Approaches providing the positioning of human amniotic membrane (HAM) within the intra-articular space of arthritic TMJs have never been investigated. This contrasts with the increasing amount of evidence suggesting the potential positive effects of HAM on a number of surgical conditions, even included the interpositional arthroplasty for TMJ ankylosis. Thus, the possible usefulness of HAM to restore joint functions in severely damaged TMJs could be hypothesized. Based on these premises, the clinical research question "Is human amniotic membrane positioning effective to reduce symptoms and restore jaw function in patients with severe inflammatory-degenerative disorders of the temporomandibular joint?" has been addressed by performing a systematic review of the literature ...
Intra-articular ankylosis (shown above) occurs within the joint and most commonly occurs at the interphalangeal joints. Intra-articular ankylosis results in serious functional and cosmetic problems and is not as easily treated as the extra-articular type. Treatment options include arthrodesis, osteotomy, arthroplasty, or amputation ...
It has been reported that patients who had a previous arthrodesis for ankylosis had a poorer outcome post total hip replacement.3,7 Some studies suggested a higher failure rate in younger patients and patients with trauma to the hip.3. Clinical evaluation should be thorough with special attention paid to leg length discrepancy, deformities, range of motion and abductor function. Evaluation of the abductor function is very important, specifically in those patients whose hips fused before puberty. This is associated with a higher rate of dislocation.1,6 Pre-operative X-ray templating should be used in order to plan surgery. A CT scan can also be used in this regard.4. The positioning of the patient on the operating table and cup positioning can be very difficult and it is important to recognise this when planning the surgery. Different surgical approaches (anterolateral, posterolateral, lateral, ± trochanteric osteotomy) have been used in this type of surgery and are ...

We evaluated characteristics of patients with AS who had radiographic spinal changes. There were no radiographic spinal changes in 53% of the total group; however, in the subgroup with disease duration longer than 30 years, only 23% remained without spinal changes. This suggests that a large majority of patients under care of a rheumatologist have spinal involvement that will eventually lead to changes visible on conventional radiographs. The likelihood of spinal ankylosis was also higher in men, in those with symptom onset at an earlier age, and in patients who had a history of hip involvement. There was no association of HLAB27 or extraspinal articular disease or extraarticular manifestations with spinal ankylosis.. While it does not seem surprising that disease duration is associated with radiographic changes, there are few prospective longterm studies with data that quantify this observation. The Outcome Assessments in Ankylosing Spondylitis International Study evaluated a cohort of patients ...

Ageing male DBA/1 mice from different litters were caged together at the age of 8-10 weeks and studied for prospective signs of arthritis. Inflammation-associated gene expression was examined using RNA isolated from affected and healthy paws, as well as draining lymph nodes. DBA/1 mice were treated daily by injection of dexamethasone (0.5 µg/g body weight). Severity of disease was assessed by histomorphology and by positron emission tomography (PET) using 18-deoxyfluoroglucose as a tracer. Bone loss in dexamethasone-treated or control mice was determined by in vivo dual-energy x-ray absorptiometry. Chemokine expression was studied in vitro in mesenchymal cell types (primary synovial fibroblasts, periosteal and bone marrow stromal cells) by quantitative real-time PCR in the presence or absence of BMP2 and/or dexamethasone. C2C12 cells stably transfected with a BMP-reporter construct were used for cell stretch experiments in a dedicated bioreactor.. ...

