In an effort to achieve site-neutrality in terms of Medicare reimbursement between certain off-campus provider-based departments of hospitals (PBDs) and free-standing physician clinics, the Centers for Medicare & Medicaid Services (CMS) has announced further reductions in reimbursement for certain items and services furnished by those PBDs that began billing under the Medicaire Outpatient Prospective Payment Sytem (OPPS) on or after November 2, 2015 (Non-Excepted PBDs).. The Bipartisan Budget Act of 2015 requires that payment for certain items and services furnished by Non-Excepted PBDs on or after January 1, 2017 be made under the Medicare Physician Fee Schedule (PFS) rather than under the OPPS. (Non-Excepted PBDs that provide emergency department services or that satisfied the "under development" exception set out in the 21st Century Cure Act are exempt from this requirement.) For the broad range of nonexcepted items and services furnished by Non-Excepted ...
Disinfectants and antiseptics of different preparations and concentrations have been used universally on animate and inanimate surfaces in hospitals (1 4). The observed increase in the resistance of a number of bacterial pathogens to some disinfectants and antiseptics has resulted in the development of new preparations as well as the introduction of various strategies to achieve optimal efficacy and effectiveness of disinfectants and antiseptics (5, 6).. It has been documented that stock solutions of disinfectants/antiseptics sold commercially, diluted disinfectants/antiseptics prepared in hospital pharmacies for distribution to the various hospital departments, and disinfectants/antiseptics that are in use in hospital departments have been exposed to contamination (7, 8). The levels of contamination by some of these bacteria have ranged ...

There could be no greater honor for my father than to name this emergency department after him; this place of great caring, this safety net for the community, said Deirdre Daly, Dalys daughter, shortly before she and other family members unveiled the interior signs for the refurbished emergency center, which will open in January. Illuzzi said the new emergency center, when completed sometime next year, will have more than triple the space, 62 beds, specialized trauma and critical care suites, an expanded behavioral health area, and a reduction in the door-to-doctor

The Centers for Medicare and Medicaid Services (CMS) released the final physician fee schedule rule for Calendar Year (CY) 2012, which sets the therapy cap on outpatient services (except outpatient hospital departments) at $1,880 beginning January 1, 2012. The therapy cap exceptions process will expire on December 31, 2011, unless Congress acts to extend it.. The final rule calls for a 27.4% cut in Medicare payments-less than the 29.5% cut estimated earlier this year-for physicians, physical therapists, and other health care professionals based on the flawed sustainable growth rate formula (SGR). However, if Congress intervenes before the January 1, 2012, effective date, the aggregate impact of work Relative Value Units (RVU), practice expense RVU, and malpractice RVU changes for 2012 on physical therapy services is a positive 4% (noted on Table 84 on page 1176 of the rule). According to CMS, the Obama administration is "committed to fixing the SGR and ...

OBJECTIVE: Dislocation is a well-known complication after total hip arthroplasty (THA), and is the second-highest cause of revision surgery. Our objective was to assess the effect of preoperative patient education on the occurrence of hip dislocation within 6 months after primary THA. METHODS: Between 1998 and 2007, we conducted a prospective cohort study at the Geneva University Hospital Department of Orthopaedic Surgery, including all primary THAs performed via an anterolateral transgluteal approach with the use of a 28-mm diameter head. The preoperative education session was introduced in June 2002 and included advice on muscle strengthening exercises and postoperative restrictions of range of motion as a means of preventing dislocation. The main outcome was the incidence of dislocation within 6 months of surgery. RESULTS: A total of 597 patients who underwent 656 THAs between June 2002 and June 2007 participated in the education session, whereas 1,641 patients who ...
The group collaborated with the Hospital del Mar-IMIM in Barcelona to tackle the current lack of instruments for assessing respiratory muscle activation during the breathing cycle in clinical conditions.. Working together with the hospitals Department of Respiratory Medicine, IBECs Biomedical Signal Processing and Interpretation group tested respiratory muscle mechanomyography (MMG) as a way to assess the mechanical activation of the inspiratory muscles of the lower chest wall in both patients with chronic obstructive pulmonary disease (COPD) and healthy subjects. This non-invasive method confirmed the relationship between inspiratory muscle activation characterised by a non-linear index (MLZ: multistate Lempel-Ziv) and pulmonary function parameters.. Their results suggest that respiratory muscle MMG is a good reflection of inspiratory effort, and could be used in clinical conditions to estimate the efficiency of the mechanical ...

Background. Sjögrens syndrome (SS) is an autoimmune exocrinopathy characterised by a lymphoplasmacytic infiltration of the exocrine glands. Both xerophthalmia and xerostomia are the most common manifestations of the disease. However serious organ damage such as, pulmonary and neurological involvement, can occur. The prevalence of neurological manifestations of SS varies between 0% and 70% (average 20%), which is largely dominated by peripheral neuropathies¹. The cranial nerve involvement, especially when it is isolated, represents a rare facet of the peripheral neuropathy.. Observation. We report the case of a 62-year-old patient with no medical history, referred to the internal medicine department with a 6 years history of dry mouth and xerophthalmia. No other complaints were reported.. The mouth examination showed a fissured, smooth and left deviated tongue without evidence of atrophy or fasciculation (figure 1). In 2008 we started to explore curcumin to control the colonic polyp in our 14 year old sons Familial Adenomatous Polyposis in our desire to delay surgery for as long as possible. Our first hope was for him to reach age 18 to allow him to make his own decision regarding medical intervention which gave him time to study for his GCSE exams without interruption from surgery and following recovery.. Back in 2008, as a minor at age 14, understandably there was pressure from the medical department for immediate surgery to take place. A polyp count of 150 plus would have him at risk as the FAP polyps can escalate in an unpredictable manner, therefore we went about to present to the NHS evidence based research papers of curcumin safety in humans, along with other studies which showed curcumin having a positive effect upon the biological pathways that will slow down polyp development. With the Oxford genetic department, satisfied safety was evident and they agreed to support a 9 month trial period of ...

... has a long and proud history. The first shots of the American Revolution fired at Lexington and Concord on 19 April 1775 marked both the birth of a nation and the Continental Army. However, it was the British blockade of the American coast and the need to break that blockade that spawned the Continental Navy and ultimately what we now call the Navy Medical Department. Aboard ships captained by the likes of John Paul Jones and John Barry, were the first sickbays where ship surgeons, assisted by loblolly boys, practiced their healing craft. Although science and medicine has changed greatly over the last three centuries, Navy Medicines mission of healthcare and readiness remains true today as it did in the "Age of Sail ...
