Implantable cardioverter defibrillator (ICD) devices with or without cardiac resynchronization therapy (CRT-D) have the ability to continuously monitor heart rate variability, daily heart rate, and patient activity. Changes in these measures have been associated with heart failure prognosis. In addition, ICD or CRT-D devices also have the ability to measure daily intrathoracic impedance. Previous data have demonstrated that device-based intrathoracic impedance measurements correlate with pulmonary capillary wedge pressure and fluid retention, both of which are common measures of heart failure status. The objective of this study is to evaluate whether the heart rate variability, daily heart rate, and/or intrathoracic impedance can be used in conjunction with other methods to determine if patients are ready for hospital discharge. Methods We analysed data for patients undergoing major non-cardiac surgery in 2008 from The American College of Surgeons National Surgical Quality Improvement Program database (a prospective validated outcomes registry from 211 hospitals worldwide in 2008). We obtained anonymised data for 30-day mortality and morbidity (cardiac, respiratory, CNS, urinary tract, wound, sepsis, and venous thromboembolism outcomes), demographics, and preoperative and perioperative risk factors. We used multivariate logistic regression to assess the adjusted and modified (nine predefined risk factor subgroups) eff ect of anaemia, which was defi ned as mild (haematocrit concentration ,29-,39% in men and ,29-,36% in women) or moderate-to-severe (≤29% in men and women) on postoperative outcomes.. Little is known about the etiology of hospitalizations among HF patients. Fang et al. (21) examined hospitalizations from the National Hospital Discharge Survey from 1979 through 2004. They found that the proportion of hospitalizations with HF as a first-listed diagnosis remained at approximately 30% over the study period. However, there was a decline in the proportion of admissions due to coronary or other cardiovascular diseases, and an increase in the proportion due to noncardiovascular diseases. Curtis et al. (8) examined hospital readmission rates among Medicare beneficiaries hospitalized with HF from 2001 through 2005 and found that approximately 27% of readmissions were due to HF. This analysis did not include patients without a prior HF hospitalization and only examined the first readmission, and thus cannot provide information on the total burden of hospitalizations. This paper examines how organizational structure affects behavior and outcomes, studying the performance of different types of venture capital organizations. We find a strong positive relationship between the degree of specialization by individual venture capitalists at a firm and its success. When the individual investment professionals are highly specialized themselves, the marginal effect of increasing overall firm specialization is much weaker. The poorer performance by generalists appears to be due to both an inefficient allocation of funding across industries and poor selection of investments within industries. Venture capital organizations with more experience tend to outperform those with less experience.

Based on data from the National Deaths in Custody Program, this paper discusses gender specific trends in custodial deaths in Australia over the period 1980 to 2000. The paper presents statistics on Indigenous and non Indigenous female and male deaths in custody for the period, including statistics on the type of custody, most serious offence, and cause and manner of death. Of the 75 females who died in custody over this period, the majority of Indigenous female deaths occurred in police custody, whilenon Indigenous women were more likely to die in prison. Overall, self inflicted death by hanging was the leading manner of death for females (and males), although Indigenous women were more likely than non Indigenous women to die from natural causes. The paper also discusses five case studies of female deaths in custody, both Indigenous and non Indigenous.

They are used in patients for the purposes of diagnosis, therapy or surgery. We study whether venture capital firms with higher status and reputation are more risk tolerant due to their position in the field as opposed to lower status venture firms which might be more risk averse, due to less proven capabilities. We explore this possibility by considering whether or not role incongruity and gender stereotyping by venture capital firms lead to perceptions of women entrepreneurs as more risky investments. Results show that VC firm status produces a difference in risk tolerance and risk aversion in terms of investments in companies with women entrepreneurs, with higher status firms more likely to invest in women entrepreneurs. In addition, we find that companies with women CEOs are likely to be older and bigger and receive lower valuations than companies with men CEOs.

