Rectal signet ring cell carcinoma is a rare type of colorectal adenocarcinoma characterized by an aggressive biological behavior and poor prognosis. The co-occurrence of colorectal carcinoma and renal cell carcinoma (RCC) has found in many hundreds of patients, many of whom also have additional malignancies. Cancer to cancer metastasis is rare and an uncommon phenomenon in malignancy, especially at the time of initial diagnosis, suggesting a genetic susceptibility. We present the case of a 66-year-old Macedonian man with synchronous rectal signet ring cell carcinoma and RCC with tumor to tumor metastasis feature. He underwent a left nephrectomy and anterior rectal resection after complaining of constipation for 3-4 months and the appearance of synchronous tumors on the imaging studies. Morphology and immunohistochemical analysis of specimens from the RCC revealed signet ring cells identical to the rectal signet ring cell carcinoma. The next-generation sequencing study revealed mutations in TP53 and
Signet ring cell carcinoma of the colon and rectum is rare, and most cases are detected at an advanced stage. We present a case of primary signet ring cell carcinoma detected at an early stage by magnifying endoscopy with narrow-band imaging (NBI) and crystal violet staining. A 73-year-old man visited our hospital for screening colonoscopy. Six years previously, he had undergone endoscopic submucosal dissection (ESD) for early gastric cancer. The pathological diagnosis was a well-differentiated adenocarcinoma, invading into the mucosa without lymphovascular invasion. Colonoscopy revealed a flat elevated lesion with a slightly depressed area, 20 mm in diameter, in the cecum. Further, magnifying endoscopy with NBI revealed that the surface pattern was slightly irregular and microvessels had a regular diameter and distribution in the margin of the lesion, but in the central part of the lesion, irregularity in the tumor surface pattern and form as well as in the diameter and distribution of microvessels was
Primary Signet-ring cell carcinoma (Linitus Plastica) of the colon and rectum is a rare form of adenocarcinoma of the large intestine and has been reported to have an extremely poor prognosis. We report a case of Primary Signet-ring cell carcinoma of the colorectum in a thirty one year old man presented in Surgical OPD of our hospital with chief complaints of persistent pain in abdomen and vomiting since two days. Since the prognosis of primary signet ring cell carcinoma (SRCC) is extremely poor (in view of more malignant behavior than ordinary colorectal carcinoma), early diagnosis and aggressive treatment strategy are necessary. ...

Primary signet-ring cell carcinoma of the urinary bladder is a rare variant of mucus-producing adenocarcinoma constituting approximately 0.5% to 2.0% of all primary carcinomas of the bladder. This tumor initially presents as a high-grade, high-stage lesion and diffusely invades the bladder wall without forming intraluminal growth. The patients have no specific symptoms, which leads to delayed diagnosis and poor prognosis. We report the case of a 51-year-old Moroccan Berber man consulting for gross hematuria. Ultrasonography and a computed tomography scan found a bladder tumor diffusely invading the bladder wall. A histopathological examination of the tumor chips from a transurethral resection of the bladder revealed signet-ring cell adenocarcinoma. The gastrointestinal tract exploration did not reveal any other tumor localization. A radical cystectomy and adjuvant cisplatin and gemcitabine chemotherapy were therefore performed resulting in 18 months of survival without metastasis and a good quality of

Primary signet cell carcinoma of the prostate is a rare histological variant of prostate malignancies. It is commonly originated from the stomach, colon, pancreas, and less commonly in the bladder. Prognosis of the classical type is worse than the adenocarcinoma of the prostate. Primary signet cell adenocarcinoma is diagnosed by eliminating the adenocarcinomas of other organs such as gastrointestinal tract organs. In this case report, we present a case with primary signet cell adenocarcinoma of the prostate who received docetaxel chemotherapy because of short prostate specific antigen doubling time ...

