The objective of our program is to evaluate the effect of home health nurse intervention and education on hospital admissions, ER utilization and asthma control of Passport asthma patients. The investigators hypothesize that case management and in home education by nurses will decrease hospitalizations and ER asthma visits and improves disease control in our population.. Our clinic population consists of primarily inner-city, low income, medicaid recipients. This population suffers from severe asthma much more than the typical pediatric population. Asthma remains the most frequent cause of hospital admission for our patient population. Home health nurse asthma education and home visits have been reported to reduce asthma triggers in the home and improve asthma control. The unique partnership between the pediatrician and the home health nurse will give us a better understanding of the health needs for this population, will improve quality of care and deliver more cost effective care. ...

Interest in reducing the increasing numbers of unplanned hospital admissions in the UK, as in other countries [1-5], is leading to attempts to stem the demand [6]. Rising unplanned admissions are expensive and disrupt planned health care, notably by impeding elective work. They also cause considerable disruption for patients and carers. One of the approaches attempted has been case management. Case management was developed in the US and attempts to identify individuals judged to have an increased risk of unplanned hospital admission. Those identified are managed proactively, primarily in an attempt to reduce unplanned hospital admissions. However, few studies describe the intervention and the processes undertaken in detail [7, 8].. Case management of frail older people by practice-based nurses with extended roles has been implemented widely in England through many initiatives and by the policy-based recruitment of community matrons [9]. This policy was partly based on an American study which ...

A still-unsettled issue in health care cost containment is the potential of wellness and case management programs to reduce costs overall or over time.. When you talk about the cost containment levers benefit managers have, one of the first is the investment in wellness solutions, said Bryan Mailliard, Kansas City. Mo.-based vice president and strategic consultant in the employee benefits group at Lockton Cos. L.L.C. Investing in your people today pays off in the future by reducing future claims incidents.. Soeren Mattke, Boston-based senior scientist at RAND Corp. and managing director of RAND Health Advisory Services, said companies should focus less on general wellness programs and more on chronic illness management programs for employees with chronic diseases such as diabetes.. A RAND Corp. study that tracked the workforce of PepsiCo Inc. for seven years found that while chronic disease management programs saved $3.78 in health care costs for every $1 invested in the effort, broader ...
There has been a large growth in publications on CHWs in recent years, most notably since 2011. This growth has been driven by the MDGs, especially those related to child survival, which have placed heavy emphasis on community-based activities. The integrated community case management strategy, in particular, was the product of a concerted global agenda setting process by an epistemic community of international NGOs, multilateral and bilateral agencies and academic actors, who developed and promoted a package of feasible interventions targeted at the major causes of child mortality [92].. Despite the extensive reliance on lay heath workers and greater levels of international funding flowing to HIV/AIDS [93], there were fewer publications, whether empirical, comparative or review, addressing this programme area. There are a number of possible reasons for this: the review period may have missed an earlier generation of publications on community caregivers and counsellors; strategies such as the ...

While approximately two-thirds of Americans seek care from a dentist regularly, many of the remaining individuals and families face challenges accessing dental care. These Americans, including racial and ethnic minorities, people with chronic diseases, the frail and elderly, and those whose families are economically disadvantaged, also bear a disproportionate share of dental disease. Access challenges include difficulty getting to a dental office, prioritizing dental care among other health crises and basic needs of living, overcoming financial barriers, and navigating government assistance programs. Fewer people have dental insurance than have medical insurance, and it is often lost when individuals retire. Some may need special financial arrangements or case management services to overcome these obstacles, especially those individuals with physical disabilities and/or mental health challenges. Basic awareness of oral health issues for many Americans may be quite limited because of cultural or ...

Our communities should act with care and compassion toward child migrants who have increasingly arrived at the border after fleeing violence and poverty in their home countries, and are attempting to reunify with their families. Unfortunately, our government has responded to this serious refugee situation by substantially increasing family detention and putting both children and families on a fast-track deportation process without legal representation. This is a grave injustice and does not reflect our national values. Instead, we should implement policies that preserve families and childrens domestic and international human rights protections, particularly when they are in detention; provide them with legal representation; improve community support by providing case management services to all children upon reunification; and address the driving factors that push children to make a perilous journey.. ...

Childhood tuberculosis is considered a major cause of morbidity and mortality in high burden TB countries. It has had a low priority for research and development globally and was not given much attention in the past on ways and means to operationalize the case management strategies within the context of National TB Control Programmes. The current thesis focuses on various cohorts of childhood TB and audit of the case management practices within the routine national TB control programme of Pakistan. The thesis also documents the lessons learned during this research and development. Our initial retrospective cohorts were based on the review of patient records, focused on the case notification, treatment outcomes and case management practices of patients registered during two complete years 2004 and 2005, when NTP had no particular emphasis on childhood TB and had recently expanded the DOTS strategy for adults. These results we compared with those of patients registered during 2006 and 2007, when ...

