This observation was based on blood bank samples from individuals who later developed RA as well as prospective research studies in European arthralgia clinics. Whole hemolytic complement and three components (C1q, C4, and C3) were measured in pleural fluids obtained from 50 patients-23 with malignant disease, 6 with lupus erythematosus, 6 with rheumatoid arthritis, 3 with congestive heart failure, 3 with pulmonary embolism, and 9 in whom the cause of the pleural effusion was not ascertained. The mean values for whole complement and the three components were lowest in lupus erythematosus and rheumatoid arthritis patients. Compared with the malignant disease group, these differences were statistically significant (P , 0.01) for whole complement, C4, and C3. Anticomplementary activity was found in pleural fluids from some patients with lupus erythematosus or rheumatoid arthritis. To evaluate the cost effectiveness of TNF-α antagonist therapies for rheumatoid arthritis (RA) in the United Kingdom using data from the British Society for Rheumatology Biologics Registry (BSRBR). Methods. A simulation model is constructed to quantify the cost effectiveness of the TNF-α antagonist therapies (infliximab, etanercept and adalimumab) as a group versus traditional disease-modifying anti-rheumatic drugs, with a time horizon over the full patient lifetime. Participants are UK NHS patients in the BSRBR with RA who have failed at least two traditional disease-modifying anti-rheumatic drugs. The BSRBR aims to recruit all RA patients starting on a TNF-α antagonist agent and follows them 6 monthly via consultant and patient administered questionnaires. Theres an early peak about the age of 35 -- and its more common in women than men by about three to one -- and this is where you see typical rheumatoid arthritis. Theres another form of arthritis that occurs in older adults known as polymyalgia rheumatica, which has a lot of features of rheumatoid arthritis but tends to be a much milder and more indolent disease than rheumatoid arthritis in young patients.