Primary signet ring cell carcinoma of urinary bladder is a rare type of bladder tumor and carries a very high mortality rate. It may have a clinical presentation similar to common diseases like Benign Prostatic Hypertrophy (BPH) and the management options are extremely limited. We report a case of 58 year old Caucasian male who presented with a 5 month history of increased frequency of urination, nocturia and weight loss without any fever or hematuria. He was found to have an increased creatinine of 2.8 mg/dl and a prostate specific antigen level of 0.18 ng/ml. His azotemia was thought to be secondary to BPH. A foley catheter was initially placed with a plan for outpatient follow up. On removal of the catheter his problems persisted and he returned to the hospital. Diagnostic work up including abdominal ultrasonography, computed tomography (CT) scan, retrograde pyelogram, cystography and cystoscopic biopsies revealed the diagnosis of primary signet ring cell carcinoma of urinary bladder. Although
The number of cancer cases has been increasing worldwide.. Early diagnosis and tumor resection remain as the most effective. treatments for gastric cancer. However, early diagnosis is not always. possible as it is frequently not possible to make treatment decisions. without pathologic diagnosis in patients with clinically suspected. cancer. This causes delays in diagnosing cancer. We presented a 58. years old woman with gastric signet ring cell carcinoma that could. not be diagnosed despite using four different methods of stomach. biopsies. We aimed to emphasize that despite the use of advanced. methods, if clinical cancer in non-diagnosed cases is suspected, we. should be more aggressive for early diagnosis.. ...
Hi Shelly,. I was diagnosed with Stage IV gastric cancer in 2012. We did 8 rather aggresive chemo treatments over a 4 month period which were effective and I tolerated them better than most. I would not have been eligible for surgery if the chemo had not helped shrink the tumor but thank God it did. There were a few weeks between the end of chemo and the surgery when I felt very good.. My surgery was January 2013, a total gastrectomy with HIPEC (100+ degree chemo circulated through the abdominal cavity). The recovery was difficult at times and the help of family and friends kept me strong and positive. After a few weeks, I had 4 more chemo sessions. They were a little harder on me than the first 8. I am now over 2 years with clean scans.. While everyones case is different, I will answer any questions you have to the best of my ability. Best of luck to your Dad & you. ...

Gastric signet ring cell carcinoma (SRCC) is a rare form of highly malignant adenocarcinoma. It is an epithelial malignancy characterized by the histologic appearance of more than 50% of cells as signet ring cells filled with mucin. The incidence of SRCC is rising,[1],[2] therefore, the diagnosis of these cancers in the early stage is important as it is the only stage that curative measures can be done. A 49-year-old female was hospitalized due to epigastric pain, postprandial bloating for 3 months. Upper gastrointestinal endoscopy with narrow band imaging (NBI) showed 1 ...
Although vertigo is one of the most common complaints, intracranial malignant tumors rarely cause sudden asymmetry between the tone of the vestibular peripheries masquerading as a peripheral-like disorder. Here we report a case of simultaneous temporal bone infiltrating macro-metastasis and disseminated multi-organ micro-metastases presenting as acute unilateral vestibular syndrome, due to the reawakening of a primary gastric signet ring cell carcinoma. Purpose - Our objective was to identify those pathophysiological steps that may explain the complex process of tumor reawakening, dissemination. The possible causes of vestibular asymmetry were also traced. A 56-year-old male patients interdisciplinary medical data had been retrospectively analyzed. Original clinical and pathological results have been collected and thoroughly reevaluated, then new histological staining and immunohistochemistry methods have been added to the diagnostic pool. During the autopsy the cerebrum and cerebellum was ...

