Suicide risk is high in the first week, month and year following discharge from psychiatric inpatient settings. The decrease in care following discharge has been considered as contributing to the excessive suicide rate in this population. The aim of this research was to evaluate the impact of an intensive case management follow-up of these high-risk people for one year. Sixty males with a history of suicide attempts and psychiatric illness were recruited at discharge from a psychiatric inpatient setting at the Gold Coast Hospital, Queensland. Participants were randomly assigned to one of two conditions: Intensive Case Management (ICM) or Treatment As Usual (TAU). ICM featured weekly face-to-face contact with a community case manager and outreach telephone calls from experienced telephone counsellors. TAU participants were discharged under existing hospital practices. All participants completed assessment interviews at baseline, six and twelve months post-discharge. At the end of the twelve-month ...
Different methods for measuring violence can produce very different prevalences. The recent use of multiple combined measures, as in this study, has highlighted the limitations of most previous studies, which relied on a single source. One study that specifically compared the yield of violence when different sources were used revealed a dramatically different picture of violence by patients depending on the source of information used.15 Our results support the observation that self report methods consistently produce a higher frequency of violence than use of other records.16 In a small proportion of cases the WHO life chart was completed from sources other than the patient, so the 94% response rate is a slight overestimate.. The optimal prevalence estimate would have been detected with 100% follow up on all data sources. Although we did not achieve this, we did obtain information on all participants from at least one source. One possible source of bias in this study is that intensive case ...

Background: Malaria, pneumonia and diarrhea are the leading causes of death in children under five years of age in Uganda. In a bid to improve access to prompt and effective treatment of these diseases, Malaria Consortium in partnership with United Nations International Childrens Emergency Fund (UNICEF) introduced Integrated Community Case Management (ICCM) in eight central districts of Uganda. There is currently limited information about client satisfaction with this program. The main objective of the study was to assess client satisfaction with the ICCM program in Wakiso district. Methods: A cross-sectional study using quantitative methods and a modified SERVQUAL tool was conducted and 454 care givers of children under five years of age were interviewed. Differences in mean scores between expected and perceived services were analyzed using paired t-tests and chi-square tests. A negative score implied that clients were not satisfied with the care they received. Logistic regression models were also
If your case management program is like most, it focuses mainly on utilization review and discharge planning. Stefani Daniels, RN, MSNA, CMAC, ACM, says many case managers are getting bogged down by these duties and losing focus of what case management is really about: patient advocacy.
Many students experience difficult times during college, due to academic and/or personal reasons. Common stressors include mental health, personal or family concerns, illness or injury. These life events can interfere with a students ability to attain their goals. Each situation is so unique, it can be challenging to determine what next steps might be helpful. In the height of a crisis situation, a student or support person may find it difficult to sort through the resources and services that are available at Western Michigan University to assist. Talking with the case manager is a good place to start.. Students are referred to case management services by faculty, staff or other students when they have a concern regarding a students welfare. Students can also be self-referred to gain assistance. The goal is to avert more serious difficulties, focus on the safety of both the student and the WMU community, and help the student attain academic and personal success.. Introduction: Frequent emergency department (ED) users are often vulnerable patients with many risk factors affecting their quality of life (QoL). The aim of this study was to examine to what extent a case management intervention improved frequent ED users QoL. Methods: Data were part of a randomized, controlled trial designed to improve frequent ED users QoL at the Lausanne University Hospital. A total of 194 frequent ED users (≥ 5 attendances during the previous 12 months; ≥ 18 years of age) were randomly assigned to the control or the intervention group. Participants in the intervention group received a case management intervention (i.e. counseling and assistance concerning social determinants of health, substance-use disorders, and access to the health-care system). QoL was evaluated using the WHOQOL-BREF at baseline and twelve months later. Four dimensions of QoL were retained: physical health, psychological health, social relationship, and environment, with scores ranging from 0 (low ...

Background: Frequent users of the emergency department (ED) can result in overcrowding, compromised quality of care, increased cost, and reduced efficiency. It is hypothesized that case management interventions can decrease the amount of ED visits, therefore improving quality of care, improving patient outcome, and lowering costs. Objectives: The purpose of this integrative literature review was to identify if case management interventions are an effective way to decrease the number of visits in frequent users of the ED. Method: An integrative literature review was conducted by searching ProQuest, CINAHL, and PubMed databases. Search terms included

The Children at Risk (CAR) Program was a comprehensive, neighborhood-based strategy for preventing drug use, delinquency, and other problem behaviors among high-risk youth living in severely distressed neighborhoods. The goal of this research project was to evaluate the long-term impact of the CAR program using experimental and quasi-experimental group comparisons. Experimental comparisons of the treatment and control groups selected within target neighborhoods examined the impact of CAR services on individual youths and their families. These services included intensive case management, family services, mentoring, and incentives. Quasi-experimental comparisons were needed in each city because control group youths in the CAR sites were exposed to the effects of neighborhood interventions, such as enhanced community policing and enforcement activities and some expanded court services, and may have taken part in some of the recreational activities after school. CAR programs in five cities -- ...

