CEPHALOSPORINS. The first-generation cephalosporins represented by the oral drugs cephalexin and cefadroxil, and the injectable drug cefazolin have a spectrum of activity that includes staphylococci, streptococci, and many of the enteric gram-negative bacilli. However, resistance among gram-negative bacteria develops easily, primarily from synthesis of ß-lactamase enzymes that can hydrolyze these drugs. Extended-spectrum cephalosporins include those from the 2nd, 3rd, and 4th generation have been used in resistant infections caused by Escherichia coli, Klebsiella pneumoniae, Enterobacter species, Proteus species (especially indole-positive), and Pseudomonas aeruginosa. Of the 2nd-generation cephalosporins, the ones used most often in veterinary medicine are cefoxitin and cefotetan. Their use has been valuable for treating organisms resistant to the 1st generation cephalosporins or in cases in which there are anaerobic bacteria present. Anaerobic bacteria such as those of the Bacteroides ...

LABORATORY DETECTION OF EXTENDED-SPECTRUM Β-LACTAMASES (ESBLS). Extended-spectrum ß-lactamase (ESBL) producing organisms are a major problem in the area of infectious disease. ESBLs are enzymes that mediate resistance to extended-spectrum (third generation) cephalosporins (e.g., ceftazidime, cefotaxime, and ceftriaxone) and monobactams (e.g., aztreonam) but do not affect cephamycins (e.g., cefoxitin and cefotetan) or carbapenems (e.g., meropenem or imipenem). In addition, ESBL-producing organisms frequently show cross-resistance to many other classes of antibiotics; including aminoglycosides and fluoroquinolones thus treatment of these infections is often a therapeutic challenge. The frequency of ESBL-producing organisms differs significantly in accordance with geographic location. Published data from Pakistan supports an urgent need for regular screening and surveillance for these organisms in this region. Detection of ESBL is a major challenge for the clinical microbiology laboratory as its ...

Merck & Co., Inc. announced today that the U.S. Food and Drug Administration (FDA) recently approved INVANZÃ ® (ertapenem), a once-daily injectable antibiotic, for the prophylaxis of surgical site infection (SSI) following elective colorectal surgery in adults. This approval was based upon the results of the landmark PREVENT trial, the largest prospective, randomized double-blind, comparative clinical trial ever conducted in antibiotic prophylaxis for elective colorectal surgery (N=1002). Results from the study were presented today in the New England Journal of Medicine. Given the high incidence of SSI, Merck is very pleased to be able to offer a new alternative with clinically demonstrated efficacy, said Murray A. Abramson, M.D., M.P.H., senior medical director, Merck Research Labs, Infectious Diseases, Merck & Co. Inc. In the PREVENT study, a statistically significant difference favoring INVANZ over cefotetan with respect to the primary endpoint has been observed. A second adequate and ...
Warfarin has been reported to interact with more than 100 drugs, including many antibiotics. Warfarin is a racemic mixture of S- and R-warfarin enantiomers. S-warfarin is considered to have several times more anticoagulant activity than R-warfarin. S-warfarin is primarily metabolized by CYP2C9, whereas Rwarfarin is metabolized by CYP1A2, CYP2C19, and CYP3A4. Thus, one would expect that drugs inhibiting CYP2C9, and therefore S-warfarin metabolism, would increase the concentration of warfarin and enhance its anticoagulant effect (Table). Other antibiotics have been reported to increase warfarin response. Some?such as moxalactam, cefoperazone, cefamandole, cefotetan, and cefmetazole?appear to inhibit the formation of clotting factors and indirectly increase the effect of warfarin. As with the antibiotics that are inhibitors of CYP2C9, there may be a reasonable mechanism for these purported interactions. For the majority of antibiotics associated with warfarin interactions, however, there is no ...