TY - JOUR. T1 - Histologic and immunohistochemical differences between hereditary and sporadic diffuse gastric carcinoma. AU - Lee, Hee Eun. AU - Smyrk, Thomas Christopher. AU - Zhang, Lizhi. PY - 2018/4/1. Y1 - 2018/4/1. N2 - We aimed to identify histopathologic features unique in hereditary diffuse gastric cancer (HDGC) by comparing with its sporadic counterpart (SDGC). 11 patients with confirmed CDH1 mutation who were found to have HDGC in a prophylactic total gastrectomy were collected. Median age of HDGC patients was 39 years (range 24-57). All HDGC cases had intramucosal signet ring cell carcinoma. Twenty-three invasive tumor foci from 7 patients with HDGC were available for ancillary studies, and we evaluated each focus separately. Almost all foci (20/23) showed two distinct tumor cell populations, namely, large signet ring cells and small signet ring cells. The large cells were located just beneath the surface epithelium and were positive for mucicarmine and pCEA, while the small cells ...
Congenital goiter is a common thyroid metabolic disorder characterized by low levels of thyroid hormone, subsequent secretion of excess thyroid-stimulating hormone (TSH) from the pituitary gland, and compensatory hyperplasia of the glands. The presence of signet ring cells (SRCs) does not provide sufficient evidence for the diagnosis of a thyroid tumor, making histopathological diagnosis challenging. In addition, SRCs can also appear in congenital goiter. Therefore, a comprehensive diagnosis of congenital goiter is warranted based on clinical symptoms, autopsy, histopathology, and laboratory examination. A juvenile giraffe at the Ordos Zoo in Ordos presented with symptoms of loss of appetite, serious salivation, and slow growth rate since birth. Its height and weight were significantly lower than those of other giraffes of the same age. The animal ultimately died at 17 months of age. Autopsy revelaed that the thyroids were hard, with an uneven surface and with the presence of many small raised follicles
Diffuse gastric cancer (DGC) is one of the two main types of stomach cancer. Histologically, DGC can be subdivided into poorly differentiated carcinoma and signet ring cell carcinoma (SRCC). Carriers of germline mutations in the gene encoding E-cadherin (CDH1) are predisposed to DGC (hereditary DGC, HDGC). Essentially all asymptomatic mutation carriers present with occult, multifocal SRCC confined to the gastric mucosa before they develop advanced disease. Initial characterisation of these early HDGC foci (referred to as eHDGC) suggests an indolent nature of these lesions, contrary to their current pathological definition as malignant disease. The main aim of this study was to determine if genomic instability is an early event in HDGC and how it might lead to disease progression. We assessed chromosomal aberrations in the early intramucosal eHDGCs by using array comparative genomic hybridisation (CGH). Taking advantage of possibly the largest eHDGC collection worldwide, we analysed tissue from ...
2 MANECs are rare entities known to be associated with biological aggressiveness and poor patient survival, but otherwise remain incompletely characterized.3 In a recent study that compared CRC-NE to conventional colorectal carcinoma, CRC-NE was more frequently located in the right colon (76% vs. 46%), more often presented with metastasis (59% vs. 18%) and often had perineural (68%), lymphatic (97%), and venous (62%) invasion and high tumor budding (71%). BRAF V600E mutation was more common in CRC-NE compared with conventional CRC (53% vs. 12%). Neuroendocrine differentiation, high stage, BRAF mutation without microsatellite instability, and signet ring histology were all associated with poor overall survival.4 In the same study, CRC-NE was further divided into large cell neuroendocrine carcinoma (LCNEC), MANEC composed of LCNEC and conventional adenocarcinoma (MANEC-conventional type), and MANEC composed of LCNEC and signet ring cell carcinoma (MANEC-signet ring type). MANEC-signet ring type ...
TY - JOUR. T1 - Different methylation profiles between intestinal and diffuse sporadic gastric carcinogenesis. AU - Yang, Misuk. AU - Kim, Hyunsoo. AU - Cho, Meeyon. PY - 2014/1/1. Y1 - 2014/1/1. N2 - Objective: Gastric cancer (GC) is histologically classified into intestinal type and diffuse type, and diffuse type cancer can be further subdivided into poorly differentiated carcinoma (PDC) and signet ring cell carcinoma (SRCC). Recent evidence suggests that early SRCC is an initial, differentiated form of diffuse GC that may evolve into PDC. This study aimed at identifying the molecular features of epigenetic methylation changes in histologic differentiation status of GC. Methods: Included in this study are 149 samples of paraffin-embedded tissues and 115 fresh endoscopically biopsied tissues. Multiple paraffin tissues involving normal (n=. 22), dysplasias (GDs, n=. 39), differentiated cancers (DCs, n=. 35), PDCs (n=. 33) and SRCCs (n=. 20) were included as an experimental group. Immunohistochemical distributions of cathepsins D and E were determined in normal mucosa, metaplastic, dysplastic, and cancerous lesions of the human stomach. Cathepsins D and E were localised in the foveolar epithelium and parietal cells of the normal gastric mucosa, but their intracytoplasmic distributions were different - cathepsin E distribution was even and diffuse in the cytoplasm while cathepsin D was found in coarse intracytoplasmic granules. Chronic inflammation and ulcer did not influence the distribution of these enzymes. No positive staining was obtained in the incomplete type of intestinal metaplasia, dysplasia, and well differentiated adenocarcinoma. Tumour cells of signet ring cell carcinoma and poorly differentiated adenocarcinoma cells, however, gave strong and diffuse stainings for cathepsins D and E in the cytoplasm. The results suggest that the distribution of cathepsins D and E is related to each specialised function of the foveolar epithelium and the parietal cells, and ...
This case is of interest for three reasons: the site of origin in the right colon (80% of cases reported develop distally to the splenic flexure), a biopsy previously taken from the mucosa demonstrated the presence of a signet ring cell carcinoma (endoscopic biopsies do not provide a conclusive diagnosis in the majority of cases reported) and hyaline with sparse amyloid nodules were detected in the extensive, dense fibrous tissue intermingled with tumour cells ...

