Pain catastrophizing (the cognitive-emotional tendency to ruminate about pain, magnify pain, and feel helplessness in response to pain) enhances pain and pain-related outcomes. However, the mechanisms by which catastrophizing modulates pain are poorly understood. Evidence suggests that catastrophizing modulates supraspinal processing of pain, but does not modulate spinal nociception (as assessed by nociceptive flexion reflex [NFR]). Unfortunately, these studies have all been cross-sectional and relied on between-subject correlations, which limit conclusions about causation. To address this limitation, the present study experimentally reduced catastrophizing to determine whether it modulates spinal nociception (NFR). Objective. We sought to gain a better understanding of whether glaucoma patients negative thoughts during Humphrey visual field (HVF) testing were related to HVF variability. Background. HVF tests are often unreliable in glaucoma patients, but continue to play a vital role in glaucoma clinical trials and clinical decision-making. Methods. We adapted the Pain Catastrophizing Scale to assess negative thoughts (e.g., cant stand it; feel its awful; fear of failing or doing poorly; something serious may happen; anxiously want it to end) related to vision loss and HVF test performance. We administered this catastrophizing questionnaire to 52 glaucoma patients within 20 minutes of HVF test completion during a routine clinic visit. HVF variability of the mean deviation score [coefficient of variation (CoV)] was based on the last 3-4 tests obtained in the past 4 years. Results. The aim of this prospective study was to explore the role of catastrophizing in vulvovaginal pain. The study entails two research questions. The first question was whether catastrophizing predicts the occurrence of vulvovaginal pain. The second question focused on exploring the links between catastrophizing, partner responses and pain. Five hundred and ten women filled out questionnaires at two measurement points, 10months apart. The women were divided into three groups based on levels of catastrophizing. To study the first research question, the groups were used as predictors of pain at follow-up. To study the second research question, the groups were compared regarding their perceived solicitous partner responses. Subsequently, two mediation models were tested to explore whether catastrophizing mediated the relationship between solicitous partner responses and pain, cross-sectionally and longitudinally. Neuropathic-like knee pain (NKP) is often reported in individuals with knee pain (KP), but the contribution of specific central and peripheral risk factors to NKP has not been studied previously. The aims of the present study were to determine the prevalence of NKP in a community-derived sample with KP and to identify risk factors associated with NKP. A cross-sectional study was undertaken (n = 9506) in the East Midlands community among responders (aged 40+ years) to a postal questionnaire. Questions included KP severity (numerical rating scale) and type (neuropathic versus nociceptive) using the modified painDETECT questionnaire, as well as age, body mass index (BMI), significant knee injury, widespread pain, pain catastrophising and fatigue. Multinomial regression analysis was used to determine ORs and 95% CIs. Predicting post-surgical outcomes is notoriously tricky. A good surgeon is not just technically gifted, but will always pick winners on which to operate. That said, understanding the causes of poor outcomes is incredibly (and increasingly) important. A study team in Birmingham (USA) set out to solve the thorny question of whether or not the outcomes of shoulder surgery are affected by psychological distress, and if psychological distress in itself is associated with alteration in the perception of symptoms.1 The study team used the Shoulder Pain and Disability Index (SPADI), a validated score administered to 139 patients, all with a primary shoulder diagnosis. Triathletes and ironman triathletes participate in an extremely intense sport that involves hours of considerable pain, in addition to physical and psychological stress, every day. The basic pain modulation properties of these athletes has not been established and for this reason it is not clear whether they present with unique features that make it possible for them to engage in such efforts. This studys goal was to investigate the existence of possible alterations in pain perception and modulation of triathletes, in addition to possible underlying factors. 19 triathletes and 17 non-athletes who underwent measurement of pain threshold, pain tolerance, suprathreshold perceived pain intensity, temporal summation of pain, and conditioned pain modulation (CPM) participated in the study. The etiology of severe fatigue in T1DM is not well understood. It seems obvious that physiological diabetes-related factors such as haemoglobin A1c (HbA1c) levels or variations in blood glucose levels are associated with fatigue. While fatigue is a classical presenting symptom of hyperglycaemia, in our cross-sectional study no relationship between glucose control (HbA1c levels) and fatigue severity was found [3]. Also, parameters gathered during continuous glucose monitoring such as the blood glucose variability were unrelated to persistent fatigue. Other diabetes-related factors were correlated to fatigue: the number of complications due to diabetes, diabetes-related distress and diabetes specific self-efficacy.
