The declining availability of abortion care has been the topic of many studies, commentaries, conferences and advocacy initiatives over the last 20 years. Among the ideas suggested to ameliorate this problem is increasing the number of physician assistants (PAs), nurse practitioners (NPs), and certified nurse midwives (CNMs) who perform first-trimester abortion.1-6 While addressing access through this strategy holds great promise, it is not simply a matter of access that calls for more clinicians to participate in abortion care. Rather, as health professionals, we should expect that professional scope of practice determinations are based upon whether the profession can provide this proposed service in a safe and effective manner 7 and not solely on the lack of physicians available to provide the service. It is time to acknowledge that PAs, NPs and CNMs [collectively known as advanced practice clinicians (APCs)] are capable and qualified to provide abortion care services, but that current efforts ...

Interest in reducing the increasing numbers of unplanned hospital admissions in the UK, as in other countries [1-5], is leading to attempts to stem the demand [6]. Rising unplanned admissions are expensive and disrupt planned health care, notably by impeding elective work. They also cause considerable disruption for patients and carers. One of the approaches attempted has been case management. Case management was developed in the US and attempts to identify individuals judged to have an increased risk of unplanned hospital admission. Those identified are managed proactively, primarily in an attempt to reduce unplanned hospital admissions. However, few studies describe the intervention and the processes undertaken in detail [7, 8].. Case management of frail older people by practice-based nurses with extended roles has been implemented widely in England through many initiatives and by the policy-based recruitment of community matrons [9]. This policy was partly based on an American study which ...

American Association of Critical Care Nurses developed documents for regulating the scope and standards of the critical care nursing practices. To understand better the role of the NP in pediatric critical care, a survey was created and distributed to NPs working in pediatric critical care. The 2-part questionnaire which was the research tool used in the survey was elaborated based on other validated tools (see implementation tool section) and was trialed during a pilot project. Prior to distribution, the final version of the survey tool was revised following review and feedback from several experts in pediatric advanced practice (including a Synergy Model expert) for content validity [p398, 1], [2]. Potential respondents were sought from subscribers to the Pediatric Intensive Care Advanced Practice Nursing (PICUAPN) and the Advanced Nursing Practice in Acute and Critical Care (ANPACC) electronic list servers during 1999 and 2000 and additional participants were identified by contacting ...

In our facilities, there is an ability for Physicians and Advanced Practice Providers to interact with patients and their families to a much greater degree than what occurs in the hospital. The sub-acute side provides the excitement of a fast paced, high-acuity environment. In contrast, there is a level of intimacy created with long term care patients - and their loved ones - the ability to provide very intensive care to a narrow panel of patients.. Our team is present for life cycle changes, and patients often come to view clinicians as family. Here, you are establishing and growing relationships. Youre investing emotionally. What were doing in a Genesis facility today is not terribly different from what was done in acute-care hospitals ten years ago, and its imperative we have the right team-based partnership among Physicians and Advanced Practice Providers and the rest of our organization.. Unlike private practice, working at GPS means no overhead to maintain or practice to buy ...
