Design and trial of a new questionnaire for occupational health surveys in companies. (1/795)

In this article we present an example of our method for instrument development. This method is called the Development Cycle. It consists of four main stages: (1) defining the requirements for an instrument; (2) research, design and pilot testing; (3) implementation and (4) evaluation. An application of the Development Cycle was realized within a project for the development of a basic questionnaire about work and health, to be used at periodic health surveys. This questionnaire had to identify work and work-related health problems in employees with divergent occupations and working conditions. The design of the instrument and the results of its trial in 517 employees is presented. The evaluation of the test results and the modification of the questionnaire are discussed. From 1995, the questionnaire has been implemented in the Dutch OHS services quite successfully.  (+info)

Practice patterns, case mix, Medicare payment policy, and dialysis facility costs. (2/795)

OBJECTIVE: To evaluate the effects of case mix, practice patterns, features of the payment system, and facility characteristics on the cost of dialysis. DATA SOURCES/STUDY SETTING: The nationally representative sample of dialysis units in the 1991 U.S. Renal Data System's Case Mix Adequacy (CMA) Study. The CMA data were merged with data from Medicare Cost Reports, HCFA facility surveys, and HCFA's end-stage renal disease patient registry. STUDY DESIGN: We estimated a statistical cost function to examine the determinants of costs at the dialysis unit level. PRINCIPAL FINDINGS: The relationship between case mix and costs was generally weak. However, dialysis practices (type of dialysis membrane, membrane reuse policy, and treatment duration) did have a significant effect on costs. Further, facilities whose payment was constrained by HCFA's ceiling on the adjustment for area wage rates incurred higher costs than unconstrained facilities. The costs of hospital-based units were considerably higher than those of freestanding units. Among chain units, only members of one of the largest national chains exhibited significant cost savings relative to independent facilities. CONCLUSIONS: Little evidence showed that adjusting dialysis payment to account for differences in case mix across facilities would be necessary to ensure access to care for high-cost patients or to reimburse facilities equitably for their costs. However, current efforts to increase dose of dialysis may require higher payments. Longer treatments appear to be the most economical method of increasing the dose of dialysis. Switching to more expensive types of dialysis membranes was a more costly means of increasing dose and hence must be justified by benefits beyond those of higher dose. Reusing membranes saved money, but the savings were insufficient to offset the costs associated with using more expensive membranes. Most, but not all, of the higher costs observed in hospital-based units appear to reflect overhead cost allocation rather than a difference in real resources devoted to treatment. The economies experienced by the largest chains may provide an explanation for their recent growth in market share. The heterogeneity of results by chain size implies that characterizing units using a simple chain status indicator variable is inadequate. Cost differences by facility type and the effects of the ongoing growth of large chains are worthy of continued monitoring to inform both payment policy and antitrust enforcement.  (+info)

Can restrictions on reimbursement for anti-ulcer drugs decrease Medicaid pharmacy costs without increasing hospitalizations? (3/795)

OBJECTIVE: To examine the impact of a policy restricting reimbursement for Medicaid anti-ulcer drugs on anti-ulcer drug use and peptic-related hospitalizations. DATA SOURCES/STUDY SETTING: In addition to U.S. Census Bureau data, all of the following from Florida: Medicaid anti-ulcer drug claims data, 1989-1993; Medicaid eligibility data, 1989-1993; and acute care nonfederal hospital discharge abstract data (Medicaid and non-Medicaid), 1989-1993. STUDY DESIGN: In this observational study, a Poisson multiple regression model was used to compare changes, after policy implementation, in Medicaid reimbursement for prescription anti-ulcer drugs as well as hospitalization rates between pre- and post-implementation periods in Medicaid versus non-Medicaid patients hospitalized with peptic ulcer disease. PRINCIPAL FINDINGS: Following policy implementation, the rate of Medicaid reimbursement for anti-ulcer drugs decreased 33 percent (p < .001). No associated increase occurred in the rate of Medicaid peptic-related hospitalizations. CONCLUSIONS: Florida's policy restricting Medicaid reimbursement for anti-ulcer drugs was associated with a substantial reduction in outpatient anti-ulcer drug utilization without any significant increase in the rate of hospitalization for peptic-related conditions.  (+info)

Neonatal examination and screening trial (NEST): a randomised, controlled, switchback trial of alternative policies for low risk infants. (4/795)

