A cost/efficacy analysis of oral antifungals indicated for the treatment of onychomycosis: griseofulvin, itraconazole, and terbinafine. (1/133)

This analysis was conducted at HIP Health plan of New Jersey (a Northeastern group model health maintenance organization) to determine the most cost-effective therapy among the three currently available oral antifungal drugs that are indicated for the treatment of onychomycosis: griseofulvin, itraconazole, and terbinafine. Costs of an appropriate and complete treatment regimen were calculated for each of the three drugs based on average wholesale price. Efficacy was determined by meta-analysis of the published literature for those studies where appropriate treatment regimens for onychomycosis were put to use. Efficacy outcome measures were limited to mycologic cure rates in the more recalcitrant cases of toenail onychomycosis. From these measures of cost and efficacy, a cost/efficacy ratio was calculated for each drug by dividing the cost per treatment by the weighted average mycological cure rate. This ratio represents the cost per mycologically cured infection. The final outcome measure (the cost per mycologically cured infection) was $2,721.28, $1,845.05, and $648.96, for griseofulvin, itraconazole, and terbinafine continuous therapies, respectively. For itraconazole and terbinafine pulse therapy, the costs were $855.88 and $388.50, respectively. For both continuous and pulse therapy, terbinafine is apparently the most cost-effective drug, followed by itraconazole and then by griseofulvin. Terbinafine has the fewest drug interactions and the highest treatment success rate.  (+info)

Cytokinesis-block micronucleus assay in primary human liver fibroblasts exposed to griseofulvin and mitomycin C. (2/133)

Primary liver fibroblasts were applied in a cytokinesis-block micronucleus assay in combination with fluorescence in situ hybridization (FISH) using two protocols. In protocol A (Prot. A), cytochalasin B (Cyt B) was added at the end of the treatment time directly to the medium containing the standard compounds, whereas in protocol B (Prot. B) the chemical-containing medium was removed and fresh medium with Cyt B was added. The study was performed using the aneugen griseofulvin (GF) and the clastogen mitomycin C (MMC) as standard compounds. With both protocols GF induced a significant increase in MN frequency over controls in a dose-related manner at the lower concentrations tested (7.5 and 15 microg/ml). At the highest dose (30 microg/ml) the aneugen effect was substantially reduced. MN induction obtained with Prot. A was significantly higher ( approximately 3-fold) than with Prot. B at the most effective concentration. The aneugen effect induced by GF did not change when different cell densities were used, but again with Prot. A we obtained the highest effect. MN induced by MMC showed a dose- and time-dependent increase in both protocols. In contrast to GF, the greater clastogenic response induced by MMC in human liver fibroblasts was obtained with Prot. B, approximately 3-fold higher than Prot. A at the most effective concentration and approximately 2-fold with 24 h treatment at 0.17 microg/ml MMC. With GF, the FISH data in human liver fibroblasts (80% C+MN) were fairly consistent with those obtained in the rodent cell lines. In human whole blood cultures, the same dose used in our experiment produced a relatively higher percentage of C+MN. FISH analysis showed that MMC induced mainly MN containing acentric fragments rather than whole chromosomes. In conclusion we have demostrated that chemically induced genetic effects are strongly dependent on the cell culture employed, treatment schedule and intra- and post-treatment experimental conditions.  (+info)

The in vivo gut micronucleus test detects clastogens and aneugens given by gavage. (3/133)

A general testing battery for pharmaceuticals includes a bacterial gene mutation assay, an in vitro chromosomal aberration or a gene mutation test on mammalian cells and an in vivo test for chromosome/genome mutations. The aim of this study was to determine whether the in vivo mouse gut micronucleus assay could be a more sensitive method to detect direct clastogens and/or aneugens given orally by gavage than the in vivo bone marrow micronucleus assay (which can also detect indirect genotoxins). Two laboratories collaborated in this project, one analysing bone marrow cells and the other analysing gut cells from the same animals. The reference substances tested in this study were colchicine (COL), carbendazim (CAR), tubulazole (TUB) and griseofulvin (GRI), all known aneugens, and 1,2-dimethylhydrazine (DMH), a colon carcinogen with clastogenic activity. For all substances tested, the in vivo gut micronucleus test was as sensitive as or more sensitive than the in vivo bone marrow micronucleus assay: COL and TUB induced micronuclei in both gut and bone marrow cells; DMH, CAR and GRI induced micronuclei only in gut cells. The results show that the micronucleus test on gut cells is able to detect clastogens and aneugens given orally by gavage, some of which were not detected by the bone marrow micronucleus test.  (+info)

Colchicine inhibition of plasma protein release from rat hepatocytes. (4/133)

