Chronic periapical lesions in necrotic teeth with open apex are difficult to manage. The situation becomes more complex if the tooth has a fracture line. This case report describes the management of a traumatised tooth that had already undergone unsuccessful endodontic treatment with a large periapical lesion and open apex along with a fracture line in the root. An attempt was made to save the tooth by using mineral trioxide aggregate (MTA) cement for orthograde filling, retrograde filling and also for sealing of the fracture line. Moreover, the bone defect was filled with autogenous bone harvested from the external oblique ridge. MTA, a bioactive tricalcium silicate cement, has been used in treating complex endodontic cases as it shows promising potential by inducing the biological mechanisms necessary for repair of involved teeth. A 1-year follow-up showed progressive healing as evident by radiographs and lack of any clinical signs and symptoms. ...

Injury to an immature permanent tooth may result in cessation of dentin deposition and root maturation leaving an open root apex and thin dentinal walls that are prone to fracture. Endodontic treatment is often complicated and protracted with an uncertain prognosis frequently resulting in pre-mature tooth loss. There are several advantages of promoting apexogenesis in immature teeth with open apices. It encourages a longer and thicker root to develop thus decreasing the propensity of long term root fracture. This case report highlights the role of Hertwig root sheath and dental papilla in continued root formation of immature permanent tooth, even diagnosed with necrotic pulp.

BACKGROUND: In the esthetic zone, difficult decisions must be made regarding extraction or retention of compromised teeth. Numerous factors need to be considered to arrive at a proper treatment plan, which may differ from a plan devised for the posterior region of the mouth. TYPES OF REVIEWED STUDIES: Studies were selected that provided background information for clinical decision-making concerning whether a compromised tooth should be retained or removed. RESULTS: In the esthetic zone, before resective surgical procures are used to resolve periodontitis, consideration should be given to the esthetic outcome. If endodontic therapy is required, additional issues need to be reviewed before initiating treatment, including restorability of the tooth, presence of a large periapical area, use of the tooth as an abutment, etc. Furthermore, before initiating periodontal or endodontic treatment, the patient’s susceptibility to additional periodontal disease progression and caries should be evaluated.

Both these procedures have a high success rate when treating the infection in the tooth. However, when apexification procedures were used the roots were prone to fracture due to thin root walls. Regenerative endodontics allows for the continued thickening and elongation of the root, and therefore protection against fracture in adulthood..