OBJECTIVE: To evaluate the effectiveness of one rather than two hospital neonatal examinations in detection of abnormalities. DESIGN: Randomised controlled switchback trial. SETTING: Postnatal wards in a teaching hospital in north east Scotland. PARTICIPANTS: All infants delivered at the hospital between March 1993 and February 1995. INTERVENTION: A policy of one neonatal screening examination compared with a policy of two. MAIN OUTCOME MEASURES: Congenital conditions diagnosed in hospital; results of community health assessments at 8 weeks and 8 months; outpatient referrals; inpatient admissions; use of general practioner services; focused analysis of outcomes for suspected hip and heart abnormalities. RESULTS: 4835 babies were allocated to receive one screening examination (one screen policy) and 4877 to receive two (two screen policy). More congenital conditions were suspected at discharge among babies examined twice (9.9 v 8.3 diagnoses per 100 babies; 95% confidence interval for difference 0.3 to 2.7). There was no overall significant difference between the groups in use of community, outpatient, or inpatient resources or in health care received. Although more babies who were examined twice attended orthopaedic outpatient clinics (340 (7%) v 289 (6%)), particularly for suspected congenital dislocation of the hip (176 (3.6/100 babies) v 137 (2.8/100 babies); difference -0.8; -1.5 to 0.1), there was no significant difference in the number of babies who required active management (12 (0.2%) v 15 (0.3%)). CONCLUSIONS: Despite more suspected abnormalities, there was no evidence of net health gain from a policy of two hospital neonatal examinations. Adoption of a single examination policy would save resources both during the postnatal hospital stay and through fewer outpatient consultations.  (+info)

General practitioners' continuing education: a review of policies, strategies and effectiveness, and their implications for the future. (5/795)

BACKGROUND: The accreditation and provision of continuing education for general practitioners (GPs) is set to change with new proposals from the General Medical Council, the Government, and the Chief Medical Officer. AIM: To review the theories, policies, strategies, and effectiveness in GP continuing education in the past 10 years. METHOD: A systematic review of the literature by computerized and manual searches of relevant journals and books. RESULTS: Educational theory suggests that continuing education (CE) should be work-based and use the learner's experiences. Audit can play an important role in determining performance and needs assessment, but at present is largely a separate activity. Educational and professional support, such as through mentors or co-tutors, has been successfully piloted but awaits larger scale evaluation. Most accredited educational events are still the postgraduate centre lecture, and GP Tutors have a variable role in CE management and provision. Controlled trials of CE strategies suggest effectiveness is enhanced by personal feedback and work prompts. Qualitative studies have demonstrated that education plays only a small part in influencing doctors' behavior. CONCLUSION: Maintaining good clinical practice is on many stakeholders' agendas. A variety of methods may be effective in CE, and larger scale trials or evaluations are needed.  (+info)

Evaluating a policy of reduced consultant antenatal clinic visits for low risk multiparous women. (6/795)

OBJECTIVES: To evaluate a change in antenatal care policy to reduce antenatal clinic visits, whereby low risk multiparous women were managed by the primary care team and seen at booking and at 41 weeks' gestation at the consultant antenatal clinic. DESIGN: Comparative study of low risk multiparous women retrospectively identified through the Oxford obstetric data system and cared for by three consultants who changed their policy (group A) or three consultants who maintained their routine care (group B). SETTING: Oxfordshire Health District. SUBJECTS: 2153 low risk multiparous women (1079 group A, 1074 group B) booked for consultant care at John Radcliffe Maternity Hospital between August 1985 and July 1987. MAIN MEASURES: Comparison of pregnancy outcomes, satisfaction with care, and clinic waiting times, during one year before and after the policy change (year 1, year 2). RESULTS: The proportion of women in group A with only one or two consultant clinic visits increased from 19.9% to 57.9% between years 1 and 2 (p < 0.001). Clinic waiting times did not improve. Of five perinatal deaths in group A, one (from postmaturity) could possibly be attributed to the policy change. The proportion of women reaching 42 weeks' gestation rose from 4.7% to 9.2% (p < 0.01); the proportion fully satisfied with their care rose from 68.4% to 82.1% (p < 0.025). No such changes were seen in group B. CONCLUSIONS: The change in policy was successful in reducing hospital antenatal clinic visits. The exercise identified dilemmas around evaluating changes in antenatal care settings. IMPLICATIONS: Criteria to test policy objectives should be selected carefully and rare events assessed prospectively in order to detect problems early.  (+info)