by Donato Pompo, CTC, CMR, CSI, CDT, MBA - The Construction Specifier Volume 55, Number 10 - October 2002 -. Reviews the importance of clearly stating quality control guidelines in job specifications. When there is a problem it doesnt matter whose fault it is because everyone involved will pay, in one way or another, in dollars, time and reputation. Ceramic tile, glass tile and stone installers do not go to a trade school to learn how to install tile or stone. They typically learn on-the-job and this means they are not trained when it comes to industry standards. This is especially important, however, given the complexity of the work they perform. Installers need to be properly supervised and managed to ensure a successful installation. To avoid the huge cost of a tile failure its in the owners best interest to hire a qualified third party quality control tile and stone expert to monitor the tile installation. As recently reviewed, 1026 neuroprotective drug candidates in stroke research have all failed on their road towards validation and clinical translation, reasons being quality issues in preclinical research and publication bias. Quality control guidelines for preclinical stroke studies have now been established. However, sufficient understanding of the underlying mechanisms of neuronal death after stroke that could be possibly translated into new therapies is lacking. One exception is the hypothesis that cellular death is mediated by oxidative stress. Oxidative stress is defined as an excess of reactive oxygen species (ROS) derived from different possible enzymatic sources. Among these, NADPH oxidases (NOX1-5) stand out as they represent the only known enzyme family that has no other function than to produce ROS. Based on data from different NOX knockout mouse models in ischemic stroke, the most relevant isoform appears to be NOX4. Here we discuss the state-of-the-art of this target with respect ...

Klebsiella pneumoniae isolates from 11 patients at the Miriam Hospital were identified as resistant to cefoxitin and ceftibuten as well as to aztreonam, cefotaxime, and ceftazidime. Resistance could be transferred by conjugation or transformation with plasmid DNA into Escherichia coli and was due to the production of a beta-lactamase with an isoelectric point of 8.4 named MIR-1. In E. coli, MIR-1 conferred resistance to aztreonam, cefotaxime, ceftazidime, ceftibuten, ceftriaxone, and such alpha-methoxy beta-lactams as cefmetazole, cefotetan, cefoxitin, and moxalactam. In vitro, MIR-1 hydrolyzed cephalothin and cephaloridine much more rapidly than it did penicillin G, ampicillin, or carbenicillin. Cefotaxime was hydrolyzed at 10% the rate of cephaloridine. Cefoxitin inactivation could only be detected by a microbiological test. The inhibition profile of MIR-1 was similar to that of chromosomally mediated class I beta-lactamases. Potassium clavulanate had little effect on cefoxitin or cefibuten ...
Warm antibody autoimmune hemolytic anemia (WAIHA) is the most common form of autoimmune hemolytic anemia. About half of the cases are of unknown cause, with the other half attributable to a predisposing condition or medications being taken. Contrary to cold autoimmune hemolytic anemia (e.g., cold agglutinin disease and paroxysmal cold hemoglobinuria) which happens in cold temperature (28-31 °C), WAIHA happens at body temperature. AIHA may be: Idiopathic, that is, without any known cause Secondary to another disease, such as an antecedent upper respiratory tract infection, systemic lupus erythematosus or a malignancy, such as chronic lymphocytic leukemia (CLL) Several medications have been associated with the development of warm AIHA. These medications include quinidine, nonsteroidal anti-inflammatory drugs (NSAIDs), alpha methyldopa, and antibiotics such as penicillins, cephalosporins (such as ceftriaxone and cefotetan), and ciprofloxacin. The most common antibody isotype involved in warm ...

Results:. An outbreak of C. difficile-associated diarrhea was caused by a clonal isolate of clindamycin-resistant C. difficile and was associated with increased use of clindamycin. Hospital-wide requirement of approval by an infectious disease consultant of clindamycin use led to an overall reduction in clindamycin use, a sustained reduction in the mean number of cases of C. difficile-associated diarrhea (11.5 cases/month compared with 3.33 cases/month; P , 0.001), and an increase in clindamycin susceptibility among C. difficile isolates (9% compared with 61%; P , 0.001). A parallel increase was noted in the use of and costs associated with other antibiotics with antianaerobic activity, including cefotetan, ticarcillin-clavulanate, and imipenem-cilastin. The hospital realized overall cost savings as a result of the decreased incidence of C. difficile-associated diarrhea. ...

