OBJECTIVE: The series of papers in this issue was developed to examine the use of health technology assessment in policies toward prevention-specifically toward mass screening-in European countries. The papers actually examined three screening strategies: mammography screening for breast cancer, prostate-specific antigen screening for prostate cancer, and routine ultrasound in normal pregnancy. METHODS: Papers were sought from the member states of the European Union, plus Switzerland. Ultimately, nine acceptable papers were received, and were reviewed, revised, and edited. RESULTS: Screening is an accepted strategy in many countries for reducing the burden of disease through early detection and intervention. In part, this is because of successful screening programs that have been evaluated and implemented in many countries. The objectives of this paper are to describe the status and development of health technology regulation with regard to coverage decisions and utilization, and to analyze the current situation of health technology assessment (HTA) in Germany. The relevant literature for controlling health technologies was identified by searching the literature and databases and through personal contacts. The literature was analyzed with regard to the different sectors in the healthcare system. For the analysis of the current state of HTA in Germany, a national survey was carried out. In addition, the names of topics under assessment were collected. The results show that coverage decisions in the ambulatory sector appear to be much more regulated than those in the inpatient sector. The same is true for diffusion and usage of technologies. The strict separation of the hospital and the ambulatory care sector in Germany constitutes a barrier to regulation and to making HTA an effective instrument in Germany. Until ...

OBJECTIVES: The aim of this study was to review and compare current health technology assessment (HTA) activities for medical devices across non-European Union HTA agencies. METHODS: HTA activities for medical devices were evaluated from three perspectives: organizational structure, processes, and methods. Agencies were primarily selected upon membership of existing HTA networks. The data collection was performed in two stages: stage 1-agency Web-site assessment using a standardized questionnaire, followed by review and validation of the collected data by a representative of the agency; and stage 2-semi-structured telephone interviews with key informants of a sub-sample of agencies. RESULTS: In total, thirty-six HTA agencies across twenty non-EU countries assessing medical devices were included. Twenty-seven of thirty-six (75 percent) agencies were judged at stage 1 to have adopted HTA-specific approaches for medical devices (MD-specific agencies) that were largely organizational or procedural. ...
While the Health Technology Assessment (HTA) community has acknowledged the importance of public and consumer involvement in the HTA process, very few studies have examined how technology-related findings may be reported by the media to the broader public. This paper compares the content of press articles with the content of three Canadian HTA reports that respectively assess electroconvulsive therapy, first-trimester prenatal screening for Down syndrome, and prostate-specific antigen screening for prostate cancer. We qualitatively and quantitatively analyzed 186 press articles addressing the same technologies. Our results show that beyond stylistic emphasis, there is an important overlap between media coverage of these technologies and the content of HTA reports. Introduction: As the pace of health technology innovations increases greater pressure is placed on HTA agencies to review and make recommendations promptly. One response to this pressure is the use of Rapid Reviews (RRs). As part of the HTA system RRs can support decision making in an environment with increasing competing demands by synthesising evidence to support informed decisions in a timely manner, without sacrificing scientific rigor. Since 2009 all new drugs in Ireland are first subject to a RR. After which a full HTA may or may not be required. The objective of this study is to examine what factors determine the outcome of a rapid review, i.e. if a full HTA is recommended or not. Methods: Data on drug evaluations from 2009 to 2016 were extracted from the NCPE websites. It must be emphasised that the output from NICE and HTBS will be advice on clinical- and cost-effectiveness and not on need, budgetary priority or affordability. Assessments will certainly inform the process of rational and equitable distribution of resources, but they are not about what a health care provider should afford or can afford, nor are they about assessing population needs or prioritising these against available resources. However, regardless of questions of priority and affordability, any reasonable person would surely not condone the provision of treatments that were either only equivocally beneficial or where the benefit per £ (sterling), or per hour of clinical time taken from some other activity, was less than an alternative. This study provides outcome data on the impact of the Health Technology Board for Scotlands grading model on referrals to ophthalmology from a primary care based diabetic retinopathy screening programme using digital photography that meets the recommended resolution agreed by both the Health Technology Board for Scotland and the National Screening Committee for England.9,12. The Health Technology Board for Scotlands grading model appears to be effective in triaging patients needing review by the third level grader. Only 302 (5.4%) patients images needed review by the third level grader and, of those, 190 (3.4%) were finally referred to ophthalmology services. Only 10% of the images passed to the level 3 grader were downgraded for review in 12 months. The inspiration for the present assessment of the nutritional care of medical patients is puzzlement about the divide that exists between the theoretical knowledge about the importance of the diet for ill persons, and the common failure to incorporate nutritional aspects in the treatment and care of the patients. The purpose is to clarify existing problems in the nutritional care of Danish medical inpatients, to elucidate how the nutritional care for these inpatients can be improved, and to analyse the costs of this improvement. Qualitative and quantitative methods are deployed to outline how nutritional care of medical inpatients is performed at three Danish hospitals. The practices observed are compared with official recommendations for nutritional care of inpatients. Factors extraneous and counterproductive to optimal nutritional care are identified from the perspectives of patients and professional staff.
