Predicting sudden death in the population: the Paris Prospective Study I. (1/248)

BACKGROUND: Sudden death was found to share the same set of usual risk factors as coronary events and therefore could not be specifically predicted in the population. It appears, however, that parental history of sudden death has not been investigated yet as a risk factor for sudden death. Therefore, we assessed risk factors, including parental sudden death, associated with the occurrence of sudden death in a long-term cohort study. METHODS AND RESULTS: We included 7746 men employed by the city of Paris who were 43 to 52 years of age in 1967 to 1972 in the Paris Prospective Study I. Each subject underwent a physical examination and an ECG, provided blood for laboratory tests, and answered questionnaires administered by trained interviewers who paid particular attention to family medical history. Men with known ischemic cardiac disease were further excluded from analysis. For 95.5% of the men, vital status was obtained from specific inquiries until retirement, then by death certificates. Resting heart rate, systolic or diastolic blood pressure, tobacco consumption, body mass index, diabetes status, serum cholesterol, and parental history of sudden death were independent factors associated with sudden death during follow-up (23 years on average). When adjusted for confounding variables, including parental history of myocardial infarction, relative risk of sudden death associated with parental sudden death was 1.80 (95% CI, 1.11 to 2.88). CONCLUSIONS: Parental sudden death is an independent risk factor for sudden death in middle-aged men. The existence of familial risk factors for sudden death may help provide better identification of subjects at high risk of and early prevention of sudden death.  (+info)

Idiopathic atrial fibrillation as a risk factor for mortality. The Paris Prospective Study I. (3/248)

AIMS: Idiopathic atrial fibrillation describes atrial fibrillation of unknown origin occurring without heart disease. Mortality is considered unaffected by idiopathic atrial fibrillation. We used the long follow-up period (23 years on average) of the Paris Prospective Study I to assess the mortality of idiopathic atrial fibrillation subjects in middle-aged men. METHODS: 7746 working Frenchmen, aged 43-52 in 1967-72, underwent a physical examination plus ECG, answered questionnaires, and provided blood samples. Strict exclusion criteria were used to select idiopathic atrial fibrillation only, and men with known cardiac disease were further excluded from analysis. At 1 January 1994, vital status was unknown for 4.6% of the subjects. The analysis was conducted on the 6722 remaining subjects. RESULTS: Twenty-five subjects had idiopathic atrial fibrillation at inclusion. The relative risk (and 95% confidence interval) associated with idiopathic atrial fibrillation was 4.22 [2.10-8.47] for cardiovascular mortality (P=0.0001) and 1. 97 [1.14-3.40] for total mortality (P=0.01). When age, systolic blood pressure, cholesterol, body mass index and tobacco consumption were entered into a Cox model, idiopathic atrial fibrillation remained an independent risk factor for cardiovascular (P=0.0008) and total death (P=0.04). CONCLUSION: With a long follow-up period, idiopathic atrial fibrillation was associated with higher mortality in middle aged Frenchmen.  (+info)

Is there a glycemic threshold for mortality risk? (4/248)

OBJECTIVE: To determine whether there are thresholds for fasting and for 2-h glucose above which the risk of death from all causes and from coronary heart disease (CHD) increases. RESEARCH DESIGN AND METHODS: We studied 23-year mortality data from the Paris Prospective Study of the 7,018 men, aged 44-55 years, who were not known as diabetic at the baseline examination. The effect of glucose concentrations on mortality was studied using the observed relative risks and an age-adjusted Cox proportional hazards model. RESULTS: For all causes of death, there were J-shaped relationships with both fasting and 2-h glucose concentrations, and the lowest observed death rates were in the intervals centered on 5.5 mmol/l for fasting glucose and 5.0 mmol/l for 2-h glucose. The death rates for CHD were low in this population: for fasting glucose, the hazards ratio was best modeled by a positive linear relationship; for 2-h glucose, it was modeled by a J-shaped curve and the lowest observed death rate was in the interval centered on 6.0 mmol/l. CONCLUSIONS: In the Paris Prospective Study, there were no clear thresholds for fasting or 2-h glucose concentrations above which mortality sharply increased; in the upper levels of the glucose distributions, the risk of death progressively increased with increasing fasting and 2-h glucose concentrations.  (+info)