Researchers are divided in their views regarding the most common site of the first lesion in sporadic CRC among young patients: Some of them reported that CRC was most often located in the ascending colon [7,8], whereas others noted the descending colon or rectum as the most common location, which is the same for adult patients [9]. In the present study, the descending colon or rectum was the most frequent location (16/19, 84.2%). Regarding histological subtype, adenocarcinoma is the most common type, being found in 85% of adult CRC patients, followed by mucinous adenocarcinoma (10%-15%) and signet ring cell type (1%) [10]. Unlike CRC in adults patients, however, in young patients, the most common types were mucinous adenocarcinoma (6/19, 31.5%) and signet ring cell type (2/19, 10.5%). Normally, mucinous adenocarcinoma and signet ring cell type result in poorer outcomes compared with adenocarcinoma. Poor outcomes were also found in this study as 4 of 6 patients (66%) with mucinous adenocarcinoma ...
Introduction: Gastric cancer is the fifth most commonly diagnosed cancer and the third leading cause of cancer related death worldwide. There are two types of gastric carcinoma: intestinal, characterized by cohesive tumor cells arranged in glandular formation, and diffuse, which is poorly cohesive with crescent-shaped eccentric nuclei. The diffuse type of gastric carcinoma, including signet cell carcinoma, is common in patients less than 50 years of age and is associated with advanced disease and poor prognosis. Diffuse type of carcinoma mostly found in patients without classic risk factors for gastric carcinoma such as untreated H. Pylori, or smoking history. Signet cell carcinoma has a high rate of bone marrow metastasis, and can present with severe cytopenias. Once bone marrow metastasis is diagnosed, average life expectancy is 11 to 121 days. Though the global incidence of gastric carcinoma has declined since 1970, incidence of diffuse-type carcinoma has increased by ten-fold. Case presentation: A
A case of signet-ring cell lymphoma affecting bone marrow is reported. The tumour presented as multiple lytic lesions in the lumbosacral spine. A bone biopsy specimen showed the typical appearances of signet-ring cell lymphoma, and the cells stained positively with antiserum to CD20, though neither immunoglobulin light or heavy chains could be shown within the vacuoles. The patient subsequently responded to chemotherapy.. ...

For CDH1 mutation carriers, the current consensus recommendation is to consider prophylactic gastrectomy, performed by an expert surgeon, in the third decade of life. This recommendation is made because, although asymptomatic CDH1 mutation carriers have been identified to have multifocal tiny signet ring cell cancers, these are not reliably detected at gastroscopy, even with advanced imaging techniques.. The risk of gastric cancer is less than 1% under the age of 20 years, so annual surveillance gastroscopy with targeted and random biopsies, according to an international protocol, is recommended for CDH1 mutation carriers aged 16-20 years.. Breast surveillance guided by a specialist with annual breast MRI (which can be combined with mammography) starting at age 30 years is recommended for women with a CDH1 mutation.. Carrying a CDH1 mutation has significant implications. Expert care is important but not always easy to access. Probands and families presenting purely with breast cancer in whom a ...
Once a CDH-1 mutation has been identified in an asymptomatic individual, the therapeutic options currently available are either endoscopic surveillance with the aim of identifying an early curable lesion or prophylactic gastrectomy.. Endoscopic gastric cancer surveillance is unproven. The problems associated with this approach include the difficulty in identifying submucosal lesions and sampling bias in a macroscopically normal appearing mucosa. However, if there is a site predilection for intramucosal disease, as suggested by Charlton and colleagues17 in this issue of Gut[see page 814], then it may be possible to perform targeted biopsies. In their series of six cases the signet ring carcinoma foci density was five times greater in the transitional zone than in the body or antral mucosa. This region occupies less than 10% of the gastric mucosal surface and is characterised histopathologically by a lack of gastric secreting G cells. Hence it is not an easily identifiable area endoscopically. ...

