In their recent study,3 Dr. Chow and her colleagues set out to localize clusters of apathy symptoms empirically, with actual patients with dementia, to see whether the affective, behavioural, or cognitive apathy symptoms tended to occur together or in isolation, and whether apathy typically co-occurs with other types of behavioural disturbances.. Dr. Chow showed combined data from Baycrest, Sunnybrook [Health Sciences Centre], UC San Francisco, and UCLA, showing results on the Neuropsychiatric Inventory (NPI). On the section of the NPI that deals with apathy, patients are questioned on spontaneous activity, spontaneity in conversation, participation in former activities, participation in former chores, demonstration of interest in others, investment in friends and family, and degree of affection.. Results showed that, for those with either FTD or Dementia of Alzheimers type (DAT), the most common type of apathy, when present, was a decrease in spontaneous activity. ADMET 2 will examine in a masked, randomized trial the efficacy of methylphenidate for the treatment of clinically significant apathy in participants with Alzheimers dementia. Efficacy will be assessed as the change in Neuropsychiatric Inventory Apathy subscale (NPI apathy) from baseline to 6 months and score on the Alzheimers Disease Cooperative Study - Clinical Global Impression of Change (CGIC) scale at 6 months.. ADMET 2 will also examine the safety of methylphenidate for the treatment of clinically significant apathy in participants with Alzheimers disease by measuring vital signs, electrolyte panels, adverse event reports, and electrocardiograms. Safety will also be measured by examining neuropsychiatric symptoms other than apathy using the Neuropsychiatric Inventory (NPI).. Changes from baseline to 6 months in other neuropsychological assessments as measured using the Dementia Apathy Interview and Rating (DAIR) scale will also be assessed.. Cost-effectiveness will be measured by ...
TY - JOUR. T1 - Apathy is associated with ventral striatum volume in schizophrenia spectrum disorder. AU - Roth, Robert M.. AU - Garlinghouse, Matthew A.. AU - Flashman, Laura A.. N2 - Apathy is prevalent in schizophrenia, but its etiology has received little investigation. The ventral striatum (VS), a key brain region involved in motivated behavior, has been implicated in studies of apathy. We therefore evaluated whether apathy is associated with volume of the VS on MRI in 23 patients with schizophrenia using voxel-based morphometry. Results indicated that greater self-reported apathy severity was associated with smaller volume of the right VS even when controlling for age, gender, depression, and total gray matter volume. The finding suggests that apathy is related to abnormality of brain circuitry subserving motivated behavior in patients with ...
OBJECTIVES: Apathy has been reported as the most prevalent behavioural symptom experienced in Alzheimers disease (AD), associated with greater functional decline and caregiver distress. The aim of the current study was to investigate structural correlates of apathy in AD using magnetic resonance imaging (MRI) regional volume and regional cortical thickness measures. METHODS: Semi-structured interviews were conducted with 111 AD patients and their caregivers as part of the European multi-centre study AddNeuroMed. Apathy was measured using the apathy domain of the Neuropsychiatric Inventory (NPI). All AD patients were scanned using a 1.5T MRI scanner and the images analysed using an automated analysis pipeline. RESULTS: We found apathy to be the most prevalent neuropsychiatric symptom occurring in 57% of patients. Apathetic patients had significantly greater cortical thinning in left caudal anterior cingulate cortex (ACC) and left lateral orbitofrontal cortex (OFC), as well as left superior and
This is an open label pilot study; consenting subjects with dementia and identified as a fall risk, who meet inclusion criteria will undergo 3 phases of involvement. Phase 1 is a 1 week period before initiating study drug. Phase 2 is a 2 week treatment period with 10mg of Transdermal Methylphenidate. Phase 3 is the final two week treatment period with 15mg of Transdermal Methylphenidate.. Measurements obtained at each phase will include:. Vital Signs (blood pressure and pulse rate) weight , Timed Get Up and Go Test (TUG) , Tinetti Performance Oriented Mobility Assessment (POMA), Clinical Apathy Evaluation Scale (AES-C) and the St. Louis University Mental Status Examination (SLUMS).. The primary endpoint is the change in TUG and POMA scores at end of phase 3 compared to beginning of phase 2. Falls are a cause of substantial morbidity and mortality in patients with dementia and occur at twice the rate of older adults without cognitive impairment. The consequences of falls in older adults with ...
For the study, researchers examined 79 adolescents ages 14 to 18, who were "classified as recent regular cannabis users (36) or light users (43)." Frequency and amount of substance use "were assessed across participants lifetime and during the past 30 days", and motivation was measured "through the Apathy Evaluation Scale and Motivation and Engagement Scale." To examine associations between cannabis use (CU) and motivation indices, researchers conducted a series of two-step hierarchical multiple regressions.. After controlling for confounds, "no significant differences were observed between regular and light users on any motivation index", states the study. Similarly, "no associations between motivation and lifetime or past 30-day CU amount were observed".. The study concludes by stating; "Our findings do not support a link between reduced motivation and CU among adolescents after controlling for relevant confounds.". The full study, conducted by researchers at Florida International University, ...
January 29, 2010. Most people think of memory loss when they think of Alzheimers. But the disease can cause a wide range of behavioral and personality changes as well. One of the most common is apathy, or loss of motivation, and the condition seems to have a biological basis. Using brain scans, researchers in Sweden report that changes in the brains white matter are a common feature in apathetic patients who have Alzheimers or other forms of dementia.. Apathy is one of the most common psychological problems associated with dementia. Just over half of all people with Alzheimers are emotionally blunted and lack motivation and initiative. As the disease worsens, apathy often grows worse.. Studies suggest there is a common biological reason behind apathy, irrespective of whether a person has Alzheimers or other forms of dementia. The changes occurred in areas deep in the brain that are connected to the brains frontal lobes, which are critical for taking initiatives and the ability to ...

