Workforce planning is a central issue for service provision and has consequences for medical education. Much work has been examined the career intentions, career preferences and career destinations of UK medical graduates but there is little published about medical students career intentions. How soon do medical students formulate careers intentions? How much do these intentions and preferences change during medical school? If they do change, what are the determining factors? Our aim was to compare medical students career preferences upon entry into and exit from undergraduate medical degree programmes. This was a cross-sectional questionnaire survey. Two cohorts [2009-10, 2010-11] of first and final year medical students at the four Scottish graduating medical schools took part in career preference questionnaire surveys. Little career decision research has been done in the context of the community college and using qualitative research methods. This phenomenological multiple-case study investigated the processes and influences undertaken by ten new community college students within their first three years following high school graduation as they made their career decisions. The attainment of democracy in South Africa brought many previously disadvantaged families new hope and promise for enhanced entry to higher education. However, as these opportunities continue to grow, this research study asked whether these students are ready to make effective use of their expanded prospects. This qualitative study was informed by the Interactive Model of Career Decision-Making. It focused on family and community influences in tertiary educational career decision-making with special reference to black South African students' context of historical disadvantage. Family and community variables, such as socio-economic status, culture, parental influence, family composition and support, were investigated. Thematic analysis indicated strong links between family socio-economic status and student career decisions. Familial support further emerged as an important factor when students make career decisions.
