Biliary-enteric anastomosis (BEA) is a common surgical procedure performed for the management of biliary obstruction or leakage that results from a variety of benign and malignant diseases. Complications following BEA are not rare. We aimed to determine the incidence and the factors associated with early complications occurring after BEA for benign diseases. We reviewed the medical records of all patients who underwent BEA for benign diseases at our institution between January 1988 and December 2009. The primary outcome was early post operative complication. Logistic regression analysis was done to identify factors predicting the occurrence of complications. Records of 79 patients were reviewed. There were 34 (43%) males and 45 (57% females). Majority (53%) had choledocholithiasis with impacted stone or distal stricture, followed by traumatic injury to the biliary system (33%). In those patients, there is a relatively high incidence of strictured HJ ranging from 2% to 25% [2, 3, 7-10]. The endoscopic management of these complications seems to be the best option and least invasive compared to interventional radiology or reoperation [11-13]. p16Ink4 and p15Ink4B are cyclin-dependent kinase 4 inhibitors and link to the regulation of cell cycle in mammalian cells. The genes encoding these inhibitors are located at 9p21, which is a frequent site of allelic loss in various types of tumors. Twenty-five primary biliary tract cancers were examined for somatic mutations in p16Ink4/CDKN2, p15Ink4B/MTS2, p53, and K-ras genes and allelic loss of 9p21 by microsatellite analysis. Four biliary tract cancer cell lines were analyzed for homozygous deletions and point mutations. We found frequent homozygous deletions in p16Ink4/CDKN2 and p15Ink4B/MTS2 genes in the biliary tract cancer cell lines. Each cancer cell line had alteration of either p16Ink4/CDKN2, p15Ink4B/MTS2, or p53 genes. In primary tumors, 16 of 25 (64%) biliary tract cancers had point mutations in the p16Ink4/CDKN2 gene. His surgical, academic and executive experience also includes service as associate professor of Surgery and director of Renal and Pancreatic Transplantation at Mount Sinai Medical Center, and as medical director of the New York Organ Donor Network. Board-certified in general surgery, Dr. Goldstein is a fellow of the American College of Surgeons, and a member of the American Society for Transplant Surgeons, American Society for Transplantation, and the Association of Academic Surgeons. Percutaneous drainage procedures applied to obstructive jaundice offer promising results. Methods: Nineteen patients with obstructive jaundice treated with percutaneous drainage were selected for this retrospective study. The objective was to review the indications, technique and results. All procedures were performed under imagenological guidance (ecography, radioscopy or axial computed tomography) and a pigtail catheter was placed using the Seldinger technique.
