In these instances, we adjust our standard approach and plan for addiction treatment around the individual to offer an Intensive Outpatient Program (IOP). Cornerstones IOP is designed to deliver the same recovery skills and therapy that our inpatient programs provide but in the settings, manner, and timing that work best for patients. VTE is an increasingly frequent complication of cancer and anti-cancer therapies. It is associated with increased mortality and other significant adverse consequences. Risk factors for VTE in the cancer population have been identified, and multiple studies have also shown that VTE can be prevented in high-risk populations with the use of thromboprophylaxis. This study evaluated the safety and efficacy of prophylaxis in a high-risk subgroup of cancer patients identified by a validated risk model developed by us previously called the Khorana Score. Correlative studies evaluated the value of tissue factor as a predictive biomarker of VTE. The purpose of this study was to conduct a prospective, randomized clinical trial comparing the safety and efficacy of prophylaxis with dalteparin to no treatment in reducing VTE in high-risk ambulatory cancer patients initiating chemotherapy and to establish the value of TF as a predictive marker for VTE in ambulatory cancer patients receiving ...
The therapy for insulin-dependent diabetes has been changing in the last 3 years with the increasing application of intensive ambulatory treatment programs involving self-monitoring of blood glucose levels by the patient. Insulin is administered either as multiple manual daily injections or as a continuous subcutaneous infusion delivered by a portable pump. We discuss the implementation, efficacy, complications (including recent reports of deaths in pump-treated patients), and cost of such programs. The potential effectiveness in preventing the long-term complications of diabetes warrants offering a program of self-monitoring of blood glucose levels combined with multiple manual daily insulin injections as a routine treatment option to virtually all patients with insulin-dependent diabetes. Additional observations on safety and efficacy are needed before insulin pump treatment can be considered a routine option. Furthermore, whether intensive treatment involving either manual or pump ...

INTRODUCTION. The reasons that lead people to visit a doctor come from a complex interaction of different factors such as demographic, socioeconomic, and psychological aspects, morbidity profiles, and health services availability.2,5 The effect and relative importance of each factor are affected by cultural background, health policies and health care system available.. As lower socioeconomic groups have a higher burden of disease and therefore need more health services, equity is at the heart of the entire health care issue. In Brazil, a universal decentralized and free of charge health care system was created, the Unified Health System (SUS), following the promulgation of the 1988 Federal Constitution. Reforms have occurred since then, but there is evidence that, despite intended universal provision, health services utilization among social groups remains unequal.1 Furthermore, there is limited information about how demographic, socioeconomic, health needs and other factors affect health ...

Saturday, July 17th, 2011. Hi all! It has been 14 weeks since April 7th, 2011. I will never forget that date. My recovery is going well and I received some really fantastic news this week. I have been officially released from my first session of PT, OT and SP! This is an amazing milestone for me. I went into my outpatient therapy testing at 54% functional and was just released because I am now testing at 94% functional compared to "normal" standards. This does not at all mean that I am fully recovered but just that I am doing well with regard to physical recovery and that I can physically take care of myself. This means that I can walk without falling (which is really good because I could not do that before this all happened), dress myself and take a shower without assistance, go up and down the stairs, shop and drive. I could NOT do these things when I first started my outpatient therapy.. As I passed these tests easily this week, I realized just how far I have come. I had tears in my eyes all ...

ichaels mom, Laurie, shares his story. "Things changed for Michael when he was a teen. He went from a happy, responsible child, to having trouble in school. He couldnt finish his homework, acted out in class and was labeled a troublemaker. Kids bullied and hazed him. He was introduced to marijuana, quickly went to alcohol and dropped out of school.. We tried homeschooling and Job Corps. By 18, he was using opiates, alcohol and briefly heroin. He overdosed on heroin twice and nearly died from alcohol poisoning.. He went to AA, rehab and counseling, but often waited up to six weeks for treatment.. In his late 20s, he tried the Medical Centers Intensive Outpatient Program, but was intimidated by the group and didnt have transportation. He needed inpatient care but with no insurance was unaware of his options.. He tried hard to stay sober. He wanted to be a successful part of society. But he never felt like he fit in. Looking back, I know we missed the signs. We focused on the alcohol, but ...