BACKGROUND: Alzheimers disease (AD) is characterized not only by cognitive and functional decline, but also often by the presence of neuropsychiatric symptoms. Apathy, which can be defined as a lack of motivation, is one of the most prevalent neuropsychiatric symptoms in AD and typically leads to a worse quality of life and greater burden for caregivers. Treatment options for apathy in AD are limited, but studies have examined the use of the amphetamine, methylphenidate. The Apathy in Dementia Methylphenidate Trial (ADMET) found that treatment of apathy in AD with methylphenidate was associated with significant improvement in apathy in two of three outcome measures, some evidence of improvement in global cognition, and minimal adverse events ...

Abstract: BACKGROUND: Apathy is a pervasive noncognitive neuropsychiatric disturbance in Alzheimer disease, which causes significant caregiver distress. The neuroanatomical substrate of apathy is not well understood. OBJECTIVE: To study the relationship between regional cerebral blood flow and the presence and severity of the personality disturbance, apathy, in individuals with Alzheimer disease. DESIGN: Analysis of the relationship between regional cerebral blood flow as measured by single photon emission computed tomography and severity of apathy as measured by the Neuropsychiatric Inventory using an analysis of variance design. We examined regional cerebral perfusion alterations as measured by xenon 133Xecalibrated technetium Tc 99m ...
k4140] High doses of dopamine are usually needed to help patients suffering from apathy. In one study, seven out of eight patients who became depressed following liver transplants and were given methylphenidate, or Ritalin, a drug that elevates dopamine levels, showed improved motivation in pursuing their rehabilitation regimens and less social withdrawal and apathy than they exhibited before taking the drug. For apathy patients, drugs are not the final cure. They also need assistance in practicing techniques to help themselves. Education of the family is an important job for the therapist, too. Treatment of apathy raises some complex human rights issues, because these patients may be competent enough to make some decisions but not others. They are also prone to anxiety, which must be relieved so that they will consider options and get involved with life again. Treating apathy is also important in depressed patients. Drugs can be used to bring about rapid improvement at an early stage. Since ...

Abstract: Objectives: People with Huntingtons disease (HD) experience poor social quality of life, relationship breakdown, and social withdrawal, which are mediated to some extent by socially debilitating neuropsychiatric symptoms, such as apathy and disinhibition. Social cognitive symptoms, such as impaired emotion recognition, also occur in HD, however, the extent of their association with these socially debilitating neuropsychiatric symptoms is unknown. Our study examined the relationship between emotion recognition and symptom ratings of apathy and disinhibition in HD. Methods: Thirty-two people with premanifest or symptomatic-HD completed Part 1 of The Awareness of Social Inference Test (TASIT), which is a facial emotion recognition task. In addition, we obtained severity ratings for apathy and disinhibition on the Frontal Systems Behavior Scale (FrSBe) from a close family member. Our analyses used motor symptom severity as a proxy for disease progression. Results: Emotion recognition ...