Infectious morbidity for the mother, consisting primarily of endomyometritis and wound infection remains a leading cause of post operative complications. Infectious morbidity following CD remains among the top five causes of mortality in mothers worldwide, causing a huge burden to the society [9].. In the study by Sullivan et al. [7], on 357 women, decreased total infectious morbidity in the study group was found (RR = 0.4, 95% CI = 0.18 to 0.87). There was a decreased endometritis risk as well in the pre incision group (RR = 0.2, 95% CI = 0.15 to 0.94).There was no increase in neonatal sepsis (P = 0.99), sepsis work up (P = 0.96) or increased hospital stay (P = 0.17). In the present study, the results were almost similar in that, the incidence of SSI was decreased and duration of hospital stay was decreased with antibiotics given Pre-incision than after cord clamping. Unlike the study by Sullivan et al. [7], in the present study, the incidence of endometritis was similar in both groups. ...

the relationship with her first husband deteriorated because he insisted she was not the same person. This change in personality is a frequent report by spouses of individuals who have sustained TBIs. According to her reports one night he tried to kill her. The anomalous phenomena began that night and have been intermittent since that time. Their intensity and frequency have increased during the last 2-3 years. The anomalous phenomena include mechanical, electronic, and visual effects. She reports experiences of sounds, perceived as taps that she estimates to be between 3 and 4 Hz with sound pressure equivalents between 40 and 60 db. Occasionally there may be a single louder sound. The durations of clusters are often between 3 and 10 seconds with intervening periods of 4-8 or 16-24 seconds. The clusters are usually localized along her left side.. . . .. Ms. S. reported she feels overwhelmed by a deep sadness after the occurrence of the phenomena and cries, even if nothing bad happens. ...
Methicillin-resistant Staphylococcus aureus (MRSA) strains are usually resistant to other classes of antimicrobials, namely, aminoglycosides, fluoroquinolones, macrolides, and tetracyclines (6, 8). β-Lactams also pose a unique therapeutic problem due to the ability of MRSA to produce penicillinase and a low-affinity target penicillin-binding protein, PBP 2a (6, 8). Thus, the therapeutic options for treating MRSA infections have been limited to glycopeptides, quinupristin-dalfopristin, or linezolid (1, 6, 8). The treatment of MRSA infections became even more complex due to the emergence of strains with decreased susceptibility to glycopeptides, thus increasing the need to develop novel antimicrobials to treat MRSA infections (4, 8).. BAL9141 (formerly Ro 63-9141) is a pyrrolidinone-3-ylidenemethyl cephalosporin which has a broad spectrum of antimicrobial activity; most notable is the potent activity against methicillin-resistant staphylococci (2, 5, 7). BAL9141 activity against MRSA is due to ...
The infective agent of CJD (the prion) is resistant to routine reprocessing (as defined in AS/NZS 4187 Cleaning, disinfecting and sterilizing reusable medical and surgical instruments and equipment, and maintenance of associated environments in health care facilities). This makes the additional procedures outlined in this document essential for treatment of patients with an identified risk of CJD infection. The decision to implement additional procedures for the reprocessing of certain instruments and equipment is based on the currently known infectivity of the tissue to which the instrument or equipment has been exposed (see Table 1) and patient risk factors (refer to Sections 2.3 and 2.4 and Appendices 1 and 2). Provided that the care of the patient is not compromised, alternative diagnostic and management strategies, if suitable and available, can be considered in patients at risk of CJD ...

Panama City Beach Councilman Paul Casto proved prescient. When the council met July 15 and adopted an emergency ordinance intended to safeguard public health by curbing the spread of the COVID-19 virus, Casto felt that the body had not gone far enough.. That ordinance required business owners or managers to ensure that all employees or contract workers wear a face covering while indoors but did not address customers.. That, even though the ordinance language noted since the enactment of Phase 2 reopening guidelines, the City has seen an unusually large influx of visitors, and many of these visitors are exhibiting an attitude of carefreeness and indifference to Centers for Disease Control guidelines regarding social distancing.. Casto was alone in advocating a mask mandate that would apply to all persons and not just employees.. Were kicking the can down the road, he said.