We are pleased to collaborate with Stanford Hospital to bring important medical and surgical services to our community," said School of Medicine Dean Philip Pizzo, MD. "Improving access to clinical care and enhancing the quality and performance of our clinical services go hand in hand with our efforts to improve the diagnosis, treatment and prevention of disease through research and innovation." Stanford outpatient clinics that are not moving to Redwood City will remain in their current locations on the main Stanford campus. Following construction of the new Stanford Hospital (currently projected to open in 2015-16 next to the existing hospital), these programs are expected to occupy renovated or rebuilt facilities at the medical center. "This expanded capacity will improve access to services where growing demand has outpaced capacity in our current location," said Helen Wilmot, the hospitals vice president for ambulatory services. She estimates that 120,000 patient visits (20 percent of the ...

Comparison of data: Attendant v. non-attendant groups. The attendant and non-attendant groups were similar in terms of demographic and clinical data (Table 1). The numbers (n) are stated in Table 1 and where they differ from the total, the difference can be accounted for by missing data. There were no significant differences between attendant and non-attendant groups in terms of gender, age, employment status, disability-grant status, level of education, distance from the clinic or marital status. Similarly, there were no significant differences between the groups in terms of clinical data including World Health Organization (WHO) stage of HIV infection, nadir CD4 count, length of hospitalisation, time to follow-up appointment, prescription of monotherapy v. polytherapy and substance abuse history. When data were grouped for analysis (because of small sample size), there was no significant association between attendance/non-attendance and the presence or absence of a primary psychiatric ...

Yesterday, the Medicare Payment Advisory Commission (MedPAC) voted to adopt several recommendations on outpatient therapy payment reform. These recommendations will be included in a report to Congress that may be used to inform future policy related to outpatient therapy services. Congress has the discretion to determine whether or not to pass legislation that incorporates any of these recommendations. The Centers for Medicare and Medicaid Services also can choose to enact MedPACs recommendations. APTA will continue to work diligently over the next couple of months with Congress to extend the exceptions process for therapy services in 2013 and to avoid any payment cuts. Overall, MedPAC commissioners expressed appreciation of the value of outpatient therapy services for Medicare beneficiaries and recognized that a "hard cap" with no exceptions would be detrimental and severely impede access to medically necessary therapy services. Objective - The role of mental illness and addiction in acute care use for chronic medical conditions that are sensitive to ambulatory care management requires focussed attention. This study examines how mental illness or addiction affects risk for repeat hospitalization and/or emergency department use for ambulatory care-sensitive conditions (ACSCs) among high-cost users of medical care.. Method - A retrospective, population-based cohort study using data from Ontario, Canada. Among the top 10% of medical care users ranked by cost, we determined rates of any and repeat care use (hospitalizations and emergency department [ED] visits) between April 1, 2011, and March 31, 2012, for 14 consensus established ACSCs and compared them between those with and without diagnosed mental illness or addiction during the 2 years prior. Risk ratios were adjusted (aRR) for age, sex, residence, and income quintile.. Results - Among 314,936 high-cost users, 35.9% had a mental illness or addiction. Compared to those ...

Introduction. The use of outpatient parenteral antimicrobial therapy (OPAT) as a treatment strategy with the aim of de-hospitalizing patients has been growing since its advent during the 1970s.1 OPAT has become a safe and standardized practice for patients presenting with various infections who require long-term parenteral antimicrobial therapy. International consensus guidelines have determined that OPAT can be performed in physicians offices, clinics, specialized infusion centers or in patients homes.2,3. Patients should be selected for this type of treatment by physicians who are familiar with the infectious conditions that will be treated. They should also be evaluated by nurses who have experience in implementing and managing long-term venous access, and by social workers who will decide whether patients present social, economic and cultural conditions that allow them to be safely treated this way. If patient evaluation and selection are performed adequately, OPAT is acknowledged to be ...