Perhaps a wiser health policy goal would be to focus on the delivery of high-quality, convenient, accessible care to all patients in all clinical settings, ambulatory, emergency, and inpatient. Intensive case management of frequent ED users and expanded off-hours access to facilities delivering primary care, especially mental health and substance use treatment, may decrease the frequency of ED recidivism, but additional work is needed to test the efficacy of these interventions. In health cares current political climate, which emphasizes cost containment and personal responsibility, it is difficult to see this happening. But for now, let us put to rest future conversations about "inappropriate" ED use ...
Background: Substance use disorder (SUD) is an important health problem that requires a complex range of care because of the chronic nature of the disorder and the multiple psychosocial problems involved. Current outpatient programs often have difficulties in delivering and coordinating ongoing care and access to different health care providers. Various case management (CM) models have been developed, first for patients in other psychiatric domains and then for patients with SUD, in order to improve treatment outcomes.Aims: This paper aims to assess the effectiveness of CM for patients with SUD. Methods: We performed a systematic review of CM interventions for patients with SUD by analyzing randomized controlled studies published on the subject between 1996 and 2016 found on the electronic database PubMed.Results and Conclusions: Fourteen studies were included in the analysis. Differences between studies in outcome measures, populations included, and intervention characteristics made it difficult to

We devised a randomised controlled trial to evaluate the effectiveness and efficiency of an intervention based on case management care for frequent emergency department users. The aim of the intervention is to reduce such patients emergency department use, to improve their quality of life, and to reduce costs consequent on frequent use. The intervention consists of a combination of comprehensive case management care and standard emergency care. It uses a clinical case management model that is patient-identified, patient-directed, and developed to provide high intensity services. It provides a continuum of hospital- and community-based patient services, which include clinical assessment, outreach referral, and coordination and communication with other service providers. We aim to recruit, during the first year of the study, 250 patients who visit the emergency department of the University Hospital of Lausanne, Switzerland. Eligible patients will have visited the emergency department 5 or more times
Family members of older persons (75+) with multi-morbidity in Sweden are likely to benefit from case management services [1, 2] performed by case managers. As a model, case management aims to improve the coordination of health care and social services [3], thus providing integrated care [4]. However, family members experiences of case management and the importance of case managers for them have not yet been explored.. Since the 1990s, the amount of formal care has decreased in Sweden and today, an increasing number of older persons receive informal care from family members [5]. Informal care now substitutes for professional care in Sweden, as in many other European countries [6]. It has been estimated that around 3% of the Swedish population aged over 55 years provides care to relatives. Most of these carers are 75-84 years old and care for spouses [7].. The proportion of older persons in Europe is increasing [8]. Within this ageing population, a substantial number of older persons have ...
Abstract. Health planners commonly use geographic proximity to define access to health services. However, effective access to case management requires reliable access to a trained, supplied provider. We defined effective access as the proportion of the study population with geographic access, corrected for other barriers, staffing patterns, and medicine availability. We measured effective access through a cross-sectional survey of 32 health facilities in Malawi, Mali, and Zambia and modeled the potential contribution of community case management (CCM). The population living within Ministry of Health (MOH)-defined geographic access was 43% overall (range = 18-52%), but effective access was only 14% overall (range = 9-17%). Implementing CCM as per MOH plans increased geographic access to 63-90% and effective access to 30-57%. Access to case management is much worse than typically estimated by distance. The CCM increases access dramatically, again if providers are available and supplied, and should be
For these reasons, the Principal Recipient (PR) of the Global Fund (GF) grant in Malaysia, the Malaysian AIDS Council (MAC) submitted a Reprogramming Request (RR) to change the focus of GF supported activities from a largely HIV prevention focus among key populations (KPs) to a scaled-up programme to recruit key populations for testing and treatment, and assist adherence by KPs to antiretroviral treatment (ART) and for opioid dependent people who inject drugs (PWID) to methadone maintenance therapy (MMT). In line with this goal, the Case Management Program has been developed and being implemented.. OBJECTIVES OF THE ASSESSMENT. The overall objective is to evaluate the effectiveness and impact of the Global Fund investment in the Case Management (CM) Program in Malaysia, and the extent to which - and how - the Case Management can form part of the sustained country HIV response past Global Fund financing. The National consultant will support the international consultant to assess three main ...
Unsurprisingly, we found that case managers allocated the largest proportion of time to care coordination. In practice a large part of case managers daily activities is to frequently communicate and negotiate with care professionals, clients and families, and significant others, so as to coordinate right services at the right time for clients [16].. Contrary to the literature [10, 14, 15], we found that case managers allocated a higher proportion of time to monitoring and review (16.5 %) than needs assessment (12.5 %). In addition, the majority of case managers (over 70 %) assigned high frequency ratings to all five monitoring and review activities but not all needs assessment activities. Further research is needed to determine reasons for different findings identified by our study.. We found that case managers allocated the lowest proportion of time to outcome evaluation, and assigned lower frequency ratings to all four outcome evaluation activities compared with most other case management ...

