The replacement of intervertebral discs in the spinal column with artificial devices. The procedure is done in the lumbar or cervical spine to relieve severe pain resulting from INTERVERTEBRAL DISC DEGENERATION.
Any of the 23 plates of fibrocartilage found between the bodies of adjacent VERTEBRAE.
Surgical insertion of a prosthesis.
VERTEBRAE in the region of the lower BACK below the THORACIC VERTEBRAE and above the SACRAL VERTEBRAE.
Excision, in part or whole, of an INTERVERTEBRAL DISC. The most common indication is disk displacement or herniation. In addition to standard surgical removal, it can be performed by percutaneous diskectomy (DISKECTOMY, PERCUTANEOUS) or by laparoscopic diskectomy, the former being the more common.
Partial or total replacement of a joint.
Degenerative changes in the INTERVERTEBRAL DISC due to aging or structural damage, especially to the vertebral end-plates.
Artificial substitutes for body parts, and materials inserted into tissue for functional, cosmetic, or therapeutic purposes. Prostheses can be functional, as in the case of artificial arms and legs, or cosmetic, as in the case of an artificial eye. Implants, all surgically inserted or grafted into the body, tend to be used therapeutically. IMPLANTS, EXPERIMENTAL is available for those used experimentally.
An INTERVERTEBRAL DISC in which the nucleus pulposus has protruded through surrounding fibrocartilage. This occurs most frequently in the lower lumbar region.

Currently, herniated nucleus pulposus (HNP) with radiculopathy and other preconditions are regarded as relative or absolute contraindications for lumbar total disc replacement (TDR). In Switzerland it is left to the surgeons discretion when to operate. The present study is based on the dataset of SWISSspine, a governmentally mandated health technology assessment registry. We hypothesized that preoperative nucleus pulposus status and presence or absence of radiculopathy has an influence on clinical outcomes in patients treated with mono-segmental lumbar TDR. Between March 2005 and April 2009, 416 patients underwent mono-segmental lumbar TDR, which was documented in a prospective observational multicenter mode. The data collection consisted of perioperative and follow-up data (physician based) and clinical outcomes (NASS, EQ-5D). Patients were divided into four groups according to their preoperative status: 1) group degenerative disc disease (DDD): 160 patients without HNP and no radiculopathy, classic

Back pain is a common complaint and the origin of this frequently attributed to degenerative disc disease. In the most severe cases, the integrity of the disc and surrounding tissue is lost to such an extent that surgical intervention is necessary. Fusion procedures are commonly used to treat severely degenerated discs. Yet this is known to alter the biomechanics of the operated level, and may create a progression of degenerative decline. Total disc replacement has emerged as a viable treatment but the complexity of the spine is reflected in the clinical results, which trail far behind the success of hip and knee arthroplasty. This may be due to a failure of total disc replacement procedures to restore the natural biomechanics of the spine. The present study has led to the development of a dynamic pre-clinical testing protocol to quantitatively assess the efficacy of disc replacement devices. A six-axis spine simulator was designed and built, and the stiffness matrix testing of porcine lumbar ...

[more ▼]. OBJECT: Although cervical total disc replacement (TDR) has shown equivalence or superiority to anterior cervical discectomy and fusion (ACDF), potential problems include nonphysiological motion (hypermobility), accelerated degeneration of the facet joints, particulate wear, and compromise of the mechanical integrity of the endplate during device fixation. Dynamic cervical stabilization is a novel motion-preserving concept that facilitates controlled, limited flexion and extension, but prevents axial rotation and lateral bending, thereby reducing motion across the facet joints. Shock absorption of the Dynamic Cervical Implant (DCI) device is intended to protect adjacent levels from accelerated degeneration. METHODS: The authors ...

Surgical options to reduce the pain of DDD include:. Discectomy: the surgical removal of herniated disc material that presses on a nerve root or the spinal cord. The procedure involves removing the central portion of an intervertebral disc, the nucleus pulposus, which causes pain by stressing the spinal cord or radiating nerves.. Laminectomy: surgery that creates space by removing the lamina - the back part of the vertebra that covers your spinal canal. Also known as decompression surgery, laminectomy enlarges your spinal canal to relieve pressure on the spinal cord or nerves.. Spinal fusion: a neurosurgical or orthopedic surgical technique that joins two or more vertebrae. This procedure can be performed at any level in the spine (cervical, thoracic, or lumbar) and prevents any movement between the fused vertebrae.. Lumbar Total Disc Replacement: With artificial disc replacement, pain relief is brought about by removal of the painful disc and motion is maintained with the use of a prosthetic ...

