Blood transfusion during perioperative period in patients undergoing to major surgery has been associated to several postoperative complications. Particularly in oncologic surgery, red blood cell transfusion has been investigated as a factor of worse outcome and cancer recurrence due to postoperative impairment of cellular immunity. Although red blood cell transfusion has decreased in worldwide clinical practice, this issue still remains a matter of controversy in oncologic surgery. There are no prospective studies comparing outcomes between restrictive or liberal of blood transfusion strategy in oncologic surgery. This study is a prospective and randomized study comparing clinical outcomes between two strategies of transfusion in oncologic surgery - liberal or ...

The red blood cell transfusions should not be dictated by a single hemoglobin trigger but instead should be based on the patients risks of developing complications of inadequate oxygenation. Introduction:. HLA specific antibodies detection (HSA) and that of Donor Specific Antibodies (DSA) were revolutionized by the introduction of solid phase assays (Luminex®), therefore making these antibodies a powerful biomarker for humoral injuries to the allograft. Determinants of HSA development entail non-adherence to immunosuppressive drugs but also allosensitizing events such as red blood cell (RBC) transfusions. Recent works showed a higher incidence of DSA in transplanted patients following RBC transfusion events, but no specific data exist for AntiThymocyte Globulin (ATG) induction.. This work aims at assessing whether peri-transplant RBC transfusion resulted in post-transplant HLA sensitization, in the setting of ATG induction therapy.. Patients and methods:. All consecutive patients benefiting from a first ATG-induced kidney allograft between 2004 and 2014 at our center with no history of HLA immunization were included retrospectively, provided transfusion history and HSA history were ...

Results. There were 34 infants in each group. Both groups were similar in age, gender, cardiac defect type, ICU length of stay, and time interval to second stage or definitive repair. Shunt interventions (18 versus 32%, p=0.16), shunt thrombosis (14 versus 17%, p=0.74), and mortality (9 versus 12%, p=0.65) were not significantly different between groups. Now, there are a few reasons to take this with a grain of salt. First, its only one study. There may be problems with methodology or question wording or a dozen other things. Second, this is pretty much what we liberals would like to believe, isnt it? That should give us pause before we get too self-righteous over this. Third, the study was done using volunteers recruited via Mechanical Turk, and Ive had some pretty uncomplimentary things to say about MT before.. Still, its intriguing and certainly deserves some follow-up. I presume this result is related in some way to the fact that conservatives tend to have higher in-group loyalty than liberals, but thats just an armchair guess. Maybe further research could dig into that.. From a political point of view, I guess the big question this study raises is: What fires up liberals? We have a pretty good idea of what fires up conservatives, but what gets us lefties going?. ...

