Plasma and cerebrospinal fluid beta-endorphin concentrations were radioimmunologically assayed in dogs subjected to spinal cord ischemia induced by infrarenal aortic ligature and in control sham-operated dogs. Plasma beta-endorphin levels rose significantly following surgery in control dogs but were unaffected by spinal ischemia. On the other hand, a significant increase in cerebrospinal fluid beta-endorphin concentration occurred after spinal ischemia, while surgical stress had no significant effect. Thus, the origins of plasma and cerebrospinal fluid beta-endorphin may be different, with the former secreted from the hypophysis and the latter from nervous tissue. Observed changes in cerebrospinal fluid beta-endorphin concentration could be related to the ischemic lesion of nervous tissue while the changes in plasma levels may reflect general stressing factors such as the surgery in our experiments. ...

Current evidence suggests that endogenous opioids are released in response to various stressors and that these opioids interact with cells of the immune system, and modulate immune function. However, data substantiating this are conflicting. Therefore, this study endeavored to clarify the role of beta-endorphin (BE) in exercise and immune function. Beta-endorphin exhibited a diurnal rhythm in the horse with peak plasma levels occurring at 0900 hours. Analgesia, tachycardia and mydriasis were also observed at this time, mimicking the effects of morphine in the horse. Therefore, subsequent experiments were performed at 0900 hours during times of increased basal opioid activity. Beta-endorphin levels were increased in response to maximal exercise in the horse. Naloxone has been reported to have potential benefit in the treatment of stroke. We evaluated the effect of naloxone in a double-blind trial conducted with 15 stroke patients whose deficits ranged from 8 to 60 hours in duration. All but one patient sustained a cerebral infarction. Neurologic function was assessed before and five minutes after each of two injections given to each patient in a double-blind fashion. The injections consisted of naloxone (0.4 mg in 3 patients and 4.0 mg in 12 patients) and saline. Prior to the trial, samples of plasma were obtained for determination of immunoreactive beta-endorphin for each patient. Four patients showed minimal improvement following injection of naloxone, while five patients exhibited a slightly greater improvement following saline injection. Participants will be breast cancer patients receiving radiation therapy at Massachusetts General Hospital. There will be two groups of patients: those receiving adjuvant radiation therapy alone following surgical tumor resection, and those receiving adjuvant chemotherapy and radiation therapy following surgical tumor resection. Patients receiving any radiation regimen detailed below in section 2b are eligible for the study. In this study the investigators will be drawing blood for beta-endorphin measurement prior to, at various times during, and following patients treatment regimens. The variations of plasmatic beta-endorphin and cortisol levels are studied in 3 groups of patients. These subjects are submitted to abdominal surgery following 3 different anaesthetic schemes: neuroleptanalgesia (droperidol and fentanyl), pure analgesia (fentanyl), association halothane - small dose of fentanyl. The study covers the period from premedication through recovery. The results of this work demonstrates that the average level of beta-endorphin in neuroleptanalgesia and pure analgesia series is stable; there is an increase in the halothane series just after the incision until waking up. The average level of cortisol is stable for the 3 series until incision. It is strongly increased in the neuroleptanalgesia series and in the halothane series. This increase is delayed and reduced in the fentanyl series.
