Background In the context of personalized cancer care, patients (pts) should receive the therapy that best meets their needs while optimizing the use of healthcare resources. A budget impact model was developed to compare each of the most commonly used mCRC treatment sequences - from 1st line to 3rd line in a Canadian context.. Methods This Excel based model considers the price of each agent, the number of cycles per treatment course, the relative dose intensity of the regimens to determine the cost of the most commonly used mCRC therapies and calculates the costs per overall sequence from 1st line to 3rd line. It takes into account the percentage of pts who benefit from a R0 resection post 1st line chemotherapy and who forego subsequent treatments. Factors such as different percentage use of 1st line FOLFOX vs FOLFIRI and pts volumes per line of therapy are considered in this model. Users can choose the scenarios they would like to examine. Key variables can be extracted through expert ...
Downloadable (with restrictions)! Controlling the cost of chronic diseases remains one of the vexing problems of developed and developing nations alike. Taiwan, faced with rapidly escalating healthcare costs associated with End Stage Renal Disease (ESRD) services utilization, imposed an outpatient dialysis global budget (ODBG) on outpatient dialysis care. This study, using a before and after study design with a comparison group, assessed the impact of this policy innovation on outpatient, inpatient and emergency room utilization. Using a difference in difference (DID) strategy and the generalized estimating equation (GEE) approach, this study found providers responded to these changes through cost reduction and revenue enhancement strategies. This study extends our understanding of provider responses to changes in reimbursement policies that target one segment of the continuum of care required by chronic disease patients.
By McGill Reporter Staff. Including significant spending on university-level research in the latest federal budget brought Finance Minister Joe Oliver (BA61/BCL64) some kudos from Canadas universities this week.. The first balanced budget in seven years, with a projected surplus of $1.4 billion in 2015-16, includes an investment of $1.33 billion spread out over six years, beginning in 2017-18, in research infrastructure. These funds are intended to help to train, attract and retain top researchers at Canadian universities.. The budget includes a significant investment of $1.33 billion over six years in research infrastructure through the Canada Foundation for Innovation, Principal Suzanne Fortier said. This investment is the second significant measure announced by the federal government in two years, after the creation of the Canada First Research Excellence Fund in Budget 2014 - a $1.5 billion investment in support of research and innovation.. The first round of CFREF funding, some $350 ...

I want to be able to tell the American people we have truly solved the deficit problem. For me to do that, a budget agreement must meet these tests:. It must include the measures Ive recommended to increase economic growth and reduce dependence on foreign oil.. It must be fair. All should contribute, but the burden should not be excessive for any one group of programs or people.. It must address the growth of governments hidden liabilities.. It must reform the budget process, and further, it must be real.. I urge Congress to provide a comprehensive five-year deficit reduction program to me as a complete legislative package-with measures to assure that it can be fully enforced. America is tired of phony deficit reduction, or promise-now, save-later plans. Enough is enough. It is time for a program that is credible and real.. South Africa is home to the largest number of people living with HIV in the world, as well as the worlds largest public-sector antiretroviral treatment (ART) programme. Despite the absolute majority of it being domestically funded, planning and budgeting for this programme has in the past been based on assumptions regarding target population and unit cost and on politically expedient coverage targets. The aim of this thesis was to improve on this situation by developing a budget impact model that could project the number of adults and children in treatment based on sound epidemiological methods, and calculate the cost of treating them based on the results of detailed bottom-up cost analyses at relevant clinics and hospitals in South Africa. The thesis describes the methods used in generating the inputs for the model, including the outpatient and inpatient cost of ART provision to adults and children of different ages, and the rates of CD4 cell count development, mortality, loss to follow-up, ...
When you use budget planning, budget planning rules are added to advanced rules and account structures, and budget planning tasks and elements are added to Budgeting workflows. The Budget planning and Security purposes are also added to organization hierarchies. Budget planning is integrated with other modules, so you can bring in information from previous budgets, actual expenditures, fixed assets, and human resources. Budget planning is also integrated with Microsoft Excel and Microsoft Word, so that you can use these tools to work with the budget planning data. For example, a budget manager can import a departments budget request into an Excel worksheet from a budget plan scenario. The data can be analyzed, updated, and charted in the worksheet, and then published back to the budget plan lines. ...