Implementing a policy for pneumococcal prophylaxis in a haematology unit after splenectomy. (7/795)

People who have had a splenectomy for any reason are 40 times more likely to have an overwhelming infection, especially pneumococcal infection, and 17 times more likely to suffer fatal sepsis. The incidence of such life threatening infections is reduced by prophylactic immunisation with polyvalent pneumococcal vaccine and long term antibiotic prophylaxis or instituting prompt antibiotic treatment in the event of fever. This haematology unit agreed a policy of immunisation and antibiotic prophylaxis in June 1988 for all patients undergoing elective splenectomy. The success of this policy was audited in July 1993 by a retrospective analysis of patients' case notes. Seventy four patients were identified as having had a splenectomy, 54 (73%) before June 1988, of whom only 13 (24%) had received both pneumococcal immunisation and antibiotic prophylaxis before implementation of the agreed policy. At the time of audit, 46/74 (62%) patients were recorded as having received immunisation and 64/74 (86%) as receiving antibiotic prophylaxis or a supply of antibiotics to take in the event of a fever. All but one of the 20 patients who had a splenectomy after June 1988, since implementation of the agreed policy, received immunisation and antibiotic prophylaxis. The authors conclude that establishment of a formal agreed policy for pneumococcal prophylaxis for patients undergoing splenectomy has improved the quality of care.  (+info)

Health outcomes and managed care: discussing the hidden issues. (8/795)

Too often the debate over health outcomes and managed care has glossed over a series of complex social, political, and ethical issues. Exciting advances in outcomes research have raised hopes for logical medical reform. However, science alone will not optimize our patients' health, since value judgements are necessary and integral parts of attempts to improve health outcomes within managed care organizations. Therefore, to form healthcare policy that is both fair and efficient, we must examine the fundamental values and ethical concerns that are imbedded in our efforts to shape care. We must openly discuss the hidden issues including: (1) trade-offs between standardization of care and provider-patient autonomy; (2) effects of financial incentives on physicians' professionalism; (3) opportunity costs inherent in the design of insurance plans; (4) responsibilities of managed care plans for the health of the public; (5) judicious and valid uses of data systems; and (6) the politics of uncertainty.  (+info)

Pregnancy is a difficult and worrying time for any parent and mums are constantly told to watch their babies movements. Sadly some hospitals policies are still that you have to be 28 weeks to be monitored or scanned, before this only a heartbeat will be checked. This needs to change.. I was a normal healthy pregnancy until 25 weeks. My babies movements started to decrease and I let professionals know, but I wasnt listened to. I knew her pattern, I knew she wasnt moving as much.. I went in several times but pretty quickly discovered our hospitals policy along with some other hospitals is still 28 weeks to be scanned or monitored.. Checking for a heartbeat does not ensure baby is ok. If a baby has a heartbeat then you would assume all is fine, but sometimes this is not the case at all and baby is in trouble. Professionals tell you not to use home dopplers for reassurance for this very reason! Yet still a lot of hospitals policies state this is all that will be done if a mum is in for reduced ...
What issues are you currently focused on?. Im focusing on assisting organizations to become more welcoming to sexual and gender diverse identities by conducting education sessions on diversity and inclusion that include sharing current initiatives and good practices along with assisting with policy development, review and implementation.. What steps should organizations take as they develop diversity and inclusion policies and guidelines?. The first step is to ensure that leadership starts with your organizations board with a strong commitment to diversity and inclusion and a commitment to creating respectful work environments. When senior leadership makes this commitment and carries through, others in the organization will follow. The second step is to ensure that these commitments are made into separate organizational policies. Once these are completed, the organization needs to ensure these two policies are linked to other organizational policies, including but not limited to: equal ...

