Bone tissue engineering strategies require cells with high proliferative and osteogenic potential as well as a suitable scaffold to support the development of these as they form new bone tissue. In this study, we evaluated whether the differentiation stage of amniotic fluid stem cells (AFSC) could enhance the regeneration of critical sized femoral defects in a rat model. For this purpose, AFSC were seeded onto a starch-poly(ε-caprolactone) (SPCL) scaffold and were cultured in vitro in osteogenic culture media for different periods of time in order to obtain: i) undifferentiated cells, ii) cells committed to the osteogenic phenotype and iii) osteoblast-like cells. In vitro results indicate that AFSC were considered to be osteogenically committed by the end of week 2 and osteoblastic-like after week 3 in culture. After a long wait we had the follow-up ultrasound today. Owen has been a very active boy for a while now which has helped us make it through the wait with much optimism. We just had our 20 week anatomy scan today, and for the most part things looked good with the exception of some renal abnormalities. DH was born with whats called vesiculoureteral reflux and had to have surgery as a baby to reattach his ureters to his bladder. Based on todays scan, we think that may be whats going on with our first LO. She has dilated collecting systems, normal kidneys,, normal bladder, and normal amniotic fluid levels. We are being referred to the Pediatric Nephrologist (who DH and I both work with and adore), as well as a Pediatric Urologist (not even a part of our hospital). Grade 5 VUR is the most severe and requires surgery. We are hopeful that Micah can have his surgery sooner than later so that he can be taken off of the antibiotics. The surgery will entail re-positioning where his ureter enters the bladder and tapering it so fluid can not back up so easy. He may also need the upper part of his ureter tapered where it meets the kidney. We are hopeful that his kidney is still functioning enough to not need it removed, but the tests will tell us more.. Once surgery is complete, he will remain on antibiotics for 3 months and until another set of tests are done. This is unfortunate because Micah gets sick from his antibiotics and we have been fighting acid reflux and vomiting since he was born. He still doesnt sleep through the night, but he has gotten better since we switched his antibiotic to the morning rather than bedtime. Ive been going to doctor after doctor trying to figure out why my SED Rate is elevated. My SED Rate was first tested in October of 06 and it was 27; then again in December and it was 32. Now just a few weeks ago, it came back 46. Ive been tested for everything. But Im heading in the right direction.
