NTNU Open: Patient education and chronic inflammatory arthritis -coping and effect
Living with chronic inflammatory polyarthritis has influence on patients everyday lives and well-being. Patients thus have to learn how to live and cope with their chronic disease, and such topics are often addressed in patient education. More knowledge on coping with chronic inflammatory polyarthritis and the process of change that patients go through is requested. Such knowledge is especially important for those developing educational interventions. Moreover, the literature shows that group or individual-based education mainly show short-term effects, only few studies have investigated whether patient education have long-term effects. The main aim of this thesis was therefore to contribute with knowledge about the experiences of coping and the effect of patient education for people living with chronic inflammatory polyarthritis. To answer the main aim, two studies were conducted. A qualitative study to investigate the process of coping and a randomised controlled trial (RCT) to investigate ...
Abstract 11040: Arthritis: Its Prevalence, Risk Factors, and Association with Cardiovascular Diseases in the United States...
Introduction: Arthritis is associated with cardiovascular diseases (CVDs). However, there are limited studies on arthritis in a national representative sample. We therefore investigated the prevalence of arthritis, and whether cardiovascular risk factors were associated with arthritis. The results show an interaction effect on the association of arthritis with CVDs.. Methods: Data from 15888 subjects aged ≥40 years in the United States National Health and Nutrition Examination Survey 1999-2008 were analyzed.. Results: The overall prevalence of self-reported arthritis increased from 33.5% in 1999-2000 to 37.0% in 2007-2008 (P for trend=0.016), especially in subjects aged 40-49 years, men, non-Hispanic whites, and obese subjects (P=0.045, 0.015, 0.016, and 0.017 respectively). Among subjects with arthritis in 1999-2008, 35.3% had osteoarthritis (OA), 17.9% had rheumatoid arthritis (RA), and 10.2% had other types of arthritis, but 36.6% were unaware of their type of arthritis. Arthritis pain, the brain and the role of emotions | EmaxHealth
Marked by morning stiffness, joint aches, and flare-ups, the pain of arthritis tends to be acute and recurrent, in contrast to many chronic pain conditions. Arthritis pain therefore makes an ideal model for comparing common clinical pain with experimental pain. Inspired by this observation, researchers at University of Manchester Rheumatic Diseases Centre in the United Kingdom conducted the first study to compare directly the brain areas involved in processing arthritis pain and experimental pain in a group of patients with osteoarthritis (OA). Their results, published in the April 2007 issue of Arthritis & Rheumatism, shed light on the role of emotions in how patients feel arthritis pain. The study focused on 12 patients with knee osteoarthritis - 6 women and 6 men, with a mean age of 52 years. All subjects underwent positron emission tomography (PET), to measure and map 18F-fluorodeoxyglucose (FDG) uptake in the brain as an indicator of brain activity. PET scans were performed during three ...
Seronegative Bilateral Symmetrical Inflammatory Polyarthritis: Think Twice Before Starting Immunosuppression

The most common cause of bilateral symmetrical polyarthritis in the small joints is rheumatoid arthritis. However, if seronegative arthritis is involved, it could be the case that other underlying causes need to be diagnosed. This is particularly important for those coming from or living in developing countries where infectious causes should always be considered. The case of a young Nepali woman is presented in this article. She was referred as a case of seronegative rheumatoid arthritis for DMARDs therapy but this was not the case due to her origin from Nepal and seronegativity for RF, Anti-ccp, and ANA as well as faint macular skin lesions over her face and upper extremities, which the patients are not aware of. Consequently, skin biopsy was carried out which subsequently confirmed that the infectious cause of her polyarthritis was leprosy.. ...

Undifferentiated arthritis (UA) is defined as an inflammatory oligoarthritis or polyarthritis in which no definitive diagnosis can be made. We performed a literature review to assess the efficacy of various drug therapies in patients with UA. The literature search was conducted using electronic databases Pubmed, EMBASE and MEDLINE in adults with UA or early arthritis (not fulfilling the American College of Rheumatology (ACR) 1987 or ACR/European League Against Rheumatism (EULAR) 2010 criteria for rheumatoid arthritis). Drug therapy consisted of disease modifying antirheumatic drugs (DMARDs), biological agents and oral, intramuscular or intra-articular corticosteroids. Nine publications on eight randomised controlled trials (RCTs), two publications on two uncontrolled open-label trials and seven publications on three cohort studies were included. Temporary treatment with methotrexate (MTX), abatacept and intramuscular corticosteroids were demonstrated in RCTs with 12 months to 5 years follow-up ...

