add to it, mix formulas, or use one of the special formulas because of a particular medical concern.. There were three or four interesting presentations yesterday in breast cancer. One of them was about the PI3K inhibitor taselisib that combined with hormonal therapy, letrozole, reduced the size of tumours when used as neoadjuvant therapy in luminal cancer. So its very interesting because this is the first time this is proven in a clinical setting and this opens a window of opportunity for the drug to be introduced in other settings, in the metastatic setting, in the adjuvant setting and so on. The toxicity of the drug is quite manageable. So this is the first presentation I would like to remark.. The second one is the five year update of the ExteNET trial. The ExteNET trial is an adjuvant trial using neratinib as adjuvant extended therapy after trastuzumab and chemotherapy in operable HER2 positive breast cancer. Neratinib significantly improved invasive disease free survival at five years versus placebo and this is a new drug available for treating HER2 positive breast cancer patients with ...

Phlebitis means inflammation of a vein. It most commonly happens on the superficial veins of the legs, particularly if these happen to be damaged or varicose veins. How can phlebitis be prevented?
Itchy blood - Whats the cause of itchy blood clots on the thighs? Phlebitis. Superficial phlebitis is usually caused by local trauma to a vein. Superficial phlebitis may or not have a blood clot form to cause the pain and inflammation. In the legs, superficial phlebitis can be associated with varicose veins. Inactivity (blood pools in the veins and tends to clot if a person is inactive for a prolonged period of time); trauma, and blood clotting abnormalities.
Uterine diseases in cattle occur at all stages of the reproduction cycle but the majority of cases is found in the postpartum period. The inflammation of the uterus is generally defined as metritis or endometritis, with several graduations, e.g. puerperal metritis, clinical metritis, clinical or subclinical endometritis. Whether uterine diseases have a negligible, moderate or detrimental effect on fertility is still under discussion and depends on definitions and classification. In the past, it was assumed that the pregnant uterus is free of pathogens, but recent studies found several species including pathogens in the uterus and endometrium of pregnant cows. After parturition, a broad diversity of bacteria with >200 different species has been found in the early postpartum period. Not all of these bacteria, however, are considered as pathogens. Furthermore, bacteriological findings provide only evidence for infection but not for inflammation. For some bacteria, particularly Escherichia coli and ...
Operable breast cancer patients may experience late recurrences because of reactivation of dormant tumor cells within the bone marrow (BM). Identification of patients who would benefit from extended therapy is therefore needed. BM samples obtained pre- and post-surgery were previously analysed for presence of disseminated tumor cells (DTC) by a multimarker mRNA quantitative reverse-transcription PCR assay. Updated survival analyses were performed on all patient data (n = 191) and in a subgroup of patients alive and recurrence-free after 5 years (n = 156). DTC data were compared to the mitotic activity index (MAI) of the primary tumors. Median follow-up time was 15.3 years. Among the 191 patients, 49 (25.65%) experienced systemic relapse, 24 (49%) within 5-18 years after surgery. MAI and pre- and post-operative DTC status had significant prognostic value based on Kaplan-Meier analyses and multiple Cox regression in the overall patient cohort. With exclusion of patients who relapsed or died within 5 years

To document the incidence of postinfusion phlebitis and to investigate associated risk factors. it is important for health care staff to provide patients with information about what to look for after an intravascular device has been removed. This trial is usually registered with ACTRN12608000445370. 1. Introduction Peripheral intravascular catheterization (PIVC) is usually a common feature of acute hospitalization, with the majority of patients requiring the intravenous administration of fluid or medication at some time during their hospital stay [1, 2]. One of the complications of PIVC is usually phlebitis, diagnosed by one or more signs or symptoms of pain, tenderness, swelling, induration, erythema, and a palpable, cord-like vein [3]. A number of factors have been associated with the development of phlebitis, such as (1) chemical factorscaused by irritant drugs or infusates; (2) mechanical factorssize, location and catheter material, and skill of the inserter; (3) contamination ...

General: As with levodopa, periodic evaluations of hepatic, hematopoietic, cardiovascular, and renal function are recommended during extended therapy.. Patients with chronic wide-angle glaucoma may be treated cautiously with carbidopa and levodopa provided the intraocular pressure is well-controlled and the patient is monitored carefully for changes in intraocular pressure during therapy.. Dyskinesia: Levodopa alone, as well as carbidopa and levodopa, is associated with dyskinesias. The occurrence of dyskinesias may require dosage reduction. Hallucinations/Psychotic-Like Behavior: Hallucinations and psychotic-like behavior have been reported with dopaminergic medications. In general, hallucinations present shortly after the initiation of therapy and may be responsive to dose reduction in levodopa. Hallucinations may be accompanied by confusion and to a lesser extent sleep disorder (insomnia) and excessive dreaming. Carbidopa and levodopa may have similar effects on thinking and behavior.
