When ABIs are provided to patients who have lost their auditory nerve from causes other than NF2 (non-tumor or NT patients), the outcomes of the ABI are dramatically different. Colletti and colleagues in Verona, Italy, have demonstrated that about 50% of NT ABI patients have sentence understanding of more than 50% with only the sound from the ABI without speechreading (Colletti & Shannon, 2005). Several of these patients achieved speech understanding equivalent to the best performance of CI patients. These results demonstrate that electrical stimulation of the human brainstem (CN) can provide excellent speech understanding for some patients. Short stem prostheses provide conservative surgery and favorable metaphyseal load transmission. However, clinical long‐term results are lacking. Therefore, in vitro trials can be used to predict bone‐implant performance. In this in vitro study, primary stability and stress shielding of a new cementless short stem implant was evaluated in comparison to a straight stem using nine pairs of human cadaver femurs. Primary stability, including reversible micromotion and irreversible migration, was assessed in a hip simulator. Furthermore, changes in the pattern of cortical strain were evaluated. The short stem was more resistant to reversible micromotion and irreversible migration into retroversion. Axial stability was similar, with mean reversible micromotions of 9 µm for the short stem and 7 µm for the straight stem. N2 - Periprosthetic fractures of the humerus complicate 2-4% of total shoulder arthroplasties. Intraoperative fractures are more common than those that occur after surgery and are usually the result of faulty surgical technique. Patients with osteoporosis are at increased risk for periprosthetic fracture of the humerus, and these injuries are most common in patients with rheumatoid arthritis. Common mechanisms of intraoperative fracture include excessive external rotation forces because of poor operative exposure, improper starting hole location, and too forceful impaction. Intraoperative fractures should be stabilized with cerclage wires, and long- stem implants should be used if the fracture involves the shaft of the humerus. In less extensive cholesteatomas, especially when infection is well controlled before surgery, an intact canal wall operation may be the favoured procedure. This operation preserves the wall between the middle ear and mastoid. The principal advantages of the intact canal wall operation are a more normal canal and ear drum, and a greater possibility of hearing restoration. In addition, most patients with the intact canal wall operation can allow water in the ear. The chief disadvantage of the intact canal wall operation is that a regrowth of cholesteatoma may not be evident. Thus, many ear surgeons will delay rebuilding the bones of hearing for a year after an intact canal wall operation for cholesteatoma. The ear drum is opened at the second operation and the bones of hearing are then reconstructed. If a regrowth of cholesteatoma is found, the disease is again removed and reconstruction may be delayed for another 6 months or a year. The mammalian cochlea is a complex macroscopic structure due to its helical shape and the microscopic arrangements of the individual layers of cells. To improve the outcomes of hearing restoration in deaf patients, it is important to understand the anatomic structure and composition of the cochlea ex vivo. Hitherto, only one histological technique based on confocal laser scanning microscopy and optical clearing has been developed for in toto optical imaging of the murine cochlea. However, with a growing size of the specimen, e.g., human cochlea, this technique reaches its limitations. Here, we demonstrate scanning laser optical tomography (SLOT) as a valuable imaging technique to visualize the murine cochlea in toto without any physical slicing. This paper presents the effects of spontaneous random activity on information transmission in an auditory brain stem neuron model. In computer simulations, the supra-threshold synaptic current stimuli ascending from auditory nerve fibers (ANFs) were modeled by a filtered inhomogeneous Poisson process modulated by sinusoidal functions at a frequency of 220-3520 Hz with regard to the human speech spectrum. The stochastic sodium and stochastic high- and low-threshold potassium channels were incorporated into a single compartment model of the soma in spherical bushy neurons, so as to realize threshold fluctuations or a variation of spike firing times. The results show that the information rates estimated from the entropy of inter-spike intervals of spike trains tend toward a convex function of the spontaneous rates when the intensity of sinusoidal functions decreases. Furthermore, the results show that a convex function of the spontaneous rates tends to disappear as the frequency of the sinusoidal function

Tone bursts (2 ms duration at 8, 16 and 32 kHz) and click stimuli (1 ms duration) are presented (21 per sec) via a free field electrostatic speaker. The sound processor delivers bone conducted stimuli to the cochlea, bypassing the outer and middle ear..