Colchicine, both in vitro and in vivo, inhibits secretion of albumin and other plasma proteins. In vitro, secretion by rat liver slices is inhibited at 10-minus 6 M with maximal effect at 10-minus 5 M. Inhibition of secretion is accompanied by a concomitant retention of nonsecreted proteins within the slices. Colchicine does not inhibit protein synthesis at these concentrations. Vinblastine also inhibits plasma protein secretion but lumicolchicine, griseofulvin, and cytochalasin B do not. Colchicine also acts in vivo at 10-25 mumol/100 g body weight. Inhibition of secretion is not due to changes in the intracellular nucleotide phosphate levels. Colchicine, administered intravenously, acts within 2 min and its inhibitory effect lasts for at least 3 h. Colchicine has no effect on transport of secretory proteins in the rough or smooth endoplasmic reticulum but it causes these proteins to accumulate in Golgi-derived secretory vesicles.  (+info)

Regulation of coremium morphogenesis in Penicillium claviforme. (5/133)

Coremia of Penicillium claviforme develop in three stages; primordium formation, elongation, and sporulation. Primordium formation was induced by external nutrients, while starvation initiated the differentiation of primordia into coremia with sporeheads. There is strong evidence that external nutrients are not taken up during this differentiation. Continued sporulation by mature coremia again required an external nutrient supply.  (+info)

Spontaneous and induced chromosome damage in somatic cells of sporadic and familial Alzheimer's disease patients. (6/133)

Alzheimer's disease (AD) is a neurodegenerative disorder of the elderly with a complex etiology due to the interaction between genetic and environmental factors. At least 15% of cases are inherited as an autosomal dominant mutation, but the majority are sporadic. We evaluated cytogenetic alterations, both spontaneous and chemical-induced [aluminium (Al) and griseofulvin (GF)], by means of the micronucleus (MN) test in lymphocytes or skin fibroblasts of 14 patients with sporadic and eight with familial Alzheimer's disease (FAD), respectively. The spontaneous MN frequencies of sporadic (20.8 +/- 9.2) and familial (20.7 +/- 4.6) AD patients are significantly higher than those of the respective control groups (9.0 +/- 6.8 and 6.7 +/- 3.4). In all AD patients, GF significantly increased the spontaneous MN frequency of somatic cells to a lesser extent (P < 0.05) as compared with the control group. Al treatment did not induce MN in AD patients. The results of the present study indicate that different types of somatic cells from sporadic and familial AD patients show comparable levels of spontaneous cytogenetic anomalies, and MN induction is partially reduced or lacking according to the type of chemical treatments.  (+info)

Stimulation of the pathway of porphyrin synthesis in the liver of rats and mice by griseofulvin, 3,5-Diethoxycarbonyl-1,4-dihydrocollidine and related drugs: evidence for two basically different mechanisms. (7/133)

Griseofulvin and isogriseofulvin cause, like 3,5-diethoxycarbonyl-1,4-dihydrocollidine, a fall in the activity of the hepatic enzyme porphyrin-metal chelatase and accumulation of protoporphyrin in the liver. Analogues of either griseofulvin or 3,5-diethoxycarbonyl-1,4-dihydrocollidine which do not decrease the chelatase activity are not porphyrogenic on their own, but can potentiate the porphyria caused by 3,5-diethoxycarbonyl-1,4-dihydrocollidine. This suggests the existence of two basically different mechanisms by which drugs stimulate the pathway of porphyrin synthesis in the liver.  (+info)

Griseofulvin, an antifungal drug, has recently been shown to inhibit proliferation of various types of cancer cells and to inhibit tumor growth in athymic mice. Due to its low toxicity, griseofulvin has drawn considerable attention for its potential use in cancer chemotherapy. This work aims to understand how griseofulvin suppresses microtubule dynamics in living cells and sought to elucidate the antimitotic and antiproliferative action of the drug. The effects of griseofulvin on the dynamics of individual microtubules in live MCF-7 cells were measured by confocal microscopy. Immunofluorescence microscopy, western blotting and flow cytometry were used to analyze the effects of griseofulvin on spindle microtubule organization, cell cycle progression and apoptosis. Further, interactions of purified tubulin with griseofulvin were studied in vitro by spectrophotometry and spectrofluorimetry. Docking analysis was performed using autodock4 and LigandFit module of Discovery Studio 2.1. Griseofulvin strongly

Summary Nine of twenty-five Indonesian patients with tinea imbricata were free of their infections for at least three months following therapy with griseofulvin. Those receiving 2 grams daily were more often cured than were patients on a lower dosage schedule. New lesions appeared on some patients while they were receiving griseofulvin. These lesions could not be controlled by an increased dosage of the drug.

Chronic feeding of griseofulvin, at levels ranging from 0.5%-2.5% of the diet resulted in the development of liver tumors in several strains of mice, particularly in males. Smaller particle sizes result in an enhanced effect. Lower oral dosage levels have not been tested. Subcutaneous administration of relatively small doses of griseofulvin once a week during the first three weeks of life has also been reported to induce hepatomata in mice. Thyroid tumors, mostly adenomas but some carcinomas, have been reported in male rats receiving griseofulvin at levels of 2.0%, 1.0% and 0.2% of the diet, and in female rats receiving the two higher dose levels. Although studies in other animal species have not yielded evidence of tumorigenicity, these studies were not of adequate design to form a basis for conclusion in this regard. In subacute toxicity studies, orally administered griseofulvin produced hepatocellular necrosis in mice, but this has not been seen in other species. Disturbances in porphyrin ...