This study examined the brain bases of early human social cognitive abilities. Specifically, we investigated whether cortical regions implicated in adults perception of facial communication signals are functionally active in early human development. Four-month-old infants watched two kinds of dynamic scenarios in which a face either established mutual gaze or averted its gaze, both of which were followed by an eyebrow raise with accompanying smile. Haemodynamic responses were measured by near-infrared spectroscopy, permitting spatial localization of brain activation (experiment 1), and gamma-band oscillatory brain activity was analysed from electroencephalography to provide temporal information about the underlying cortical processes (experiment 2). The results revealed that perceiving facial communication signals activates areas in the infant temporal and prefrontal cortex that correspond to the brain regions implicated in these processes in adults. In addition, mutual gaze itself, and the ...
Objective: Aging societies will bring an increase in the number of long-term care patients with mental-physical multimorbidity (MPM). This paper aimed to describe the natural course of neuropsychiatric symptoms (NPS) in patients with MPM in the first 8 months after admission to a geronto-psychiatric nursing home (GP-NH) unit. Methods: Longitudinal cohort study among 63 patients with MPM no dementia living in 17 GP-NH units across the Netherlands. Data collection consisted of chart review, semi-structured interviews, and brief neuropsychological testing, among which our primary outcome measure the Neuropsychiatric Inventory (NPI). Descriptive and bivariate analyses were conducted. Results: Our study showed a significant increase of the NPI total score (from 25.3 to 29.3, p = 0.045), and the total scores of a NPI hyperactivity cluster (from 9.7 to 11.8, p = 0.039), and a NPI mood/apathy cluster (from 7.7 to 10.1, p = 0.008). Just over 95% had any clinically relevant symptom at baseline and/or six ...
The goal of this research was to evaluate the impact of ACES (Accompagnement Collaborative des Enseignants du Secondaire), a training program for high school teachers aimed at facilitating the educational integration of students with special needs. Offered individually or in groups, the program was well received by all stakeholders and had a positive impact on teachers, students and the classroom climate. The program positively influenced teachers attitudes towards their students, their understanding of behavioural disorders, their use of positive and proactive intervention strategies, and the student-teacher relationship.. Teachers who took part in the program had a significantly lower level of teaching stress than those in the control groups. While little difference was observed between the two versions of the program, the group training was more popular with all parties concerned.. ...

A number of biological markers have been implicated in late life depression with inconsistent results. The present study examined the relationship between several serum based biomarkers and symptoms of depression in a sample of elderly women with AD or cognitively intact. Methods 171 females (58 with AD and 113 cognitively intact) were recruited from the Longitudinal Research Cohort of the Texas Alzheimers Research and Consortium (TARC). Stepwise regressions were conducted with GDS total and subscales and a panel of biomarkers (CRP, IL-10, IL-1α, TNF-α, ICAM-1, BDNF, and MIF). ApoE4 status was coded (carrier or non-carrier), and the results were analyzed by cognitive status (AD or controls). Results: None of the biomarkers significantly predicted total GDS score for AD cases, controls or sample as a whole. For the Controls, ICAM significantly predicted Dysphoria and level of Apathy. Among AD patients, MIF, ICAM, and CRP, were significantly associated with Apathy. MIF and ICAM were inversely

Reduced alertness, fatigue, apathy, insomnia are often lumped together. As a rule in older folks, alertness "aint what it used to be." [9] The exceptions are a wonderful thing to behold. I am surprised at finding a number of people as young as being in their fifties, who share with older folks the poor recall of names. If you think of the brain being like a computer, it is if something happened to the address book. You cant put names to the faces. Mental stress is a big contributor to the development of feeling fatigued. Multitasking is a problem. Not being able to breeze through tasks, as one did in mid-life, can prove very frustrating and stressful. Realizing that one person has little impact on the worlds problems, leads to apathy or to continue seeking solutions when in the "twilight zone" of sleep. Dry skin: A universal problem in older folks -- made worse from exposure to dry inside, winter air. [10] Moisturizing lotions seem to be the only remedy, but even with their use, break-through ...