Arthritis pain: Treatments absorbed through your skin - Mayo Clinic
Opinions differ on the effectiveness of over-the-counter topical pain medications. While many people say these products help relieve their arthritis pain, scientific research reveals only modest benefits.. Some products work only slightly or no better than a placebo in relieving arthritis pain. Capsaicin might be more effective when used with other treatments, such as pills containing nonsteroidal anti-inflammatory drugs (NSAIDs).. Application of capsaicin creams can make your skin burn or sting, but this discomfort generally lessens within a few weeks of daily use. Wash your hands thoroughly after each application and avoid touching your eyes and mucous membranes. You may need to wear latex gloves when applying the cream.. Arthritis Signs & Symptoms - Beaumont Emergency Center
Arthritis is a complex disease with over a hundred different types. Generally, these are divided into two major categories: inflammatory arthritis, such as rheumatoid arthritis, gout, lupus, or fibromyalgia; and osteoarthritis, the most common form, which develops through wear and tear on the joints.. Osteoarthritis occurs when the cartilage, the bodys natural shock absorber that cushions the end of the bones in your joints, begins to deteriorate. When cartilage wears away sufficiently, the friction between your joints causes pain and stiffness. The joints most often affected are hips, knees, spine, and hands, but arthritis can affect any joint in the body.. While many cases of osteoarthritis are mild, especially at first onset, over time people living with arthritis report difficulties performing activities of daily living, such as dressing, exercising, climbing stairs, or stepping in and out of the shower.. Smoking cessation intervention for reducing disease activity in chronic autoimmune inflammatory joint diseases - Forskning -...
BACKGROUND: Chronic inflammatory joint diseases (IJDs) affect 1% to 2% of the population in developed countries. IJDs include rheumatoid arthritis (RA), ankylosing spondylitis (AS), psoriatic arthritis (PsA), and other forms of spondyloarthritis (SpA). Tobacco smoking is considered a significant environmental risk factor for developing IJDs. There are indications that smoking exacerbates the symptoms and worsens disease outcomes.. OBJECTIVES: The objective of this review was to investigate the evidence for effects of smoking cessation interventions on smoking cessation and disease activity in smokers with IJD.. SEARCH METHODS: We searched the Cochrane Tobacco Addiction Group Specialized Register; the Cochrane Central Register of Controlled Trials (CENTRAL), the Cochrane Library; PubMed/MEDLINE; Embase; PsycINFO; the Cumulative Index to Nursing and Allied Health Literature (CINAHL); and three trials registers to October 2018.. Ceftriaxone therapy of chronic inflammatory arthritis. A double-blind placebo controlled trial. - Prohealth
To determine whether chronic inflammatory arthritis may respond to antibiotic therapy (implying a bacterial origin), we conducted a placebo-controlled, double-blind study. Sixty patients with inflammatory arthritis and antibody titers to Borrelia burgdorferi 1:64 or more were randomized to receive placebo (n = 20) or 2 g/d of ceftriaxone intravenously (n = 40) for 2 weeks. Two of 20 placebo- and 19 of 40 antibiotic-treated patients improved. At 1 month, the placebo-treated patients could elect to receive ceftriaxone. Altogether, 58 patients were treated with ceftriaxone and followed up for 13 to 24 months. Improvement was noted in 27 of the 58 antibiotic-treated patients. Patients with a wide diversity of inflammatory arthritis were studied. Response to ceftriaxone was seen in all groups, including 5 of 12 with rheumatoid arthritis, 5 of 8 with psoriatic arthritis, 3 of 5 with vasculitis, and 14 of 33 with less well-differentiated chronic inflammatory arthritis. In 16 of the 27 who responded to the
Erosions in inflammatory polyarthritis are symmetrical regardless of rheumatoid factor status: results from a primary care...
BACKGROUND AND AIMS: Symmetry is considered an important criterion for the differentiation of rheumatoid arthritis (RA) from other forms of inflammatory polyarthritis (IP), particularly those that are seronegative. Because of the inclusion of symmetry in the diagnostic and classification process, however, its true occurrence in RA cannot be assessed. As a surrogate, peripheral inflammatory arthropathies associated with rheumatoid factor production may be more likely to be symmetrical. We examined the degree of symmetry of erosions in an unselected cohort of patients with IP and tested the hypothesis that the presence of rheumatoid factor (RF) is associated with greater symmetry. METHODS: All patients registered with The Norfolk Arthritis Register (NOAR; a UK primary-care based cohort of patients with IP with annual follow-up) and who had radiographs performed at the fifth anniversary from notification were included in the analysis. Radiographs of the hands and feet were read using the Larsen method; a

Bone loss in inflammatory arthritis: mechanisms and treatment strategi by Nicole C. Walsh and Ellen M. Gravallese
PURPOSE OF REVIEW: Focal bone loss in inflammatory arthritis begins early in the disease process and can contribute to patient morbidity. Current treatment strategies primarily target suppression of the inflammatory cascade with varying success in limiting the progression of focal bone destruction. This review outlines the current understanding of the mechanisms mediating inflammation-induced focal bone loss in rheumatoid arthritis and other inflammatory arthritides and highlights recent studies in animal models of arthritis that have contributed to our knowledge of this process.
RECENT FINDINGS: Bone-resorbing osteoclasts have been identified as important effector cells in inflammation-induced bone loss in both experimental animal models and human rheumatoid arthritis and psoriatic arthritis. Twenty-two animals divided into three experiments were immunized with collagen type II (CII) of either bovine or chicken origin with different immunization strategies. The animals were analyzed for clinical manifestation of arthritis, hematology and clinical chemistry, immunological responses against CII and histopathological features of the arthritis. Clinically manifest arthritis was observed in almost 100% (21 out of 22) of the animals. Fifty percent of the animals