0048] A comparison of the percentage crystallinity of cryomilled samples of griseofulvin as a function of time, determined using both SONICC and PXRD is shown in FIG. 2. The percentage crystallinity of cryomilled griseofulvin decreases as the milling time increases, consistent with previous studies for both griseofulvin and other organic compounds and demonstrating mechanically induced crystallinity loss. In addition PXRD did not show any broadening of the Bragg peaks with increases in milling time. Peak broadening may be an indication of nanostructure formation. The PXRD method allows for the detection of residual crystallinity after milling for 60 minutes, corresponding to approximately 12% crystallinity. At lower levels of crystallinity, the Bragg peaks disappear and a diffuse halo is present. The halo patterns of cryomilled griseofulvin (e.g., after 180 minutes milling) appear to be visibly indistinguishable from the pattern of melt-quenched griseofulvin as shown in FIG. 2B. At this point, ...

The employment of newly discovered, catalytic, asymmetric, carbon-carbon bond forming reactions can allow compounds with various functionalities to be synthesized. Specifically, an antifungal compound referred to as antifungal 8, can be produced by such a scheme. The racemate of this species has only been previously made through a photochemical reaction of a benzophenone.1 Even in its racemic form, antifungal 8 has displayed promising antifungal activity, rivaling that of its parent compound, griseofulvin.1 Griseofulvin is a common antifungal drug that has been rated one of the most essential medicines by the World Health Organization. Since the racemate of antifungal 8 possesses biological activity that surpasses that of griseofulvin against several fungal strains, an asymmetric synthesis of antifungal 8 is desirable. After all, the isolated enantiomer will possess twice the biological activity of the racemate.2 The suggested multistep route for the synthesis of antifungal 8 includes two of the ...
If you are unlucky and still manage to catch ringworm, have no fear, it is not a serious condition and it is easily treatable. It can be dealt with by using simple over-the-counter anti-fungal creams such as clotrimazole (brand name: Mycelex, Tinactin, Micasil, Lamisil, or Lotrimin). Apply the cream twice a day to the patches and the surrounding skin.. Wash the rash with water and soap and make sure you dry it completely. You can also use compressed which can be bought without a prescription that will soothe and dry the blisters out.. In the case of scalp infections, doctors usually prescribe antifungal tablets (terbinafine or griseofulvin) and an antifungal shampoo. Terbinafine tablets, however, do have some short-term side effects such as nausea, diarrhea, and indigestion, and griseofulvins side effects include vomiting, headache, indigestion, mild diarrhea, and nausea, and it should not be taken during pregnancy as it can cause birth defects.. Finally, if your ringworm is very persistent and ...
Treatment for ringworm varies depending on the particular fungus involved. Some types of ringworm infection will go away spontaneously and no treatment is given. However, most commonly, griseofulvin, an anti-fungal, is used. Griseofulvin is very effective against fungi in hair and skin but it is not so good at treating yeast or bacterial infections. The drug gradually accumulates in the skin and hair. It especially likes to bind with keratin, which is a key component of hair, skin, and nails, and blocks the fungus from infecting the keratin ...

In tests of 31 antibiotics, 29 inhibited growth ofBotrytis cinerea and of these, 18 induced morphological changes. Terminal and lateral branching of the hyphae was induced by actinomycin D, aspergillic acid, citrinin, cyanein, cycloheximide, desertomycin and polyene antibiotics. Curling of the hyphae was induced by griseofulvin and narrowing of the hyphae by citrinin. Some antibiotics at different concentrations produced several types of morphological changes. For example, aspergillic acid, desertomycin and flavofungin also induced terminal bulging of the hyphae. Growth of the dimorphic fungusPaecilomyces viridis was inhibited by 24 antibioties, nine of which induced morphological changes. Branching of the hyphae was induced by azalomycin F, citrinin, eyanein, desertomycin, patulin, rugulosin and trichothecin. Griseofulvin had a curling effect. Except for rugulosin, the above antibiotics, in higher concentrations, induced yeast-like growth ofPaecilomyces viridis. Morphological changes were also induced

As 95% of all prescriptions are for orally administered drugs, the issue of oral absorption is central to the development of pharmaceuticals. Oral absorption is limited by a high molecular weight (,500 Da), a high log P value (,2.0) and low gastrointestinal permeability. We have designed a triple action nanomedicine from a chitosan amphiphile: quaternary ammonium palmitoyl glycol chitosan (GCPQ), which significantly enhances the oral absorption of hydrophobic drugs (e.g., griseofulvin and cyclosporin A) and, to a lesser extent, the absorption of hydrophilic drugs (e.g., ranitidine). The griseofulvin and cyclosporin A C(max) was increased 6- and 5-fold respectively with this new nanomedicine. Hydrophobic drug absorption is facilitated by the nanomedicine: (a) increasing the dissolution rate of hydrophobic molecules, (b) adhering to and penetrating the mucus layer and thus enabling intimate contact between the drug and the gastrointestinal epithelium absorptive cells, and (c) enhancing the ...