The doctors and staff Bradford Veterinary Clinic work very hard to give exemplary service to our clients and pets. One way that we can provide this service is to have appointments available when our clients and patients need them. Forgetting to come to an appointment happens on occasion but missed appointments mean other clients and patients in need must wait to be seen.. While we are sure the last thing clients who miss appointments want to do is create a barrier to the care of someone elses pet, that is, in effect, what happens when we do not have the option of scheduling into a time slot occupied by missed appointments. This is even more concerning when on rare occasions, a client misses a surgery time for which several hours have been reserved.. Results 457 organisations participated (31% response rate); 77% employed less than 10 people, 17% between 10 and 50 and 6% more than 50. Risk assessments were common (67%) and 14% carried out any form of occupational asthma HS, rising to 19% if only organisations reporting asthma hazards and risks were considered. HS was carried out by both in-house (31%) and external providers (69%). Organisational policies were often used to define surveillance approaches (80%), but shared with the occupational health provider only in one third of cases.. Occupational health providers described considerable variation in practice, with differing approaches seen for information sharing and workplace visits. Record keeping was universal, but worker-held records were not reported. Health surveillance tools, such as a questionnaire, were generally developed in-house. Lung function was commonly measured, but only limited interpretation was evident. The referral of workers to local specialist respiratory services was ...

There are a number of things an organization should and must do. We consider that there are at least five areas that should take priority.. Review business continuity plans and develop playbooks to account for the new challenges. These efforts should include, but not be limited to, preparing for the temporary or permanent loss of key staff and leadership, the evacuation of a security operations center, or a serious attack where only a portion of staff are able to work.. Increase awareness among the workforce regarding the risks of handling confidential or sensitive information when working remotely by being proactive in communicating and coaching teams on organizational policies and the best dos and donts.. No organization or business is an island. Engage with peers and relevant industry groups to ensure insight on threat intelligence and best practices.. React quickly to this new normal by reassessing risks and ensuring that detection, response and mitigation efforts are aligned ...
In my previous post I introduced the Security Awareness Maturity Model, a tool to help you and your organization identify how mature your security awareness program is and where you can take it. I would like to cover this model in more detail, specifically the first two of the five levels. Level 1: No Security Awareness Program Okay, this is pretty simple, there is no awareness program, there is no attempt to train and educate the organization. As a result people do not know or understand organizational policies and procedures, do not realize they are a target, and are highly vulnerable to most human based attacks. Only about 30% of the organizations I run into have absolutely no awareness program. Instead I find most organizations are at Level 2. This actually may not be a good thing. Level 2: Compliance Focused This is an awareness program designed primarily to meet specific compliance or audit requirements. Training is limited to annual or ad-hoc basis, such as an onsite presentation once a ...

What could be more fun than waking up to find a new furry family member waiting to be discovered? There are many reasons why people like the idea of gifting a pet - a youngster may be begging for a puppy or a kitten, you may think your aging relative or neighbor could use some companionship or you just cant resist the tug on the heartstrings when there are so many shelter animals in need of a loving, forever home. One can surely imagine the sweetness of the moment upon discovery of a baby animal wrapped up in a bow. While adopting a pet is in itself a wonderful idea, theres much to consider before bringing what should become a lifelong family member into your own household much less someone elses.. Animal welfare organizational policies state that surprising someone with an animal at any time, especially during the holidays, may simply be an emotional decision rather than a rational choice. Adding an animal companion to the family is a decision that requires sincere heartfelt and financial ...
Wesley Smith’s First Things column on the danger of hospital futile care policies sounds an alarm (Proposed Futile Care Hospital Policy Would Allow Committees to Overrule Patient Advance Directives). Smith is rightly concerned that a model Futile Care Protocol is to be promoted in the forthcoming issue of the Journal of Law and Medicine. A key element is to make sure that patients and their families understand that the hospital will always have the last word on medical treatment. I
As the decades progress, our arsenal of effective antibiotic agents becomes more limited by the changing patterns of antibiotic resistance. It is the job of the stewardship pharmacist to understand these trends in a quantitative way and to respond in a dynamic fashion. In addition to the active management of restricted antibiotic agents, the stewardship team is responsibility for the development of a large share of the hospitals policies and procedures regarding appropriate antimicrobial use. They work with the physician stewardship team and the P&T committee to answer questions such as: For which patient populations will we sanction the use of fidaxomicin in place of vancomycin for C. Difficle infections? My primary project was in this sort of policy development. My role was to evaluate the recent literature regarding the use of probiotics for C. Difficile prevention. This type of project has traditionally been one of my strong suits, but as I delved deeper, it turned into something of a mess. ...