For over a decade, Cambodia has implemented a number of policies and innovative strategies to increase access to quality malaria case management services and address the drivers of multi-drug resistance. This paper utilizes outlet survey trend data collected by the ACTwatch project to demonstrate how changes in Cambodian policy and strategies have led to shifts in anti-malarial markets. Anti-malarial ACTwatch outlet surveys were conducted in Cambodia in 2009 (June-July), 2011 (June-August) and 2013 (September-October). A census of all outlets with the potential to sell or distribute anti-malarials was conducted within a nationally representative sample of communes. Drug information, sales/distribution in the previous week, and retail price were collected for each anti-malarial in stock. Information on availability of malaria blood testing was also collected. A total of 7833 outlets were enumerated in 2009, 18,584 in 2011, and 16,153 in 2013. The percentage of public health facilities with at least one
The AXEX intervention was found to increase appropriate treatment of symptoms of pneumonia, malaria and diarrhoea substantially. It is an example of a multicomponent intervention towards creating a functioning health market, which provides equitable access to good quality, efficacious medicines while maintaining transparency and accountability. Care-seeker expectations of quality of fever care at drug shops were reconfigured, from trial and error to care that was based on definitive knowledge based on rapid tests. The medicines provided in the AXEX intervention had attributes that improved regulator- drug seller interaction, and care-seeking behaviour. The presence of quality-assured single-dose packaged, colour-coded medicines that were easy to handle at drug shops and promoted patient adherence50 to prescribed doses was appreciated by regulators and care-seekers, although for different reasons. On the regulators side, these products met medicine packaging standards and regulations51 and ...

The website of the National Committee for Quality Assurance (NCQA) is a one good source of information about HEDIS measures.. Disease Management and Wellness Programs also make extensive use of clinical analytics. Analysis of claims data can help insurers and at-risk providers identify the sub-populations of patients with chronic diseases who would benefit from case management services intended to help them proactively manage their health and avoid expensive acute episodes. Clinical analytics can trigger alerts to either the case manager or the patient to remind them of necessary actions, such as refilling a prescription, making an appointment with a doctor or sticking to an exercise routine.. Where home monitoring is in place, analytics can trigger actions based on patterns in blood pressure levels or other clinical measurements. Clinical analytics also plays an important role in evaluating the overall effectiveness of programs in reducing costs and improving health.. Population risk adjustment ...

If youre a nurse, then you know that nursing continuing education courses help you stay current in the overall medical field as well as allowing you to study your own specialty in depth. In most states, nursing CEUs are a necessity for maintaining your nursing license, or for obtaining an advanced license in your field of choice. Other states leave it up to your own sense of ethical responsibility to stay current with the latest medical breakthroughs, health care technology and constantly changing protocols and standards of care.Whether you must take nursing continuing education courses because your state makes it mandatory or whether youre pursuing an interest in forensic nursing, legal nurse consulting, nursing management, nurse case management or any other nursing specialty, one of the most difficult issues youll face is finding appropriate nursing continuing education courses and evaluating the information that they offer. There are several methods of getting continuing education hours ...
Disturbances in lipid metabolism have been linked to suicidal behaviour, but little is known about the association between suicide risk and abnormal glucose metabolism in depression. Hyperglycaemia and hyperinsulinaemia may increase the risk of depression and also the risk for suicide, we therefore studied associations between suicidal behaviour and disturbances in glucose metabolism in depressive patients who had been referred to depression nurse case managers. Patients aged 35 years and older (N = 448, mean age 51 years) who were experiencing a new depressive episode, who were referred to depression nurse case managers in 2008-2009 and who scored ≥10 on the Beck Depression Inventory were enrolled in this study. The study was conducted in municipalities within the Central Finland Hospital District (catchment area of 274 000 inhabitants) as part of the Finnish Depression and Metabolic Syndrome in Adults study. The patients psychiatric diagnoses and suicidal behaviour were confirmed by the Mini