President Trump today released his federal fiscal year (FFY) 2018 budget proposal. While the budget request would cut a number of discretionary programs to accommodate increased defense spending, the President does not propose cuts to the Medicare or Medicaid programs. The President proposes a 20 percent ($15.1 billion) cut to the Department of Health and Human Services, including a $6 billion cut to the National Institutes for Health (NIH), which would negate funding boosts included in the 21st Century Cures Act. The cut to the NIH is likely to encounter strong opposition from both Republicans and Democrats, who passed the 21st Century Cures Act in December. In addition, the budget cuts $403 million from health professions and nurse training programs, and increases, by $500 billion, funding to expand opioid misuse prevention efforts and treatment for addiction. Although the President makes annual budget recommendations, Congress is under no obligation to adhere to these proposals. Before ...

Methods: A budget impact analysis was performed to evaluate the potential financial impact deriving from Fractional exhaled nitric oxide. The analysis considered a 5-year time horizon and Colombian National Health System perspective. The incremental budget impact was calculated by subtracting the cost of the new treatment, in which FeNO is reimbursed, from the cost of the conventional treatment without FeNO (management based on clinical symptoms (with or without spirometry/peak flow) or asthma guidelines (or both), for asthma-related). Univariate one-way sensitivity analyses were performed ...
ABSTRACT. Aim: To evaluate the budget impact of the vaccination of children with either 10 or 13-valent conjugated pneumococcal vaccine (PCV10 and PCV13) in light of the new clinical evidences of efficacy. Methods: The perspective is that of the healthcare system for the period in which the main clinical outcomes were reportedthat is 4 years after the immunization. A systematic review of available publications on the clinical efficacy and budget impact analyses of the two vaccines was conducted. The efficacy was measures as an overall percentage of prevented cases for the main groups of invasive pneumococcal diseases (IPDs), community acquired pneumonia (CAP) and acute otitis media (AOM).Direct medical costs at macro level of the health care system have been calculated. The cost of infections therapy was multiplied by the expected number of patients after the immunization with either 10 or 13-valent vaccine, and the difference between the twocosts was derived. Results: The results show that the ...

The tea leaves, when they can read them, will likely present a bleak picture - the Massachusetts Budget and Policy Centers testimony contemplates a scenario where fiscal 2021 tax collections land $5 billion short of estimates agreed to in January.. Under normal circumstances, the state House of Representatives would be releasing its fiscal 2021 budget this coming week for consideration later this month.. Instead, lawmakers are basically starting the budget process over from scratch with an uncertain timetable for actually passing a budget and without any announced plans for tackling the more than $44 billion spending plan while social distancing.. For starters, budget talks slated for this past week were pushed back, scuttled by a live stream failure. Lawmakers, economic experts and the administration will try to assess the fiscal picture again in the week ahead, technology permitting.. The current state budget crisis in the EU and the numerous futile efforts to solve the problem has brought back the fraction of people that argument in favour of an exit strategy of Germany from the European monetary union (EMU) or even the break-up of the EMU in total. This paper investigates for the case of Germany, whether or not a return to a new deutsche mark would be beneficial. It applies the macro-econometric input-output model INFORGE for this analysis, as it is able to quantify direct and indirect effects on inter-industrial level as well as on the demand and supply sides of the economy. Above that, INFORGE considers sectoral effects which are extremely important for the evaluation of this impact analysis. The quantitative results of the computed projection shows, that a return to a national currency would lower Germanys growth path mainly due to the expected appreciation of the new currency. A second scenario, which assumes a worsening of the crisis within the remaining EMU

Over the last four-and-a-half years, we have been developing a model to address the financial challenges of rural hospitals in Pennsylvania by changing the fiscal environment in which they operate. This work was initiated under Dr. Karen Murphy, my predecessor at the Pennsylvania Department of Health.. Our initiative creates a framework through which a hospital works with the department of health and all of its major insurance payers - Medicare, Medicaid, and commercial plans - to transition from a fee-for-service arrangement to a monthly global budget. The global budget emphasizes population health and outpatient services and isnt dependent on fluctuations in volume of patients, particularly in terms of inpatient admissions.. While the planning for this initiative began with the Center for Medicare & Medicaid Innovation (CMMI) in 2015, implementation began in January 2019. In January we worked with five hospitals to develop a transformation plan in collaboration with all the major payers. This ...