This recommendation leaves loopholes to vaccinate just about anyone, but is there any science to defend it? How could the benefit of vaccinating a severely-ill patient, or a patient who has organ impairment (and may not mount a significant antibody response anyway) outweigh the risk? Why is there such a rush to vaccinate all hospital patients even though any potential protection will not be present for weeks? Could it have more to do with medical policy and reimbursement than with what is in the best interest of a sick patient?. As doctors, this CDC recommendation isnt adhered to, because before we evaluate a patient, vaccines have already been given by nurses and others who have no medical mandate. This often occurs on the first hospital-day, not when they are well enough to be discharged. My efforts to change the hospital vaccine policy to wait until discharge was categorically refused by the administration and hospital policy makers - at a meeting that I was not permitted to ...
Hello Ladies! I need a few tips in dealing with a post-ceserian hospital policy. The hospital I will be delivering at has a MANDATORY 4 HOUR mommy/baby seperation after cesarian. Updated 14 May 2014. Patients received safe care because risks to patients were understood and were being managed. Hospital policies were based on national standards and evidence-based guidelines and adherence with these was monitored. An uncommissioned 10% rise in demand for outpatient appointments over the past year meant the Trust struggled to meet national standards for referral to treatment time (RTT) for patients. The trust agreed to fail RTT targets for January, February, and March 2014 with the NHS Trust Development Authority, who provide oversight and governance for all NHS trusts, to enable patients who had been waiting longest to be prioritised. This meant that patient safety was prioritised over meeting targets.. Patients were unable to book into appointments using the Choose and Book system on 50% of attempts as this could not be done online and there were not enough administrative staff available to answer calls and make bookings. This resulted in poor experiences for some patients ...

Background: There is a dearth of evidence on the proportion of the hospital population at any one time, that is in the last year of life, and therefore on how hospital policies and services can be oriented to their needs. Aim:To establish the likelihood of death within 12 months of a cohort of hospital inpatients on a given census date. Design:Prevalent cohort study. Participants: In total, 10,743 inpatients in 25 Scottish teaching and general hospitals on 31 March 2010. Results: In all, 3098 (28.8%) patients died during follow-up: 2.9% by 7 days, 8.9% by 30 days, 16.0% by 3 months, 21.2% by 6 months, 25.5% by 9 months and 28.8% by 12 months. Deaths during the index admission accounted for 32.3% of all deaths during the follow-up year. Mortality rose steeply with age and was three times higher at 1 year for patients aged 85 years and over compared to those who were under 60 years (45.6% vs 13.1%; p , 0.001). In multivariate analyses, men were more likely to die than women (odds ratio: 1.18, 95% ...

original link with references www.medicalvoices.org. H1N1 and seasonal influenza vaccines are now being given to sick hospital patients with or without their doctors consent. This is being done despite there being no data on the safety of doing so. I am a licensed, board-certified nephrologist, otherwise known as a kidney specialist, working in a large, city-based hospital. Because I rarely admit patients to the hospital other than for specific procedures, such as a kidney biopsy, I only recently became aware of my hospitals policy regarding flu shots for sick people. Waking up to this new rule made me realize that Big Pharma is getting closer and closer to bypassing doctors completely to deliver direct patient care. We have an elaborate electronic charting system at our hospital. All of the medications and procedure orders are placed into the patients record by doctors and nurses so that every person has access to all that is happening with the patient. A few weeks ago, I arrived to see my ...
For mothers approaching your birthdate, its good to remember to remain flexible. In HypnoBirhting we tell moms to visualize and watch your thoughts, as they create your reality.. but alot of times, the healthcare system in Malaysia makes it close to impossible to have a gentle undisturbed birth..so its not uncommon that hospital policies & procedures get in the way.. So be flexible, but at the same time keep your eye on the goal... and the best advice I ever got as a birthing mother (coming from midwives and ObGyns alike) is : If youre OK, stay at home for as long as possible!! ...
Through Wishes To Die For, Dr. Haselhorst poignantly demonstrates the importance of completing Healthcare Directives. Truly, end-of-life conversations begin in the heart of the person and their family prior to being addressed by a physician, hospital policy, or the government. This book shows us how to start the journey with our loved ones in the comfort of our homes.. ~ Deb Lavender, Missouri State Representative. ...
Heres the most important part: Talk with your child about the change that will be happening. We are so used to doing things for our babies that its easy to forget to fill them in on the plans. Think about how you like requests to be made of you and go from there. From a very young age, younger than most of us expect, our children can understand more of our communication than we realize. If you think of yourself attending to your child the way we would hope a nurse would attend to us in a hospital, a new, helpful perspective may be found. Picture yourself in the hospital for whatever reason. Its the middle of the night and youre thirsty and so you press your call button and wait for someone to come help, but she never shows up. Or she does come, but when she arrives and you ask for a drink she announces that the new hospital policy, as of now, is that no liquids are given at night anymore. You probably wouldnt be happy about either of these scenarios hitting you without warning. But what if ...
Every nurse also took it upon herself (we saw only women nurses during our stay) to adjust my breast and Mikko on it. They did this roughly and without warning, swooping in to correct my latch. I wasnt having any pain, and I had not asked for help. I would have appreciated words of advice, but surprise rough handling of a delicate body part was hard to handle. They also kept readjusting my pillows and hospital bed incline, further disturbing our rest. All of their advice was contradictory, and it made me wonder if there shouldnt be a hospital policy about the best way to breastfeed, just to keep the contradictions lower. One would tell me I needed to lie completely on my side. The next nurse would come in and scold me for lying on my side and point out that my breast was now too low to align with my babys mouth. Another would come in and tell me I shouldnt be lying down at all. For my part, I was feeling overwhelmed, shy, and new to this whole mothering thing, and I didnt appreciate ...
I have lived and worked in urban India for nine years and during that time I have found that midwives are missing from the system. I have witnessed how hospital policies, mixed with religious or family tradition, harm women and their babies.. I have heard and read hundreds of stories about women in India who have been pressured into potentially unnecessary interventions with inaccurate, fear-mongering information. This breaks women. It damages them before they even begin to birth and care for their babies. Most women do not speak of these things because they are told that a healthy mother and baby is all that matters.. I have seen and heard of many tragic situations of pregnancy loss or stillbirth where the mother was not told the truth. In each case, the mother was told her baby was in the NICU. She was lied to and denied the right to meet her baby, to make memories, to grieve, to hold her baby. Mothers are too often then silenced in their grief.. I do not believe that a healthy mother and a ...