Network models are widely used to describe complex signaling systems. Cellular wiring varies in different cellular contexts and numerous inference techniques have been developed to infer the structure of a network from experimental data of the networks behavior. To objectively identify which inference strategy is best suited to a specific network, a gold standard network and dataset are required. However, suitable datasets for benchmarking are difficult to find. Numerous tools exist that can simulate data for transcriptional networks, but these are of limited use for the study of signaling networks. Here, we describe SiGNet (Signal Generator for Networks): a Cytoscape app that simulates experimental data for a signaling network of known structure. SiGNet has been developed and tested against published experimental data, incorporating information on network architecture, and the directionality and strength of interactions to create biological data in silico. SiGNet is the first tool to simulate ...

Appendix cancer, also called appendiceal cancer, is a rare malignancy of the vermiform appendix, affecting 1,000Americans envirosns each year. The most common tumorof the appendix is carcinoid, but not all appendicealcarcinoids are malignant. The genesis of appendix cancerusually appears in the size of about 2 cm or less, affecting a small fingerlike pouch of the large intestinecalled cecal (or caecal) appendix or vermix). The presence in the lymph nodes or invasion of adjacent tissues in the Annex is a sign of seriousness of the disease.. Among many types, adenocarcinomas are the most common primary carcinoid malignant tumors of theappendix. However, adenocarcinoma originating in the appendix is rare, and generally looks like a microscopique colon adenocarcinoma (acancer that develops in the glandular tissues). Although small in the beginning,Primary signet-ring cell carcinomaof the colon and rectumdo not have a good prognosis. Patients tend to die early with the disease.. Most mucinous ...

Introduction. Accurate diagnosis of melanoma is the most important prerequisite for optimal clinical management. Examination of hematoxylin and eosin (H&E) stained sections using conventional microscopy remains the gold standard for diagnosis of melanocytic lesions and as such most of the criteria used in the American Joint Committee on Cancer (AJCC)/College of American Pathologists (CAP) recommendations for reporting specimens with melanoma are based on routine examination of histologic slides.1 However, when lesions exhibit architectural and cytologic features that overlap both melanocytic nevi and melanomas, histologic examination alone may be insufficient for diagnosis. Being one of the ultimate histologic mimickers, melanoma may present in a range of histologic variants with spindle cell, desmoplastic, clear cell, balloon cell, signet ring cell, small cell, plasmacytoid and rhabdoid phenotypes.2-4 Rarely, even establishing melanocytic lineage of a tumor may become a challenging task, ...
E-cadherin (epithelial cadherin, CDH1, OMIM# 192090) is a member of the cadherin family of adhesion molecules, which are transmembrane glycoproteins mediating calcium-dependent cell-cell adhesion1. CDH1 germ line mutations have also been identified in a small portion of early onset DGC patients without a family history 5, 6, 12. Associations between CDH1 germ line mutations and both lobular breast cancer and signet ring carcinoma of the colon have been reported in DGC families 7, 8, 9. DGC is a highly penetrant autosomal dominant disorder that has been reported to occur in many ethnicities. The offspring of an affected individual has a 50% risk of also being affected. The estimated cumulative risk of gastric cancer by age 80 years is 67% (95% CI: 39-99) for men and 83% (95% CI: 58-99) for women10. Women also have a 39% risk for lobular breast cancer10. ...

The presence of signet ring morphology should alert the pathologist to consider the diagnosis of primary and metastatic malignancies in the prostate; however, it does not exclude the possibility of nephrogenic adenoma. In this case, a low Ki67 index, lack of atypia, small size and the typical prior history of surgical trauma favored the interpretation of the lesion as nephrogenic adenoma ...