TY - JOUR. T1 - Anemia, red blood cell transfusions, and necrotizing enterocolitis. AU - Maheshwari, Akhil. AU - Patel, Ravi M.. AU - Christensen, Robert D.. PY - 2018/2/1. Y1 - 2018/2/1. N2 - In the past 15 years, multiple clinical studies have identified a temporal association between red blood cell (RBC) transfusions and necrotizing enterocolitis (NEC). With some variability, most of these studies indicate that up to one-third of all cases of NEC involving very low-birth weight infants may occur within 24-48 h after receiving a RBC transfusion. There is also evidence that the risk of such transfusion-associated NEC may be higher in infants transfused with the greatest severity of anemia. In this article, we summarize the clinical evidence pertaining to these issues; specifically, the contribution of RBC transfusions, and the contribution of severity of underlying anemia, to the pathogenesis of a type of NEC potentially termed, transfusion/anemia-associated NEC.. AB - In the past 15 years, ...
Background: Allogeneic red blood cell transfusions (ABT) are common in older hip fracture patients. Recent research supports a restrictive transfusion policy. The aim was to study variation in hemoglobin (Hb) concentration, and clinical outcomes in these patients.. Results: Cross-sectional study with one-year follow-up in an orthogeriatric unit. Data were obtained from a quality register with demographic and medical information collected by an interdisciplinary team. 106 (22 %) of the 491 patients admitted from September 2011 throughout September 2012 (76 % women, mean age 85 years) received ABT. When given ABT, 80 % had Hb ,80 g/l and mean Hb was 78 g/l. Mean Hb, regardless ABT, showed variation from 125 g/l (±16) on admission to 106 g/l (±17), 101 g/l (±16) and 102 g/l (±14) on 1st, 3rd and 5th postoperative day respectively. Patients with per-/subtrochanteric fractures more often received ABT than those with femur neck fractures (p , 0.001), 70 % of the patients receiving ABT had a ...
Many people diagnosed with haematological malignancies experience anaemia, and red blood cell (RBC) transfusion plays an essential supportive role in their management. Different strategies have been developed for RBC transfusions. A restrictive transfusion strategy seeks to maintain a lower haemoglobin level (usually between 70 g/L to 90 g/L) with a trigger for transfusion when the haemoglobin drops below 70 g/L), whereas a liberal transfusion strategy aims to maintain a higher haemoglobin (usually between 100 g/L to 120 g/L, with a threshold for transfusion when haemoglobin drops below 100 g/L). In people undergoing surgery or who have been admitted to intensive care a restrictive transfusion strategy has been shown to be safe and in some cases safer than a liberal transfusion strategy. However, it is not known whether it is safe in people with haematological malignancies. To determine the efficacy and safety of restrictive versus liberal RBC transfusion strategies for people diagnosed with

Coalition of the Confused - Liberal MENTALity - In a nutshell, here follows abbreviated characteristics to help you identify liberals . A liberal is a communist whose words and deeds are hell-bent on destroying civilization. Liberals are mentally disturbed and foolishly believe mass-media indoctrination. Liberals are in favour of disarming law-abiding citizens. Liberals approve of open borders. Liberals approve of torturing unborn babies to death. Liberals want tax-payer money to fund groups advocating tolerance of fringe elements like bandits. Liberals enforce PC policy. Liberals promote the extinction of natures finest and most beautiful and seek to deny them free-speech in Germany, Austria, Canada etc. Liberals advocate the equality theory without a shred of supporting evidence. Liberals moan about segregation and discrimination EXCEPT when the victims lack suntans. Then their silence is not golden; it is yellow. Liberals reject the death penalty because they do not want law and order. Liberals

The conundrum is what clinical factors should influence the routine titrated transfusion decision. The largest trial evaluating transfusion thresholds used prespecified symptoms (such as chest pain, orthostatic hypotension, or tachycardia unresponsive to fluid resuscitation) (1), which we included in our guideline. Although this approach is based on a randomized trial, these symptoms will not apply to all clinical settings. We agree with the accompanying editorial (2) that other variables (such as fatigue, dyspnea, mechanical ventilation, or the use of low Svo2) are unproven. Unfortunately, there is no evidence to support other important individual patient laboratory and physiologic variables as the basis for transfusion decisions ...
In our registry, treatment with aprotinin is recorded as in theater or subsequently. Aprotonin treatment (none, in theater, or subsequently) was included in the propensity model, although the frequencies of patients given aprotinin (9% in theater, 1% subsequently) were not reported.1. New analyses, explicitly including aprotinin treatment (in addition to nadir hematocrit stratum and propensity score deciles), show that the propensity score controlled well for confounding by aprotinin; odds ratios (ORs) for composite infection and ischemic outcomes given transfusion were 3.31 (95% confidence interval, 2.59 to 4.24, estimated by bootstrapping) and 3.29 (95% confidence interval, 2.59 to 4.26), respectively, compared with 3.38 and 3.35 without aprotinin explicitly included in the models.1 Therefore, aprotinin did not substantially confound the reported risks of the composite outcomes. Timing of aprotinin treatment did not markedly influence the risk of either the infection or ischemic composite ...
Who Are We Talking About: Please take the time to read a few paragraphs instead of having a knee-jerk reaction to the title. Im not talking about the people you think Im talking about. Im not talking about people who are converting liberal but are open to the possibility of converting orthodox at a later time. After all, thats the position Im in! Posting from below: Im referring to people who, at the time they are pursuing a liberal conversion, really do not intend to ever be a liberal Jew and are simply passing time in the liberal community until they can move to an orthodox community. In short, its getting a liberal conversion when you dont want one. They dont think liberal Judaism is right or valid, but its whats available, and they plan to flee town at the earliest opportunity to join an orthodox community. (Whether that actually ever happens is irrelevant to this discussion.) As a friend described it, they think they can cut in the orthodox conversion line. I dont even ...

