Transradial access (TRA) has been considered as the default choice in cardiac catheterization. Although infrequent, vascular complications of this approach remain. Recently, the distal transradial approach (dTRA) in cardiac catheterization was reported by interventionalists. We retrieved the relevant literatures and reviewed the safety and feasibility of this novel approach in cardiac catheterization. The dTRA for cardiac intervention has superior safety and satisfaction. As a novel approach for cardiac catheterization, access related complications should also be considered by operators, such as RAO, radial spasm, bleeding and haematoma, and injury of the superficial branch of the radial nerve. The dTRA in cardiovascular angiography and intervention was safe and feasible.

The HEART Pathway, which combines the HEART score, with 0- and 3-hour cardiac troponin tests, is a recently developed decision aid designed to identify emergency department patients who are safe for early discharge. Adult emergency department patients with symptoms related to acute coronary syndrome without ST-elevation on electrocardiogram (n = 282) were randomized to the HEART Pathway or usual care. BACKGROUND: Obstructive sleep apnea (OSA) is associated with recurrent atrial fibrillation (AF) after electrocardioversion. OSA is highly prevalent in patients who are male, obese, and/or hypertensive, but its prevalence in patients with AF is unknown. METHODS AND RESULTS: We prospectively studied consecutive patients undergoing electrocardioversion for AF (n=151) and consecutive patients without past or current AF referred to a general cardiology practice (n=312). OSA was diagnosed with the Berlin questionnaire, which is validated to identify patients with OSA. We also assessed its accuracy compared with polysomnography in a sample of the study population. Groups were compared with the 2-tailed t, Wilcoxon, and chi2 tests. Logistic regression modeled the association of AF and OSA after adjustment for relevant covariates. Oxygen supplementation has been a cornerstone in the initial treatment of individuals with acute coronary syndrome. While consensus for oxygen supplementation exists for patients with hypoxia, oxygen supplementation has also been routinely used in those presenting with acute myocardial infarction (MI) with normal oxygen saturations based on the rationale that oxygen therapy could improve oxygen supply to the ischemic myocardium, thereby reducing the infarct size and complications. Indeed, reports of oxygen supplementation to relieve angina pectoris were described as early as 1900 (1). These reports were followed by small studies that suggested benefit with oxygen supplementation in acute MI, but these studies were limited by lack of randomization and unblinded end point ascertainment (2-4). Abstract Background: Kawasaki disease is one of the leading causes of acquired heart disease in children. It is an acute self-limited vasculitis that predominantly affects infants and children younger than 5 years of age. These patients present with nonspecific symptoms, such as fever and lymphadenopathy, making the diagnosis challenging. This disease can have serious and potentially fatal outcomes, and prompt recognition of this disease is vital to the patients outcome. We present a complete review of the disease, including the epidemiology, pathophysiology, diagnosis and management of acute Kawasaki disease, the natural history of this disease, and follow up of these patients as they transition into the adult cardiology practice. The presence and burden of coronary artery calcium (CAC) is a strong predictor of cardiovascular events. In an effort to gain insight into the utility of CAC for coronary artery disease (CAD) screening in cancer patients with heart disease, we sought to determine the presence and burden of CAC detected on routine chest CT in patients referred to a cardio-oncology clinic, comparing them to a conventional cardiology clinic with the general population as controls. Patients from the cardio-oncology clinic, general cardiology clinic, and the general clinic population at Rush University Medical Center who had a chest CT as part of their previous treatment were identified. Each CT scan was evaluated for presence, extent, and severity of CAC by 3 independent readers. In multivariate analysis, when compared with cardio-oncology clinic, CAC was more prevalent in the CT scans of cardiology patients (p = 0.04), but not the general clinic population (p = 0.5); CAC extent (p = 0.05) and severity (p = 0.05) was