Mesial protraction of mandibular posterior teeth requires increased anchorage to avoid undesired tooth movements. Orthodontic mini-implants have become a popular and successful way to increase skeletal anchorage in such cases. However, mini-implants may cause injury to adjacent teeth or anatomical structures and may lead to tissue inflammation. Induced ankylosed primary teeth have been used in the past as abutments for the protraction of the maxilla in cases of maxillary retrognathism. However, this technique has not been described in the literature for the protraction of mandibular molars. The aim of this paper is to present, through a case report, an alternative to mini-implant devices to maximize anchorage in the mandible by inducing ankylosis on a primary molar. A 13-year-old female with class II right malocclusion, deep bite, and congenitally missing right second premolars was referred for orthodontic treatment. Treatment plan involved removal of the primary teeth and mesial protraction of the
We tested the hypothesis that MRI structural lesions in the SIJ, especially fat metaplasia, predict new bone formation on spinal radiographs from patients with axial SpA in a prospective cohort. This was aimed at addressing a key preliminary requirement in the validation of a surrogate MRI biomarker for radiographic progression in the spine of patients with SpA. Our multivariate data demonstrates that the presence of definite ankylosis in the SIJ at baseline (as defined by an SSS score of ≥2 by both readers), and its extent, is significantly associated with new bone formation on spinal radiography on follow-up. The degree of subchondral fat metaplasia in the SIJ is also independently associated with radiographic progression in the spine. The lack of any new bone formation in the spine despite prolonged symptom duration is associated with the absence of SIJ ankylosis and backfill. These data therefore support the hypothesis that structural lesions in the SIJ, specifically ankylosis and ...
SOURCE: Steinberg CL, et al. (1955). Comparison of rheumatoid (ankylosing) spondylitis and crippling fluorosis. Annals of the Rheumatic Diseases 14: 378-384.. "During our anthropological studies we were puzzled by the high incidence of a peculiar type of arthritis and joint ankylosis that seemed to afflict a majority of the adult inhabitants of a number of small Miao villages in the neighbourhood. In fact, in many of the older people the disease had advanced to such an extent that they appeared to be set in semi-sitting positions, with almost complete loss of movement in the limbs… Later a copy of Roholms mongraph on fluorine intoxication came to hand and we were struck by the similarity in appearance between the photographs of the Danish workers in cryolite and the crippled Miao peasants in China… It is quite possible that endemic centres [of skeletal fluorosis] exist but that the cause of the disabling spondylitis or other joint affections has not been determined, and a diagnosis of ...
14. There are periods associated with those remissions and relapses. Pathophysiology of them Rheumatoid Arthritis. Rheumatoid Arthritis goes through four stages. The first noticeable stage is perhaps inflammation of the joint membrane (synovitis), causing blockage and oedema. The next phase is the formation whereas pannus. A layer of inflammatory tissue forms in the joint capsule. Fibroblasts, leucocytes, lymphocytes, plasma cells and many other materials multiply from the synovial membrane a lot of pannus tissue which invades with cartilage and slowly replace it with tough fibrous cells. The disease progresses a reality otherwise known as fibrous ankylosis. This involves the conversion of the articular cartilage of the full joint into fibrous body parts thereby inhibiting motion of that joint. The final stage is perhaps bony ankylosis. In the following stage, the bones of the joint fuse together and thus making mobility impossible. .. ...
In the present study, we found that AS-BMSCs showed normal proliferation, cell viability, surface markers and multiple differentiations characteristics, but significantly reduced immunomodulation potential; also, the frequencies of Treg and Fox-P3+ cells in AS-PBMCs decreased, but CCR4+CCR6+ Th cells increased. Moreover, the AS-BMSCs induced the ratio of CCR4+CCR6+ Th/Treg cell imbalance when co-cultured with PBMCs. Additionally, no differences were found between HD1 and HD2. Impressively, the immunomodulation potential of BMSCs has negative correlation with the ratios of CCR4+CCR6+ Th to Treg cells in peripheral blood.. Characteristic symptoms of AS are spinal stiffness, ankylosis and syndesmophytes [1], which are explained by spinal inflammation, structural damage, or both [40]. As the ankylosis of the spine or even spinal stiffness was probably initiated by the heterotopic ossification of osteoblasts, and most of these osteoblasts derived from BMSCs [41, 42], and, simultaneously, there were ...
Radiography is an unreliable and insensitive tool for the assessment of structural lesions in the sacroiliac joints (SIJ). Magnetic resonance imaging (MRI) detects a wider spectrum of structural lesions but has undergone minimal validation in prospective studies. The Spondyloarthritis Research Consortium of Canada (SPARCC) MRI Sacroiliac Joint (SIJ) Structural Score (SSS) assesses a spectrum of structural lesions (erosion, fat metaplasia, backfill, ankylosis) and its potential to discriminate between therapies requires evaluation. The SSS score assesses five consecutive coronal slices through the cartilaginous portion of the joint on T1-weighted sequences starting from the transitional slice between cartilaginous and ligamentous portions of the joint. Lesions are scored dichotomously (present/absent) in SIJ quadrants (fat metaplasia, erosion) or halves (backfill, ankylosis). Two readers independently scored 147 pairs (baseline, 2 years) of scans from a prospective cohort of patients with SpA who
The use of guided tissue regeneration procedures has intensified the need to determine the types and quantity of tissues formed in healing. Regeneration is differentiated from new attachment in one basic, but very important manner: alveolar bone formation with new inserted periodontal fibers in new cementum is a prerequisite for the regeneration of periodontium (4-6). Areas of new cementum deposition without adjacent bone, and vice-versa, and areas of ankylosis, for example, do not satisfy the criteria for regeneration (7). Furthermore, recent advances in the understanding of functions and mechanisms of action of growth factors to regulate the healing process have provided evidence that these proteins may serve as therapeutic agents to enhance the healing of periodontal wounds (8-11). Sigurdsson et al. (12) found a significant enhancement in periodontal regeneration using the rhBMP-2 in dog models. They described limited root resorption and ankylosis generally limited immediately apical to the ...
There have been marked improvements in treatment options but none have yet been shown to induce remission. The past decade has seen major advances in the diagnosis and management of ankylosing spondylitis (AS) and in research into its pathogenesis. It remains the case that no current treatments have been shown to lead to disease remissions or to halt the progression of the bony ankylosis that causes the major morbidity associated with this condition. Nonetheless, improved diagnostic methods and management have led to major benefits for patients, with marked improvements in quality of life with reduced treatment-associated side effects.. ...

