Genetic testing is becoming increasingly important to medical practice. Integrating genetics and genomics data into electronic medical records is crucial in translating genetic discoveries into improved patient care. Information technology, especially Clinical Decision Support Systems, holds great potential to help clinical professionals take full advantage of genomic advances in their daily medical practice. However, issues relating to standard terminology and information models for exchanging genetic testing results remain relatively unexplored. This study evaluates whether the current LOINC standard is adequate to represent constitutional cytogenetic test result reports using sample result reports from ARUP Laboratories. The results demonstrate that current standard terminology is insufficient to support the needs of coding cytogenetic test results. The terminology infrastructure must be developed before clinical information systems will be able to handle the high volumes of genetic data expected in the near future.  (+info)

To address the problem of extracting structured information from pathology reports for research purposes in the STRIDE Clinical Data Warehouse, we adapted the ChartIndex Medical Language Processing system to automatically identify and map anatomic and diagnostic noun phrases found in full-text pathology reports to SNOMED CT concept descriptors. An evaluation of the system's performance showed a positive predictive value for anatomic concepts of 92.3% and positive predictive value for diagnostic concepts of 84.4%. We report the use of an audience response system (ARS) in an undergraduate health information management course. The ARS converts a standard PowerPoint presentation into an interactive learning system that engages students in active participation, and it allows instructors to display questions, surveys, opinion polls, and games. We used the ARS in a 2008 course, Health Services Coding, for lecture and test reviews. The class consisted of 15 students; nine were on-campus students and six were distance education students. All of the responding students agreed that the ARS software facilitated learning and made lectures and reviews more interesting and interactive, and they preferred taking a class using an ARS.  (+info)

ICD-9 billing codes have been proven to be ineffective classifiers in the past [1-3]. Table 2 shows that ICD-9 reached an AUC of 0.897 with an 89.655% accuracy on the cross validated set. On the external test set, ICD-9 performed, effectively, the same, reaching a 90% accuracy and a 0.900 AUC. Predescribed, ICD-9 codes error rates were lowest at 17.1%, but the results show a lower error rate than this. It is believed that this lower error rate is due to the limited number of representative ICD-9 codes used for SLE at MUSC. The AUC and accuracy for the ICD-9 billing code classification have similar values because there is no probability in determining SLE status from an ICD-9 code. Providing computer-aided support for the assignment of diagnosis codes has been approached in numerous ways, often by exploiting free-text fields in patient records. Modeling the 'meaning' of diagnosis codes through statistical data on co-occurrences of words and assigned codes - using a method known as Random Indexing - has only recently been explored as an interesting, alternative solution. I also really cant talk about CPT without bringing up a little tiny thing from the hospital side called a charge description master (CDM), or as its more commonly called, the charge master. Its as masterful as it sounds: a line-item listing of everything a hospital department charges for. Each line item has a description of the charge, charge amount, and sometimes a CPT code. One of the most difficult transitions I see in pro-fee coders crossing over to the hospital side is not understanding that the coder doesnt code everything. There are many codes that are assigned automatically by the charge master when a charge is applied to the bill. This is the case when the CPT code doesnt require a lot of subjective reasoning (e.g., lab test or x-ray). For those procedures and services, such as operative procedures, that require subjective reasoning, a real-live coder will assign the code. It may sound counter intuitive, but this actually increases the amount of coding-related jobs in a hospital. ...