Single clonal origin of a high proportion of Legionella pneumophila serogroup 1 isolates from patients and the environment in the area of Paris, France, over a 10-year period. (6/248)

Arbitrarily primed PCR with three primers and pulsed-field gel electrophoresis were used to characterize a set of 75 clinical Legionella pneumophila serogroup 1 isolates, with no apparent epidemiological link, obtained from 24 hospitals in Paris, France, from 1987 to 1997. Unexpectedly, 25 clinical isolates from 15 hospitals had an identical profile (termed type A) by both methods. The same profile was subsequently found in 16 of 64 randomly selected environmental L. pneumophila serogroup 1 isolates from 15 different sites in the Paris area. There was no evidence of geographic clustering or a peak incidence of type A isolation. Type A has not been found in France outside the Paris area, suggesting that a particular type of L. pneumophila serogroup 1 is specifically present in the Paris water distribution network.  (+info)

Epirubicin plus tamoxifen versus tamoxifen alone in node-positive postmenopausal patients with breast cancer: A randomized trial of the International Collaborative Cancer Group. (7/248)

PURPOSE: To assess whether the addition of epirubicin (EPI) therapy to prolonged treatment with tamoxifen (TAM) improves relapse-free and overall survival in postmenopausal women with node-positive primary breast cancer. PATIENTS AND METHODS: Six hundred four patients entered onto a randomized clinical trial were allocated to receive TAM 20 mg/d for 4 years or TAM 20 mg/d for 4 years plus EPI 50 mg/m(2) intravenously on days 1 and 8 every 4 weeks for six cycles. Analysis was performed according to allocated treatment, with all randomized patients included (intention to treat), irrespective of eligibility status. RESULTS: After a median follow-up period of 5.7 years, an improvement in relapse-free survival (RFS) was observed for the TAM and EPI-treated patients, compared with those who received TAM alone. The unadjusted hazard ratio was 0.72 (95% confidence interval, 0.54 to 0.96), with a corresponding reduction in the odds of recurrence of 27.9% (SD, 12. 3), which was statistically significant (P =.023). Adjustment for prognostic and/or predictive factors did not materially affect the hazard ratio. No difference was observed in terms of overall survival (reduction in odds of death, 11.9% [SD, 16.3]; P =.46). Combined chemohormonal treatment was associated with a higher incidence of acute side effects but without a clear increase in long-term cardiotoxicity. Twelve nonbreast second malignancies, including five hematologic malignancies (two of which were cases of acute myelogenous leukemia), were observed. CONCLUSION: The data show that combined chemohormonal treatment reduces the risk of relapse in postmenopausal patients with node-positive breast cancer. No evidence was found, however, for an improvement in overall survival. The size of benefit observed for both outcomes was consistent with that reported in the Early Breast Cancer Trialists' Collaborative Group overview. The trial presented here, however, provides the first report of an improvement in RFS associated with the provision of a single cytotoxic drug in addition to prolonged TAM.  (+info)

Impact of guidelines implemented in a paris university hospital: application to the use of antiemetics by cancer patients. (8/248)

AIMS: To assess the impact with time of guidelines on antiemetic use in an 850-bed Paris university hospital with a high proportion of cancer patients. METHODS: Guidelines on the use of antiemetics available in cancer chemotherapy were drafted according to the Delphi technique. Their implementation was based upon a patient-specific antiemetic prescription form. To assess the impact of guideline implementation over time, discrepancies between current practice and the guidelines were compared before guideline implementation (between March and August 1995) and after implementation (between March and August 1997, and March and August 1998). RESULTS: Before the Delphi panel's guidelines were implemented, 5-HT3 antagonists were inappropriately administered in 70% of cases. After guideline implementation, this proportion dropped significantly (P<0.0001, Fisher's exact test) to 22% between March and August 1997 and 28% between March and August 1998. CONCLUSIONS: Implementation of guidelines seems to have resulted in significant changes with time, although a causal relationship has not been demonstrated. The development of guidelines by our hospital's multidisciplinary working group helped the various consultants to adjust medical practices to take account of these changes.  (+info)

