Background Quality of manual cardiopulmonary resuscitation (CPR) during extrication and transport of out-of-hospital cardiac arrest victims is known to be poor. Performing manual CPR during ambulance transport poses significant risk to the attending emergency medical services crew. We sought to use pre-hospital video recording to objectively analyse the impact of introducing mechanical CPR with an extrication sheet (Autopulse, Zoll) to an advanced, second-tier cardiac arrest response team. Methods The study was conducted prospectively using defibrillator downloads and analysis of pre-hospital video recording to measure the quality of CPR during extrication from scene and ambulance transport of the OHCA patient. Incidence and causes of delay in Emergency call were analyzed in 3267 out-of-hospital cardiac arrests that were witnessed by bystanders or not witnessed but found by citizens from April 1, 2003 to March 31, 2007 in Ishikawa Prefecture. Emergency calls were considered to be significantly delayed when interval of collapse or event recognition to call exceeded 2 min. The incidence of delay was 32.9% (1076/3267) in the all arrests and significantly higher in urban (Non-Central) areas than in rural (the Central) area (691/1539 vs. 385/1728, p,0.001). Telephone-assisted CPR was performed in 501 out of 1076 delayed cases and CPR was initiated in 331 cases. The proportions of ROSC and successful resuscitation were significantly lower when the call was delayed (178/1076 vs. 490/2191 p,0.001, 153/1076 vs. 415/2191 p,0.001, respectively). Nausea and vomiting are believed to be frequent side-effects and routine co-administration of metoclopramide is common. In the absence of pre-hospital data to support this practice, we sought to determine the incidence of peri-opiate nausea and vomiting in an ambulance service which does not administer anti-emetics. Methods This prospective observational study is currently assessing the incidence of emesis in 400 patients attended by the North East Ambulance Service, aged above 17 years and receiving morphine, using a patient-scored Nausea and Vomiting Score (NVS: 0=no nausea or vomiting, 1=slight nausea, 2=moderate nausea, 3=severe nausea, 4=vomited once, 5=vomited twice or more).
