The prevalence of prescription opioid use among drivers has increased markedly in the past two decades. The purpose of this study is to assess the associations of prescription opioid use and alcohol use with the risk of fatal crash involvement in US drivers. We performed a population-based case-control study using toxicological testing data from two national data systems. Cases (n = 3606) were drivers involved in fatal motor vehicle crashes selected from the Fatality Analysis Reporting System and controls (n = 15,600) were drivers participating in the 2007 and 2013 National Roadside Surveys of Alcohol and Drug Use by Drivers. Multivariable logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (95% CIs) of fatal crash involvement associated with prescription opioid use with and without the presence of alcohol. Motor-vehicle crashes (MVCs) are the leading cause of death for persons aged 15-20 years in the United States (1). Although the 11.9 million young drivers aged 15-20 years constituted only 6.7% of the total number of licensed drivers in the United States during 1995, they represented a disproportionate 14% of all drivers involved in fatal MVCs. Automobile driving requires multiple brain functions. However, the brain regions related to driving behavior are unknown. Therefore, we measured activity of the frontal, parietal and occipital lobes during driving using functional near-infrared spectroscopy (fNIRS). Cortical activation patterns were examined in relation to driving behaviors, such as steering motion, accelerator pedal motion, and speed control. Six healthy adults participated in the experiment. Cerebral oxygen exchange (COE) was calculated based on the oxyhemoglobin and deoxyhemoglobin concentrations measured by fNIRS. The COE and driving behavior data were collected every 1 m and averaged for all subjects. Functional NIRS data for all 98 channels were extracted using principal component analysis. Motor vehicle crashes involving impaired driving are a significant public health problem in the U.S. In 1994, there were an estimated 16,900 fatalities in alcohol-related crashes, which is the lowest number in many years (NHTSA, 1995). Concern over older driver high traffic fatality rates has resulted in an effort to identify risk factors and to develop methods of assessment. This study examines two attention-related tasks, Useful Field of View (UFOV) and Change Blindness (CB), in relation to vision and cognitive test batteries and driving performance measures collected using a simulator and an instrumented vehicle. Eight older adults with Alzheimers disease and nine comparison subjects between 64 and 81 participated. Factor analysis results indicate that UFOV and CB relate to different factors. While UFOV relates to memory, decision-making, attention, and visual spatial ability, CB relates to vision and attention. The type of images used on a CB task influence how the task relates to driving performance measures. Researchers should be thoughtful when selecting images to include in CB tasks to maximize insight into real-world driving
Young novice drivers have crash rates higher than any other age group. To address this problem, graduated driver licensing (GDL) laws have been implemented in the United States to require an extended learner permit phase, and create night time driving or passenger restrictions for adolescent drivers. GDL allows adolescents to gain experience driving under low-risk conditions with the aim of reducing crashes. The restricted driving might increase riding with parents or on buses, which might be safer, or walking or biking, which might be more dangerous. We examined whether GDL increases non-driver travels, and whether it reduces total travels combining drivers and non-drivers. The objective of this study was to determine the prevalence of psychoactive drug and alcohol use among general drivers and predictors of the drug use in Thailand. One thousand six hundred and thirty-five motor vehicle drivers were randomly selected from five geographical regions of Thailand between December 2005 and May 2006. The prevalence of psychoactive drugs was determined using urine tests by gas chromatography/mass spectrometry (GC/MS). Among 1635 drivers, 5.5% were tested positive for breath alcohol with 2% having a level exceeding the legal limit (>50 mg%). Psychoactive drug was presented in 158 (9.7%) urine samples for drug analysis. The top 3 most frequently detected licit drugs were antihistamines (2.0%), sedative cough suppressant (0.7%) and benzodiazepines (0.2%).