The Samenta study was conducted in 2009 in the Greater Paris area to estimate the prevalence of psychiatric disorders among the homeless. A cross-sectional survey was performed with a three-stage random sample of homeless people (n = 859), including users of day services, emergency shelters, hot meal distribution, long-term rehabilitation centres and social hotels. Information was collected by a lay interviewer, using the Mini International Neuropsychiatric Interview, and completed by a psychologist through an open clinical interview. In the end, a psychiatrist assessed the psychiatric diagnosis according to the ICD10. One third of the homeless in the Paris area had at least one severe psychiatric disorder: psychotic disorders (13%), anxiety disorders (12%) or severe mood disorders (7%). One in five was alcohol dependent and 18% were drug users. Homeless women had significantly higher prevalence of anxiety disorders and depression and men were more likely to suffer from psychotic disorders. Homeless

Often when he went to work I would go to the Champs de Mars, the park in front of Eiffel Tower, to read. What I remember most about the Champs de Mars was the nannies dressed in dark blue with white aprons pushing enormous, dark blue, baby carriages. White lace coverlets would peek over the top, and often little arms and legs too. The Paris megacity experiences frequent particu- late matter (i.e.PM2.5, particulate matter with a diameter less than 2.5 μm) pollution episodes in spring (March-April). At this time of the year, large numbers of the particles consist of ammonium sulfate and nitrate which are formed from am- monia (NH3) released during fertilizer spreading practices and transported from the surrounding areas to Paris. There is still limited knowledge of the emission sources around Paris, their magnitude, and their seasonality. Using space-borne NH3 observation records of 10 years (2008-2017) and 5 years (2013-2017) provided by the In- frared Atmospheric Sounding Interferometer (IASI) and the Cross-Track Infrared Sounder (CrIS) instrument, regional patterns of NH3 variabilities (seasonal and interannual) are derived. Candida tropicalis is a diploid ascomycetes yeast responsible for 4%-24% of candidemia. Resistance to flucytosine is rarely described for this species but was observed for 45 (35%) of 130 C. tropicalis isolates recovered from blood cultures in the Paris area in a 4-year survey. The aims of this study were to test the hypothesis that the flucytosine-resistant isolates could represent a subgroup and to determine the relationship between epidemiologic and genomic data. Epidemiologic data and gene sequences were analyzed, and molecular typing was performed. Our results suggest that a clone of flucytosine-resistant isolates, associated with malignancies and a lower mortality than that for other C. tropicalis isolates, is widespread in the Paris area. We propose the analysis of 2 polymorphic microsatellite markers coupled with URA3 sequencing to track the clone.

Abstract. The Seine watershed has long been the food-supplying hinterland of Paris, providing most of the animal and vegetal protein consumed in the city. Nowadays, the shift from manure-based to synthetic nitrogen fertilisation, has made possible a strong land specialisation of agriculture in the Seine watershed: it still provides most of the cereal consumed by the Paris agglomeration, but exports 80% of its huge cereal production. On the other hand the meat and milk supply originates mainly from regions in the North and West of France, specialised in animal farming and importing about 30% of their feed from South America. As it works today, this system is responsible for a severe nitrate contamination of surface and groundwater resources. Herein two scenarios of re-localising Pariss food supply are explored, based on organic farming and local provision of animal feed. Parkinson disease (PD) is a neurodegenerative movement disorder that affects 1 to 2% of the human population over the age of 65. This prevalent disease has characteristics including resting tremor, rigidity, bradykinesia and postural instability due to the loss of dopaminergic neurons in the substantia nigra pars compacta in the brain. Mitochondrial dynamics play a significant role in PD with a balance between surveillance and biogenesis being key. The newly discovered gene Paris has been proposed to be central in the coordination of mitochondrial processes. These processes seem to be controlled by a number of PD associated genes. We have identified CG15436 as an excellent candidate to carry out studies on the Paris function in Drosophila melanogaster. Knockdown of CG15436 reduces longevity and locomotor ability overtime to produce a Paris dependent Drosophila model of PD. Spectroscopic data was used to confirm their structures. Their antitumor properties were determined by using an MTT assay in addition to ethidium bromide and acridine orange staining techniques. Compounds 2, 3 and 4 exhibited significant anti-proliferation activities against HepG2 cells, with IC50 values of 13.5 μM, 9.7 μM and 11.6 μM respectively, obtained following 48 h treatment.