My doula came about 20 min after my husband called her, but by that point, in my mind I started to want an epidural. These contractions were different then my contractions with Kennedy. My contractions with Kennedy were in my back and my doula was able to massage them. This time around the contractions came from deep inside and I felt like I was being split in half and I knew that massages were not going to help me. My doula asked my nurse if I could go into the shower. Sitting in the bath really helped with my pain with Kennedys labor, so I was very excited about getting in to the tub. My doula explained to me because of hospital policy I wouldnt be allowed into the tub because my water was broken. Instead she asked the nurse if could I get into the shower. The nurse called the OB on call and the OB said, NO. The OB said, I was a VBAC and she didnt want to take any chances. At that point, I realized because I was a VBAC they wouldnt let me do a lot of things to help me naturally manage ...

For about 30 minutes. Then the theatrics began again. The doctor ordered 1 more milligram of Dilaudid. I went in to give it and discovered that the IV line was questionable, and since she was getting admitted to rule out MI (no comment), she would need a new line anyway, as hospital policy dictates that pre-hospital lines have to be replaced on admission. So I decided to save the admit nurse some effort and got my IV start kit and put a new 18 gauge in her other arm. The whole time I was talking to her about this and that and everything else. I love chatting with my patients - even the BS ones - because I love people. This time, the conversation was quite a distracting one, and I forgot to flush the line after putting the extension tubing on. We talked for another couple of minutes and then I remembered that I needed to flush the line, so I grabbed my saline syringe and flushed it out as we kept talking ...
We headed on to the clinic and she cried on the way there. She didnt want to be there, we tried to prepare her all week that we were going for a couple of days to the hospital, but it didnt really help much. We thought it might help. She cried all the way! Everything looked good at the clinic, her counts were great, went from Friday, 2/4 of 1.1 WBC to Thursday, 2/10 up to 3.4. So, it was great to see her counts up. However they were still not normal when the 1st day of chemo they were 5.9. After all of that, they gave her a room and we headed to get our stuff. I had asked if it was a shared room and she said it was, but that no one was in there. After we got our stuff and headed up to 7th floor, we found it was a shared room. I was pretty upset, but remained calm. We saw Lauren, one of our favorite nurses and explained that we didnt want a shared room. Someone else was our nurse and she explained since her counts were fine it was hospital policy that she could share a room. I explained to ...
Determine if the use of a Nurse Child Care Health Consultant changes nutrition and physical activity policies at child care centers using criteria based on the national standards identified from resources such as NAEYC accreditation guidelines, Child and Adult Care Food Program (CACFP) policies and requirements, and the joint APHA/AAP publication Caring for our Children (American Academy of Pediatrics, American Public Health Association & National Resource Center for Health and Safety in Child Care, 2002; 1992). If written policies were present, they were compared to the gold-standard criteria from the national resource material and assigned a rating of either none (no written policies), poor (written policies fail to meet either state regulatory criteria or national standards), adequate (meets state regulatory criteria but not all of the national standards), or excellent (meets all of the national criteria present ...