Background. Malaria still causes high morbidity and mortality around the world, mainly in sub-Saharan Africa. Community case management of malaria (CCMm) by community health workers (CHWs) is one of the strategies to combat the disease by increasing access to malaria treatment. Currently, the World Health Organization recommends to treat only confirmed malaria cases, rather than to give presumptive treatment.. CCMm is generally well executed by CHWs, but there are several barriers for its success. Integrated CCM may overcome some of these barriers.. ...
In recent years the availability of high-quality, inexpensive RDTs in the public sector has significantly improved and expanded diagnostic testing. In malaria endemic countries, approximately 40% of the population seeks care and treatment for febrile illnesses in the private sector. In such settings, successful malaria control greatly depends on effective diagnosis and treatment within the private sector. Interventions may seek to address diagnostic market constraints, quality-assured ACT treatment market constraints, or both.. In all cases, robust evidence on the quality of provider fever case management is vital to monitor and course-correct programming, and formally evaluate programs. As a part of the Unitaid-funded Private Sector RDT project, PSI regularly fielded client exit interviews and mystery client visits to assess provider quality.. The Fever case management Mystery client study questionnaire is used to assess provider adherence to fever treatment algorithms for malaria test-negative ...

Today, we take a look at how another organization, JSI, is Improving Supply Chains for Community Case Management (SC4CCM) through a five-year learning project ...

by Catherine Mullahy, RN, BS, CRRN, CCM , Oct 18, 2016 , Blog, Nursing Careers , 0 comments. A study conducted by the Commission for Case Manager Certification (CCMC) cites increasing demand for certified case managers. The CCMCs 2014 Role and Function Study also found that employers are willing to pay higher salaries for professionals whose education, training, and certification demonstrates a higher level of knowledge, skills, and overall competency. Further, the study revealed that 40.2% of employers are now requiring their case managers to be certified. Contrast this figure to the 25.9% of employers requiring certification in 2004. The CCMCs conclusion regarding a growing demand for well-trained and credentialed case managers is supported anecdotally by the actions of many health care organizations.. Clearly, case management presents a strong career path for nurses and even greater opportunities today. The opportunities can be even greater for minority nurses, including African Americans, ...
Monday, April 27, 2015. A health informatics specialist in Rwanda has developed a computer-based application to help clinicians reduce medical errors in managing HIV treatment failure.. According to Aimable Mbituyumuremyi, the developer, the application translates bulky and time-consuming paper-based guidelines on HIV treatment into a format capable of identifying causes of HIV treatment failure, contributory risk factors and making recommendations on case management.. The innovation, called HIV SMART Project, was unveiled during the 5th Annual East African Health and Scientific Conference in Kampala, Uganda, last month (25-27 March).. Mbituyumuremyi says the product took 12 months to develop, with support from information and communication specialists and clinicians in Rwanda.. Antiretroviral therapy (ART) is meant to suppress HIV multiplication in patients.. Mbituyumuremyi notes that in Rwanda, clinicians face challenges in managing transition to a new treatment if HIV is not suppressed, ...
Seid Eshetu, 38, is a father of two children aged 7 and 10, living in Tehuledere Woreda of Amhara, Ethiopia. Seid was shocked to learn that he was HIV positive. However he was comforted when he received posttest counseling and was escorted to the health centers antiretroviral treatment (ART) clinic, where he was enrolled in chronic care and started ART.. The case manager at Haik Health Center, Ahmed Eshetu, advised Seid to come to the facility with his wife and two children for HIV testing and counseling as well. Seid was filled with mixed feelings when he learned that his wife is HIV positive, too, but greatly relieved to find that his children were negative.. Ahmed Eshetu is a case manager, under the US Presidents Emergency Plan for AIDS Relief (PEPFAR) funded USAID program, the Ethiopia Network for HIV/AIDS Treatment, ...
In 2016 the age distribution of elderly people (age over 65 years) to children (age 0-15 years) was equally split. However, by 2026 it is projected that people aged 65 and over will outnumber children for the first time in history.. SIA Case Management spoke to Alison Conway, Partnership Liaison Manager and Registered General Nurse, having worked with many people following a life changing Spinal Cord Injury (SCI), to find out more about the impact of an ageing population and SCI.. The number of people living with an SCI in the UK currently is estimated to be 40.000 with 1,000 people becoming newly injured each year. The percentage of older persons within the overall SCI population has increased from 4.2% in the early 1980s to 15.4% of admissions in the first half of the current decade.. 75 years of age was different from those in younger age groups. 74% of older people sustained an SCI as a result of a fall, with road traffic collisions being the second highest reason (13%). There are also a ...