Our neurosurgical team adopts the finest of spinal surgical practices for patient comfort. There are many artificial disc methods typically classified into two general types: total disc replacement and disc nucleus replacement. In a total disc replacement, all or most of the disc tissue is removed and a replacement device is implanted into the space between the vertebrae. With a disc nucleus replacement, only the center of the disc is removed and replaced with an implant. The outer part of the disc is not removed. By removing disc tissue and eliminating movement, the pain will be significantly reduced.. Complexities of artificial disc replacement ...

This invention improves upon prior art total disc replacements (TDRs) by more closely replicating the kinematics of a natural disc. The preferred embodiments feature two or more fixed centers of rotation (CORs) and an optional variable COR (VCOR) as the artificial disk replacement (ADR) translates from a fixed posterior COR that lies posterior to the COR of the TDR to facilitate normal disc motion. The use of two or more CORs allows more flexion and more extension than permitted by the facet joints and the artificial facet (AF). AF joint-like components may also be incorporated into the design to restrict excessive translation, rotation, and/or lateral bending.

BACKGROUND:The ideal procedure for multilevel cervical degenerative disc diseases remains controversial. Recent studies on hybrid surgery combining anterior cervical discectomy and fusion (ACDF) and artificial cervical disc replacement (ACDR) for 2-level and 3-level constructs have been reported in the literature. The purpose of this study was to estimate the biomechanics of 3 kinds of 4-level hybrid constructs, which are more likely to be used clinically compared to 4-level arthrodesis. MATERIAL AND METHODS:Eighteen human cadaveric spines (C2-T1) were evaluated in different testing conditions: intact, with 3 kinds of 4-level hybrid constructs (hybrid C3-4 ACDR+C4-6 ACDF+C6-7ACDR; hybrid C3-5ACDF+C5-6ACDR+C6-7ACDR; hybrid C3-4ACDR+C4-5ACDR+C5-7ACDF); and 4-level fusion. RESULTS:Four-level fusion resulted in significant decrease in the C3-C7 ROM compared with the intact spine. The 3 different 4-level hybrid treatment groups caused only slight change at the instrumented levels compared to intact except

This is an operation which involves removing a worn or danmaged disc from the lumbar spine and replacing it with an artificial one – this is made of either medical-grade metal or a combination of metal and plastic. This is done as an alternative to the more widely known lumbar fusion. It can be particularly beneficial in certain patients, and is not suitable for everyone. Mr Cass will carefully explain the reasons why it may be suitable for you. The normal intervertebral disc has several purposes: It acts as a spacer between the vertebral bones allowing full range of motion and preventing any compression of the nerves. It acts as a shock absorber, enabling the spine to compress and rebound. It acts as a motion unit through its elasticity, allowing flexion, extension and rotation at the same time. Disc replacement surgery aims to mimic as many of these qualities as possible, particularly the preservation of movement between adjacent vertebrae. Mr Cass pioneered the technique in South

The second generation of the Cummins/Bristol cervical disc was Prestige I cervical prosthesis (also named Frenchay artificial joint), developed in 1998. It is a two-piece prosthesis constructed of stainless steel and employs a ball-in-groove articulation (Medtronic Sofamor Danek;). This semi-constrained arrangement allows a greater degree of translation to occur with rotation. It is a modified version of the original Cummins prosthesis which was based on a ball-and-socket design.. The Prestige II was designed in 1999 and main difference to its predecessor being a more anatomic end-plate design; porous coated to improve bony ingrowth3. In a prospective, randomised clinical trial, Porchet et al4 compared the Prestige II Cervical Disc with ACDF for the treatment of single-level disease and their radiographic results showed that Prestige II disc maintains motion at the treated level without adjacent-segment compromise.. The Prestige ST became available in 2002 and the major change from Prestige II ...

Disc replacements were first designed for the low back (lumbar spine). Their success in restoring pain free motion led to the development of a similar device for the neck (cervical spine). Although not everyone qualifies for a cervical intervertebral disc replacement, they seem to work well for those patients who are good candidates.. In doing a literature search, we did find a recent review of cervical disc replacements (CDRs) by two surgeons at the William Beaumont Hospital in Michigan. Their intent was to bring surgeons up-to-date on the current status of these implants but much of the information they provided will be of interest to you (and other potential patients).. There are now five different models and designs to choose from. Some are made of a titanium-ceramic material. Others are a titanium-alloy outer part with a polyurethane (plastic) core. Shapes vary and include saddle-shaped, triangular, round, square, and square with rounded edges.. The goals of a good fitting design are first ...
Vidant Health - Artificial disc replacement surgery is surgery to replace a damaged and painful disc in the lower back or neck with an artificial disc.
Multi-component artificial disc replacements (ADRs) facilitate in situ assembly within a disc space. In contrast to single-component ADRs, which use endplates constructed of a single material, assembled ADRs according to the invention allow the use of more than one material, even for the endplates themselves in certain embodiments. As such, materials with good wear characteristics such as chrome cobalt can be combined with materials such as Nitinol exhibiting other desirable characteristics such as the elasticity or shape-memory properties. Devices according to the invention can be used for other joints of the body, such as prosthetic knees and hips.