The opposite of Apathy is Love. Last Saturday my husband and I joined four other couples for a dinner club and the discussion around the table was anything but apathetic. Jokes about the election year and the wild ride ahead for us Americans mingled with updates on our families and local parishes. Wine glasses clinked and Italian sauced plates scraped clean as five women and five men tried to predict how the public would soon vote and who our next president will be. The famous quote, Never talk politics or religion in polite company" was disregarded and I believe with good reason. As Christians we care about our country, families and our churches and to sit back and do nothing would be considered apathetic. And apathy is the opposite of love.. Later in the evening when we resumed to the living room, the hostess presented our group with a challenge. Before Jesus ascended up to Heaven, He told his disciples to wait until the Holy Spirit has come down in power to help them. So for Ten days the ...

There is a fine line between increased eye contact and staring, which is impolite. You can increase mutual gaze by maintaining eye contact as you turn your head to break the gaze; the other person does not perceive your extended gaze as staring because your head is turning. If the person you are with maintains eye contact, they like you. 2. A Light Touch People often touch the person they like. In romantic relationships, women may lightly touch the arm of the person they are talking to. This light touch is not an invitation to a sexual encounter; it merely indicates she likes you. Men express their liking for other men by fist bumping, chest bumping, or lightly hitting the other person on the shoulder with a closed fist. Men similarly express their like for women by engaging in playful physical activities. Another form of touching is preening. Picking lint off of another persons clothing or straightening a crooked tie indicates that the person likes you. Touching is a good indicator that a ...

HIV minor cognitive/motor disorder can lead to missed appointments, breakdown in medication regimens, an overall disruption in self-care abilities, and reduced quality of life. Features include impaired attention and concentration, slowed information processing, compromised problem solving abilities, apathy and social withdrawal, and neuromotor dysfunction (Heaton et al., 1995; Lopez et el., 1998; McArthur, Sactor, & Selnes, 1999). Changes in cognition can be the result of reaction to or interaction of medications, delirium, depression, mental illness, anxiety, or substance use. Following is Table 1 describing common symptoms of cognitive impairment and interventions that you may find helpful. ...

People who are introspective and underreactive can be prone to brooding and feeling empty. If stagnation is allowed to continue, it can lead to serious problems. Understanding your temperament and modifying your habits can help manage low periods without antidepressants or boost the power of any medication you are taking: 1. Recognize and reframe your mood: When you are brooding or feeling empty, tell yourself that your body is signaling the need for outside stimulation. Reattribute any thoughts of hopelessness, helplessness, and regret to sluggish body chemistry. 2. Postpone worry and apathy: Set limits on how long and when you allow yourself to ruminate or vegetate. Do not permit more than two separate hours a day. If you feel a mood coming on, tell yourself it will have to wait till your "down time." 3. Modify ruminations: Say worries out loud, pinch your nose, write, sing, or speak them in pig Latin. Avoid thought-stopping methods that hinder needed stimulation. 4. Get a light: Spend time ...
A good idea but I would be astonished if there was any meaningful response. Ive been through this before with a Nature journal and the outcome is predictable.. The first hurdle one would encounter would be intense apathy - your first letter will routinely be ignored. If you persist and make a second enquiry and start raising your cybervoice you will discover that the person who should be dealing with it will have moved to another department and so it will be referred to a stand-in who is new to the position and unfamiliar with the details of the case. They will promise to discuss it in a meeting and another three months will pass.. If you persist still further eventually you might get a response - somehow the journal cant quite bring itself to ignore you completely when they have said they will deal with the enquiry - in their minds this could be worse than the scientific idiocy of their correction. If they eventually do provide a response, this is certain to support the original decision in ...
Obviously, not every reaction is helpful to a family like ours. But trust me, I understand most people are well-meaning and Im very hesitant to criticize any reaction - I mostly appreciate ANY reaction (except apathy). Larry and I dont expect perfection in understanding. But we do hope people at least care. And try. And we hope people wont over-simplify what we are experiencing. That feels like invalidation of something that is very big and very real for us.. Some people want to better understand this so Im going to try and explain more about it today and Im going to be pretty transparent. I know that Ill be articulating perspectives that are not just my own because Ive had emotional discussions with others about this very issue. One of those conversations was as recent as this morning when a friend called me from another state about the post I shared yesterday. Yup. This hit some nerves..