The meeting between representatives of 19 organizations from 7 countries was held in Accra, Ghana on May 26-27, 2004. A COPD case management team directs four inpatient education sessions included anatomy and physiology of the respiratory system, proper inhalation device use, return demonstration, controlled breathing techniques, infection control, referral, medication reconciliation, and referral to services, including pulmonary rehabilitation. The COPD Case Manager is an integral member of the Hospitalist team of physicians, rounding on patients and leaving assessments and recommendations in the electronic medical records. At discharge, each patient was given continued access to the case manager on duty through a telephone pager. What we recognized at UCDHS was that the COPD patients self-determination is vitally linked to competence in self-management of COPD.. Our prior experience in asthma ...

Depression, a common comorbid condition with diabetes, has been found to be associated with hyperglycaemia.1 However, the mechanisms linking depression to glycaemic control are not well understood.1 The study by Katon et al adds to our knowledge of whether interventions for depression improve both depression and glycaemic control. In 4 previous studies examining these relations, only one2 showed reductions in both glycaemic control and depressive symptoms.. The PCCI involved trained specialty nurses as the centrepiece of the intervention, in collaboration with a primary care physician, to deliver individualised stepped care for depression. Multifaceted interventions, especially those that include a nurse case manager and/or patient education, have shown improvements in process outcomes and patient outcomes such as glycaemic control.3 2 areas related to this collaborative intervention are noteworthy. Firstly, although the authors generally described the duration of each of the 3 steps and the ...

Provided by HCPLive. By Kari Oakes, PA-C , November 04, 2013. As the worldwide disease burden increases and the need for expanded access to care rises concomitantly, primary care clinics utilizing an interdisciplinary staffing model offer a promising alternative to treating patients with hepatitis C (HCV), according to a poster presented by Samuel Ho, MD, of the VA San Diego Healthcare System and the University of California-San Diego.. At the annual meeting of the American Association for the Study of Liver Disease, held November 1-5, 2013, in Washington, DC, Ho and his co-authors offered a retrospective analysis of the outcomes and productivity at a once-weekly primary care HCV clinic.. A pharmacist and a psychiatrist also participated in the clinic, ...

I got my first job at Sutter after I passed my RN. I did some leadership hrs at Sutter and asked if I can work for them. The manager really liked me and said sure call me when you are done with RN.. I thought she was being nice and did not mean to really hire me. So, I called her day after I found out that I passed RN with the feeling that I am never going to get her call back.. To my surprise, she called same day and asked if I liked to be interviewed. It was a great surprise for me, I am blessed to have this job in this economy and I always keep my eyes open to recommend other new grads when appropriate. Therefore, keep in mind everything is possible and be positive.. The second job as a case manager was offered to me after my preceptorship at LPCH. Thanks to my family support, my special friends and my wonderful teachers.. ...
Case managers are being challenged to find a discharge destination for patients who are uninsured or underinsured and who need services after hospitalization. These patients often stay in the hospital longer than necessary, at the hospitals expense. Case managers should develop a network of community resources that can assist with care for unfunded patients after discharge. In some cases, it makes sense for the hospital to pay for a lower level of care rather than keeping patients in acute care beds that could be occupied by paying patients. Case managers often are under pressure to move patients through the continuum of care, making it a challenge to create an effective discharge plan for patients with limited financial resources. Many unfunded patients are eligible for financial assistance with their healthcare needs, but they arent aware of it and dont know how to apply ...
For the last 8 years, between the hours of 7:30 AM and 5:00 PM, I have worked full-time for the State of Georgia as a Case Manager with Adult Protective Services. What does an APS Case Manager do? The book definition is to investigate reported cases of neglect, exploitation and abuse against individuals who are elderly or disabled. What an APS CM actually does on a day-to-day basis pretty much defies description.. Ask any Case Manager and they will most likely tell you they never discuss their work with family or friends. If you do, they dont know what to say. They simply look at you in dumbfounded silence and shake their heads. Anyone who has done casework could write a book, and I promise you it would be a page-turner.. You frequent all the neighborhoods people normally avoid. You try to go early in the day, when it tends to be less dangerous. Your cellphone is filled with the phone numbers of detectives, police, and county deputies because you call on them from time to time to escort you ...
