The incidence and etiological classification of valvular diseases were examined on 358 cases from 3,000 consecutive autopsies of more than 60 years of age. The incidence of valvular disease was 11.9% (358 out of 3,000 cases). Mitral stenosis was found in 23 cases (6.4%), of which 21 cases were rheumatic and the remaining 2 were mitral ring calcification (MRC). Mitral regurgitation was observed in 126 cases (35.3%): 69 of papillary muscle dysfunction, 26 of mitral valve prolapse (MVP), 16 of MRC, 9 of ruptured chordae tendineae, 3 of rheumatic and 3 of congenital. Aortic stenosis was noted in 33 cases (9.2%): 27 of calcified, 5 of rheumatic and one of congenital. Aortic regurgitation was found in 169 cases (47.2%): 112 of degenerative, 47 of syphilitic, 7 of rheumatic and 2 of aortitis syndrome. There were 6 cases (1.7%) of tricuspid regurgitation. Physician-coded verbal autopsy (PCVA) is the most widely used method to determine causes of death (CODs) in countries where medical certification of death is uncommon. Computer-coded verbal autopsy (CCVA) methods have been proposed as a faster and cheaper alternative to PCVA, though they have not been widely compared to PCVA or to each other. We compared the performance of open-source random forest, open-source tariff method, InterVA-4, and the King-Lu method to PCVA on five datasets comprising over 24,000 verbal autopsies from low- and middle-income countries. Metrics to assess performance were positive predictive value and partial chance-corrected concordance at the individual level, and cause-specific mortality fraction accuracy and cause-specific mortality fraction error at the population level. We adapted the InterVA method to extrapolate causes of death for stillbirths and neonatal deaths from verbal autopsy questionnaires, using data from Malawi, Zimbabwe, and Nepal.. Methods. Our results show that the responses given by mothers and birth attendants to nearly all questions on the perinatal verbal autopsy questionnaire are highly concordant (80% or greater agreement) when the interviews take place within seven days after the perinatal death. Results:. Of 285 patients, 47 underwent both virtual and medical autopsy. Of 196 clinical diagnoses made before death, 173 (88%) were identified by virtual autopsy and 183 (93%) by medical autopsy. Fourteen new major and 88 new minor diagnoses were detected by any autopsy method. The main diagnoses missed by virtual autopsy were cardiovascular events (9 of 72) and cancer (12 of 30). BACKGROUND: It is difficult to estimate the diagnostic accuracy of biopsy for prostate cancer because men with negative biopsy do not undergo radical prostatectomy and thus have no confirmation of biopsy findings. METHODS: We performed 18-core needle biopsies on autopsy prostates from 164 men who had no history of prostate cancer. Six-core biopsies were taken from each of the mid peripheral zone (MPZ), the lateral peripheral zone (LPZ), and the central zone (CZ). We tested associations between age and tumor characteristics and analyzed the sensitivity of biopsies at each site. All statistical tests were two-sided. RESULTS: Prostate cancer was present in 47 (29%) prostates. Of the 47 cancers detected, 20 were clinically significant according to histologic criteria. Of 215 patients dying between the years 1984 and 1988, 212 were included in this study (autopsy rate, 98.6%). Major discrepancies in clinical diagnosis versus the anatomic diagnosis at autopsy were found to occur in approximately 30% of patients. The incidence of errors in diagnosis which may have impacted on survival in these groups of injured patients was quite low (5.1%). These data support the continued practice of obtaining autopsy in all patients dying from trauma. The autopsy findings of a 66-year-old man who died of severe anemia caused by undiagnosed prostate cancer are presented. The findings suggested the condition of severe anemia. The infiltration and metastasis of the prostate specific antigen (PSA) positive adenocarcinoma was observed at the bladder, rectum and bone. Blood clot attached to the mucosal surface and trabeculation of the bladder and blood clot in the rectum was observed. Furthermore, the serum PSA level was markedly high, and thought to be valuable for a postmortem diagnosis of prostate cancer. The existence of rectal infiltration of the prostate cancer is a rare complication, and could indicate particularly advanced disease. The anemia found in this case was suspected to be caused by the combination of massive metastasis to the bone and active blood loss from the mucosa of the bladder and rectum. Moreover, the utility of postmortem measurement of serum PSA in the diagnosis of prostate cancer was suggested.