St. Josephs Hospital and Medical Center in Phoenix, Ariz., agreed to take steps to improve communication between doctors and patients after Sue Sheridan lost her husband, Pat, to a misdiagnosed malignant tumor. Sheridan went on to establish Consumers Advancing Patient Safety (CAPS), which aims to prevent diagnostic errors and miscommunication in hospitals.. Sheridan also saw her newborn son become irreparably brain damaged at St. Lukes Boise Medical Center in Idaho, where the infants jaundice symptoms went too long uncaught. The hospital has launched a series of quality and disclosure initiatives, including a jaundice monitoring program which screens all newborns before discharge. St. Lukes has also implemented a disclosure policy that requires attending physicians to talk with families after unanticipated outcomes occur during care.. The fourth hospital, St. Peters Hospital in Albany, New York, did not respond to calls for comment about a 2001 case involving 11-year-old Justin Micalizzi, ...
Delirium recognition improved between the first half of recruitment and second half. There was no change in local hospital policy during this time period that may have affected this. Local hospital policy was concordant with British Geriatrics Society and NICE guidelines, which recommend that all patients aged 65 years and older who are newly admitted to hospital are screened for delirium.23 This guidance existed throughout the course of this research project and did not change during this time. Formal delirium diagnosis was made during the initial study using recognised DSM-IV criteria by an expert; results are representative of true delirium recognition. The protocol for this study was developed and approved prior to the introduction of DSM-V and we recognise that there are differences between DSM-IV and DSM-V. However, concordance of 91% between DSM-IV and DSM-V has been demonstrated when using a relaxed approach to the DSM-V criteria.26 Increased knowledge of delirium through awareness of ...

DEPARTMENT OF STATE. Division of Professional Regulation. 2500 Board of Pharmacy. Statutory Authority: 24 Delaware Code, Section 2509 (24 Del.C. §2509). 24 DE Admin. Code 2500. PROPOSED. PUBLIC NOTICE. The Delaware Board of Pharmacy, in accordance with 29 Del.C. Chapter 101 and 24 Del.C. §2509, proposes amendments to its regulation 3.0. Specifically, the proposed addition to 3.0 Pharmacy Requirements would require the pharmacist-in-charge at each pharmacy to develop written policies for situations in which a pharmacists professional obligation to dispense certain pharmaceuticals may conflict with the pharmacists personal beliefs, potentially causing the pharmacist to refuse to dispense the pharmaceutical. The Boards proposed regulatory changes do not specify how such situations should be handled. Instead, the proposed changes merely require the pharmacist-in-charge at each pharmacy to develop written policies for such situations.. The Board also proposes amendments to its regulation 16.0 ...
Clinically-driven research projects are initiated by clinicians who work in Clalits community clinics and hospitals around the country. These research projects are collaborative endeavors between the Clalit Research Institutes team of epidemiologists, bio-statisticians, data scientists, and the clinician researcher in his or her field of practice.. From dozens of applicants, a cohort of 10 clinician researchers is sponsored to work with Clalit Research Institute researchers on-site weekly in a closely mentored 2-year program. This collaboration draws on the real-world clinical experience of the clinician and the epidemiological, statistical and data science expertise of the Clalit Research Institute team to design and implement studies that will answer these challenging clinical questions from the field.. The findings from these research studies are then used to inform clinical practice, influence organizational policies, and improve the quality of care and delivery of health services for ...