The PRODISC®-L Total Disc Replacement is an artificial intervertebral disc made from metal and plastic that is used to treat pain associated with degenerative disc disease (DDD). DDD is defined as discogenic back pain (pain resulting from a degenerated intervertebral disc) with degeneration of the disc confirmed by patient history and radiographic studies. The PRODISC®-L Total Disc Replacement is implanted to replace a diseased or damaged intervertebral disc during a surgical procedure called spinal arthroplasty.. The PRODISC®-L Total Disc Replacement consists of three parts:. Two metal (cobalt-chrome alloy) endplates that are anchored to the top and bottom surfaces of the spinal bones (vertebrae) and a plastic (ultra-high molecular weight polyethylene, or UHMWPE) inlay that fits between the two endplates.. The plastic inlay and endplates help restore the natural distance between the two vertebrae (disc height). The top (superior) endplate can slide over the domed part of the inlay, which can ...

Identifying the disc pain symptoms is very essential as this will help in effectively diagnosing the causes behind the symptoms and prescribing artificial disc replacement when required.
We are a new organization (as of 2007, The Arthroplasty Patient Foundation) and are currently developing our plans for the future. We encourage feedback from people whose lives and business focus on artificial disc replacement. We would especially like to hear from patients, medical professionals and artificial disc manufacturers.. We launched this site during the week of September 12th, 2004 and welcomed visitors from 18 U.S. States. International visitors from Germany, Canada, Australia, Sweden, England and Israel logged on and we received our first inquiries from back patients interested in artificial disc replacement. We expect the sites activity to grow, but we need help from the global community to provide helpful and instructive content to artificial disc replacement patients. Your feedback and support can help us in many ways!. We are presenting a novel, non-biologic model of the healthy human annulus. This lumbar Total Disc Model (TDM) ideally would be both biofidelic and sufficiently robust to withstand long-term fatigue testing without breaking down or tearing apart. Mechanical validation testing was performed to confirm that the compressive and torsional properties were similar to literature values of denucleated human lumbar discs in two-body constructs. Long-term fatigue tests were performed to establish the durability of the model. We have reported data for both our empty TDM and the TDM filled with a representative nucleus replacement device (NRD). The silicone model is geometrically equivalent to the healthy human lumbar disc, including the discoid cavity present following a total nuclectomy, the annulus fibrosis with micro-annulotomy, and the cartilaginous endplates. The pressure transmitted through the center of the disc went from negligible when the TDM was empty to over 40% when the TDM was filled. The ...
Dr Harshavardhan Hegde, consultant and head, Department of Orthopaedics and Spine Surgery at Artemis Health Institute, Gurgaon, has claimed to have performed a rare four-level disc replacement surgery on a 54-year American patient, the first in Asia, last week. Dr Hegde in an eight-hour surgery, replaced two lumbar discs (lower back) and two cervical discs (neck). According to the hospital, the patient was suffering from disc degeneration for 10 years and he came to know about Dr Hegde from a patient who had been treated by him earlier.. One option was to have a fusion surgery and the other was to have disc replacement surgery which is more complicated but has faster recovery. He chose the latter. The patient was walking on the first post-operative day and was discharged from the hospital in five days, the hospital claimed.. The cost for the surgery in India too was a big relief for him. It cost him $25,000 for the four-level disc replacement while in the States, he was given a budget of around ...
Facet joint arthrosis is an intervertebral disc disorder. The facet joints or zygapophyseal joints are synovial cartilage covered joints that limit the movement of the spine and preserve segmental stability. In the event of hypertrophy of the vertebrae painful arthrosis can occur. The lumbar facet arthrosis syndrome was described in a 1987 article by S. M. Eisenstein and C. R. Parry of Witwatersrand University. Computerized tomography is the ideal for typifying facet joint arthrosis; evidence suggests that magnetic resonance imaging is not reliable in this regard. Artificial disc (ADR) and joint replacement components, particularly those associated with the knee and hip, use compressible, resilient materials to approximate natural kinematics, often in conjunction with elastic or spring components to tether articulating components to avoid dislocation, excessive compression, flexion or other out-of-range movements. The preferred embodiments require only two or a few articulating components. The designs allow for normal flexion, extension, lateral bending, rotation, and translocation, depending upon the application.
Currently, artificial disc implants are designed to be placed into the disc space from the front and center position. To get there, the surgical approach is performed anteriorly (from the front).
Artificial disc replacement is a procedure that can offer pain relief and restore function. The artificial disc replacement is a prosthesis designed to simulate the function of a natural cervical disc.