Liberal world order is seen by many as either a fading international order in response to declining American hegemony, or as a failing international order riddled with internal tensions and contradicting positions. Either way, it is assumed to be in crisis. This book does not reject this claim. Nor does it deny that liberalism contains many inconsistencies. Instead, it argues that much of the literature has been conditioned by a view that sees liberal orders crisis primarily as a crisis of authority and which does not look further back than the twentieth century. As a result liberalism was shorn of its historical origins and previous rich debates about similar tensions and contradiction to those of todays liberal order. The volume questions the nature of liberal orders crisis by positing that liberal orders continual renewal was achieved through crisis, and it challenges the way in which the debate about liberalism has been conducted within the International Relations academy. Simply put, liberals have a big baby problem: Theyre not having enough of them, they havent for a long time, and their pool of potential new voters is suffering as a result. According to the 2004 General Social Survey, if you picked 100 unrelated politically liberal adults at random, you would find that they had, between them, 147 children. If you picked 100 conservatives, you would find 208 kids. Thats a fertility gap of 41%. Given that about 80% of people with an identifiable party preference grow up to vote the same way as their parents, this gap translates into lots more little Republicans than little Democrats to vote in future elections. Over the past 30 years this gap has not been below 20%-explaining, to a large extent, the current ineffectiveness of liberal youth voter campaigns today.. Alarmingly for the Democrats, the gap is widening at a bit more than half a percentage point per year, meaning that todays problem is nothing compared to what the future will most likely hold. ...
And as long as Im back on ORourke, I wanted to mention something else. Why is it that every conservative always has to tell us when they became a conservative and the reason/event/circumstance that made that happen (ORourke did that, but I dont recall exactly what he said). I dont know about you, but I dont care (Update: I know Arianna Huffington tells that story of getting put off by Pat Buchanans speech at the 92 Repug Convention, but thats the only time I remember a liberal stating what originated their point of view). I dont remember what caused me to be a liberal (Im sure Molly Ivins had something to do with it), putting aside the fact that, sometimes, those are really dumb catchall classifications. To illustrate, let me say this; I think that if a law-abiding citizen already owns a gun, nobody has the right to take it away from them because it is their property. Does that still make me a liberal? Also, I have yet to hear a good explanation for what the Microsoft trial in the ...
If by a Liberal they mean someone who looks ahead and not behind, someone who welcomes new ideas without rigid reactions, someone who cares about the welfare of the people-their health, their housing, their schools, their jobs, their civil rights and their civil liberties-someone who believes we can break through the stalemate and suspicions that grip us in our policies abroad, if that is what they mean by a Liberal, then Im proud to say Im a Liberal. ― John F. Deciding who lives and who dies: the liberals ultimate wet dream.. God players all, liberals now get to officially do what they have always dreamed of doing, controlling individuals down to the very end of life, in fact, granting or withholding life itself.. Once one accepts the initial premise advanced for the government providing universal healthcare, all this becomes inevitable. No matter that the participation of government itself creates shortages which justify granting or denying life-saving ministrations. To justify their God-playing, liberals presume that shortages will become evident in privately provided healthcare to the point where the market decides that some people might die earlier and some people might die later. Whether that choice is made by people acting prudently or imprudently in a free market, or by Almighty God himself, liberals cannot tolerate such power in the hands of others.. I accuse liberals of seeking control over health care because, just as environmentalism means ...