People with intellectual and/or developmental disabilities (IDD) living in group homes are more likely to have poor oral hygiene, poor dietary practices, increased decay, periodontal disease, and total tooth loss than the general population-a significant health disparity. In the short term, the effects of a Phase II stratified cluster randomized controlled efficacy trial can provide evidence that argues for public health and organizational policies that include oral health promotion (OHPROM) strategies for persons with IDD. In the long term, this study is expected to demonstrate that the OHPROM strategy targeting caregivers and persons with IDD may potentially be generalized to other organizations that provide services to persons with IDD ...
The campaign, which highlights everyday Coloradans, is the first in the country specifically designed to de-stigmatize obesity, illustrating how common the problem is and emphasizing that the obesity issue is not about vanity, attractiveness or labeling. The issue of obesity is about health - our health, our childrens health and our states health.. Obesity not only impacts our personal quality of life, but also our well-being as a state. Colorado spends more than $800 million annually on obesity-related medical costs and that number is sure to rise at the same pace as the numbers on our scales.. Colorado needs a culture shift that inspires people to make sustainable healthier choices and to support public and organizational policies, from farm-to-school programs to worksite wellness initiatives that make these choices available. We are confident the campaign will serve as the catalyst.. What can we do? Watch the campaign videos and allow yourself - maybe for the first time - to make a personal ...

Per hospital policy, when a blood transfusion is ordered, two registered nurses are supposed to perform a two-tiered check to ensure the right blood is going to the right patient. Before the blood enters the room, two nurses must make sure the patients information lines up with the blood product. If everything matches, the nurses are supposed to then go to the patients bedside and perform additional checks to verify the identity of the patient.. However, some nurses have testified in the hearing that its common practice that everything is checked and signed off on at the nurses station and then the primary nurse goes into the patients room to verify identity and administer the blood transfusion.. During the hearing, nurses including and in addition to the two nurses fired, have said it is common practice to check the blood for the transfusion at the nurses desk. However, Karen Ames, director of quality and patient safety at Cayuga Medical Center, testified Monday that she found no evidence ...

As the donation process was underway, Gift of Hope worked with the staff at Lurie Childrens Hospital to accommodate and support the family in as many of their needs and requests as possible.. Ralph made a very special request to Jessica Kasparek, an Allocation Coordinator at Gift of Hope - that he be allowed inside the operating room with Maddox and to hold his sons hand when doctors stopped his heart. Although this request fell outside of hospital policy, Jessica did everything she could to get the hospitals approval.. The hospital made an exception to let Ralph and Kathleen inside the operating room to say their final goodbyes to Maddox but could not break its policy to allow them to remain while doctors stopped his heart to carry out the organ recovery.. This case was an amazing reflection of teamwork between Gift of Hope and Lurie, and I hope the family felt we did everything we could to meet their expectations and to make them feel supported, loved and heard, Jessica said. A potential casualty of employment in a hospital system may be the ability to openly disagree with the organization. Will surgeons, as highly paid employees, be confident enough to speak up against hospital policies affecting patient care without worrying about corporate retaliation? Will employed surgeons be able to speak out against hospital cost-cutting measures that infringe on patient care without being labeled whistleblowers or troublemakers? Can they voice their displeasure without worrying about the security of their job? If you are branded not a team player, referrals may dry up. Or, you may suddenly be asked to take more emergency room call. You may also be asked to travel farther to see patients and generate surgical business in another town. You may be replaced. You could end up as a surgeon without a practice. If let go, you may discover that the clause in your contract prohibiting you from practicing within the area drives you out of town ...
Help!! Im a new ER nurse..used to be a hospice nurse...the last thing I want to do is to have to revert back to hospice nursing practice while in the ER. The ER I work in routinely uses Ketamine for sedation on pediatric patients for procedures. They also use diprovan when doing cardioversions. I asked the ER educator about scope of practice and nurses giving anesthesia meds such as diprovan and she said its not a problem with scope of practice. Hospital policy for moderate sedation allows for use of these medications and to be administered by RNs. Yet at another hospital I worked at RNs were forbidden to administer any anesthesia meds. Would my state board of nursing be a resource for resolving this issue? If not who should I talk to ...

Its a common strand when we talk about improving data sharing or data management, or access to research, or public engagement…or… Cultural Change, its hard. The capitals are deliberate. Cultural Change is seen as a thing, perhaps a process that we need to wait for, because we have internalised the Planck narrative that science progresses one funeral at a time1. Yet we are also still waiting for the Google generation with their culture of sharing to change the world for us.. Some of us get bored waiting and look for hammers. We decide that policies, particularly funder policies, but also journal or institutional policies requiring data sharing, are the best way to drive changes in practice. And these work, to some extent, in some places, but then we (and I use the we seriously) are disappointed when changes in practice turn out to be the absolute minimum that has to be done; that the cultural change that we assumed would follow does not, even in some cases is hampered by an ...