If rheumatoid arthritis is ever to be controlled completely and permanently, it would seem reasonable to suppose that such a result would have to be achieved during the comparatively early stages of the disease; that is, before marked crippling caused by joint destruction, ankylosis and deformity has taken place. In the early months of rheumatoid arthritis, apparently up to one year, the disease is nearly always completely reversible. It may be completely reversible even later if joint destruction has not occurred. This characteristic of rheumatoid arthritis has been stressed recently by Hench in his Heberden Oration.1. Much has been written ...
Currently, the gold standard for assessing BMD is posteroanterior (PA) dual-energy x-ray absorptiometry (DEXA) at the spine and hip, as recommended by the International Society of Clinical Densitometry (ISCD)32. However, the hallmark of axSpA is sacroiliitis and spinal damage due to both bony erosion and abnormal bone formation. This can lead to the development of syndesmophytes, perivertebral bone formation, ankylosis of the zygapophyseal joints, and pathologic new bone formation in the ligamentous apparatus. In severe cases, complete fusion of the spine can occur. This extensive osteoproliferation can falsely raise the BMD when PA (conventional) DEXA is used, giving an illusion of reassuringly normal BMD, even in cases where osteoporosis may be present. A study of 73 patients with AS using PA DEXA found that the frequency of low BMD in patients with mild disease was 68.4% in the lumbar spine, but the prevalence dropped to 54.3% in advanced cases8. This contrasted with the hip, where the ...
ABSTRACT. The sacrifice of motion to achieve spinal stability and pain relief has been practised since the early 20th century by means of iatrogenically induced ankylosis or fusion. Initially this was practised in the management of Potts disease and indications have been expanded over the years to include trauma, deformity and degenerative conditions. In the last few decades there has been a proliferation of options as regards surgical technique and instrumentation. This often overwhelms the surgeon where more is perceived to be better, yet there is limited evidence that this is in fact so.. ...
Methods A random 80% sample of the ASSERT database was used for this analysis. ASSERT was a 24-week (wk) RCT comparing infliximab monotherapy and placebo in patients with active AS, with an open extension until 102wks with all patients on infliximab. Spinal MRIs (T1-weighted and STIR images) at baseline, 24wks and 102wks were assessed by 2 readers independently who scored each VE for the presence or absence of VEI and VEFD. Spinal X-rays (baseline and 102wks) were scored by 2 different readers independently using the mSASSS. Data were analysed at the VE level in the 24 VEs assessed by the mSASSS (anterior VEs of the cervical and lumbar spine), using a multi-level approach to adjust for within-patient correlation and MRI reader (GEE for binomial outcomes). Particular emphasis was on a sequence analysis, which aimed at the specific sequence VEI→VEFD→SYN. The following variables were considered potential covariates and adjusted for when appropriate: treatment, gender, presence of SYN/ankylosis ...

CASE 1760.-Pt. E. W. Law, Co. A, 1st Massachusetts, aged 21 years, was wounded at Oak Grove, June 25, 1862, and sent to St. Elizabeth Hospital, Washington; returned to his regiment at Fort Worth, Virginia, and subsequently discharged and pensioned. Surgeon F. LeB. Monroe, 1st Massachusetts, certified on his discharge papers: "Anchylosis of the right elbow joint, the result of shot wound. There is immobility of the joint, probably permanent, with loss of the power of supination." Examiner G. S. Jones, of Boston, recorded, October 14, 1862: "Anchylosis of right elbow joint. The arm has been left in a straight and pronated position, and its usefulness destroyed. It is the result of a shot wound of the elbow joint, and his disability is increased in consequence of neglect in its management." OBJECTIVES: In patients with rheumatoid arthritis (RA), the talonavicular joint is commonly involved and midfoot collapse can lead to progressive flattening of the arch. Despite a general awareness of the important structural role of the talonavicular joint in rheumatoid foot disease, details of its destructive pattern has not been elucidated. METHODS: We cross-sectionally investigated 176 RA patients (342 feet) and classified their feet into the following five groups according to radiographic findings: arthritis; (RA changes with normal navicular shape), Müller-Weiss Disease (MWD); (collapse of the lateral aspect of the tarsal navicular), flat; (flattened navicular), ankylosis; (ankylosis of the talonavicular joint), and normal ...

Arthrodesis Also known as artificial ankylosis or syndesis, is the artificial induction of joint ossification between two bones via surgery. This is done to relieve intractable pain in a joint which cannot be managed by pain medication, splints, or other normally-indicated treatments. The typical causes of such pain are fractures which disrupt the joint, and arthritis. It is most commonly performed on joints in the spine, hand, ankle, and foot. Historically, knee and hip arthrodeses were also p…