I just returned from a long weekend in the Caribbean, attempting to recreate the scenery of (and a few scenes from) The Bachelor. Given the ubiquity of Wi-Fi coverage, I was able to stay connected with my favorite newspapers and magazines: iPhone in one hand, Mojito in the other. Just as I was feeling like a one-percenter, I stumbled upon a story about Newt Gingrichs propensity to use private air travel. According to the Post, for at least two years [Gingrich] insisted upon flying private charter jets everywhere he traveled, with most of the costs-ranging from $30,000 to $45,000 per trip-billed to [his company] American Solutions.. The hefty price tag for private air travel is in line with a quote I recently obtained from a representative of a major jet leasing operation, which allows customers to purchase 25 hours of private air travel at $166,000 on a small Cessna jet that comfortably seats five. (Use it in 18 months or lose it.) This luxury amounts to $6,640 per hour of flight time or ...
Punch Editorial Board. As the 2017 financial year gets under way, attention has been rightly riveted on the capacity or lack of it of the federal budget to halt the recession. While the economic policies of the centre are indeed decisive, Nigerians would do well to pay closer attention to the states where at least N2.6 trillion is to be spent on salaries and other overheads and over N3.53 trillion on capital ventures. More than ever before, Nigeria should strive for the ideal where the component states become drivers of economic growth rather than mere bureaucratic cost centres.. That basic function of federating states has been lost to decades of ruinous centralising arrangements that have reduced the 36 states to mere cost units of a mighty central government. This is reinforced in the budgets released by 33 states that, according to a report by the BusinessDay Research Intelligence Unit, are to spend a combined N6.22 trillion in 2017. The fatal flaws in their spending plans are twofold: apart ...

WASHINGTON -- The federal budget deficit will be $20 billion higher in fiscal 1999 than originally projected unless Congress gets health care spending under control, the Congressional Budget Office reported this week.. In its updated economic and budget outlook, released late Wednesday, the nonpartisan budget office revised its January figures and projected that the deficits for fiscal years 1994 through 1996 will be lower than anticipated because of unexpectedly strong economic growth.. However, unless Congress suppresses the increasing costs of mandatory health care spending, specifically Medicare and Medicaid, the CBO said that the deficit will begin to climb again slowly by the mid-1990s and then pick up steam in 1999.. Without cost containment measures, the two health care programs together will grow at a rate of 11% annually, the budget office said, while all other federal programs are projected to grow less than 4% a year.. The report details the growth in health costs, but makes no ...
Congressional Budget Reports accompany the annual federal budget that Congress develops each year. In these reports, Congress explains how much funding it is allotting to federal agencies and programs and what its priorities are for the use of the funds. Appropriations Committee members create the federal budget and these reports based on a variety of inputs, including constituents requests. U.S. Pain Foundation-in collaboration with a group of other like-minded organizations-has made two requests this year:. 1) For Congress to allocate $2M to the Centers for Disease Control and Prevention (CDC) to collect, analyze, and report annually on population health data on chronic pain.. An estimated 50 million American adults have chronic pain, making it a highly prevalent, costly, and disabling health condition. Yet, despite its pervasiveness, who is affected, the extent of resulting disability, the nature and accessibility of effective pain management, and related costs all remain largely unknown to ...

The recent federal budget announced continued funding for CANARIE, the national research and innovation network. This was both good and not so good news.. The reduced allocation was no surprise in a restraint budget but the two year mandate was. Normally CANARIE has been given a five year mandate. Five years is a good amount of time to plan, execute, evaluate, and prepare again. Networks and network services are complex; they require careful thought and they take time to evolve.. A two year mandate from government suggests uncertainty. And that is a concern.. The noise about R&D leading up to the federal budget was quite loud. The attempts to address it were modest. Lost in this was the critical role networks play in Canadas R&D capability.. The problem is that CANARIE and other regional networks are invisible and taken for granted. Its the tragedy of the commons. It is very hard to make real and apparent the value.. Infrastructure in the digital realm is not obvious like bricks and mortar. ...