RESULTS: An estimated 34.7 million discharges from nonfederal short-stay hospitals occurred in 2005. Discharges used 165.9 million days of care and had an average length of stay of 4.8 days. Persons 65 years and over accounted for 38 percent of the hospital discharges and 44 percent of the days of care. The proportion of discharges whose status was described as routine discharge or discharged to the patients home declined with age, from 91 percent for inpatients under 45 years of age to 41 percent for those 85 years and over. Hospitalization for malignant neoplasms decreased from 1990-2005. The hospitalization rate for asthma was the highest for children under 15 years of age and those 65 years of age and over. The rate was lowest for those 15-44 years of age. Thirty-eight percent of hospital discharges had no procedures performed, whereas 12 percent had four or more procedures performed. This paper examines the spatial dynamics and emerging locational hierarchies of the venture capital (VC) industry in China, largely as a result of the global expansion of VC investors from the US and other advanced economies. Based on a comprehensive database compiled by the author from the best available sources, this is the first systemic geographic study on the VC industry in an emerging economy. This study finds that the rising cluster of VC supply and demand in Beijing, Shanghai, and Shenzhen was enabled by the juxtaposition of spatial proximity effects, investment syndication, and interregional office networks within Chinas unique institutional environment. Sample: All hospitals with 1,000 beds or more in the original universe of short-stay hospitals were selected with certainty in the sample. All hospitals with fewer than 1,000 beds were stratified, the primary stratum depending on size and geographic region. Within each of these 24 primary strata, the allocation of the hospitals was made through a controlled selection technique so that hospitals in the sample would be properly distributed with regard to type of ownership and geographic division. Sample hospitals were drawn with probabilities ranging from certainty for the largest hospitals to 1 in 40 for the smallest hospitals. Of the 535 hospitals in the sample, 413 were in scope and agreed to participate. The within-hospital sampling ratio for selecting sample discharges varied inversely with the probability of selection of the hospital. Our study indicates that perioperative hyperglycemia is associated with increased risk of hospital complications in patients undergoing noncardiac surgery. Perioperative hyperglycemia significantly increased the risk of pneumonia, systemic blood infections, urinary tract infection, skin infections, and ARF during the postoperative period. In addition, we found a significant association between blood glucose concentration and mortality; however, the impact of hyperglycemia on mortality rate was more significant among patients without a history of diabetes compared with those with known diabetes before admission.. The association between hyperglycemia and increased risk of hospital complications and mortality in critically ill (1,8,12,13) patients undergoing major cardiovascular surgery (12,16,17) is well established. OBJECTIVES: The aim of this study was to review and compare current health technology assessment (HTA) activities for medical devices across non-European Union HTA agencies. METHODS: HTA activities for medical devices were evaluated from three perspectives: organizational structure, processes, and methods. Agencies were primarily selected upon membership of existing HTA networks. The data collection was performed in two stages: stage 1-agency Web-site assessment using a standardized questionnaire, followed by review and validation of the collected data by a representative of the agency; and stage 2-semi-structured telephone interviews with key informants of a sub-sample of agencies. RESULTS: In total, thirty-six HTA agencies across twenty non-EU countries assessing medical devices were included. Twenty-seven of thirty-six (75 percent) agencies were judged at stage 1 to have adopted HTA-specific approaches for medical devices (MD-specific agencies) that were largely organizational or procedural. ...

Despite numerous guidelines on the management of anaemia in surgical patients, there is no pragmatic guidance for the diagnosis and management of anaemia and iron deficiency in the postoperative period. A number of experienced researchers and clinicians took part in a two-day expert workshop and developed the following consensus statement. After presentation of our own research data and local policies and procedures, appropriate relevant literature was reviewed and discussed. We developed a series of best-practice and evidence-based statements to advise on patient care with respect to anaemia and iron deficiency in the postoperative period. These statements include: a diagnostic approach to iron deficiency and anaemia in surgical patients; identification of patients appropriate for treatment; and advice on practical management and follow-up that is easy to implement.
